1.Comparative study on the characteristics of the high site visual field injuries and the visual nerve injuries after the head trauma

Objective To Study the characteristics of the high site visual field injury and the LED-VEP after the head trauma for the purpose of the medico-legal appraisals. Methods 11 cases with cortical blindness resulted from the head injury were studied and then compared with 20 cases suffered from optical nerve injuries with respect to the clinical manifestations and characteristics of the visual field injury and LED-VEP. Results As compared with the characteristics of the optical nerve injuries, besides the subjective complain of lowering the visual field, there were not any positive findings in pupil reflex of light and retina examination in case of the high site visual field injuries. The electrical visual field test revealed the homony-mous hemianopia while the internal segment optical nerve injury manifested unilateral visual field defect. 8 out of 11 cases with cortical blindness, occipital cortex were injured and the flash VEP demonstrated the slight prolongation of the latent period of the PI wave of both eyes, while the PI amplitudes were normal. 3 out of 11 cases were suffered from the subcortical injuries and were manifested with the prolongation of the latent period of the PI wave and the amplitude of the PI wave were significantly reduced. Conclusion Bilateral homonymous hemianopia were the characteristics of the high site visual field injury while the abnormal rate of the flash VEP of the high site visual field was quite low. So that, homonymous hemianopia may be helpful for the diagnosis of cortical blindness. At the same time CT scanning and the electroen-cephalography (EEG) also must be considered comprehensively in the medico-legal appraisals.
2.Effect of streptozotocin on learning memory and JNK signaling pathway in APP/PS1 transgenic mice

Objective To investigate the effect of streptozotocin (STZ) on learning,memory and expression of p-MKK4,p-JNK and p-Jun in APP transgenic mice.Methods Three-month-old APP/PS1 mice were intraperitoneally injected with STZ.Learning and memory function was performed by Morris water maze.The changes of neuronal morphology in hippocampum were observed by Nissl staining.The expression of MKK4,JNK and c-Jun were detected in hippocampal tissues by Western blotting.Results In the hidden platform experiment of Morris maze,the escape latencies and the pathway length of STZ-induced diabetic mice were longer than those in controls from the 2nd to 5th day(P < 0.05 or P < 0.01).The Nissl staining showed that some neurons in hippocampum of STZ-treated mice were displayed changes of injury.Compared with the vehicle control,the expression levels of pJNK and p-c-Jun were increased by 217.78 ± 23.49 and 189.27 ± 17.80,respectively (P < 0.01).Conclusion STZ can decrease the learning and memory function and injury the neurons,which maybe relate to the JNK signal pathway.
3.Radiation protection shielding computation of gamma photons produced in positron annihilation

Objective To study the best method of radiation protection shielding calculation for the gamma photon produced in positron annihilation.Methods With 18F as the typical of nuclide,different methods or literature about constant and the calculation of the TVL or recommended values were adopted to calculate and analyze the shielding thickness by lead as block material.Results The shielding thickness by the dose constant was calculated by point source model (0.142 μSv· m2· MBq-1 · h-1) and TVL of lead recommended by NCRP was the biggest:at the control area the unit thickness for lead shield was 7.562 × 10-4 mm· MBq-1 for one patient.While by the dose constant recommended by AAPM TG108 (0.092 μSv·m2 ·MBq-1 ·h-1) and lead TVL recommended by IAEA was the minimum.The unit thickness for lead shield at the control area was 4.982 × 10-4 mm · MBq-1 · patient-1,with difference of 36.2%.Conclusions The dose constant and the TVL value of lead from AAPM.TG 108 are recommended for the calculation.In shielding design attention should be paid to the distance protection,the separate toilet with shielding for patients,and the separate corridors beteen doctors and patients.
4.Effect of the evidence-based nursing educational programme for preventing ventilator-associated pneumonia in critical care nurses

Objective To establish the evidence-based nursing educational programme for preventing ventilator-associated pneumonia (VAP) and evaluate the effects in critical care nurses. Methods The evidence-based nursing practice guidelines for preventing VAP from 2008 American Association of Critical Care Nurses was used as training materials,which guided the curriculum design. Seventeen critical care nurses were trained. The difference of the nurses' knowledge and practice was evaluated before and after training by an evaluation scale of nursing quality criteria for preventing VAP. Results After the training,nurses' knowledge and practice of preventing VAP were improved(Z=-3.624,P=0.000). Except the factor score of handwashing,there were significant differences on the factor scores of body position care,endotracheal tube care,enteral nutrition,maintaining the tube cuff pressure,oral care and ventilator equipment management (P<0.01). Conclusion The educational programme of evidence-based nursing for preventing VAP can improve the nurses' knowledge and practice of preventing VAP. Furthermore,it can promote the application of evidence-based nursing for preventing VAP,and thereby to improve the quality of nursing,and provide guidance for the training of critical care nurses and continuing education.
5.Drug inhibition of wear particles induced osteolysis in rabbits

[Objective]To study the possibility of drug inhibition of wear particle induced osteolysis and aseptic prosthesis loosening in vivo.[Methods]4?8 mm transverse holes were made in femur condyle and tibia plateau of 12 adult rabbits bilaterally.One milligram of CoCr alloy(mean 5.38 ?m),Ti-6Al-4V(mean 3.21 ?m) and UHMWPE(12~200 ?m) particles were implanted into the holes separately.The animals were divided into 4 groups and each group had the same particles combination.Oral drug therapy twice a day was adopted with non-selective COX inhibitor(indomethacin,0.5 mg/kg BW),membrane Ca2+ channel inhibitor(nitrendipine,0.1 mg/kg BW),osteoclast inhibitor(alendronate sodium,0.1mg/kg BW) and blank control(0.9% saline) to the 4 groups separately and randomly according to the double blind principle,which started on the 2nd day postoperatively until 12 weeks at the time of sacrifice.Routine histological observation and calculation of the ratio of bone area(BA) to total tissue area(TTA) were done under the computerized analysis system.[Results]CoCr and Ti alloy particles could seldom be seen in the slices of the groups which showed normal cancellous bone.There were obvious macrophage infiltration and fibroblast proliferation round the particulate UHMWPE in the group of blank controls.It was similar in the groups of indomethacin and nitrendipine though the histological reaction was a little bit weaker and osteotoid tissue could occasionally be seen.UHMWPE particles were totally enclosed in the cancellous bone with little fibrous tissue proliferation in the group of alendronate sodium and the BA/TTA ratio was significantly higher than those at the other groups(P0.05).[Conclusion]This indicates the possibility of inhibition or prevention of osteolysis induced by wear particles by drug therapy especially when alendronate sodium is adopted,and it has significant clinical implications for controlling the most common cause of implant failure.

7.Comparative genomic hybridization technique detects the chromosomal aberration in renal carcinoma

Objective To detect the chromosomal aberration in the cells of renal carcinoma and to evaluate the rela-tionship between the chromosomal aberration and development of renal carcinoma.Methods CGH technology was used to analyze the global genomic aberration in cancer cells from 12 patients with renal carcinoma.Results All of 12 cases deeeted by CGH showed chromosomal aberrations.The common extension regions of renal carcinoma were 1p,4p,5q,7p,9p and 16p.The common deletion regions of renal carcinoma were 3q,4q,6q,9q,14q and 18q.Conclusion The hereditary material of renal carcinoma is unbalanced.The extension and/or deletion of chro-mosome may facilitate the occurrence of renal carcinoma.

Objective To examine the expression of lipoxygenases (LOXs)on human pancreatic carcinoma and their effects on proliferation and apoptosis of human pancreatic carcinoma in vitro.Methods Expression of 5- LOX and 12-LOX in the tissue of human pancreatic carcinoma and pancreatic cancer cell line (SW1990) was detected by immunohistochemical staining and RT-PCR.To examine the effects of polyunsaturated fatty acids on pancreatic cancer cell line,the proliferation rate of SW1990 cells treated with arachidouic acid (AA),docosahexaenoic acid (DHA) and eicosapentenoic acid (EPA) was analyzed by MTT and the incorporation of bromodeoxyuridine (BrdU) methods.An annexinV/propidium iodide (PI) assay with flow cytometry and in situ terminal deoxynucleotidyl transferase-mediated dUTP-digoxigenin end labeling (TUNEL) assay were used to identify potential induction of apoptosis by the unsaturated fatty acids.The effects of 5-LOX inhibitor (MK-886) and 12-LOX inhibitor (baicalein) on the proliferation of SW1990 cells were determined by above methods.Results Both 5-LOX and 12-LOX were over expressed in the tissue of human pancreatic carcinoma and the pancreatic cancer cell line.AA had a stimulatory effect on the growth of SW1990 cells while EPA and DHA had an inhibitory effect in a dose-dependent manner.The apeptosis of SW1990 cells co-cultured with DHA or EPA lasting for 24 hours was increased markedly.Furthermore, both MK-886 and baicalein in a dose-dependent-manner inhibited the growth of SW1990 cells ard induced a significant increase of apoptotic cells rate.Conclusions Expression of lipoxygenase is up-regulated in human pancreatic cancer. Polyunsaturated fatty acids have regulatory effects on the growth of pancreatic carcinoma in vitro.AA stimulates proliferation of pancreatic cancer cell line,while DHA and EPA suppress proliferation,simultaneously inducing apoptosis of pancreatic cancer cell line.Blocking the functions of lipoxygenase with its inhibitor exerts a negative regulatory effect on the growth of pancreatic carcinoma in vitro.Lipoxygenase might be a novel therapeutic target for the pancreatic cancer.
10.Effect of Auricular Acupuncture Combined with Physical Therapy on Motor Function of Stroke Patients

ObjectiveTo observe the effect of auricular acupuncture combined with physical therapy on motor function of stroke patients.Methods40 stroke patients were divided randomly into trial group and control group with 20 cases in each group. Patients of trail group were treated with auricular acupuncture combined with physical therapy; cases of control group were treated only with physical therapy. Motor function of all patients were assessed with Fugl-Meyer Assessment (FMA) before and after treatment.ResultsFMA scores of patients of trial group increased significantly compared with that of control group (P<0.01).ConclusionEffect of auricular acupuncture combined with physical therapy is better than that of single physical therapy on motor function of the stroke patients.
