Chinese Journal of Cerebrovascular Diseases 2006;3(6):257-262
Objective: Exploring the effect and safety of local intra-arterial thrombolysis (LIT) on acute central retinal artery occlusion (CRAO). Methods: Retrospective data analysis of 49 consecutive CRAO patients was performed. The visual acuity was 3.0-4.7 in 51% patients, and less than 3.0 in 49%. The averaged visual acuity was 1.5 ± 0.1. All the patients were treated with urokinase perfusion through the ophthalmology artery within the first 6 hours after CRAO attack. Conventional treatment including intra-ocular pressure decreasing, microcirculation improvement, neuroprotection and anti-coagulation were conducted. The visual acuity was detected at the 2, 28 days and 6 months after thrombolysis, and field of visions was detected at 28 days and 6 months after thrombolysis. And the thrombolysis related complications were observed. The correlation of the visual outcome after thrombolysis with the patient's ages, the time thrombolysis when started, and the residual visual acuity at admission were calculated. The complication which was related to cerebral angiography and LIT was explored. Results: Improvement of visual function was achieved after thrombolysis. The averaged visual acuity was 4.2 ± 0.2, 4.4 ± 0.3, 4.6 ± 0.5 at the 2,28 days and 6 months after thrombolysis respectively. 24.5% patients regained visual acuity of > 4.8, and visual field deficit was less than 30% in 34.7% at 28 days. 6 months later, 36.7% patients regained visual acuity of > 4.8. Field deficit was less than 30% in 44.9% patients. The pretreatment residual visual acuity was the significant predictor of good recovery of visual acuity (r=0.292, P < 0.05). There was no ischemic attack and hemorrhagic complication which correlated with cerebral angiography and LIT. Conclusion: LIT is an effective and safe method for CRAO patients. The pretreatment residual visual acuity is the significant predictor of good outcome.

Objectives Understanding the characteristic changes of Keshan disease (KD) in different epidemic period to provide reference basis for prevention and teatment. Methods On the basis of medical record as fundamental element, the relative conditions of Keshan disease's prevailing and spreading period were compared. Results In high incidence years, familial aggregation [accounted for 12%(6/50)] and seasonal aggregation were found, and KD cases occurred mainly from May to September, which was 78% (39/50)of the total cases in the whole year. Circulatory dysfunction(gallop rhythm, pulmonary role, jugular venous engorgement, cyanosis of lips) was more severe in high incidence years than that in low incidence years(X2=8.53,P<0.01). The average age of incidence was (4.07±1.46) years old in high incidence years and (6.11±2.71) years old in low incidence years. The type constitution in high incidence years was significantly different from that in low incidence years (X2=40.68, P<0.01), and chronic type of KD accounted for 22.85%(707/3094),46.09%(53/115), respectively, in high and low incidence years. Conclusions Making a further research of seizure of disease, and improving diagnosis and cure management level are also the important content for prevention and cure research work of Keshan disease at right time.

Objective To study the mechanism of glycerol in treatment of trigeminal neuralgia. Methods After chronic constriction injury was produced to the infraorbital nerve and the model of trigeminal neuralgia in rats was successfully established, the trigeminal ganglion and preganglionic rootlets were exposed through infratemporal fossa approach. In the experimental group, pure glycerol was instilled into the preganglionic rootlets, while in the control group physiologic saline instead. The electrophysiologic response and the results of the electron microscopy were observed at different time points after glycerol injection. Results The latency of sensory evoked potential in the experimental group was longer than that in control group. Under the electron microscope, demyelination and degeneration were very similar in two groups, but disruption of neuraxon was only observed in the experimental group. Neuraxon regeneration occurred 10 weeks later. Conclusion Retrogasserian glycerol rhizotomy in treatment of trigeminal neuralgia was through disruption of myelinated nerve fiber that is reversible.

Objective To explore the mechanism of blood-stage treatment in patients with psoriasis vulgaris through studying the level change of IFN-?, IL-4, IL-10 and IL-17 in patients with blood-stage treatment during activity and quiescence period separately. Methods 20 patients with psoriasis vulgaris in active stage(blood heat syndrome)and 20 patients with psoriasis vulgaris in resting stage(blood stasis syndrome) were recruitedto observe the treatment effects by the PASI score, and to observe thechange of IFN-?, IL-4, IL-10, IL-17 before and after treatment in the serum by ELISA. Results The PASI scores of two groups were both significantly decreased after treatment (blood heat syndrome group t=6.685, P<0.01;blood stasis syndrome group t=4.959, P<0.01). The levels of cytokines were significantly different between patients in the periods of activity and quiescence. Onvarying degrees, the levels of cytokines of two groups were improved after treatment. The levels of cytokines IFN-?, IL-17 in blood heat group significantly decreased(t=3.024, P<0.01;t=2.543, P<0.05). The levels of cytokines IL-17 drop but the levels of cytokines IL-4 raised in blood stasis group, that were significantly differentwith the levels before treatment(t=2.417, P<0.05; t=2.547, P<0.05). Conclusion The levels of INF-?, IL-4, IL-10 and IL-17 could be effectively modulated with blood-stage treatment in treating psoriasis vulgaris.
