1.Locking compression plate fixation for the acetabular fractures

[Objective]To explore the clinic results of the locking compression plate fixation for the acetabular fractures.[Method]Twenty-three patients were treated in the author's department, 16 of them were male and 7 were female patients.The ages ranged from 21 to 61 years, and mean age was 43.3 years.Ninteen patients had been injured in an automobile accident,3 were fell from height and 1 was crushed by a heavy object. The patients were classified according to Letournel classification.There were 7 anterior column fractures,3 posterior wall fractures,5 anterior wall fractures,2 transverse fractures,1 anterior column and posterior column fracture,1 T shape fracture, 3 posterior wall and posterior column fractures,1 transverse and posterior wall fracture.[Result]All the incisions healed during the primary procedure and all the patients were available at follow-up ranging from 3 to 24 months(average 20.1 months).The average intraoperation time was 123 minutes(50~235 minutes),and the intraoperative blood loss was 100 to 1500 ml.The patinents underwent blood transfusion from 0 ml to 1 200 ml.According to Matt's criteria, the rate of excellent and good was 73.9%(17/23).[Conclusion]The locking compression plate fixation is one of the effective methods for treatment of acetabular fractures, with the advantages of being simple and minimally traumatic.
2.Clinical report on arthroscopic reconstruction of anterior cruciate ligament with LARS artifical ligament

[Objective]To explore the operating methods and rudimental clinical effects of reconstruction of anterior cruciate ligament(ACL) with the ligament advancement reinforcement system(LARS) artificial ligaments under arthroscope.[Method]Since June of 2006,15 patients(11 males and 4 females,with an average age of 26.8 years)with acute ACL rupture have been treated with the LARS artificial ligaments.The results of preoperative Arto-scan of all patients suggested discontinuation of ACL.The average score of Lysholm on knee joint was 50 points.The operations were completed under arthroscope.Bone tunnels of the femur were located at the central point of arc on the medial surface of ectocondyle of 140 degree.Bone tunnels of tibia were located between the medial and lateral condyle on the tibial plateau、2-4 mm to summit line of fossa intercondylica.The diameter of the bone tunnels was 7.5 mm while that of LARS ligament and hollow crushing screw were 8 mm.[Result]All 15 patients were available for follow-up with an average of 18 months.No postoperative complications such as infection,ligament rupture,ligament loosening occurred.The clinical results were graded as being excellent in 8 patients,good in 5 and fair in 2 according to the Lysholm's classification.The excellent and good rate was 86.6%.[Conclusion]The anatomical reconstruction and the stability of the knee joint can be obtained after ACL reconstruction with LARS artificial ligaments.The operation under arthroscope has advantages of less-injury,rapid recovery and satisfactory curative effect.
3.Degeneration of Tibial Nerve Induced by HgCl_2 in Rats

Objective To study the adverse effect of mercury on the tibial nerves of rats. Methods Seventy SD rats (35 females, 35 males, weighed 160-200 g) were randomly divided into 4 groups. 40 rats in group 1 were used to test the acute toxicity of HgCl2, 10 rats in group 2 were given HgCl2 by gavage at 17 mg/kg(1/4 LD50) daily, in group 3 the dose was 8.5 mg/kg(1/8 LD50), 10 rats in group 4 were treated with 0.9% saline 2 ml by gavage daily, the experimental duration was (20?4) days. When the model had been developed, all the male rats were killed and the female rats in group 2 and 3 were given the chelator(DMPS) by intraperitoneal injection at 28 mg/kg for 2 periods of treatment, then were killed and the tibial nerves were examined with the electronic microscope. Results The oral LD50 of HgCl2 was 68.1 mg/kg. Seven rats in group 2 (7/10) were poisoned and two of them got pain manifestation. Six rats in group 3(6/10) were poisoned. Myelinoclasis and the changed axons were found in the tibial nerves of poisoned rats and it was more serious in the rats with pain. The chelator(DMPS) did not relieve the change. Conclusion Sub-acute poisoning of HgCl2 can induce degeneration of tibial nerves in SD rats .
4.Application of evidence-based medicine in the department of neurology

Objective To explore ways of enhancing the therapeutic level of the neurology department and reducing the economic burdens of patients. Methods Relevant materials were collected by means of evidence based medicine to optimize therapeutic schedules, with cerebral infarction being the pitching in point. Results Prior to the adoption of evidence based medicine, the average length of stay, the effective rate of treatment and the expenses for single entity diseases were respectively 17.83?13.44, 89.91% and 4 550.70 yuan, whereas after the adoption of evidence based medicine, they were respectively 15.38?11.37, 94.16% and 3 942.88 yuan. Conclusion Evidence based management is a means of scientific management. Its application in the department of neurology has produced a marked effect and it is worthy to be spread.
5.Clinical study of expert tibial nail in treatment of distal tibial fractures

Objective To study the clinical effect of expert tibial nail (ETN) in the treatment of distal tibial fractures. Methods From October 2007 to June 2008,ETN was performed in 13 patients with distal tibial fractures. There were eight males and five females, at age range of 25-47 years (33.8 on average). According to AO/ASIF classification, there were three patients with 43-A1 fractures, four with 43-B1 fractures, four with 43-B2 fractures and two with 43-C1 fractures. All the patients were with close fractures except for three patients with Gustilo-Anderson type Ⅰ fractures. Their clinical data were analyzed for assessing the clinical effect of ETN. Results All patients were followed up for a mean time of 8.4 months (range 3-13 months), which showed that all the fractures obtained stable fixation and sound healing, with no complications like breakage of ETN, wound infection, fracture nonunion or limb shortening. According to Johner-Wruhs standard, the functional results were excellent in 10 patients and good in three. Conclusion ETN has advantages of minimal invasion, shorter operation time, stronger fixation,better soft tissue protection and better functional recovery for distal tibial fractures in comparison with traditional open reduction and buttress plate fixation.
6.Nursing Care on Falls in Hospitalized Elderly Patients (review)

In China, fall ranks the top 3 among adverse events in hospital and it is the first cause of death in the elderly aged 65 and above. Fall risk factors and characteristics, nursing assessment and nursing care interventions are reviewed in this paper to explore risk factors and further improve the nursing care interventions for falls in hospitalized elderly patients.
7.Role of Hepcidin in prostate cancer patients with bone metastasis

Objective To detect the expression and correlation of serum Hepcidin,IL-6,sTfR,BMP6 diversity and explore the role of serum Hepcidin in prostate cancer with bone metastasis.Methods From January 2012 to March 2012,serum Hepcidin,IL-6,sTfR,BMP6 diversity were tested by ELISA in 25 prostate cancer patients with bone metastasis,30 prostate cancer patients without bone metastasis and 30 patients with BPH were used as the control group.The mean patient's age was 67 years (range from 55 to 75 years).In prostate cancers with bone metastasis group,the mean PSA base line was 138.0 μg/L (ranged from 20.0-1500 μg/L),the prostate cancers without bone metastasis group,mean PSA was 10.2 μg/L(ranged from 3.5-28.2 μg/L),and in BPH group,the mean PSA was 3.7 μg/L (ranged from 0.3-14.2 μg/L).Venous blood samples were taken in fasting mornings,then stored 3 ml in EDTA anticoagulant vacuum tube and centrifuged at4 ℃ for 10 min,the isolated serum were then preserved in-80 ℃ refrigerator.The competitive in-phase enzyme-linked immunoassay (ELISA) was used to detect serum Hepcidin,IL-6 and sTfR and BMP6 levels.Results Serum Hepcidin expressions in three groups were 67.7 ± 40.6 μg/L,37.5 ± 15.3 μg/L and 34.3 ± 10.7 μg/L,respectively.For prostate cancers with bone metastasis group,serum Hepcidin expression were higher than control group (P < 0.05),and associated with IL-6(22.5 ±22.1 μg/L),sTfR (5.7 ± 2.6 μg/L),BMP6 (429.3 ± 188.4 μg/L),correlation coefficients were 0.972,-0.987,0.971 (P < 0.05).Conclusions Increased serum Hepcidin level might be a sensitive index for diagnosis and prognosis of prostate cancers with bone metastasis.

Standardized training of residents is a necessary way for clinical medical personnel train-ing. Residents graduating from medical colleges and universities should accept the standardized, scientific, strict pre-service training, in order to realize role conversion from medical students to hospital physicians. This paper summarizes and analyzes the pre-service training, focusing on the training object, course design, organization implementation, evaluation, questionnaire survey, and puts forward some thoughts and sugges-tions on strengthening and improving pre-service training form, expanding the content of training, and quan-tifying the evaluation of training effect.
