1.The value of MRI in evaluation of metastatic axillary lymph nodes in breast carcinoma

Objective To investigate the value of MRI in evaluation of metastatic axillary lymph nodes in patients with breast carcinoma. Methods 60 patients with breast carcinoma underwent MRI before surgical management.Tumor maximal diameter,tumor SERmax (Maximal Signal Enhancement Ratio),the size,shape,border,signal intensity and enhancement pattern of ipsilateral axillary lymph nodes were recorded respectively.The results of MRI observation were compared with the histopathological results.Results Among the 60 patients,MRI showed that 28 cases had metastatic axillary lymph nodes,and 22 of them were confirmed by pathology.Its sensitivity,specificity and accuracy were 90.91%,78.95%,83.33% in the diagnosis of lymph nodes metastasis.Meanwhile,57 cases were underwent ultrasound examination,its sensitivity,specificity and accuracy were 59.09%,94.29%,80.70%.The sizes of lymph nodes in the patients with metastatic lymph nodes and the patients with no metastatic lymph nodes were(1.20±0.59)cm, (0.85±0.24)cm respectively.There was statistically significant difference between the two groups(t=3.258,P=0.001<0.05). There was statistically significant difference between the above two groups in the tumor maximum of SER(t=4.167,P=0.000<0.05),SERmax in metastatic lymph nodes were higher than in no metastatic ones.Irregular shape and border,inhomogeneous signal intensity and heterogeneous enhancement patterns were suggestive of metastatic lymph nodes.Conclusion MRI is an effective imaging modality in evaluation of axillary lymph nodes status in patients with breast carcinoma comprehensively before operation.
2.Femoral supracondylar varus osteotomy combined with Giebel blade plate on treatment of valgus deformity of the knee

[Objective]To investigate the method and efficiency of operative femoral supracondylar varus osteotomy combined with new blade plate on treatment of valgus deformity of knees.[Method]Thirty-seven patients with 43 knees with valgus deformity were treated with femoral supercondylar varus osteotomy with Giebel blade plate(Link company/Germany)from Oct.2000 to Oct.2005.The study group consisted of seven men with 8 knees and thirty women with 35 knees,the average age of the patients was 31.2 years(range,fifteen to seventy years).Eleven patients with 13 knees had lateralcompartment osteoarthritis,Ahlback stage Ⅰ eight patients with 10 knees and stage Ⅱ three patients 3 knees.The full-length Ap view of the lower extremity and the weightbearing AP view of the knee were obtained before and after operation,as well as the femoral angle and femoral-tibial angle were evaluated.The Giebel blade plates were taken out average 1.5 year after operation and the patients were followed-up,bone union and the change of the correction angles and the pain of the knees were evaluated.[Result]All deformity of the knees were well corrected,the femoral angle were corrected from 71.7?(62?~75?) to 82.1?(78?~85?),the average correction angle was 10.4?(8?~21?).With average 1.5 year follow-up thirty four patients with 40 knees got good bone union,two knees had non-union and we changed the internal fixationwith bone autc-grafting and got good results,as well as lost follow-up for one knee.The pain relieved in 10 knees with lateral osteoarthritis and disappeared in 3 knees after operation.[Conclusion]Femoral supracondylar varus deformity of the knee,the technique is simple and damage is slight,fixation is safety and effective.
3.Management of massive acetabular bone defect with impaction bone grafting plus mesh in revision total hip arthroplasty

[Objective]To investigate the method and effeciency of management massive acetabular bone defect with impaction bone grafting plus mesh in revision total hip arthroplasty[Method]Twenty-one patients 21 hips with massive acetabular bone defect were treated with impaction bone grafting plus mesh during Dec 2001 to Jan 2006,including 6 men and 15 women,the average age were 51.9 years at the revision operation(range,38~77 years).The acetabular defects of the patients in this study group were type III(combined defect)according to the American Academy of Orthopaedic Surgeons(AAOS)classification scheme.At first the combined defect was transformed into cavitary defect with metal mesh,then the contained acetabulum was tightly packed with deep-frozen morselized cancellous allograft chips(size 7~10mm),the anatomy of the hip was reconstructed and the cup was inserted after pressurizing the cement directly onto the graft.The AP view of the hip at 3 days,3 months,1 year and yearly after the operation were obtained,cup migration according to the teardrop and radiolucent line were evaluated,at the same time the Harris hip score were also evaluated.[Result]The mean follow-up time was 2.8 years,the mean Harris hip score improved from 46.4 points preoperatively to 81.3 points at the final evaluation,no cup migration were found,except for one dislocation and one sciatic nerve injury,as well as lost follow-up for one hip.[Conclusion]Impaction bone grafting plus mesh is an effective approach to treat massive acetabular bone defect in revision total hip arthroplasty,it can restore bone stock and help the cup to be placed to the anatomic position,as well as provide execellent prothesis stability.
4.Relation of expression and significance of IL-25,IL-12 and IgE in children with allergic rhinitis

OBJECTIVE To investigate the relation of expression and significance of IL-25,IL-12 and IgE in children with allergic rhinitis.METHODS The concentration of IL-25,IL-12 were measured in 34 children with allergic rhinitis and 34 health control subjects by enzyme-linked immunosorbent assay(ELISA),and compare to them and relationship.RESULTS The concentrations of IL-25,IL-12,IgE in periodicitical allergic rhinitis(AR)were(42.64? 11.77)ng/L,(38.52?8.32)ng/L,(563.2?141.7) kU/L,respectively.And in persistence AR they are(57.51?5.36)ng/L,IL-12(29.31?11.69)ng/L,(625.6?104.3)kU/L.However,in control group,the concentrations of IL-25,IL-12 and IgE were(33.42 ?9.78)ng/L,(49.31?11.69)ng/L(209.7?113.2) kU/L,respectively.There were significant difference in periodicitical AR and persistence AR,periodicitical AR and control group,persistence AR and control group(P
5.Association of body mass index and waist circumference with dyslipidemia among adults in Suzhou Industrial Park District

Objective To study the association of body mass index (BMI) and waist circumference (WC) with dyslipidemia and the risk of dyslipidemia at different BMI and WC level among adults in Suzhou Industrial Park District. Methods A total of 6219 participants were chosen by stratified random cluster sampling method. Four streets were selected form the district first, then one community as a cluster was selected randomly from each street. Questionnaire survey, physical examination and laboratory tests were done by all subjects. Results The prevalence of dyslipidemia was 35.9%;the rates of overweight, obesity and central obesity were 36.6%, 9.6% and 49.5% respectively. The prevalence of dyslipidemia in underweight group, normal weight group, overweight group and obesity group were 13.6%, 30.2%, 47.7%and 59.0% for male, and 17.2%, 27.3%, 38.6%和 48.8% for female respectively. The prevalence of dyslipidemia for male and female both rose with BMI (χ2=139.848, P<0.001; χ2=92.387, P<0.001). The prevalence of dyslipidemia in the high waist circumference group and normal group were 50.2%and 30.9%for male, and 40.8%and 23.8%for female respectively. Prevalence of groups with high waist circumference for male and female were significantly higher than normal groups (χ2=108.669, P<0.001; χ2=110.642, P<0.001). Pearson correlation analysis showed that total cholesterol (TC), triglyceride(TG), and low density lipoprotein cholesterol (LDL-C) were positively correlated with BMI ( r=0.153, P<0.001;r=0.227, P<0.001;r=0.192, P<0.001), and were also positively correlated with WC(r=0.138, P<0.001; r=0.234, P<0.001; r=0.159, P<0.001). High density lipoprotein cholesterol(HDL-C) was negatively correlated with BMI (r=-0.189, P<0.001) and WC (r=-0.185, P<0.001). Logistic regression analysis after age adjustment showed that, compared to the group with BMI<24 kg/m2 and WC<85 cm(male)/WC<85 cm (female), odds ratios (OR) for male in group with BMI<24 kg/m2 and WC≥85 cm, group with BMI≥24 kg/m2 and WC<85 cm and group with BMI≥24.0 kg/m2and WC≥85 cm were 1.602, 1.834 and 3.064 respectively, and ORs for female in group with BMI<24.0 kg/m2 and WC≥80 cm and group with BMI≥24.0 kg/m2 and WC≥80 cm were 1.703 and 2.381 respectively, however, the OR for female in group with BMI≥24.0 kg/m2 and WC<85 cm was not statistically significant (P>0.05). Conclusions BMI, waist circumference and dyslipidemia were closely correlated. Waist circumference is more important than BMI for female.
6.Studies on Establishment of Co-expression System and Function of Human Complement Regulatory Proteins MCP and CD59

Recombinant expression vector pcDNA3-MCPCD59DP containing human membrane complement regulatory proteins(hCRPs) MCP and CD59 cDNA was constructed successfully by using two independent promoters.After transfected into NIH3T3 cells with calcium phosphate-DNA precipitate method,NIH3T3 pcDNA3-MCPCD59-DP transfectants were obtained by G418 selection.Extraneous genes integration was identified by PCR.The co-expression of human CD59 and MCP at both mRNA and protein levels was confirmed by using RT-PCR and Western blot analysis.Human MCP and CD59 cDNA were integrated in NIH3T3 pcDNA3-MCPCD59-DP genomic DNA after continuous 30 times passages,indicating that NIH3T3 pcDNA3-MCPCD59-DP were stable cell lines.Human complement-mediated cytolysis assays showed that NIH3T3 cells transfected stably with pcDNA3-CD59,pcDNA3-MCP,and pcDNA3-MCPCD59-DP were protected from C-mediated damage and co-expressed human MCP and CD59 provided more excellent protection against C-mediated attack as compared with either CD59 or MCP expressed alone.The dicistronic vector represents an effective and efficacy strategy to overcome C-mediated damage and has potential therapeutic value for effectively controlling complement activation and finally for preventing hyperacute rejection in clinical gene therapy.
7.Influence of ifenprodil pretreatment on focal cerebral ischemia-reperfusion-induced iNOS expression and activity and apoptosis in the ischemic penumbra in rats

Objective To explore the possible mechanism for the neuroprotective effect of ifenprodil by investigating its effects on inducible nitric oxide synthase (iNOS) expression and activity and apoptosis in the ischemic penumbra following focal cerebral ischemia-reperfusion (I/R) in rats.Methods Fifty-four adult male SD rats weighing 280-320 g were randomly divided into 3 groups ( n = 18 each) : I sham operation group (group S) ; II focal cerebral I/R group (group I/R) and Ⅲ ifenpradil preconditioning group (group IF) received intraperitoneal ifenprodil 10 mg/kg before focal cerebral I/R. Focal cerebral I/R was induced by middle cerebral artery occlusion (MCAO) . A 3-0 nylon thread with rounded tip was inserted into right internal jugular vein and threaded cranially until resistance was met. MCAO was maintained for 2 h. At 48 h after reperfusion, the animals were assessed for neurological function which was scored (0 = no functional deficit, 4 = unable to crawl, unconscious) and then decapitated. The brains were immediately removed for microscopic examination and determination of iNOS protein expression and activity, NO content and apoptosis in the ischemic core (IC) and penumbra (IP). Results Ifenprodil pretreatment significantly decreased the cerebral infarct size and neurological scores in group IF as compared with group I/R. In group I/R the iNOS activity was increased compared with group S.The iNOS activity and NO content were significantly lower in IP than in IC in group IR and IF. The TUNEL-positive cells were also mainly confined to IP. Compared with group I/R, in group IF the iNOS protein expression was significantly down-regulated in IC and IP and the iNOS activity and NO content in IC and IP were suppressed and TUNEL-positive cells were significantly reduced in IP. Conclusion Ifenprodil pretreatment has protective effect against cerebral I/R injury by inhibiting iNOS protein expression in IP, suppressing iNOS activity and NO content and reducing apoptosis.

9.Nicotine inhibits the inflammation induced by 6-hydroxydopamine lesion in the striatum of SD rat

Objective To explore the mechanism through which nicotine protects dopaminergic neurons against 6-OHDA toxicity in SD rat. Methods Rats received nicotine or saline treatment (two doses tested,0. 2 rag/ kg and 2 rag/ kg, 5 injections i.p. per day at 2-h intervals). On day 8after the treatment, a single injection of 20μg of 6-OHDA was administered into right striatum.Nicotine or saline was administered continuously daily until animals were killed. The dopaminergic neurons and CD3, CD4 and CDS-positive lymphocytes were analyzed quantitatively using immunohistochemistry. Microglia activation was quantified by IBA1 immunofluorescence. Results The loss of dopaminergic neurons induced by 6-OHDA in the substantia nigra was significantly less severe in the nicotine treatment group (at both 0. 2 and 2 mg/kg groups) than that in the saline treated group. In the striatum, we observed that the number of CD3, CD4 and CD8-positive lymphocytes reduced significantly in the nicotine treated animals as compared to saline controls. Otherwise, nicotine inhibited CD4 and CD8-positive lymphocytes infiltration equivalently. Quantitative immunofluorescenee analysis indicated the microglia activation was inhibited obviously in nicotine treatment. Conclusions Our data suggest that nicotine may have a neuroprotective effect against dopaminergic lesion induced by 6-OHDA by inhibiting the inflammation.
