1.Design and application of a mobile ultrasound guided puncture vehicle

In daily medical work, it is common to carry out bedside ultrasound guided puncture surgery, which is easy to miss and inconvenient to operate when transporting medical devices. The department of emergency of the First Hospital of Jiaxing designed a kind of mobile ultrasound guided puncture vehicle, and obtained the National Utility Model Patent of China (ZL 2020 2 0100473.X). The utility model includes a car body, a work table, an ultrasonic machine, a storage cell and a material basket. The side of the car body is provided with an opening, and both sides of the opening surface are provided with a turnover mechanism, and the rotation axis of the turnover plate is horizontally arranged. The utility model can facilitate the operation of puncture bags, disinfection articles, monitors, oxygen bottles and other medical equipment, and can expand the use area of the operating platform, increase the storage function and store more. This device has good practical value.
2.Analysis of pathogens and drug resistance of lower respiratory tract infection in children with severe acute respiratory syndrome

Objective To investigate the distribution of pathogens in acute severe lower respiratory tract infection and drug resistance.MethodsFrom October 2014 to December 2016, selected 441 children patients with acute severe lower respiratory tract infectionin tianjinfifth centralhospital were drawn lower respiratory tract secretions from the oral endotracheal intubation of the disposable suction tube or container, bacterial culture and identification, to determine the type of bacteria, and drug susceptibility test.ResultsA total of 642pathogens were co-cultured in 441 children, of which 391 were Gram-negative bacteria, accounting for 60.90% (391/642), including Pseudomonas aeruginosa (17.45%), Acinetobacterbaumannii (14.80 %), Klebsiellapneumoniae (13.86%);Gram-positive bacteria were 150 strains, accounting for 23.36% (150/642), mainly including Staphylococcus aureus (15.58%).Gram-negative bacteria are mostly resistant bacteria and multidrug-resistant bacteria, piperacillin and imipenem resistance is relatively low.Gram-positive bacteria on penicillin, gentamicin, erythromycin and other drug resistance are high, resistance to vancomycin are low, including Staphylococcus aureus, Staphylococcus epidermidis, Enterococcus faecalis Resistant drug resistance were 0.00%.ConclusionThe pathogens of lower respiratory tract infection in children with acute severe disease mainly include Pseudomonas aeruginosa, Acinetobacterbaumannii and Klebsiellapneumoniae and other Gram-negative bacteria, the clinical application should be based on the results of pathogen species and susceptibility test to choose reasonable antimicrobial drugs for children patients.
3.Reliability and Validity of Communication Apprehension Scale in College Students

Objective:To do a psychometric test of PRCA-24(Personal Report of Communication Apprehension,24 items)to provide an instrument to detect the interpersonal communication anxiety of Chinese college students.Methods:The English version was translated into Chinese and back-translated.After checked the meaning of the original version and the back translation,data were collected from a sample of 208 college students from Dezhou College,including test-retest with two weeks interval.58 of the 208 received Social Avoidance and Distress Scale for criterion validity.Explorative factor analysis was used for item analysis and correlation analysis for criterion validity.Results:The revised scale was divided into three factors.Factor analysis indicated that the three factors could explain 53.8% of the variance with loading between 0.444~0.820.The coefficient of Cronbach's ? was 0.88.The test-retest correlations of the three factors were 0.66,0.73,0.79 respectively and that of the total scale was 0.85(P

Posterior cortical atrophy (PCA) is a kind of neurodegenerative dementia,which is characterized by the progressive decline of visuoperceptual,visuospatial,reading and writing ability and praxic skills.Neuroimaging usually shows atrophy or metabolic decrease in posterior brain regions.The most common neuropathologic changes of PCA are amyloid plaques deposition and neurofibrillary tangles in posterior cortex.In general,PCA is considered as a variant form of Alzheimer's disease.
5.Treatment evaluation criterion for gastric cancer

In the treatment evaluation criterion of gastric cancer,overall survival(OS)is a dependable endpoint.Besides response cate(RR),disease-free survival(DFS),time to progression(TTP),progression free survival(PFS)have become major endpoints in the system.In addition,patient-reported outcomes(PRO)has received more and more attention.Biomarkers can also serve as a secondory endpoint.Adverse reaction may directly influence the continuity of the treatment and can not be neglected.The combined use of these endpoints can make the evaluation more objective.
6.Intravenous glucose combined with furosemide in the treatment of heart failure

Objective To explore the clinical effect of intravenous injection of 50% glucose combined with furosemide in the treatment of heart failure.Methods 71 patients with intractable heart failure were selected.According to the random number table,they were divided into the observation group (35 cases) and control group (36 cases).On the basis of conventional treatment,the observation group received continuous intravenous infusion of furosemide 100mg combined with intravenous injection of 50% glucose 100ml.The control group was given intermittent injection of furosemide 300mg,3 times/day,100mg/each time.Treatment effects were compared between the two groups.The cardiac function,24h urine volume,BNP and ultrasound beckoning figure index including left ventricular end-diastolic diameter (LVEDD),ejection fraction (EF) before and after 72h treatment of the two groups were compared.Results The total effective rate of the observation group was 82.86%,which was significantly higher than 61.11% of the control group (x2 =4.15,P < 0.05).After treatment,cardiac functional grading,24 hour urine volume,BNP,EF and LVEDD of two groups were significantly improved,and compared with the control group,the improvement of the observation group was more significant,the differences were statistically significant (t =3.23,2.98,3.87,3.12,3.67,P < 0.05).The incidence rate of adverse reaction of observation group was 8.57%,that in control group was 13.9%,there was no statistically significant difference between two groups (x2 =0.97,P > 0.05).Conclusion Effect of furosemide combined with intravenous injection of 50% glucose in the treatment of congestive heart failure is reliable,the cardiac function recovered well,and has high application value.
7.The research and practice of double-cuff method used to calculate pulse wave velocity

Objective:A new method of measuring pulse wave velocity,double-cuff method,has been proposed based on oscillometric method and used to practice to validate its reliability in this paper.Methods:In this method,two cuffs are used to collect pulse wave signals of humerus and oar artery respectively.We analyse the two signals'time difference ?t by choosing wave boot as the pulse beating' start point.Pulse wave velocity can be calculated according to the formula PWV=?S/?t,in which ?S is the distance between humerus and oar artery and can be measured by a soft ruler.Results:The double cuff method,which has the advantages of simple operation,speedy measurement and low cost,can measure pulse wave velocity reliably,accurately,fast and simply.Conclusions:The pulse wave velocity measured by this new method,which can reflect the differences between normal people and critical or already hypertension suffers,is significant in forecasting hypertension.

9.Atherosclerosis occurs in patients undergoing maintenance hemodialysis

BACKGROUND:Atherosclerosis is a common complication in diabetic nephropathy and hemodialysis patients. The effect of hemodialysis duration and other relative factors on the atherosclerosis of patients with diabetic nephropathy needs to be further observed and explored. OBJECTIVE:To observe the atherosclerosis in the patients with different hemodialysis durations, then to evaluate the effect of hemodialysis duration and other relative factors on atherosclerosis of patients with diabetic nephropathy. METHODS:The intima-media thickness of the carotid artery in the diabetic nephropathy hemodialysis group, non-diabetic nephropathy hemodialysis group and end-stage renal disease non-hemodialysis group was detected with high-frequency ultrasonic testing, and the healthy volunteers were as the normal control group. The relationship between the intima-media thickness of the carotid artery in the hemodialysis patients and hemodialysis duration was analyzed, and the changes of insulin resistance in each group were compared. RESULTS AND CONCLUSION:Compared with the normal control group, the intima-media thickness of the carotid artery in the diabetic nephropathy hemodialysis group and non-diabetic nephropathy hemodialysis group was increased (P<0.01);there was no significant difference of intima-media thickness of the carotid artery between 24 hours diabetic nephropathy hemodialysis group and end-stage renal disease non-hemodialysis group (P>0.05);the intima-media thickness of the carotid artery in the 60 months diabetic nephropathy hemodialysis group was higher than that in the 24 hours diabetic nephropathy hemodialysis group (P<0.05);the intima-media thickness of the carotid artery in the 60 months diabetic nephropathy hemodialysis group was significantly higher than that in the end-stage renal disease non-hemodialysis group (P<0.01). There was no significant difference of intima-media thickness of the carotid artery between 60 months diabetic nephropathy hemodialysis group, end-stage renal disease non-hemodialysis group and 24 hours diabetic nephropathy hemodialysis group (P>0.05). The homeostasis model of assessment-insulin resistance value in the 24 hours diabetic nephropathy hemodialysis group was slightly lower than that in the diabetic nephropathy non-emodialysis group (P<0.05), and the homeostasis model of assessment-insulin resistance value in the 60 months diabetic nephropathy hemodialysis group was significantly lower than that in the diabetic nephropathy non-emodialysis group (P<0.01). The development of atherosclerosis on diabetic nephropathy patients can be affected by hemodialysis to some extent. And the effect is correlated with the hemodialysis duration.
