1.Preparation and application of multicellular spheroid in cancer therapy

Multicellular spheroid (MCS) can simulate many aspects of the in vivo physiological and pathological conditions in many aspects,better reflect the in-vivo behavior of cells in tumors.So it's increasingly accepted as a valuable tool for evaluating the efficacy of therapeutic intervention including chemotherapy,radiotherapy,immunotherapy and combined therapy.Various spheroid co-culture approaches have been presented to study heterologous cells interaction in solid tumors.The present review briefly introduces the methodology and applications of MCS with focus on the up-to-date information.
2.Enhancing effect of DHEA ON DbcAMP-induced cell differentiation

Aim To observe whether DHEA has enhancing effect on DbcAMP -induced differentiation of NG108-15 cells, including neurite outgrowth, and study its possible mechanisms. Methods NG108-15 cells (a h ybrid cell line of mouse neuroblastoma and rat glioma) were used as a substitute for primary culture neuron in vitro. The morphology of NG108-15 cells was o bserved and neurite outgrowth was determined in an inversed microscope after treatme nt with various drugs. Gelatin-substrate gel electrophoresis was used to detect gelatinases (MMP-9 and MMP-2). Results ① DHEA and DbcAMP inhibited NG108-15 proliferation.②DHEA had enhancing effect on the promoting activity of neuronal differentiation and neurite outgrowth by DbcAMP. DbcAMP could increase neurite elongation of NG108-15 cells. Compared with this, the combined treatment with DHEA and DbcAMP significantly enhanced the neurite outgrowth of NG108-15 cells, including neurite length and numbers of cells with neurite, in a DHEA dose-dependent manner. ③ MMPs were involved in neuronal differentiation. DbcAMP induced the increase in MMP-9 and MMP-2 activities and such elevation was enhanced by DHEA in a dose-dependent manner. Conclusion DHEA enhances the effect of DbcAMP in promoting the neurite outgrowth of NG108-15 cells, which might be related to the increase in MMP-9 and MMP-2 activities.

AIM: To explore the mechanism of ET-1, NO and PGI 2 release from coronary artery endothelial cells (CAEC) induced by acute hypoxia. METHODS: Bovine coronary artery endothelial cells were cultured and [ 45 Ca 2+ ] was used to investigate the difference of calcium uptake between normoxia group and hypoxia group (3% O 2). The contents of ET-1, NO and PGI 2 in media of normoxia group, hypoxia group and hypoxia + verapamil group were measured 24 h after hypoxia. Objective To study the cyfluthrin resistance and potential mechanisms of Anopheles sinensis in Nanchang Chang-bei International Airport,Nanchang City,Jiangxi Province. Methods The resistance levels of the local An. sinensis were de-tected by WHO drug resistance bioassay. During the bioassay,the dying mosquitos were classed as sensitive mosquitos,and the survival ones were classed as resistant mosquitos. The P450 monooxygenase activity and glutathione S-transferase activity were detected and compared between the two groups. At the same time,the death time of each sensitive mosquito was recorded,and the correlations between the death time and the P450 monooxygenase activity and glutathione S-transferase activity were ana-lyzed,respectively. Results The bioassay mortality of the local An. sinensis was 59.5%. The differences of the P450 monooxy-genase activities among the resistant mosquitos,sensitive mosquitos and laboratory sensitive mosquitos had statistical signifi-cances(F=151.89,P<0.01),the resistant mosquitos>sensitive mosquitos>laboratory sensitive mosquitos. The differences of glutathione s-transferase activities among the three groups had no statistical significance(F=0.72,P=0.49). There existed positive correlation between the mosquito death time and the P450 monooxygenase activity,and the regression equation was y=79.479+1.512x with the correlation coefficient of 0.88,while there was no correlation between the mosquito death time and the glutathione S-transferaseactivity. Conclusion The An. sinensis in Nanchang Changbei International Airport has been resistant to cyfluthrin,and the promotion of P450 monooxygenase activity maybe one of the reasons for the resistance.
5.Diagnosis and treatment of pancreatic sinistral portal hypertension with splenomegaly

Objective To study the diagnosis and treatment of pancreatic sinistral portal hypertension with splenomegaly.Method From January 2010 to December 2012,twenty-six cases of pancreatic sinistral portal hypertension with splenomegaly were retrospectively analyzed.Results The underlying pancreatic diseases were chronic pancreatitis (n=12,46 %),pancreatic pseudocyst (n=5,19%),mass of pancreatic body and tail (n=6,23%),autoimmunie pancreatitis (n=2,8%) and pancreatic trauma (n=1,4 %).All the 26 patients presented with splenomegaly,but there was no evidence of cirrhosis or abnormal hepatic function.Three cases presented with a history of upper gastrointestinal bleeding.Gastroscopy or upper gastrointestinal visualization revealed all 26 patients had varices in the gastric fundus and 6 patients had inferior segment oesophageal varices simultaneously.19 patients underwent operative treatment,2 patients underwent splenic artery embolization and 5 patients were treated conservatively.22 patients were followed up for 6 months to 30 months,the follow-up rate was 85 %,and there were no upper gastrointestinal bleeding.Hypersplenism disappeared in all the 19 patients after operation.2 patients succumbed to pancreatic malignant tumor.Conclusions Splenomegaly is one characteristic clinical manifestation of pancreatic sinistral portal hypertension.It could be cured by operation.The choice of treatment depends on the patient's condition.The management of the primary pancreatic disease should also be emphasized.
6.Evaluation on feasibility of Roche e601 for detecting hepatitis B virus surface antigen

Objective To evaluate the of feasibility of Roche e601 for detecting hepatitis B virus surface antigen(HBsAg).Meth-ods To evaluate the feasibility of Roche e601 for detecting hepatitis B virus surface antigen(HBsAg).Results The coefficient of variation(CV)of inter-run and between-run from low and high value specimens was lower than the requirements of manufacturers;the negative and positive coincidence rates in detecting 60 external quality assessment controls were 100%(40/40)and 100%(20 /20),respectively;the detection threshold values of the system was 0.01 IU/mL;the results detected by this detecting system in 40 samples,in which the concentration of HBsAg was 0.8
7.Sinistral portal hypertension caused by body and tail tumors of the pancreas in 40 cases

Objective To evaluate the clinical characteristics,diagnosis and treatment method of pancreatic sinistral portal hypertension caused by tumors in the body and tail of the pancreas.Methods A retrospective review of 40 patients diagnosed with pancreatic sinistral portal hypertension at Peking Union Medical College Hospital from January 2007 to December 2012 was performed.Results The Initial symptoms were epigastric pain and discomfort (n =12),emaciation (n =5),low-back pain (n =4),splenomegaly (n =2),hematemesis and melena (n =4),hypoglycemic coma (n =1).All 40 patients had splenomegaly and varices in the gastric fundus with normal liver function.8 had combined esophageal varices.24 had hypersplenism and 26 had elevated serum CA19-9 level.25 patients received surgical intervention and 15 were treated conservatively.Pathology confirmed malignancy in 29 patients and benign lesions in 11.Thirty-five patients (35/40,88%) were followed up for 12 to 72 months.For patients undergoing surgery,hypersplenism and varices in the gastric fundus were all relieved.There was no upper gastrointestinal bleeding occurred during follow-up.For patients treated conservatively,hypersplenism remained stable and among them 4 patients had upper gastrointestinal bleeding,and successfully treated by medication,therapeutic endoscopy and interventional therapy.Conclusions Patients with pancreatic sinistral portal hypertension caused by tumor in the body and tail of the pancreas can be cured successfully by surgery.In those patients portal hypertension can present as initial clinical manifestation.
8.ERCP and EST after Billroth gastroenterostomy: a report of 168 cases

Objective To determine the value of ERCP and EST after Billroth gastroenterostomy. Methods ERCP was used in 31 patients after Billroth- Ⅰ gastroenterostomy, 12 of whom received EST. It was in 137 patients after Billroth-Ⅱ gastroenterostomy, of the 34 received EST and 4 EPBD.Results Billroth- Ⅰ gastroenterostomy ERCP was successfully performed in 28 out of the 31 patients and EST in 11 out of the 12 patients. Billroth- Ⅱ gastroenterostomy ERCP was successfully performed in 109 out of the 137 patients and EST in 31 out of the 38 patients. There were no serious complications in patients receiving endoscopic treatments. Concluasion The success rates of ERCP and EST are high in patients with bile duct lithiasis after Billroth-gastroenterostomy. Endoscopic treatment or cholangioduodenostomy has good therapeutic effects.
9.Effects of postconditioning with electric vagal stimulation on myocardial ischemia-reperfusion injury in rats

ObjectiveTo investigate the effects of postconditioning with electric vagal stimulation on myocardial ischemia-reperfusion (I/R) injury in rats.MethodsSixty male SD rats weighing 250-350 g were randomly divided into 3 groups (n = 20 each):group sham operation (group S); group myocardial I/R (group I/R) and group electric vagal stimulation postconditioning (group POES).Myocardial I/R was induced by occlusion of left anterior descending branch of coronary artery for 30 min followed by 120 min reperfusion in groups I/R and POES.In group POES right cervical vagus nerve trunk was stimulated for 30 min with continuous electric rectangular pulses (2 ms,10 Hz) starting from 15 min of myocardial ischemia.The voltage of the pulses was adjusted to decrease HR by 10% of the baseline HR before stimulation.MAP,HR and RPP (MAP× HR) were recorded before (baseline) and at 1 and 10 min of ischemia and 30,60 and 120 min of reperfusion.Arterial blood samples were collected from 10 rats in each group at 120 min of reperfusion for determination of serum concentrations of cTnI,CK-MB,TNF-a,high mobility group box 1 protein (HMGB1),ICAM-1,IL-1,IL-6 and IL-10 (by ELISA).The animals were then sacrificed and myocardial infarct size was measured by Evans blue and TTC staining.Another 10 rats were sacrificed at 120 min of reperfusion for determination of myocardial contents of TNF-α,HMGB1,ICAM-1,IL-1,IL-6 and IL-10 (by ELISA).ResultsI/R induced myocardial infarct,significantly increased serum concentrations of cTnI,CK-MB,TNF-α,HMGB1,ICAM-1,IL-1 and IL-6 and significantly increased myocardial contents of TNF-α,HMGB1,ICAM-1,IL-1,IL-6 and IL-10 in both ischemic and non-ischemic regions in group I/R as compared with group S.Electric vagal stimulation significantly decreased myocardial infarct size and serum concentrations of cTnI,CK-MB,TNF-α,HMGB1,1CAM-1,IL-1 and IL-6 in group POES compared with group I/R.Myocardial contents of TNF-α,HMGB1,ICAM-1,IL-1 and IL-6 were significantly decreased while myocardial IL-10 content was increased in both ischemic and non-isehemic regions in groups POES compared with group I/R.ConclusionPostconditioning with electric vagal stimulation can attenuate myocardial I/R injury by inhibiting inflammatory response in rats.
10.Operation opportunity of laparoscopic cholecystectomy for senile patients with acute cholecystitis combined with underlying disease

Objective To explore the operation opportunity of cholecystectomy for senile patients with acute cholecystitis combined with underlying disease.Methods 82 elderly patients with acute cholecystitis complicated with underlying diseases underwent cholecystectomy were selected,they were divided into two groups according to the different operation time:the early operation group and the delayed operation group,41 cases in each group.The patients in early operation group were given operation in 72h,while the delayed operation group was given operation more than 72h after attack.The operation time,intraoperative blood loss,intraoperative tissue adhesions,intraoperative exudation,hospitalization time,cost of hospitalization and complications of the two groups were observed and com-pared.Results The operation time of the two groups were (45.32 ±1 2.23)min,(50.89 ±1 3.34)min,there was no significant difference (P >0.05),the intraoperative tissue adhesions of the delayed operation group was more than the early operation group,and the intraoperative hemorrhage and exudation in the early operation group were significantly higher than those of the delayed operation group (P <0.05),and the hospital stay and cost of hospitalization were sig-nificantly lower than those of the delayed operation group (P <0.05).1 case in the early operation group occurred pulmonary infection,3 cases with incision infection,the incidence of complications was 9.76%;1 case in the delayed operation group underwent laparotomy operation,2 cases in the delayed operation group had pulmonary infection,inci-sion infection in 4 cases,the incidence rate of complication was 1 4.63%,there was no significant difference between the two groups (P >0.05).Conclusion The optimal operation timing of acute cholecystitis combined with basic dis-eases in the elderly patients underwent cholecystectomy is within 72h.For patients who can not be immediately given operation due to various reasons,delayed cholecystectomy is safe and feasible.
