BACKGROUND:Non-heart beating donors have become the most promising source of donor in recent years. Tofurther improve donor preservation solution, on the basis of conventional storage solution hypertonic citrate adenine solution, artificial cordyceps was added to prepare artificial cordyceps compound solution. It is hoped to reduce donor injury and to elevate donor quality by antioxidation. OBJECTIVE: To investigate the preservation effect of artificial cordyceps compound solution on isolated renal from non-heart-beating rats. METHODS: Medulas of 39 Sprague-Dawley rats were injured. Withoutin vitro respiratory and circulatory supports, non-heart-beating models were established and randomly divided into three groups: artificial cordyceps compound group (n=15), hypertonic citrate adenine solution group (n=15) and physiological saline group (n=9). The kidneys of rats in different groups were stored in artificial cordyceps compound solution, hypertonic citrate adenine solution and physiological saline at 4℃. RESULTS AND CONCLUSION: Compared with the hypertonic citrate adenine solution group, after 6 and 12 hours of preservation, morphological injury was mild, apoptotic index, malondialdehyde, Bax and Caspase-3 expression levels decreased, superoxide dismutase activity and Bcl-2 expression increased; after 24 hours, the degree of injury was similar, malondialdehyde content decreased, and superoxide dismutase activity increased in the artificial cordyceps compound group. These findings indicated that the preservation effect of artificial cordyceps compound on antioxidation and inhibition of apoptosis was better than that of hypertonic citrate adenine solution in non-hearting beating rats.

Objective To evaluate the value of stereotactic core needle biopsy(SCNB)in diagnosis of breast lesions.Methods Forty-seven cases were punctured with computer-assisted stereotactic system, spring-loaded biopsy guns and 16 G core needles.The record of each item was collected,including clinical manifestations,descriptions of the mammographic characteristics(such as calcification,mass and architectural distortion),the pathology of the SCNB and the surgical pathology or mammographic follow-up data.Then the results of SCNB were analyzed based on the comparison of SCNB pathology and the surgical pathology.The reason that SCNB failed and misdioagnosed was inferred from the relationship of SCNB accuracy and the X-ray characteristics.Results Forty-four cases were punctured successfully,3 cases failed.Thirty-one patients were operated soon after biopsy.The results of 27 SCNB cases agreed well with the final pathology but the other 4 did not.Conclusions SCNB as an accurate,time-saving and cost- effective method,is also minimally invasive and hardly changes the architecture of the breast.SCNB can diagnose breast lesions in advance,reduces the number of surgical biopsy,and is promising in clinical application.

Objective Aortic arch obstruction is a commonly associated malformation in the patients with Taussig-Bing anoma- ly.Herein,we reported our outcomes of single stage corrections in patients with the Taussig-Bing anomaly associated with aortic arch obstruction.Methods Between May,2000 and Dec,2006,12 patients with Taussig-Bing anomaly associated with aortic arch obstr- uction (5 patients associated with interrupted aortic arch) underwent arterial switch operation with baffling of the left ventricle to neo- aorta.The corrections of aortic arch obstruction included extended resection combined with end to end anastomosis to aortic arch or end to side anastomosis to ascending aorta.Results The hospital mortality rate was 25% (3/12).The ventilating time and ICU stay were (7.4?2.1) days and (11.7?4.6) days,respectively.No reoperation because of residual anomalies was required.Conclusion Tanssig-Bing anomaly,especially associated with aortic arch obstruction,is different from transposition of great artery.The opti- mized operative indications,techniques and the managements of aortic arch obstruction are discussed in the article.

Objective To develop a portable device to monitor and record the exercise quantity of navy serviceman. The device was composed of control components, a oscillator clock, buttons, 3D acceleration sensor and a organic light-emitting diode (OLED) screen. The monitoring involved in two modes of exclusive use and use combined with exercise apparatus.Methods The device could be connected with PC for data transmission through USB interface. Results Trials proved that the device met the desired requirements and could be used for the data acquisition and monitoring of the exercise quantity of the serviceman.ConclusionThe device can record and evaluate the physical fitness training of navy serviceman.

Aim: To study the effect of diabetes mellitus and insulin treatment on rat penile nitric oxide synthase content.Methods: Male Wistar rats were divided at random into two groups: the Control ( n = 8) and the Diabetic ( n =17). Diabetes mellitus was induced by intraperitoneal injection of streptozotocin. The diabetic animals were then ran domly divided into two subgroups: diabetic rats without insulin treatment ( n = 7) and diabetic rats with insulin treat ment ( n = 10). The neuronal nitric oxide synthase (nNOS) in the penile corpus cavemosum were assayed by immrmo histochemical staining with specific antibody to nNOS and the nNOS-positive nerve fibers were counted semiquantita tively under a high power microscope. Results: The nNOS- positive nerve fibres in diabetic rats with treatment was higher than that in diabetic rats without treatment ( P ＜ 0.05) and lower than that in the controls ( P ＜ 0.01 ). The nNOS-positive nerve fibres in diabetic rat without treatment were also lower than that in the controls ( P ＜ 0.01). Con clusion: In streptozotocin-induced diabetic rats, the nNOS content in the penile corpus cavernosum was significantly decre~ed. Insulin treatment at the dose level employed partially restores the penile nNOS content in these rats.

AIMTo analyze the chemical constituents of cortex moutan by liquid chromatography coupled with electrospray ionization mass spectrometry.

METHODSAn on-line optimized HPLC-DAD/MS2 technique was employed.

RESULTSIn the negative ion detection mode, 38 components such as monoterpene glucosides, galloylglucoses and acetophenones were isolated. Among them, over thirty compounds were identified, including paeonol, paeonilflorin, oxypaeoniflorin, benzoylpaeoniflorin, benzoyloxypaeoniflorin, galloylpaeoniflorin, galloyloxypaeoniflorin, mundanpioside A, C, D, E, H, etc. by the high performance liquid chromatography with diode-array detection in parallel with electrospray ionization and quadrupole-time of flight tandem mass spectrometry (HPLC-DAD/ESI-MS2).

CONCLUSIONThis method can be used to rapidly determine the constituents of cortex moutan.

OBJECTIVETo establish a HPLC fingerprint for quality evaluation of Cortex Moutan.

METHODThe HPLC chromatographic fingerprinting of 30 lots of Cortex Moutan was established and major peaks were identified by LC-MS and MS-MS.

RESULTThe HPLC fingerprint of Cortex Moutan was established, showing 15 characteristic peaks. The areas of these peaks were found to complied with the following rule: paeonol > 1, 2, 3, 4, 6-penta-O-galloyl-glucos > methyl gallate > galloylpaeoniflorin > gallic acid > oxypaeoniflorin > other compounds.

CONCLUSIONThe chromatographic fingerprinting of Cortex Moutan with high specificity can be used to control its quality and monitor lot to lot consistency.

Objective To propose a new blood purification modality-hemodialysis with plasma- based dialysate (HD-PBD) plus high volume hemofiltration (HVHF) for patients with liver failure, and to evaluate the effect of this treatment on plasma cytokines.Methods Twelve patients with liver failure were included in this study.All patients received HD-PBD therapy in the first 6 hours,and then were treated with HVHF for 24 hours with the same filter (AV600).The levels of TNF-?,IL-1?, IL-6 and IL-8 in plasma before and after HD-PBD plus HVHF for 6 and 24 hours were examined respectively by ELISA,and changes of clinical parameters were observed at the same time point. Serum bilirubin,total bile acids (TBA),serum ammonia,blood urea nitrogen (BUN) and serum creatinine (Scr) were detected before and after treatment.Arterial blood gas analysis and the concentration of electrolytes were monitored before and after treatment.Results (1)HD-PBD for 6 hours was more effective than HVHF for 24 hours in removal of serum bilirubin and TBA(P＜0.05). (2)Serum ammonia,BUN,Ser,arterial blood HCO_3~-,PCO_2,PO_2 and electrolytes did not show significant difference before and after HD-PBD (P＞0.05),but these parameters significantly changed before and after HVHF (P＜0.05).(3)The average level of serum bilirubin was sharply decreased after HVHF for 24 h following HD-PBD(P＜0.05).(4)After HD-PBD plus HVHF,there was a marked reduction of the plasma levels of TNF-?,IL-6 and IL-8.Conclusions HD-PBD plus HVHF,a newly proposed modality for patients with liver failure,can effectively decrease serum bilirubin,TBA,BUN,Scr,ammonia and cytokines,and adjust water-electrolyte as well as acid- alkali balance.It is a low-cost,safe,simple and convenient therapy.

OBJECTIVETo summarize the experience in diagnosis and treatment of primary tracheal tumors, and to improve the life quality of patients.

METHODSSixty-three patients with primary tracheal tumors treated in the First Affiliated Hospital of China Medical University during the past 40 years were included in this study, among them, there were 42 cases of malignant tumors and 21 cases of benign tumors. The 61 patients underwent surgery including tracheal sleeve resection (22), carinal resection and reconstruction (6), semi-carinal resection and reconstruction (6), tracheal resection for tracheal tumors (17); tracheostomy (4), tracheal resection, partial resection of the thyroid (goiter) and esophagomyotomy (1), tracheal tumor resection and vertical hemilaryngectomy with reconstruction of laryngeal ventricle and trachea by sternocleidomastoid flap (2), cervical trachea and laryngeal resection (1), and carinal scrape (2).

RESULTSFifty-five patients had an uneventful recovery. Eight patients suffered from postoperative complications, among them 3 patients died postoperatively.

CONCLUSIONSPrimary tracheal tumors often present atypical symptoms, are easily misdiagnosed and with poor prognosis. The main aim of treatment remains to remove the airway obstruction.

Objective To investigate the early diagnosis value of MR by detecting spinal inflammatory lesions in ankylosing spon-dylitis (AS).Methods Forty patients were involved in this study,including 20 cases with short inflammatory back pain (IBP)histo-ry (duration ≤18 months)and 20 cases with long IBP history (duration ≥24 months).MR images were analyzed retrospectively. Results Patients with a short history of IBP had 7 lesions in vertebral bodies (anterior/posterior spondylitis and spondylodiscitis) and 33 lesions in posterior spinal structures (arthritis of costovertebral joints,costotransversal joints,zygapophyseal joints and en-thesitis of spinal ligaments).Patients with a long history of IBP had 27 lesions in vertebral bodies and 24 lesions in posterior spinal structures.Patients with a short history of IBP had significantly more lesions in posterior spinal structures than in vertebral bodies with 82.5% (33/40)vs 1 7.5% (7/40),respectively (P <0.01).In contrast,patients with a long history of IBP had significantly more inflammation in vertebral bodies with 79.4% (27/34)vs 20.6% (7/34),respectively (P <0.01).Conclusion Inflammatory spinal lesions in patients with a short history of IBP are seen more often in the posterior structures.Early detection of inflammatory spinal lesions by MRI is useful for early diagnosis of AS.
