1.Advances in protective effects of thyroid hormone on myocardial mitochondria by sirtuins

Many studies have confirmed that thyroid hormone (TH) and mitochondria have protective effects on myocardium. The partial subtype of sirtuins (SIRTs) can also be able to protect myocardium by mitochondria, which includes improving ischemia reperfusion injury, improving the pathological cardiac hypertrophy and improvement of heart failure. SIRTs can achieve the protective effect for myocardium mainly through inhibiting mitochondrial permeability transition pore and mitochondrial membrane permeabilization, maintaining balance of regulation of mitochondrial morphology, promoting mitochondrial biogenesis, maintaining the function of autophagy of impaired mitochondria. At the same time, there is an interaction between TH and SIRTs. At present, the role of mitochondria has become more and more concerned in apoptosis and necrosis and its protective effect on cells. This review summarized the protective effect of SIRTs on myocardium through mitochondrial and the influence of the interaction between SIRTs and TH on myocardial protection of mitochondrial.
2.Regression of left ventricular hypertrophy with ramipril and losartan:assessment by three-dimensional echocardiography

Objective To compare the reduction in left ventricular hypertrophy (LVH) caused by monotherapy with ramipril or losartan or the combination of these two drugs,in patients with hypertensive LVH and to evaluate the value of three-dimensional echocardiography (3DE) on LVH regression.Methods Ninety-six essential hypertensive patients with LVH were enrolled in this study.All patients were randomly divided into 3 groups: ramipril group,losartan group,combination group with ramipril and losartan.Left ventricular mass(LVM) was calculated by M-mode echocardiography,two-dimensional echocardiography and 3DE at baseline and after 6 months treatment with ramipril or losartan.Results Of 96 patients enrolled,59 patients have complete follow-up datum.LVM of ramipril,losartan and combination group decreased 16.9% , 17.9% and 18.8% compared with before treatment.LVM by 3DE decreased 11.6% , 14.7% and 13.6% respectively.The difference of LVM derived from 3DE showed obviously lower than derived from M-mode echocardiography.Conclusions Applying M-mode echocardiography and two-dimensional echocardiography to evaluate the regression of LVH will overestimate the effects of drugs.Three-dimensional echocardiography may detect the changes of LVM more accurately.

Objective To evaluate the effects of angiotensin converting enzyme inhibition(ACEI) or angiotensin II receptor blocker(ARB) on coronary hemodynamics in hypertensive patients with left ventricular hypertrophy(LVH) by Doppler ultrasound.Methods Sixty essential hypertensive patients with LVH were enrolled in this study.All patients were randomly divided into 3 groups: ramipril group,losartan group and combination group.Left ventricular mass(LVM) was calculated by three dimensional echocardiography and coronary hemodynamics was evaluated by transesophageal echocardiography with left anterior descending artery.Coronary flow reserve(CFR) was calculated as the ratio of coronary flow velocity,after intravenous injection of dipyridamole,to rest peak velocity.All the indexes of the coronary hemodynamics were corrected by LVM.Results Fifty patients had complete follow up datum.The systolic blood pressure(SBP),diastolic blood pressure(DBP) and LVM were significantly decreased in ramipril,losartan and combination group after 6 months′ treatment (All P

Objective To explore the effectiveness of transcranial magnetic stimulation (TMS) combined with hyperbaric oxygen (HBO) in treating cerebral infarction.Methods Totally 120 patients with cerebral infarction were randomly divided into a control group,a treatment group A and a treatment group B,40 cases in each group.The control group was given conventional treatment,the treatment group A was treated by HBO on the basis of conventional therapy,and the treatment group B was treated by HBO combined with TMS,10 d for a period of treatment.Before and after 2 courses of treatment,all patients' neurological defect was evaluated using National Institutes of Health Stroke Scale (NIHSS).Their central motor conduction time (CMCT) and the expression of brain derived neurotrophic factor (BDNF) and nerve growth factor (NGF) were also assessed.Results After treatment,the average NIHSS score and effective rate of the treatment group B was (7.35 ± 1.98) and 92.5% respectively,significantly better than the other two groups.Moreover,the average BDNF content [(4.96 ± 1.20) ng/ml] and NGF content [(152.36 ± 18.01)pg/ml] of the treatment group B after treatment were significantly better than before treatment,and those of the other two groups after treatment.Conclusion TMS combined with HBO is effective in treating cerebral infarction.This may relate to its strengthening the expression of neurotropic factors.
7.Preliminary study of energy loss in left ventricle obtained by vector flow mapping in patients with hypertension

Objective To quantitatively analyze the energy loss (EL) in left ventricle with vector flow mapping (VFM) in hypertensive patients,and try to reveal the correlation of EL with cardiac structure and function.Methods Hypertension group:52 hypertensive individuals were enrolled in our study,divided into 4 subgroups:①14 patients with normal geometry(NG);②20 patients with concentric remodeling(CR);③15 patients with concentric hypertrophy(CH);④3 patients with eccentric hypertrophy(EH)(not enrolled in subgroup analysis).Health control group were composed of 32 healthy volunteers.LVEF,LVMI and E/e′ were measured.And the EL in left ventricle was measured at different phases and different segments based on VFM images.Results ①EL comparison:in the phrase of late diastole(DL),isovolumic contraction(IVC) and rapid ejection(RE),compared with the control group,EL in entirety and each segment of left ventricle in hypertension group increased (P<0.05);Compared with the control group,EL in entirety and each segment of left ventricle in each subgroup of hypertension increased (P<0.05);EL among different subgroups showed no statistically significant difference(P>0.05);②Correlation analysis:in the phrase of DL,IVC and RE,EL in entirety and each segment of left ventricle showed significant positive correlations with LVMI and with E/e′ (P<0.05).In the phrase of DL,EL in entirety and each segment of left ventricle showed significant negative correlations with LVEF(P<0.05).Conclusions EL measured quantitatively by VFM is more sensitive to reflect the changes of hemodynamic in hypertensive individuals.EL is closely correlated with cardiac structure and function.
8.Effects of glucocorticoids on intracellular calcium in microglial cells

Aim To explore the effects of hydrocortisone on intracellular calcium in microglial cells.Methods The intracellular calcium was measured by instantaneous scanning with confocal laser microscope(CLM) in BV-2 cells, and fluo3-AM was used to dye the intracellular calcium.Results Both hydrocortisone and nicotine could obviously increase intracellular calcium in BV-2 cells(P<0.05).It was indicated by instantaneous scanning with CLM that hydrocortisone induced the rising of intracellular calcium immediately, and reached the peak about at the fifteenth second, and sustained for 10 seconds, then declined to baseline at 200th second.The effect of hydrocortisone on intracellular calcium exhibited a highly consistency with nicotine.Antagonist of glucocorticoid receptors RU486 could not abolish the rising of intracellular calcium induced by hydrocortisone(P>0.05);but the blocker of α7 nicotinic acetylcholine receptor(α7nAChR) methyllycaconitine could suppress the rising of intracellular calcium induced by hydrocortisone(P<0.05).Conclusion Hydrocortisone enhances intracellular calcium via α7nAChR in microglial cells, which not only demonstrates the non-genomic effect of glucocorticoid, but also suggests that glucocorticoid could serve as endogenous ligand of α7nAChR.
9.Design and validation of bone marrow histological scoring system for myelodysplastic syndrome

Objective To design a bone marrow histological scoring system for myelodysplastic syndrome(MDS) in order to provide an objective evaluation of clinical diagnosis,disease development and outcome prediction. Methods HE and Gomori staining were used to observe the morphological alterations in 149 bone marrow biopsies.Immunohistochemical staining for MPO,CD235,F8,CD42b,CD61 and CD34 was performed.The scores of distinct morphological changes of MDS were valued. Objective To investigate blood flow dynamics adjacent to regional myocardial segments in coronary artery disease(CAD) patients and to explore the association of the two CAD features.Methods In 43 CAD patients,velocity vector imaging (VVI) was used to evaluate the wall motion and classify normal myocardial segments (NS),relatively abnormal and obviously abnormal wall motion segments (RS and OS) by systolic peak strain (Ss).Vector flow mapping (VFM) was used to show blood flow dynamics in the heart chamber,including blood flow (Q) heading towards or away from the apex during a heart beat and time to every peak flow (T).Results Compared with NS,both RS and OS showed a higher systole Q + (blood flow heading towards the apex during systole) and lower diastole Q + (blood flow heading towards the apex during diastole) and shortened T2 (adjusted time interval between E and A peaks).Systole Q + was positively correlated with Ss but negatively with segmental ejection fraction (SEF).Diastole Q + was negatively correlated with early and late diastolic peak velocity (Ve and Va) but positively with Ve/Va.T2 was negatively correlated with Ve but positively with Ve/Va and early diastolic peak strain rate (SRe).Conclusions VFM might be a new tool to quantitatively assess change in regional blood flow dynamics in patients with CAD.
