1.Application of inferior temporal muscle bone flap craniotomy in hypertensive cerebral hemorrhage patients with early hernia

Objective To explore the therapeutic effects of two kinds of operation mode of inferior temporal muscle bone flap craniotomy and traditional big bone flap craniotomy in treatment of hypertensive cerebral hemorrhage patients with early cerebral hernia?Methods Sixty?four cases patients of hypertensive cerebral hemorrhage with early cerebral hernia,treated in Huxi Hospital Affiliated to Jining Medical College from January 2011 to January 2015 were randomly divided into observation group ( 32 patients underwent temporal muscle bone flap craniotomy) and control group ( 32 patients underwent traditional big bone flap craniotomy)?The incidence rate of complications,mortality within 3 months,the excellent rate of GOS score on 6 months after the onset were compared?Results The mortality rate of 3 months after operation in the observation group was 9?4%( 3/32) ,in the control group was 12?5%( 4/32) ,the difference between the two groups was not significant (χ2=0?00(correction),P>0?05))?There were 7 cases had severe complications in the observation group,the incidence rate was 21?9%( 7/32 );there were 16 patients had serious complications in control group, the incidence rate was 50%( 16/32) ,the difference was statistically significant between the two groups(χ2=5?50,P<0?05)?There were 7 cases recovered well,8 cases were residual,the excellent rate of GOS score was 46?9%( 15/32) in the observation group;there were 4 cases recovered well,3 cases were residual,the excellent rate of GOS score 21?9%( 7/32) in the control group,the difference was statistically significant(χ2=4?27,P<0?05)?Conclusion For patients with hypertensive cerebral hemorrhage in basal ganglia region in the early stage of cerebral hernia,temporal muscle bone flap craniotomy hematoma removal can also effectively relieve the hernia, compared with traditional big bone flap craniotomy, can significantly reduce the incidence of complications, improve the prognosis of the patients.
2.The efficacy and safety of transdermal fentanyl in patients with chronic non-cancer pain

Objective To evaluate the efficacy and safety of transdermal fentanyl path ( Durogesic, Xian Janssen) in patients with moderate to severe chronic non-cancer pain. Methods This open prospective study was approved by the Ethic committee of the hospital. After obtaining written informed consent we studied 46 ASA I - H patients (24 male, 22 female), aged 36-82 yr, suffering from moderate to severe chronic non-cancer pain. Durogesic 25 fxg/h was used. The patch and the site of application were changed every 3 days. The dose was adjusted to the level of pain reduction and side effects. The treatment was maintained for 2-4 weeks. The pain intensity measured using VAS, quality of life including appetite, mental state and sleep etc and side effects were recorded.Results Thirty-eight of the 46 patients completed the trial. 5.A biodynamic experiment of traditional rolling massage and its experimental device development

BACKGROUND:Methods after mechanical stimulation can be combined with various kinds of cells to produce many conversion types so as to start the various physiological functions of the body. OBJECTIVE:To analysis the reason why rol ing massage can improve the intracellular concentration of Ca2+METHODS:A biodynamic experimental device was set up, composed of pressure generating device, pressure forming device, pressure display device and related data recording device. We detected the effects of rol ing massage on Ca . 2+concentration in normal skeletal muscle cells and damaged cells through the different functions of the experimental device and system software. These functions consisted of pressure display, data transmission and recording, signal acquisition and processing, pressure signal acquisition, control and processing of signal acquisition, wireless signal transmission and receiving system, display system, storage system, system implementation function. RESULTS AND CONCLUSION:The experiment results showed that rol ing massage could reduce the Ca 2+concentration in the normal skeletal muscle cells and damaged cells. These findings suggest that rol ing massage can improve the overload of intracellular Ca2+in damaged cells, and it is superior to the sinusoidal mechanical stimulus.
6.Compatibility of bone marrow mesenchymal stem cells with nano-hydroxyapatite/collagen

BACKGROUND:The microcosmic and submicroscopic organizations of tissue engineering scaffold matedals’superficial structure have all important effect on the eell adhesion and growth.By means of nano.Technique and three-dimensional porous technique,the resultant nano-hydroxyapatite/collagen(n-HAC)call imitate the component and microstructure of natural bone.OBJECTIVE:To observe the biocompatibility of human bone m arrow mesenchymal stem cells(MSCs)cultured in vitro with nHAC.DESIGN,TIME AND SETTING :Single samples observation was performed in the Experimental Center of School ofMedical Technology,Jiangsu University from September 2005 to December 2006. MATERIALD:nHAc was provided by the Material Science and Engineering Department of Tsinghua University.Humanbone marrow mesenchymal stem cells were derived from healthy adult volunteers.All the subiects signed the informedconsents. METHODS:Whole bone marrow culture and successive adherence method was used to culture MSCs in vitro,and the cells were then induced to differentiate into the phenotype of osteoblasts by the revulsants(methylprednisolone,vitamin C,β-glycerophosphate and basic fibroblast growth factor).MSCs at passage 3 were co-cultured with nHACfor 14 days.MAIN OUTCOME MEASURES:The cytological characteristics of the osteoblast were identified throue,alkalinephosphatase immunohistochemistry method and Von Kossa stain.The growth condition with or without nHAC wasevaluated through invert microscope and scanning electron microscope,respectively.RESULTS:The cultured MSCs proliferated into uniform fibroblast-like cells rapidly.MSCs reached confluence and started to form multilayers averaging from 10 to 12 days,passaged stably as well.Then the MSCs passaged from 7 to 9 days.Cytochemistry evaluation showed that MSCs in induced culture were positive for alkaline phosphatase and Von Kossa stain,and deposited calcified matrix.It showed a typical ostcoblast feature in morphology and biology.In coculture model ofMSCs with nHAC,cells would attach to the inner surface of nHAC.At 8 days,the osteoblasts proliferated in the nHAC and the secretion of the matrix was observed.Lots ofcells adheredon the surfaceand pores of nHAC at 14 days.There wereextensive prominent connections among cells. CONCLUSION:THE nHAC is suitable for MSCs to adhere,grow and proliferate,with a good compatibility.

8.Construction and identification of non-immunized human phage display library

Objective:To develop non-immunized human phage display library.Methods:The total RNA of lymphocyte cells from peripheral blood of healthy voluntee was isolated and cDNA was synthesized,and the genes of heavy variable chain (VH) and light variable chain (V? and V?) were amplified by direct PCR and half-nested PCR.By overlapping extension PCR,the genes of VH and VL (V? and V?) were linked.The linked genes of single chain Fv fragment (scFv) were ligated with the vector pCANTAB-5E and then cloned into TG1 for the scFv library construction.Results:By direct PCR and half-nested PCR,42 VH fragments,16 V? and 18 V? fragments were obtained.The size of linked scFv library genes was 750 bp and the volume of constructed scFv library was 1.35?108.The results of BstN Ⅰ analysis of scFv genes from the phage library showed that fingerprint map of the selected scFvs was different.Conclusion:The developed phage library is diversity and can be used for selecting humanized scFv.
10.The expression of molecular chaperone HSP90 and IL-6 in patients with systemic lupus erythematosus.

To explore the expression and clinical significance of molecular chaperone heat shock protein 90 (HSP90) in peripheral blood mononuclear cells (PBMC) and plasma level of interleukin-6 (IL-6) in patients with systemic lupus erythematosus (SLE), HSP90 was detected in PBMC by Western blot assay and the plasma level of IL-6 was measured by ELISA in 38 SLE patients and 20 normal controls. The correlation analysis was performed between the SLE disease activity index (SLE-DAI) and the expression of HSP90 and IL-6. The results showed that there was increased expression of HSP90 in the SLE patients. The active SLE group exhibited higher HSP90 levels (0.82+/-0.10) than the inactive SLE group (0.54+/-0.09) (P<0.01). The expression of HSP90 in normal control group (0.37+/-0.11) showed significant statistical difference as compared to both the inactive and active SLE groups (P<0.01, P<0.01, respectively). The plasma level of IL-6 exhibited a significant increase in both the inactive and active SLE groups (28.99+/-1.74 pg/mL, 44.58+/-9.15 pg/mL, respectively) compared with normal control group (P<0.01, P<0.01, respectively). The expression of HSP90 and IL-6 in SLE patients showed significant positive correlation with SLEDAI scoring (r=0.80, P<0.01: r= 0.74, P<0.01, respectively). In addition, there was a positive correlation between the level of IL-6 and HSP90 in SLE patients (r=0.86, P<0.01). The increased expression of molecular chaperone HSP90 and IL-6 may play an important role in the pathogenesis of SLE by regulating autoimmunity.
