1.Prevention and management of portal vein thrombosis following orthotopic liver transplantation

Objective To investigate the prevention and management of portal vein thrombosis following orthotopic liver transplantation.Methods Between May 1995 to September 2005,clinical data of 137 cases subject to orthotopic liver transplantation were analyzed.Among them,there were 10 cases of portal vein thrombosis(5 cases in gradeⅠ,4 cases in gradeⅡand 1 case in gradeⅢ). All patients received an eversion thromboendovenectomy(ETEV)with occlusion of the portal flow u- sing a Forgarty balloon.Ligation of the collateral circulation,especially spontaneous or surgical sple- norenal shunt,was made as approaches to improve portal flow.Heparin or low-molecule-weight hepa- rin as a prophylactic anticoagulation therapy was maintained during and after operation if prothrombin time was less than 18 s.Follow-up Doppler ultrasonography was used daily in the early postoperative period.Results After a follow-up of 2~66 months,overall incidence of portal vein thrombosis was 2.92%(4/137).Surgical thrombectomy and revascularization was carried out in 1 case.Thromboly- sis,balloon angioplasty and stent placement via hepatic artery were performed on 2 cases.No treat- ment was given in 1 patient without hepatic dysfunction and portal hypertention.Mortality related to portal vein thrombosis was 0.Conclusions Portal vein thrombosis might be avoided by performing a complete thrombectomy as often as possible,by ligation of portosystemic shunt during surgery,and by postoperative anticoagulation.Close follow-up by Doppler ultrasonography may make a prompt di- agnosis and reduce portal vein thrombosis-derived loss of grafts.
2.Clinical Observation of Locally Advanced Gastric Cancer Treated by Compound Matrine Injections Combined with Concurrent Chemoradiotherapy

Objective To study the recent clinical efficacy of compound matrine injections combined with concurrent chemoradiotherapy in the treatment of locally advanced gastric cancer. Methods Totally 65 cases of untreated locally advanced gastric cancer were randomly divided into treatment group (33 cases) and control group (32 cases). The control group was treated by the conformal intensity modulated radiotherapy combined with chemotherapy. On the basis of the control group, the treatment group was given compound matrine injections 20 mL/time, once a day for 5 weeks. The recent curative effect, the quality of life and adverse reaction of the two groups were observed. Results The recent curative effect rate of the treatment group was 66.67% (22/33), the control group was 40.63% (13/32), the difference was statistically significant (P=0.031). On hematologic toxicity, leukopenia occurred in the treatment group was 30.30% (10/33), the control group was 65.63% (21/32), the difference was statistically significant (P=0.004). KPS of 26 cases (78.79%) improved in the treatment, and 18 cases (56.25%) improved in the control group. Conclusion The effects of compound matrine injections combined with concurrent chemoradiotherapy for locally advanced gastric cancer are good, they can improve short-term efficacy, reduce side effects, and improve the quality of life.
3.The influences of fenofibrate on the expression of scavenger receptor class B type I in adipose tissue and adipocytes

Objective To observe the effects of fenofibrate on the expression of scavenger receptor class B type I(SRBI) in adipose tissue and adipocytes of hypercholesterolemia rabbits.Methods Ten male New Zealand white rabbits were fed with high cholesterol diet for 8 weeks,and were randomly divided into two groups: high cholesterol group and treatment group.The rabbits in the high cholesterol group were maintained cholesterol diet for 4 weeks and those of the treatment group were fed with the same cholesterol diet supplemented with fenofibrate(30 mg/kg/day) for 4 weeks.Five rabbits in control group were fed with normal diet for 12 weeks.Subcutaneous adipose was collected for adipocytes culture.The expressions of SRBI mRNA in adipose tissue and adipocytes were evaluated by semi-quantitative reverse transcription-polymerase chain reaction(RT-PCR).Results The serum levels of total cholesterol and low density lipoprotein cholesterol were significantly higher in the high cholesterol group and treatment group than those of the control group(P0.05).The expressions of SRBI mRNA in adipose tissue and adipocytes were up-regulated in high cholesterol group compared with those of control group(P
4.Effect of on adhesion between pancreatic stellate cell and mast cell

Objective To investigate stem cell factor(SCF)expression in activated pancreatic stellate cell(PSC)and the effect of SCF on adhesion between PSC and mast cell(MC).Methods PSC of SD rat was separated and activated by pancreatic tissue culture;the expression of SCF mRNA and protein was detected bv RT-PCR and immunachemistry.SCF was blocked with different dose of anti-SCF antibody,the rate of adhesion between PSC and MC was determined by thiomn blue stain and β-hexasaminidase assay.PSC was stimulated by 10 μg/ml mitomycin-C and was co-cultured with MC,then the growth of MC was observed at 2,3,5 day,respectively.Results Activated PSC expressed SCF mRNA and protein,the number of adhered MC in the control,1mg/ml and 10 mg/ml antibody groups was 48±10,28±8,and 23±9,respectively;the difference between antibody and control group was statistically significant(P<0.05).One day after PSC and MC was co-cultured without serum.there were MC adhesion;3 days later the rate of adhesion increased;5 days later the adhesion appeared bulk-like and lots of colonies were present,while PSC basically disappeared.Conclusions Activated PSC may synthesize,express SCF and induced adhesion and growth of MC.
5.Evaluation on tobacco control strength at or below county-level governments of Zhejiang Province

Objective To evaluate the strength of tobacco control at or below county-level governments of Zhejiang Province,to explore the strategies and measures on tobacco control,and promote a smoke-free government.Methods By multistage stratified random sampling method,12 government ageucies were selected in 4 counties.The information of tobacco control strength and smoke-free environment creation was collected by interview and field observation,and the scores were calculated by using the percentage assignment method.An intercept suuvey was carried out in four counties and there were 405 civil servants under investigation.The data were analyzed by frequency statistic and correlation analysis.Results The smoking rate of civil servants was 17.8%,and the secondhand smoking rate was 61.9%;84.7% of civil servants adnmitted the harm of smoking and 96.8% admitted the harm of secondhand smoking.Support for forbidding the cigarette advertisements was highest and increasing the cigarette price was the weakest.The scores of smoke-free environment creation of project counties showed that the average score was 50.60,Zhuji and Luqiao got a maximum score of 57.14,Haiyan got a minimum score of 42.86.The scores of tobacco control strength showed that the average score was 82.35,Zhuji got a maximum score of 92.16,followed by Luqiao,Jiande and Haiyan counties,82.35,80.39 and 74.51 respectively.The scores of tobacco control strength,include policy,political will,capacity,and tobacco control efforts were the highest in Zhuji county.The results of correlation analysis showed that the total score of tobacco control strength had significant association with the score of smoke-free environment creation,smoking rate of civil servants,and approval rate of anti smoking in public places (P<0.05).Conclusion The overall situation of tobacco control strength at or below county-level governments of Zhejiang province is good,but there is a relative shortage of smoke-free environment creation.Civil servants master the knowledge related to tobacco and support tobacco control policy.We should further promote the construction of smoke-free government.More effective smoking control measures should be conducted in order to raise the strength of tobacco control and finally implement the tobacco control legislation.
6.Epidemiological status of tobacco use among college students in Zhejiang province, China

Objective To understand the prevalence of tobacco use among college students in different populations by sex,age and major,and to provide evidence for smoking control and health decision.Methods A total of 4 638 college students selected in 21 colleges through stratified multi-stage cluster sampling from Zhejiang Province were surveyed by using questionnaire analysis.Indicators such as tried smoking,current smoking,and secondhand smoke exposure,etc.,were calculated by weight on age proportions from the numbers of college students in Zhejiang.Results The prevalence of tried smoking and currcut smoking among college students were 29.72% and 10.57%,respectively.The prevalence of tried smoking was 46.52% for men,18.25% for women;28.57% for undergraduate,31.53% for junior college;the prevalence of current smoking was 18.86% for men,3.86% for women;8.88% for undergraduate,13.23% for junior college students.The snoking rate in college students increased with age,the group of ≥21 Years was the highest (37.87%,16.09%).The lowest level of smoking was medical student (21.76%,2.19%).The prevalence of secondhand smoke exposure in outdoor and indoor places,and dormitory among students were 65.49%,60.33%,and 23.66%,respectively.In the past 30 days,college students have seen tobacco advertisement (47.38%),smoking scenes (60.20%),and got tobacco related gifts (10.40%),free tobacco products (8.93%);college students have seen information about tobacco control (60.50%),cigarette pack warning labels (58.45%),and learned the tobacco knowledge in class in the past 12 months (14.03%).Conclusion Current prevalence of smoking in college students in Zhejiang province remained at a high level,the rate of secondhand smoke exposure is high and in college students there was lack of tobacco control education in class.The results indicated that the policy about tobacco control in school was not enough and the task of health promotion should be taken more arduously.
7.Technical analysis of endovascular embolization for wide-necked aneurysms of ruptured anterior communicating arteries

Objective To investigate the safety, efficacy and key points of technology of endovascular embolization for ruptured wide-necked aneurysms of anterior communicating arteries. Methods The clinical, DSA imaging, interventional treatment and follow-up data of 35 patients with ruptured wide-neck aneurysms of anterior communicating arteries were analyzed retrospectively. Results Immediate postoperative angiography of the 35 patients found 100% occlusion were achieved in 29 (82.9%), 90% occlusion were achieved in 5 (14.3%), and 80%occlusion was achieved in 1 (2.8%). One patient had occlusion in ipsilateral anterior cerebral artery , whose blood vessel was patent after thrombosis. One patient had intraoperative aneurysm rupture. One patient had cerebral infaraction of anterior cerebral artery after the operation. Clinical follow-up was made from 6 to 60 months , and no aneurysms ruptured was found. 21 patients were followed up for 6 to 60 months with DSA. Two of them experienced recurrence, and they were not treated with supplementary packing. Conclusion Endovascular embolization for ruptured wide-necked aneurysms of anterior communicating arteries is more difficult , but it is safe and effective. Flexible choosing of various kinds of embolism technique and designing the most optimal embolism treatment plan are the keys to the improvement of embolization effect.
8.Technical analysis of endovascular embolization for ruptured tiny aneurysms of anterior communicating arteries

Objective To investigate the feasibility,efficacy and key points of individually designed endovascular embolization for ruptured tiny aneurysms of anterior communicating arteries. Methods The clinical,DSA imaging,interventional treatment and follow-up data of 21 patients with ruptured tiny aneurysms (21 aneurysms)of anterior communicating arteries from March 2009 to March 2014 were analyzed retrospectively. Thirteen of them were treated with coil embolization only,6 were embolized with microcatheter-assisted technique,and 2 were embolized with balloon-assisted technique. Results (1)One patient was failed to embolize because of technical reason. Immediate postoperative angiography found that in the other 20 patients:14 were packed densely,6 had residual aneurysm necks. (2)Intraoperative complications:One patient had acute occlusion of ipsilateral anterior cerebral artery,the blood vessel was patent after thrombolytic method;2 patients had intraoperative ruptured aneurysm. (3)The modified Rankin scale (mRS)scores of 20 patients with successful embolization at discharge:0-1 in 14 cases,2 in 4 cases, and 3-4 in 2 cases. (4)Follow-up study:20 patients were followed up for 12 months after discharge. The mRS scores:0-1 in 18 cases,2 in 1 case,and 3-4 in 1 case. Clinical follow-up for 6 to 60 months,no aneurysm ruptured. Fifteen patients were followed up for 6 to 60 months with DSA,one of them recurred,and he was treated with supplementary dense packing. Conclusion Endovascular embolization for ruptured tiny aneurysms of anterior communicating arteries is difficult,but it is feasible and effective. The proficient surgical skill,individualized surgical design and appropriate embolism are the keys to improve the embolization effect.
9.Study on the Ex Vivo Adsorption of Montmorillonite Powder to Ofloxacin

OBJECTIVE:To study the ex vivo adsorption of montmorillonite powder to ofloxacin.METHODS:Different dosages of ofloxacin and montmorillonite powder were mixed with artificial gastric juice and artificial intestinal juice,respec?tively,which were filtrated after warmed at(37?0.5)℃in water bath for1hour,the content changes of ofloxacin were de?termined by ultraviolet spectrometry.RESULTS:The adsorption rates of montmorillonite powder to ofloxacin in artificial gastric juice and in artificial intestinal juice were(99.76?0.01)%and(99.55?0.02)%,respectively.CONCLUSIONS:Montmorillonite powder has strong adsorption to ofloxacin in both artificial gastric juice and artificial intestinal juice,there?fore,which should not be administered simultaneously in the clinic.
10.Distribution of Bacteria Detected from Blood Culture of Pediatric Patients and Analyses of Antibiotic Resistance

OBJECTIVE To investigate the distribution of bacteria detected from blood culture of pediatric patients and to observe the antibiotic resistance of these bacteria. METHODS The BacT/Alert blood culture system was applied for culture.Species identification and antibiotic resistance tests were performed by the VITEK automicroscan system. RESULTS Coagulase-negative staphylococcus(CNS) and Staphylococcus aureus accounted for 67.2% and 7.8%, respectively.The proportion of Gram-negative bacteria was 14.4%.The resistant rate of CNS and S.aureus to oxacillin was 85% and 36%,respectively.The strains producing the extended spectrum beta-lactamases(ESBLs) of Gram-negative bacteria were often detected. CONCLUSIONS Gram-postive bacteria are the main pathogens detected from the blood culture of pediatric patients.Detection rate of CNS is the highest.Multiresistant strains are common.
