1.Current status of mechanical thrombectomy for acute ischemic stroke

Before irreversible neuronal damage occurs, quickly and securely recanalize vessels and restore blood flow to save the ischemic penumbra tissue are the main therapeutic target for acute ischemic stroke (AIS). Currently, the method for recanalization in early AIS is intravenous thrombolysis, how ever, the effect of intravenous thrombolysis is often not satisfactory because of the reasons, such as its short treatment time w indow , low vascular recanalization rate, and high incidence of bleeding complications. In recent years, w ith the research and development of mechanical thrombectomy device and the development of interventional technology, the therapeutic time w indow of AIS has been significantly prolonged, the vascular recanalization rate has increased significantly, the clinical outcome has improved significantly, and the treatment of mechanical thrombectomy has show n good application prospects.
2.Correlation between CD40-1C/T gene polymorphism and serum sCD40L

Objective To investigate the correlation between CD40-1C/T gene polymorphism and serum soluble CD40L (sCD40L) expression in cerebral infarction.Methods According to the inclusion and exclusion criteria,select the acute large artery atherosclerosis in patients with cerebral infarction as the case group,select the same period without a history of stroke examination subjects as the control group.Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) technology was used to detect CD40 gene polymorphism and sequencing,double-antibody sandwich enzyme-linked immunosorbent assay (ELISA) was used to detect the expression levels of serum sCD40L.Results A total of 209 patients with large artery atherosclerotic cerebral infarction (case group) and 87 subjects without history of stroke (control group) were included.The CC genotype (31.6%) and C allele frequency (53.8%) of the case group were significantly higher than that of the control group (17.2%,39.1%) (x2 =6.94,10.69,P <0.01).Case group serum sCD40L expression level was significantly higher than those in the control group [(3.97 ± 1.20) vs (2.69 ±0.88)] (t =10.19,P <0.001).Case group serum sCD40L expression level was different among CC,CT,and TT genotypes (F =19.22,P <0.001),serum sCD40L level (4.55± 1.16) of CC genotype in patients was significantly higher than that of CT (3.93 ± 1.17) and TT genotypes (3.27 ± 0.90),serum sCD40L level (3.93 ± 1.17) of CT genotype in patients was significantly higher than the TT genotype (3.27 ±0.90).The serum sCD40L expression level of the control group in the CC,CT,TT has no statistically significant differences among various genotypes [(2.91 ±0.79),(2.67 ±0.89),(2.61 ±0.91),F =0.619,P =0.541).Conclusions CD40-1C/T gene polymorphism is associated with serum sCD40L expression level in cerebral infarction,serum sCD40L level of the CC genotype was significantly higher.
3.Molecular Mechanism of Asthma Induced by Indoor Air Pollutants

During the last decade a lot of new learning about asthma was arisen along with the development of modern immunology and molecular biology,and asthma had been one hotspot in the studies of molecular biological and administer medical field.One of the new viewpoints was that airway neurogenic inflammation took important role in asthma development.For years scholars had suspected that airborne pollutants had an synergistic effect on the attack of asthma and could cause asthma attack,but they did not know the molecular mechanism.In 1991 Denish scholar Nielsen GD advanced a new hypothesis that airway irritation was mediated by capsaicin receptor.The pathological process included a molecular chain of message transfer as air pollutants_capsaicin receptor_Ca2+_substance P_neurokinin receptor.Airway neurogenic inflammation was the result of activation of the receptors.In 1997 American scholar Caterina MJ and his colleagues successfully obtained the cloned capsaicin receptor in expressing cells,this will make Nielsen GD's hypothesis living.
4.Preparation and Quality Control of PVP-I Film

OBJECTIVE: To prepare PVP-I film agent of high water solubility and to establish a method for its quality control. METHODS: The film was prepared by mixing PVP-I,glycerol, distilled water and PVA05-88. The content of povidone iodine was determined with electric titration. The stability of the preparation stored in brown glass and colorless glass was investigated.RESULTS:The prepared film was integrated, clear and even in color and size. The average recovery was 99.03%(RSD=0.75%).The film stored in brown glass showed no evident changes in appearance and content within 60d, while that in colorless glass was found to have a decrease in content and yellowish appearance. CONCLUSION: The preparation technology is simple and method of quality control is accurate and reliable.
5.Complexation of Clonazepam with Hydroxypropyl-?-cyclodextrin

OBJECTIVE:To investigate the complexation of clonazepam with hydroxypropyl-?-cyclodextrin(HP-?-CD)in aqueous solutions.METHODS:The complexation of clonazepam with(HP-?-CD),the inclusion molar ratio of host and guest molecules and thermodynamic parameters were detected using phase solubility method,equimolar series method,UV absorption spectroscopy etc.RESULTS:The solubility of clonazepam increased linearly with the increase of(HP-?-CD)level in aqueous solutions at different temperatures(25℃,35℃and45℃)and in buffers at different pH values(4.0,6.0and7.0),the phase solubility diagram was classified as type AL.CONCLUSIONS:Under the above research condition,the incl_ usion complexes of clonazepam with(HP-?-CD)can be formed spontaneously at a molar ratio of1∶1.
6.The effect of tramadol on perioperative insulin resistance in patients undergoing pulmonary lobectomy

ObjectiveTo investigate the effect of tramadol on perioperative insulin resistance in patients un dergoing pulmonary lobectomy.MethodsThirty patients undergoing pulmonary lobectomy,were randomly divided in to two groups :group T(tramadol group)and group C (control group), five minutes before skin incison, group T re ceived tramadol 1.5 mg/kg intravenously (IV), followed immediately by a continuous infusin of 0.5mg/kg per hour until the end of surgery, and the control group received normal saline instead of tramadol.Blood samples were takenfrom vein for determination of plasma blood sugar,insulin, IL-6 and TNF-α concentrations before induction of anesthe sia(T0) ,at the end of the operation(T1) ,four hours after operation(T2)and seven oclock on the next morning(T3),and the value of ISI was calculated.ResultsThe concentrations of blood sugar, insulin,TNF-α and IL-6 at T1 andT2 ,T3 increased significantly compared to baseline(T0) in both groups(P < 0.05).The concentrations of blood sugar,insulin,IL-6 and TNF-o at T1 ,T2 and T3 were significantly lower in group T than that in group C (P <0.05).Thevalue of ISI was significantly higher in group T than that in group C (P < 0.05).ConclusionTramadol could allevi ate perioperative insulin resistance in patients undergoing pulmonary lobectomy by inhibiting plasma blood sugar,insu lin,IL-6 and TNF-α.
7.The study of biocompatibility of polyurethane sponges made in China

Objective To evaluate the biocompatibility of four kinds of polyurethane sponges made in China. Methods According to ASTM Standards for Medical Devices of America, polyurethane sponges are used to perform such biological tests as cytotoxicity test, acute toxicity test, pyrogenic reactions test, stimulation test of conjunctiva and cornea, sensitization test. The data are analyzed and evaluated according to the criterion. Results Reaction scales of these polyurethane sponges in cytotoxicity are 0 or 1 level. No toxicity effects and pyrogenic reactions are observed in vivo test. No conjunctiva and cornea irritation reactions and no sensitization reactions are found. Conclusions The four kinds of polyurethane sponges have high biocompatibility and can become ideal dressings of Vacuum-Assisted Closure.

With the frequent occurrence of natural disasters, incidence rate of PTSD and sleep-disorder is rising. So, how to treat them effectively has become a hot spot. This article analyzed the literature about improvement of PTSD and sleep-disorder based on the perspective of soothing and regulating liver, and primarily discussed the effectiveness and feasibility of treating PTSD from liver, with a purpose to lay work foundation for improving common diagnosis and treatment system of PTSD and sleep-disorder.
9.A randomized study of the effect of polyethylene glycol electrolytes powder(Ⅱ)on bowel preparation in elderly patients

Objective To study the efficacy and safety of polyethylene glycol electrolytes powder (Ⅱ)on bowel preparation in elderly people.Methods A total of 120 elderly patients were divided into two groups,polyethylene glycol electrolytes powder(Ⅱ)group (the experimental group,n =60)and oral colonic salt group (the control group,n =60).The bowel preparation scores,gastrointestinal compliance,patient compliance,and safety of patients were evaluated.Results Bowel preparation scores of the experimental group (7.42 ±1.05)was much better than that of the control group(5.87 ±1.05)(t =8.10,P =0.00). The efficiency rate of bowel preparation of the experimental group was significantly higher than that of the control group(95% VS 65%,χ2 =16.88,P =0.00).Compliance of bowel preparation of the experimental group was much better than that of the control group (χ2 =7.67,P =0.01).There was no significant differ-ence in gastrointestinal compliance between the two groups(χ2 =3.08,P =0.08).No serious adverse events occurred.Conclusion The effect of bowel preparation of elderly patients with polyethylene glycol electro-lytes powder(Ⅱ)is significantly advantageous in drug intestinal tolerance,taste,and safety,which can re-place the traditional method for bowel preparation.
10.Sequential diffuse large B-cell lymphoma after peripheral T-cell lymphoma not otherwise specified: a case report and review of literature

Objective To investigate the diagnosis and treatment of sequential diffuse large B-cell lymphoma (DLBCL) after peripheral T-cell lymphoma (PTCL).Methods A case with sequential DLBCL after PTCL was reported,and the characteristics and responses of this case were analyzed.The previous literature was reviewed in order to explain the mechanism and prognosis of such type of disease.Results This patient was diagnosed as PTCL not otherwise specified (PTCL-NOS) definitely,but after a period of treatment,DLBCL was developed as a second tumor.The characteristics and onset interval were just similar to those described in the literature,in which the mechanisms were mentioned as common effects of tumor cell,microenviroment and therapies.This patient got effects through the initial treatment,but considering the poor outcome by former researchers,the prognosis needed to be closely followed up.Conclusion Sequential development of EBV-unrelated DLBCL after PTCL-NOS is very rare,and the mechanism,therapy and prognosis need further investigation.
