1.Effect of muscle stimulating instrument on muscle stone of children with cerebral palsy after selective posterior rhizotomy

ObjectiveTo observe the effect of muscle stimulating instrument in enforcing muscle stone of children with cerebral palsy after selective posterior rhizotomy(SPR).Methods106 patients were divided into two groups,treatment group(n=51) and control group(n=55).Two groups received routine rehabilitation therapy,otherwise,treatmwnt group were treated with the muscle stimulating instrument after SPR.Pretreatment and 30d,60d,180d after treatment,muscle stone of two groups was measured and compared.ResultsImprovement of treatment group in muscle stone was better than that of control group(P<0.01).ConclusionMuscle stimulating instrument is effective to improve muscle tone of children with cerebral palsy after SPR.
2.Early hematoma enlargement in patients with spontaneous intracerebral hemorrhage and related factors analysis

Objective To explore the related factors of early hematoma enlargement in patients with spontaneous intracerebral hemorrhage.Methods The clinical data of 142 patients with spontaneous intracerebral hemorrhage were analyzed.The first CT was performed within 6 h of onset and the second within 24 h of onset.Single factor analysis and multiple Logistic regression analysis were performed to determine the related factors of early hematoma enlargement.Results The incidence of early hematoma enlargement was 24.6% (35/142).Multiple Logistic regression analysis revealed that the following four factors were independently associated with early hematoma enlargement:age (OR =1.069,P =0.003),systolic pressure (OR =1.865,P =0.026),sharp of hematoma (OR =2.712,P =0.028),using mannitol (OR =2.939,P =0.020).Location of hemorrhage and volume of hemorrhage were not associated with early hematoma enlargement (P > 0.05).Conclusions Age,systolic pressure,sharp of hematoma and using mannitol are the important predictors of early hematoma enlargement with spontaneous intracerebral hemorrhage.In patients with older age,higher systolic pressure,irregularly shaped hematoma,close observation of hematoma enlargement should be made CT-scanning check.And the caution in the early use of dehydrating agent should be careful.
3.Staphylococcus in Children with Septicemia:Classification and Antibiotics Resistance

OBJECTIVE To investigate the classification and antibiotics resistance of Staphylococcus in children with septicemia in recent two years and to provide data for clinical treatment. METHODS All the blood specimens delivered by pediatrics were identified and detected the antibiotic resistance with Microscan Auto SCAN4. RESULTS Totally 268 Staphylococcus strains were collected. Among them,S. aureus was 114 strains (42.5%),coagulase-negative Staphylococcus were 154 srains. Among 114 S. aureus strains,meticillin-resistant Staphylococcus aureus (MRSA) were 79 strains (69.3%). Among 154 coagulase-negative Staphylococcus strains,meticillin-resistant coagulase-negative Staphylococcus aureus (MRCNS) were 102 strains (66.2%). The drug sensitivity results showed that meticillin-resistant staphylococcus (MRS) were highly resistant to 15 commnly used antibiotics than meticillin-sensitive Staphylococcus (MSS) and showing multi-drug resistance. No vancomycin-resistant strains were found.CONCLUSIONS Children with septicemia infected by Staphylococcus are increasing recently. Identification and antibiotic resistance of clinical isolated Staphylococcus must be strengthened,and give the effective data for clinicians.
4.Study on Therapeutic Effect and Mechanism ofQi-Zhu Er-Zhu Er-CaoTang in Treatment of Chronic Atrophic Gastritis Precancerous Lesion Based on Pathway of NF-κB (p65) and CyclinE

This study was aimed to observe the therapeutic effect and mechanism ofQi-Zhu Er-Zhu Er-Cao Tang(QZEZECT) in the treatment of chronic atrophic gastritis (CAG) precancerous lesion. A total of 56 clean grade healthy Wistar rats were randomly divided into 6 groups. In the negative control group (NC), model group,Wei-Fu-Chun(WFC) group,Ren-Zhu Jian-Wei Ke-Li(RZJWKL, RZ) group, there were 10 rats in each group. In the high-dose QZEZECT (QZ-H) group and low-dose QZEZECT (QZ-L) group, there were 8 rats in each group. CAG/PLGC model was established by MNNG in the model group, WFC group, RZ group, QZ-H group and QZ-L group. Intragastric administration of corresponding decoctions at the dose of 0.39 g·kg-1, 3.2 g·kg-1, 54.4 g·kg-1, 13.6 g·kg-1 were given to rats in the WFC, RZ, QZ-H and QZ-L groups once a day for 28 consecutive days, respectively. The same volume of normal saline was given to the NC group and the model group. Histomorphological changes of gastric mucous membrane in rats of each group were observed. Expressions of NF-κB/p65 and CyclinE protein were detected. The results showed that compared with the NC group, the degrees of infiltration and dysplasia and expressions of NF-κB/p65 and CyclinE significantly increased in the model group with statistical significance (P < 0.01). Compared with the model group, the expression of NF-κB/p65 and CyclinE in the WFC group reduced with statistical significance (P < 0.05). The expression of NF-κB/p65 and CyclinE in the RZ and QZ-L group obviously reduced with statistical significance (P < 0.01). The overall effective rates of WFC group, RZ group, QZ-H group and QZ-L group were 50%, 63.3%, 43.3% and 73.3%, respectively. It was concluded that QZEZECT can treat CAG precancerous lesion, which may take effect by improving and inhibiting pathologic changes of the transcription factor of inflammation.
5.Efficacy of Qingpeng ointment for the treatment of asteatotic eczema and its effect on skin barrier function: a self-controlled clinical study

Objective To evaluate the efficacy of Qingpeng ointment for the treatment of asteatotic eczema and its effect on skin barrier function.Methods A self-controlled clinical study was performed.Totally,78 patients with asteatotic eczema symmetrically located on both lower extremities were enrolled into this study.The left and right lower extremities of these patients were treated with Qingpeng ointment (Qingpeng group) and hydrocortisone butyrate ointment (hydrocortisone butyrate group) respectively,twice a day for 4 consecutive weeks.At the end of treatment,therapeutic effect and skin barrier function were compared between the 2 groups.Results The response rate was significantly higher in the hydrocortisone butyrate group than in the Qingpeng group after 1-and 2-week treatment (week 1:58.97% vs.39.74%,x2 =5.77,P < 0.05;week 2:76.92% vs.60.26%,x2 =5.03,P < 0.05),but insignificantly different between the 2groups after 4-week treatment (80.77% vs.87.18%,P > 0.05).Compared with the hydrocortisone butyrate group,theQingpeng group showed significantly increased water content of the stratum corneum after 4-week treatment (P < 0.05),and decreased transepidermal water loss after 2-and 4-week treatment (both P < 0.05).Conclusion Qingpeng ointment is safe and effective for the treatment of asteatotic eczema with gradually increasing and stable effects,and also has a favoring effect on the restoration of skin barrier function.
6.Treatment methods and effect of glucose metabolit disorders after liver transplantation

Objective To evaluate the efficacy and safety of glargine combined with repaglinide in the treatment of post-transplantation diabetes mellitus(PTDM).Methods PTDM patients who were treated in our hospital from Jan 1,2010 to Dec 31,2010 were enrolled in this study.They were administrated with glargine combined with repaglinide for 6 months,and their glucose level,hepatic and renal function indicators,and tacrolimus concentration were examined at baseline and 1,3,and 6 months after treatment.Results Totally 44 patients were included and given dietary control.Three cases were not given any hypoglycemic drugs,7 were administrated with glargine only,30 cases received glargine combined with repaglinide therapy,and 4 cases required intensive insulin therapy.All the patients achieved satisfactory glycemic control.The hepatic function,renal function,and serum tacrolimus concentration showed no significant change before and after repaglinide therapy(P > 0.05).Five hypoglycemic events were recorded during the treatment,in which the lowest blood glucose level was 3.7 mmol/L.No severe hypoglycemia happened.Conclusion On the basis of dietary control,glargine combined with repaglinide provides a safe and effective therapy for PTDM.
7.Development of UPLC-Q-TOF-MS/MS combined with reference herb approach to rapidly screen commercial sulfur-fumigated ginseng.

An ultra-performance liquid chromatography-quadrupole/time of flight mass spectrometry (UPLC-Q-TOF-MS/MS) combined with reference herb method was developed to rapidly screen commercial sulfur-fumigated ginseng. Sufur-fumigated ginseng reference herb was prepared using genuine ginseng by conventional procedure. Then the reference sulfur-fumigated ginseng sample was analyzed by UPLC-Q-TOF-MS/MS to identify characteristic marker components. 25-hydroxyl-Re sulfate with higher abundance was se- lected as marker compound from 8 characteristic components identified in sulfur-fumigated ginseng reference herb. The fragmentation of 25-hydroxyl-Re sulfate was extensively investigated, fragment ion m/z 879.44 with higher intensity was chosen as the characteristic ion of sulfur-fumigated ginseng. The response of ion m/z 879. 44 was improved by optimizing the MS conditions so that this ion could be used as the characteristic marker ion for screening purpose in ion extracting screening mode. The established approach was successfully applied to inspect 21 commercial ginseng samples collected from different cities in China It was found that the chemical profiles of 9 samples were similar to that of sulfur-fumigated ginseng reference herb, and the characteristic ion m/z 879. 44 of 25-hydroxyl-Re sulfate was also detected in these samples, suggesting that there were nearly 43% ginseng samples analyzed being sulfur-fumigated. This findng agreed well with the results of sulfur dioxide residues of these 21 commercial ginseng samples determined with the method documented in Chinese Pharmacopeia Compared with the method documented in Chinese Pharmacopeia, the proposed approach is more rapid and specific for screening sulfur-fumigated ginseng. SFDA of China should strengthen the enforcement to prohibit ginseng being sulfur-fumigated, so that ginseng and it preparations could be effectively and safely benefit to the health of human beings.

8.Biocompatibility of phosphorylcholine modified alginate-chitosan microcapsules

ObjectiveTo explore whether the biocompatibility of phosphorylcholine (PC) modified alginate-chitosan microcapsules could be improved. MethodsPC modified alginate-chitosan microcapsules were obtained by high-voltage electrostatic system.Bradford method was adopted to determine the adsorption amounts of bovine serum albumin by chitosan alone and PC modified chitosan.Alginate-chitosan-PC microcapsules (experimental group) and alginate-chitosan microcapsules ( control group) were respectively implanted into the peritoneal cavity of mice and retrieved 4 weeks after transplantation.Fibrosis of the capsules was evaluated by HE staining.Glucose stimulated insulin secretion (GSIS) assay was used to assess the insulin secretion response of encapsulated and nonencapsulated rat islets. Results The adsorption amount of protein was 189.4 μg/mg and 90.5 μg/mg respectively by chitosan alone and PC modified chitosan.The difference had statistical significance ( t =5.549, P < 0.05 ).In contrast to the control group, the cellular reaction on the surface of the modified microcapsules was weaker, with no obvious fibrosis found.The insulin secreted by encapsulated islets and nonencapsulated islets was( 3.298 ± 1.680 ) μIU/ml VS (4.299 ± 1.159 ) μIU/ml ( t =1.096, P > 0.05 ) in response to low-glucose stimulus and( 11.783 ± 4.175 ) μIU/ml VS ( 12.875 ± 2.268 ) μIU/ml ( t =0.514, P > 0.05 ) in response to high-glucose stimulus.Conclusions PC can improve the biocompatibility of alginate-chitosan microcapsules, with no effect on the biological function of encapsulated islets.It may be more appropriate to use modified microcapsules encapsulating islets for transplantation.
10.Perioperative management of procedure for prolapse and hemorrhoids in elderly patients with serious hemorrhoids

Objective To evaluate the perioperative management and therapeutic efficacy of procedure for prolapse and hemorrhoids (PPH) in elderly patients with serious hemorrhoids. Methods The clinical data of 52 elderly patients (aged 60 years or over) with serious hemorrhoids treated by PPH from March 2005 to November 2007 were analyzed restrospectively. Results All the operations were successfully completed. The average operation time was (15.3±3.2) min, the average blood loss was (15±1.7) ml and the average hospitalization time was (4.5±2.6) days.Patients were followed for 1-20 months with (14.2±2.4) months for average. The follow-up rate was 92.3%(48/52). Complications included urinary retention in 18 cases(37.5%), anastomotic bleedings in 2 cases (4.2%), postoperative pain of anus in 4 cases (8.4%), recurrence in 1 case (2.1%),hypoglycemia in 1 case(2.1%), and 1 case (2.1%)was found stricture in rectum at 2 months after the procedure and no case developed rectovaginal fistula. Conclusions Old age is not the contraindication for PPH treatment of the serious hemorrhoids. Proper perioperative management may decrease the postoperative complications and improve the life quality of the patients.
