Drug resistance is major obstacles in the successful treatment of malignant solid tumors. Multiple mechanisms involeve in development of cancer drug resistance. Recent research suggests dysregulation of microRNAs is associated with cancer drug resistance. The profiles of microRNAs in drug resistance cancer cells or tissues are different with sensitivity cells or tissues in various solid tumors. Restoring microRNAs could improve chemosensitivity of cancer cells. MicroRNAs expression profiles may provide a critical link for understanding mechanisms involved in chemoresistance. We can also find a specific marker for screening chemosensitivity patients through identification of the microRNAs patterns of drug resistance cells or tissues.

Objective:To explore the efficacy and adverse reaction of TOMOX regimen combined with three dimensional conformal radiotherapy in the treatment of middle and advanced esophageal cancer. Methods:51 cases of hospitalized patients with esophageal cancer were selected from January 2013 to December 2015 in our hospital who were divided into observation group (n=27) and control group (n=24) by random number table method. The control group was treated with three dimensional conformal radiotherapy, and the observation group was added with the TOMOX scheme based on the control group. Compare the efficacy of the two groups, the control period, the survival period, the toxicity and side effects. Results: The rate of complete response(CR)and partial response (PR) in the observation group was significantly higher than that in the control group, the difference was statistically significant (P<0.05). The total effective rate of the observation group was 96.30%, which was significantly higher than that of the control group 58.33% (P<0.001). The 2 year control rate and 2 years survival rate of the observation group was significantly higher than that of the control group (P<0.05). The incidence of acute radiation pneumonia and gastrointestinal reaction in observation group was lower than the control group, the difference was statistically significant (P<0.05).The neuritis rate and leukopenia of observation group was lower than the control group, but the difference was not statistically significant (P>0.05). The rate of acute esophagitis in the observation group group was higher than that in the control group, the difference was statistically significant (P<0.05).Conclusion: TOMOX combined with three-dimensional conformal radiation therapy can effectively improve the short-term efficacy and long-term survival rate, reduce adverse reactions.

Objective To observe the analgesic effects of preoperative tramadol caudal injection in patients after receiving gynaecological laparoscopy under general anesthesia. Methods A total of 60 patients scheduled for gynaecological laparoscopic operations under general anesthesia was randomly divided into two groups with 30 patients each. The Group A was given an injection of tramadol 100 mg,2% lidocaine 5 ml,ondansetron 4 mg,and normal saline 20 ml by caudal route preoperatively, while the Group B was given an intravenous drip of tramadol 100 mg and ondansetron 4 mg at 10 minutes before the end of operation. The Visual Analog Scale(VAS) pain scores and the Ramsay Sedation Scores were recorded after extubation, before the departure from operation room, and at 1,3,and 6 hours after surgery, respectively.Adverse effects of dysphoria, dizziness, drowsiness, nausea, and vomiting were recorded. Results The VAS scores were significantly lower after extubation, before the departure from operation room, and at 1 and 3 hours after surgery in the Group A than in the Group B(t=-5.961,-8.362,-4.038,-3.565;P=0.000), while the difference was not significant between the two groups at 6 hours after surgery(t=-1.403,P=0.166).The Ramsay scores were significantl higher after extubation, before the departure from operation room, and at 1 and 3 hours after surgery in the Group A than in the Group B(t=9.409,10.407,8.167,4.082;P=0.000),while the difference was not significant between the two groups at 6 hours after surgery(t=0.428,P=0.670).Significantly more patients presented dysphoria, dizziness, nausea, and vomiting in the Group B than in the Group A after extubation and before the departure from operation room. Conclusions Tramadol 100 mg caudal injection before operation can produce a better postoperative analgesic effect in patients receiving gynaecological laparoscopic operations.

OBJECTIVE: To establish headspace capillary gas chromatography for the content determination of 3 kinds of residual organic solvent (furanidine, methanol and ethanol) in betamethasone sodium phosphate raw material.METHODS: The sample was dissolved in water and n-propanol was used as internal standard. The residual solvent in betamethasone sodium phosphate was separated on HP-INNOWAX (PEG) capillary column with column temperature set at 60 ℃.The injector temperature and FID detector temperature were controlled at 180 ℃ and 250 ℃,respectively. The carried gas was nitrogen at flow rate of 1.0 mL?min-1.The splitting-ratio was 10 ∶ 1.The containers of head-space injector were in equilibrium at 80 ℃ for 30 min.Injection time was 1 min.RESULTS: With this chromatographic condition, those solvents could be separated completely. The linear range were 0.014 4~0.071 8 mg?mL-1 for furanidine,0.060~0.300 mg?mL-1 for methanol and 0.099 3~0.497 mg?mL-1 for ethanol. The average recovery were 103.7% (RSD=0.53%,n=6),95.8% (RSD=0.30%,n=6) and 95.0% (RSD=0.48%,n=6) respectively. The minimum quantitation limit were 0.057 3 ?g?mL-1,0.486 ?g?mL-1,0.145 ?g?mL-1,respectively.3 kinds of residual organic solvents were all in line with the standard stated in Chinese Pharmacopeia.CONCLUSION: The established method is simple, sensitive and accurate for the content determination of residual solvents in betamethasone sodium phosphate raw material.

Objective: Design a test phantom with a full function, simple structure and high accuracy which is used for MRI’s Quality Control. Methods: Through a intensive study in imaging principle of MRI and parameters which effect the image quality ,first come up with the phantom’s performance data, and then determine the technical solutions including the choosing material, processing technology, the machining precision, at last develop a trial set of MRI test phantom using the method of numerical control machining. Make a comparative experiment between this phantom and the similar imported one to analyze the consistency of the results by the two phantoms. Results: The result of all the technical parameters of this phantom is consistent with the result of the imported one. There are some extra test components which the imported phantom doesn’t contain. Some performance data like the low contrast resolution are superior to the imported phantom. Conclusion:This MRI test phantom can satisfy the requirement of the daily quality control work in hospital.

Objective To observe the efficacy of extracorporeal liver support by using less fresh frozen plasma in the treat‐ment of acute‐on‐chronic liver failure. Methods A total of 45 patients with acute‐on‐chronic liver failure were divided into ob‐servation group[plasma perfusion(PP) with a small amount of plasma+ plasma exchange(PE)] ,control group 1(PE) ,control group 2(PP+PE)in terms of the amount of plasma used on the day of treatment. All the patients received artificial liver treatnts 62 times totally. Results The clinical symptoms were improved in the three groups after treatments. There were significant differences in the decrease of alanine transaminase (ALT) ,aspartate transaminase(AST) and direct bilirubin(DBil)rather than the decrease of total bilirubin(TBil)and blood ammonia among the groups. No significant difference was noted in the liver and kidney function among the three groups. The improvement of the coagulation function was poor in the observation group when compared with the control group 1 and control group 2 and there were significant differences. Conclusion During the short sup‐ply of the plasma ,plasma perfusion combined with small amount of plasma can be considered to be used in artificial liver treat‐ments ,which can effectively decrease the level of TBil ,relieve symptoms and decrease the occurrence of complications.

Long non-coding RNA (lncRNA)is a kind of RNA molecule which is longer than 200 nucleotides and has no capacity of coding proteins.Accumulating evidences have indicated that several lncRNAs,such as HOTAIR,MALAT-1 ,H1 9 and BANCR may promote tumor metastasis by inducing epitheli-al-mesenchymal transition.
