1.Modified cervical single open-door laminoplasty combined with segmental lateral screw fixation for the treatment of multilevel cervical spondylotic myelopathy

Objective To compare the clinical effects of modified single open-door laminoplasty combined with segmental lateral screw fixation with conventional single open-door combined with continuous screw fixation in the treatment of multilevel cervical spondylotic myelopathy (MCSM).Methods Retrospective analysis was used to analyze the fifty-five cases of MCSM who underwent posterior operation in Lianyungang Hospital Affiliated to Xuzhou Medical University from March 2009 to January 2017.Among them,26 cases in the modified group were treated by modified single open-door laminoplasty combined with segmental lateral screw fixation;29 cases in the traditional group were treated by conventional single open-door combined with continuous screw fixation.JOA score collected before the operation and at the last time of the follow-up,the cervical spine mobility,cervical curvature,cervical axial symptom,C5 nerve root palsy and the incidence rate of screw fracture in both groups were compared.Operation time and the intraoperative blood loss were recorded.Results 55 cases were all followed up.The cervical spine mobility at the last time of the follow-up in the modified group was significantly higher than that of the traditional group ((39.63°±5.98°) vs.(36.14°±6.05°),P=0.036).The cervical curvature in the modified group was higher than that of the traditional group ((19.83°±5.61°) vs.(16.02°±7.05°),P=0.030).The operation time and intraoperative blood loss in the modified group were significantly lower than those of the traditional group ((116.3±13.2) min vs.(128.7±14.3) min,(186.7±39.8) ml vs.(228.7±42.5) ml,P=0.002,0.000).In the modified group,the proportion of patients with postoperative cervical axial symptom was 15.38%,the rate of C5 nerve root palsy cases was 8.69%,compared with 48.27% and 34.48%,respectively in the control group,the difference between the two groups was statistically significant (P=0.011,0.022).In the traditional group,2 cases of screw fracture were found at the last follow-up,while there was no such case in the modified group.Conclusion The modified cervical single open-door laminoplasty combined with segmental lateral screw fixation can obtain good effect of nerve decompression,maintain cervical mobility and cervical curvature,reduce the occurrence of postoperative cervical axial symptom,C5 never root palsy,screw fracture and other complications,is a safe and effective surgical method.

Allergic airway diseases have been identified as chronic inflammatory diseases of respiratory membranes, characterized by infiltration of many inflammatory cells, especially eosinophils. The expression of CCR3 is abundant on the cell surface of eosinophils. Increased accumulation of CCR3-driven inflammatory cells is thought to favor the development of allergy. In this review, we survey the properties of CCR3 and its ligands and highlight the roles of CCR3 signaling in allergic airway diseases.

3.Influence of excipients on the mechanical properties of microcapsules

AIM: To use the increasing rate of hygroscopic weight added to represent the mechanical properties of microcapsules, and to study the influence of excipients on the mechanical properties of microcapsules. METHODS: To develop a method of processing the microcapsules, which simulates the rubbing, shearing effect on the microcapsules during manufacturing, and calculate the increasing rate of hygroscopic weight added to study the influence of excipients on the mechanical properties. RESULTS: The excipients could affect the mechanical properties of microcapsules. CONCLUSION: The mechanical properties of microcapsules is the important quality standard, We can use it for selecting excipients used for microcapsulation.
4.Primary renal lymphoma (report of 3 cases)

ObjectiveTo study the clinical features of primary renal lymphoma.MethodsThe clinical,radiological features,treatment and prognosis of 3 cases of primary renal lymphoma are presented and discussed.ResultsSurgical intervention was carried out in 2 cases,and radiotherapy and chemotherapy were undertaken postoperatively.The other case was treated with chemotherapy alone.One patients survived more than 2 years and 2 more than 1 year.ConclusionsRadiological features are similar to those of renal cell carcinoma. Those with a preoperative diagnosis of primary renal lymphoma should be treated with chemotherapy with or without radiotherapy depending on the histological status,stage and volume of the tumor.
5.Developments of neutrophil function and the relationship between neutrophils dysfunction and periodontitis.

Polymorphonuclear neutrophil leukocyte (PMN) is an important member of the human immune cells. Recentyears, the recognition of the PMN function and the relationship between PMN and periodontitis have been updated. Besidesthe pathogens killing and phagocytosis, PMN also play an important role in immunoregulation and proresolving. The maintaining of PMN homeostasis is an intricate process and the precondition of defense function, which involves activation, adhesion, recruitment, apoptosis and efferocytosis. The regulatory mechanism of PMN homeostasis called neutrophil rheostat, it works through several cytokines and cells. Any factors that break the homeostasis will result in the damage of host immunity,and may relate to the occurrence of periodontitis. Moreover, PMN dysfunction, because of host factors or microorganism factors, is closely related to periodontitis, especially those associated with systemic diseases and gene defect.

6.Construction of tissue engineering adipose with human umbilical cord mesenchymal stem cells transfected by insulin gene and silk fibroin scaffold in vitro

Objective To study the construction of tissue engineering adipose in vitro with silk fibroin scaffold and human umbilical cord mesenchymal stem cells (hUCMSCs) transfected by recombinant human insulin gene lentivirus.Methods hUCMSCs infected with recombinant lentiviral pLenti6.3-insulin-IRES-EGFP (Group A) by the best MOI=10 were seeded in silk fibroin scaffold; hUCMSCs transfected by EGFP gene (Group B)was regarded as negative control; and then the cell-scaf-fold complexes were cultured in adipogenic differentiation medium. MTT test was used to detect whether recombinant lentiviral affected the growth and proliferation of hUCMSCs and the growth activity of hUCMSCs seeded in silk fibroin.Results After 5-7 days for adipogenic culture,the numbers of fat-like cells in group A were significantly more than those in group B (P =0.007,P<0.01).RTPCR results showed that the expression of PPARγ-2 in group A was much stronger than that in group B.MTT test showed that there was no significant difference in optical density (A)at each time between transfected group and nontransfected group (P=0.056,P>0.05).And there was also no significant difference in optical density (A)between cell group and cell-scaffold group (P =0.066,P>0.05).Conclusions Insulin gene could obviously promote hUCMSCs getting into adipose,and carrying recombinant human insulin gene lentiviral vector transfection of hUCMSCs and silk fibroin scaffolds could effectively construct tissue engineering adipose in vitro.
7.Repair of articular cartilage defects with tissue-engineered cartilaginous grafts in rabbits

Objective To observe the effect of repairing the articular cartilage defects with tissue-engineered cartilaginous grafts in rabbits. Methods A total of 60 rabbits were divided into the cartilage graft group, the pure carrier control group and the blank control group. In the cartilage graft group, the bone marrow mesenchymal stem cells (MSCs) of the rabbits were obtained by isolating and culturing the bone marrow aspirates in vitro. The culture system that facilitates the chondrogenous differentiation of MSCs in rabbits was established. The tissue engineering cartilaginous graft was composed of chondrogenetic MSCs, bovine type Ⅰ collagen and human fibrin. Then transplantations of the cartilaginous grafts were performed to repair articular cartilage defects in rabbits. Results Hyaloid cartilage was formed within the defects repaired with the grafts at 12 weeks after transplantation by analyzing the content of type Ⅱ collagen and metachromatism. In the control groups, the fibrous cartilage repair was observed first, then the fibrous tissues and bone repairs were found. Conclusions The cartilaginous graft through tissue engineering is a promising graft for repairing articular cartilage defects.
8.MRI qualitative estimation of cerebrospinal fluid flow in normal volunteers

Objective To evaluate the feasibility of the cerebrospinal fluid (CSF) flow measurement by cine phase contrast MRI. Methods Evaluation of intracranial and intraspinal CSF flow was accomplished by the use of periphery gated cine phase contrast MRI technique. Normal patterns of pulsatile flow within the ventricles, cisterns and cervical subarachnoid space were established by this technique. Results CSF flow within ventricle, cistern and subarachnoid space could be clearly displayed, and the relationships between the cardiac cycle and the CSF pulsations were demonstrated on both magnitude reconstruction and phase reconstruction MR images. Conclusions Cine phase contrast MRI is capable of demonstrating intracranial and intraspinal CSF flow. As a new noninvasive technique, it is really superior than other modalities in studying the flow pattern and direction of CSF.
9.Endovascular stenting for treatment of complex intracranial aneurysms: preliminary outcome

Objective To summarize the technique and efficiency of combined endovascular stenting and Guglielmi detachable coils (GDC) packing for the treatment of intracranial wide-necked bifurcation aneurysms and recanalized aneurysms.Methods The coronary stents were implanted across the necks of 5 intracranial wide-necked aneurysms (two at bifurcation of ICA, one at basilar artery and two recanalized PcoA aneurysms). The porous stents covered the ostia of vital side branches and perforating vessels in three cases. Microcatheters were introduced into the aneurysmal sac through the stent mesh and then followed by GDCs embolizing the aneurysms. Results The stents were deployed in parent artery in all cases. Total occlusion was obtained in 4 cases, while more than 90% in one case with patency of the parent arteries and vital perforating arteries. Follow-up angiography performed in all patients 3-6 months later revealed no intra-stent stenosis. All the patients recovered well except Ⅲ nerve palsy in one patient which diminished 3 months later.Conclusion Endovaseular therapy combined stent implantation and microcoil placement maybe a valid alteration for the treatment of wide-necked bifurcation aneurysms and recanalized aneurysms, which shows promise clinical application in the future. The ostia of vital side branches covered with stents showed no effect on hemodynamics.
10.The treatment of DAVF with stricture of drainage vein by implantation of stent

Objective Try to find out the feasibility and curative effect of the treatment of DAVF with stenosed drainage vein by implantation of stent.Methods A patient of complicated DAVF with stenosed drainage vein ,stent was implanted in the stenosed drainage vein to improve the function of vien. Results The procedure was successful. The symptom of the patient improved obviously. Cerebral angiography after six month revealed the fine shape and location of the stent. The fistulas of lateral sinus and sigmoid sinus showed no obvious change comparing with the formers.Conclusions The treatment of DAVF with stenosed drainage vein by implantation of stent is feasible. The short term curative effect is satisfactory and the long-term one is still wait for follow-up.
