1.Mid-term outcomes of Bryan cervical disc arthroplasty versus anterior cervical discectomy and fusion for cervical spondylopathy

Objective To compare the clinical outcomes of Bryan disc replacement with anterior cervical discectomy and fusion (ACDF) in patients with cervical spondylopathy. Methods Sixteen patients underwent Bryan cervical disc replacement (A group), and 35 patients underwent traditional ACDF (B group) were included in the study. Patients were followed up at regular intervals. The JOA score, SF-36, neck disability index (NDI) score and the dynamic flexion-extension radiographs were used to evaluated the oucomes.Results All the patients were followed up for more than 6 years (mean, 73.5 months). There were no severe adverse events in both groups. In A group, there were no differences between postoperative and preoperative mobility of surgical segments (P>0.05). All patients obtained bone fusions 6 month after surgery in group B.In both groups, the clinical symptoms relieved obviously after surgery. The postoperative scores of the JOA,SF-36 and NDI significantly improved compared with those of preoperative ones (P<0.05). In B group, range of motion (ROM) was significantly decreased postoperatively (P <0.01); in A group, there were no significant differences between postoperative and preoperative ROM (P>0.05). The difference between two groups regarding ROM was noted (P<0.05). Conclusion The mid-term outcomes of Bryan cervical arthroplasty are satisfied. And the cervical arthroplasty which can maintain the mobility of the segment, and decrease the incidence of the postoperative neck axial symptoms is a viable alternative to cervical spondylopathy.
2.Influence of large-dose spironolactone on concentration of angiotensinⅡin rat hepatic fibrosis

Objective To comfirm the anti-fibrotic effect of spironolactone and investigate the influence of spironolactone on concentration of angiotensinⅡ(AngⅡ) in rat hepatic fibrosis. Methods Ninety male Sprague-Dawly rats were randomly divided into three groups: control group (8 rats, subcutaneous injection of pure peanut oil, normal diet), model group (42 rats, subcutaneous injection of 40% CCl 4, 0.3 mL/100 g, twice a week, high cholesterol and low protein diet, 5% alcohol as the drink), and spironolactone group (40 rats, same dosage of CCl 4, high cholesterol and low protein diet, gastropufusion of spironolactone at the dose of 100 mg?kg -1?d -1). The animals were sacrificed at 2, 4, 6 and 8 weeks of administration. AngⅡ was assayed by radioimmunoassay. The liver tissues were stained, coded and examined. 3.Experience in the diagnosis and treatment of bronchial anastomosis after lung transplantation

Objective To assess the incidence and prognosis of the bronchial anastomosis complications following lung transplantation.Methods Between January 2003 and July 2016,all 81 cases after lung transplantation at Department of Thoracic Surgery of Shanghai Pulmonary Hospital,Tongji University were retrospectively analyzed.We analyzed the incidence and prognosis of postoperative complications of bronchial anastomosis in lung transplant recipients.Results The overall incidence of bronchial anastomotic complications was 30.9%,including anastomotic infection (18.5 %),anastomotic fistula (4.9 %),stenosis (6.2 %),and bronchomalacia (1.2 %).One case died of infection,and 3 cases died of anastomotic fistula.Conclusion The main bronchial anastomosis complications occurred early,and the prognosis was acceptable.
4.The mechanisms of apoptosis induced by curcumin in gastric cancer cells

Objective To study the mechanisms of Curcumin-induced apoptosis on human gastric cancer cell line SGC-7901.Methods SC,C-7901 cells were treated with various concentrations of Curcumin and the growth inhibition rates of it were accessed by MTT method.Apoptosis of gastric cancer cells were in- spected by flow cytometry.The expression of Fas and survivin in gastric cancer cells were evaluated by west- ern blot.Results Curcumin could effectively inhibit the growth of gastric cancer cells in dose-dependent and time-dependent manners,the sub-peak appeared and the apoptotic rate was increased.The expressions of Fas was higher in Western blot,meanwhile,the expressions of survivin was decreased.Conclusion Curcumin could significantly inhibit the growth and induce apoptosis of gastric cancer cells(SGC-7901),Curcumin could probably through up-regulating Fas and down-regulating surviving to induce apoptosis.
5.Biomechanical effect of bone cement volume and distribution on lumbar vertebral body with osteoporotic fracture

Objective To evaluate biomechanical effect of bone cement distribution on lumbar vertebral body with osteoporotic fracture.Methods Forty nine lumbar vertebrae (L1-L5) specimens were collected from 12 old cadavers.After exerting axial pressure load on every specimen,the initial intensity and stiffness were measured,and then vertebral body crush fracture models were established.According to zones where bone cement was injected in vertebrae,the specimens were divided into one control group and six experimental groups:A,B,C,D,E,F groups,i.e.unilateral anterior 2/3 group,unilateral posterior 2/3 group,unilateral whole group,bilateral anterior 2/3 group,bilateral posterior 2/3 group,and bilateral whole group,respectively,including 7 specimens in each group.In A,B,C groups,unipedicular balloon kyphoplasty was done,while in D,E,F groups,bipedicular balloon kyphoplasty was done.Then the maximum compressive strength and stiffness were measured.Results After percutaneous kyphoplasty,the maximum strength in all experimental groups was significantly greater than that in the control group.There were no significant differences in strength between A and B groups,between C,D and E groups.For the maximum strength,the results of comparison were:F group > C group,D and C groups > A and B groups.Except for F group,the stiffness in other 5 experimental groups was significantly lower than its initial value.There were no significant differences in stiffness between A,B,and C groups,between D and E groups.However,the stiffness in F group was greater than those in D and E groups,and it was greater in bilateral groups than those in unilateral groups.Conclusion Bone cement distribution in lumbar vertebral body can affect biomechanical property of vertebral body,and the bilateral distribution can cause better biomechanical effect than unilateral distribution.The ideal distribution zone of bone cement is in the anterior 2/3 of the vertebral body.
6.Correlative study between the angiotensin converting enzyme gene polymorphism and cerebrovascular diseases in Naxi population in Yunnan province

Objective To investigate the correlation between the angiotensin-converting enzyme(ACE)gene insertion/deletion(I/D)polymorphism and cerebrovascular diseases in Naxi population in Lijiang,Yunnan province.Methods Fifty-eight Naxi patients with cerebral infarction,32 Naxi patients with intracerebral hemorrhage,and 50 sex- and age-matched Naxi healthy controls were recruited.Polymerase chain reaction-restricted fragments length polymorphism was used to detect the ACE gene polymorphism and perform bidirectional sequencing Results The DD genotype and the D allele frequency in the cerebral infarction group were significantly higher than those in the healthy control group(32.8% vs.16.0%,P = 0.045 and 54.3% vs.39.0%,P =0.025);while there were no significant differences in the DD genotype and the D allele frequency between the intracerebral hemorrhage group and the control group.The different types of cerebral infarction carried different allele frequencies,they were in order of cerebral embolism < cerebral thrombosis < lacunar infarction.The DD genotype in patients with lacunar infarction(40.0% vs.30.0%,P = 0.481 )and the D allele frequencies(63.3% vs.51.2%,P =0.257)were all higher than those in patients with cerebral thrombosis,but there were no significant differences.Conclusions The ACE DD genotype carriers were susceptible to cerebral infarction in Naxi poulation in Yunan province.
7.Value of virtual touch tissue imaging in the differential diagnosis of benign and malignant thyroid nodules

Objective To explore the value of virtual touch tissue imaging (VTI) of acoustic radiation force impulse (ARFI) elastography in the differential diagnosis of benign and malignant thyroid nodules.Methods VTI features of 77 histologically proven thyroid nodules in 74 patients were analyzed and the VTI features were divided into 6 grades.The higher grade represented higher tissue stiffness.Differences in VTI grade between benign and malignant lesions were analyzed by receiver operator characteristic (ROC) curve,and the sensitivity,specificity and accuracy were calculated.Results 77 pathologically proven thyroid nodules included 61 benign and 16 malignant lesions.VTI images classification included 0 in grade Ⅰ,1 ingrade Ⅱ,1 in grade Ⅲ,9 in grade Ⅳ,3 in grade Ⅴ and 2 in grade Ⅵ for malignant diseases ; and included 18 in grade Ⅰ,28 in grade Ⅱ,10 in grade Ⅲ,1 in grade Ⅳ,2 in grade Ⅴ and 2 in grade Ⅵ for benign diseases.The best diagnostic point of VTI-grade was grade Ⅳ.When VTI-grade Ⅳ or greater was used as the diagnostic criterium for malignancy,the sensitivity,specificity and accuracy rate in differentiation between benign and malignant thyroid nodules were 87.50%,91.80% and 90.9(% respectively.Conclusions VTI is useful in the differential diagnosis between benign and malignant thyroid nodules.
8.Experimental study on relationship between TCM syndrome type and gastric mucosa cell proliferation in rat model of chronic atrophic gastritis.

OBJECTIVETo explore the relationship between TCM syndrome type and gastric mucosa cell proliferation related controlling gene protein in chronic atrophic gastritis (CAG).
METHODSExpressions of cell proliferation related controlling gene protein, including proliferative cell nuclear antigen (PCNA), epidermal growth factor receptor (EGFR) and c-myc, from gastric mucosa of CAG model rats with different syndrome types were measured by immunohistochemistry and the changes of them before and after TCM intervention were also analyzed by image analysis.
RESULTSProtein expressions of PCNA, EGFR and c-myc in gastric mucosa of CAG model rats with different syndrome types (Pi-deficiency type, Gan-stagnation type and dampness-heat type) were different to some extent, and all of them reduced significantly after TCM intervention in the model rats of all syndrome types.
CONCLUSIONExpressions of cell proliferation related controlling gene protein in gastric mucosa of CAG model rats of different syndrome types were different to some extent, which provides a certain experimental evidence for revealing the essence of TCM syndrome type of CAG and judging the prognosis of various types.

9.Diagnosis and surgical management of juxtaglomernlar cell tumor of the kidney

Objective To improve the diagnosis and treatment quality of juxtaglomerular cell tumor of the kidney. Methods Three patients(2 females, 1 male) were diagnosed with juxtaglomerular cell tumor of kidney and underwent nephron-sparing surgery. Case 1 was female, 15 years old. She presented with hypertention of 245/135 mm Hg. The serum kalium was 2.5--3.0 mmol/L. Thelaboratory examination suggested that in decubitus and standing position, the plasma renin activity (PRA) was 2.2 and 3.5 μg · L-1 · h-1 , angiotensin Ⅱ (ATⅡ) was 181.2 and 481.4 ng/L; aldosterone(ALD) was 332.4 and 747.9 pmol/L, respectively. Dynamic enhanced CT scanning demonstrated a tumor with the diameter of 1.3 cm in right kidney. Case 2 was male, 39 years old. He presented with hypertention of 180/120 mm Hg. The serum kalium was 2. 7--3.0 mmol/L. In decubitus and standing position, PRAwas8.1 and 9.2 μg·L-1 · h-1, ATⅡ was 198.3 and 279.1 ng/L, ALD was 285.3 and 761.7 pmol/L, respectively. Dynamic enhanced CT scanning showed a tumor with the diameter of 1.2 cm in right kidney. Case 3 was female, 26 years old. She presented with hypertention of 210/120 mm Hg. The serum kalium was 4. 1 mmol/L. In decubitus and standing position, PRA was 0.1 and 0.3 μg · L-1·h-1 , ATⅡ 56.2 and 71.5 ng/L, ALD 321.3 and 421.1 pmol/L, respec tively. On dynamic enhanced CT scanning, a tumor with a diameter of 3.0era was located in left kidney. Results Partial nephrectomy was successfully performed in 3 patients, including 1 (case 2) retroperitoneal laparoscopic surgery. Pathologic examination revealed encapsulated tumors in all cases. Light microscopically, the tumor consisted of clusters of polygonal cells, and the cell had centrally located nuclei and slightly eosinophilic cytoplasm. Thick walled vessels were usually present. The tumors showed positive immunostaining for actin and CD34. Three patients were followed up for 23,4, 26 months respectively and all remained normotensive without any treatment. No recurrence or metastasis occurred. Conclusions Hypertention, increased PRA, secondary aldosteronism, hypokalemia are characteristics for juxtaglomerular cell tumor of the kidney. Dynamic enhanced CT scanning has high sensitivity. Partial nephrectomy or enucleation of tumor are both effective surgical treatment. Retroperitoneal laparoscopic surgery is safe and effective as well.
10.Role of opioid receptors in protective effects of isoflurane- induced delayed preconditioning against myocardial ischemia-reperfusion injury in rabbits

Objective To investigate the role of opioid receptors in the protective effects of isoflurane-induced delayed preconditioning against myocardial ischemia-reperfusion (I/R) injury in rabbits. Methods Forty male New Zealand white rabbits weighing 2.0-2.5 kg were randomly assigned into 4 groups ( n = 10 each) : group I sham operation (S); group II I/R; group Ⅲ isoflurane + I/R (Iso) and group IV Iso + naloxone + I/R (Nal). Myocardial I/R was induced by 40 min occlusion of left anterior descending branch (LAD) of coronary artery followed by 120 min reperfusion. In group Ⅲ (Iso) 2% isoflurane in 100% O2 was inhaled for 2 h and I/R was produced 24 h later. In group IV (Nal) naloxone 6 mg/kg was given iv 10 min before 2 h of 2% isoflurane inhalation and I/R was produced 24 h later. At the end of 120 min reperfusion, infarct size (IS) and area at risk (AAR) were determined by Evan's blue and TTC staining. Myocardial ultrastructure was examined by electron microscopy. The phosphorylated p38MAPK protein expression in myocardium was determined by Western blot. Results The IS was significantly smaller in group Iso ( Ⅲ ) ( 19.7% ± 2.8%) than in I/R group ( II ) (37.8% ±1.7%) (P<0.05). The phosphorylated p38MAPK protein expression in myocardium was significantly lower in group Iso than in group I/R. Microscopic examination showed less myocardial damage in Iso group than in group I/R. The protective effects of delayed preconditioning by isoflurane was prevented by naloxone pretreatment. ConclusionOpioid receptors may be involved in the protective effects of delayed preconditioning by isoflurane against myocardial I/R injury.
