Objective To provide the convenient and efficient screening program for the early detection and diagnosis of peptic ulcer by evaluatingthe clinical value of serum anti HP -IgG detection in the diagnosis of peptic ul-cer in health examination .Methods 116 participants underwent serum anti HP -IgG detection and gastroscopy ex-amination, taking the pathological biopsy results of gastroscopy examination as the gold standard .Their consistency and the diagnostic value of serum anti HP -IgG detection were compared by the Kappa consistency test .Question-naires were filled by participants after the examinations to evaluate the comfort level and conveniences during the ex -aminations as well as their overall satisfaction , and to evaluate the value of HP -IgG detection .Results 87 partici-pants were positive in serum anti HP -IgG detection , accounting for 75%, while 29 participants were negative , ac-counting for 25%.According to the results of gastroscopy examination , 29 participants were diagnosed with duodenal ulcer (25%), and 25 participants with gastric ulcer (21.55%), and 34 participants with hybrid ulcer (29.31%), and 28 participants without peptic ulcer (24.14%).Based on the results of serum anti HP -IgG detection and gas-troscopy examination, Kappa=0.744 (p=0.073) was found by the Kappa consistency test , and the consistency was good.After the calculation , the sensitivity ( the true positive rate ) was 94.25%, and the specificity ( the true negative rate) was 79.31%.The anxiety, comfort level, conveniences and patients'satisfaction of anti HP-IgG group were bet-ter than those of control group (p<0.05).Conclusion Serum anti HP-IgG detection is worthy of the clinical promo-tion as a screening program for the early detection and early diagnosis of peptic ulcer , because it boasts of high sensitivi-ty, a good consistency in gastroscopy examination ,less anxiety , convenience and satisfaction for patients .

Objective To study the clinical significance of molecular markers and routine coagulation tests in patients with cerebral infarction and to set up a serial programs of laboratory diagnosis,monitoring and treatment of cerebral infarction.Methods Prothrombin fragment 1+2(F1+2),thrombin antithrombin III complex(TAT), D-dimer(D-D),Von willebrand Factor(vWF),antithrombin(AT), protein C(PC),prothrombin time(PT),activated partial thromboplastin time(APTT) and thrombin time (TT) were determined in 90 patients with cerebral infarction and 60 normal control subjects.Results The levels of F1+2,TAT,D-D,vWF were significantly higher in patients with cerebral infarction than that in control subjects. But the levels of PC,AT,PT,APTT,TT in patients with cerebral infarction were no different from that in control subjects.Conclusion There were hypercoagulable states in patients with cerebral infarction.The activity of prothrombin is higher, thrombin is generated more, the activity of fibrinolysis is higher too, but anticoagulation system is not sufficiently activated .Meanwhile, perhaps endothelial lesion would be the main role of coagulation system activating and pathogenesis.Molecular markers such as F1+2,TAT,D-D,VWF can be as diagnositic signs but routine coagulable tests can not display the hypercoagulable states in patients with cerebral infarction.

This article has shown the experiences to achieve more effective teaching results by integration of multiple teaching patterns,including case-based,problem-based learning,in the teaching of water-electrolyte metabolism and acid-base balance disorders.

OBJECTIVE:To optimize the toxicity attenuation processing technology of Amorphophallus sinensis Belval.. METH-ODS:With the overall score of taste stimulation and rabbit eye irritation of A. sinensis Belval. as the index,and based on the sin-gle factor method,orthogonal test was designed to investigate the the influences of the amount of saturated calcium hydroxide solu-tion,heating time and heating temperature on the processing effect,and verification tests as well as irratation comparison before and after processing were conducted. RESULTS:The amount of saturated calcium hydroxide solution and heating temperature had significant influence on the processing effect. The optimal processing technology was to add saturated calcium hydroxide solution 30 times as much as the amount of medicinal materials at 100 ℃ and heat it for 30 min. The verification tests showed overall scores of 8.05,8.44 and 8.37(RSD=2.5%,n=3). The average overall scores before and after processing were 0.12 and 8.54(n=10)re-spectively. CONCLUSIONS:The medicinal materials processed by the optimal technology have lower stimulation and irritation. The optimal technology is stable and reliable.

Objective To detect the serum growth hormone(GH),insulin-like growth factor-I(IGF-Ⅰ),insulin-like growth factor binging protein-3(IGFBP-3) levels in children with hypothyroidism and explore the change regulation of GH-IGF axis and thyroxine.Methods Fifty-six hypothyroidism children were divided into congonital hypothyroidism(CH)(14 cases) and Hashimoto's thyroiditis(32 cases).Serum GH,IGF-Ⅰ,IGFBP-3 levels of 56 hypothyroidism children before and after luothyroxine sodium L-T4 treatment and 50 normal children were detected with radioimmunoassay or enzyme-linked immunosorbent assay methods.Results Serum IGF-I,IGFBP-3 levels in 9 CH newborn group were lower,and they were normal after thyroxine substitute therapy.But those of 5 children with CH and 32 cases with Hashimoto's thyroiditis were not significantly changed.Conclusions The GH-IGF axis is inordinate in the CH children,which is an important reason of stature short.Early thyroxine substitute therapy is very helpful to maintain normal growth and development of children with CH.

Aim To explore the effects of hydrocortisone on intracellular calcium in microglial cells.Methods The intracellular calcium was measured by instantaneous scanning with confocal laser microscope(CLM) in BV-2 cells, and fluo3-AM was used to dye the intracellular calcium.Results Both hydrocortisone and nicotine could obviously increase intracellular calcium in BV-2 cells(P<0.05).It was indicated by instantaneous scanning with CLM that hydrocortisone induced the rising of intracellular calcium immediately, and reached the peak about at the fifteenth second, and sustained for 10 seconds, then declined to baseline at 200th second.The effect of hydrocortisone on intracellular calcium exhibited a highly consistency with nicotine.Antagonist of glucocorticoid receptors RU486 could not abolish the rising of intracellular calcium induced by hydrocortisone(P>0.05);but the blocker of α7 nicotinic acetylcholine receptor(α7nAChR) methyllycaconitine could suppress the rising of intracellular calcium induced by hydrocortisone(P<0.05).Conclusion Hydrocortisone enhances intracellular calcium via α7nAChR in microglial cells, which not only demonstrates the non-genomic effect of glucocorticoid, but also suggests that glucocorticoid could serve as endogenous ligand of α7nAChR.

This study was designed following eyeball extirpation to evaluate the therapeutic effects of hydroxyapatite and to explore the causes and preventing from exposure during orbital implantation. A total of 23 cases with autologou sclera and muscular suture selected from Department of Ophthalmology, Affiliated Hospital of Luzhou Medical College between January 2005 and December 2007 were retrospectively summarized and analyzed. Seventeen cases were implanted initially and the other 6 cases received secondary implantation. For the implantation with scleral-wrap, a 360? peritomy, limbic sclerotomy and full keratectomy were performed with corneoscleral scissors. Uveal tissue was completely removed. Posterior sclera was radially incised, and the hydroxyapatite orbital implant was implanted. Anterior sclera was sutured completely. Other implantations without using sclera-wrap were performed with the hydroxyapatite orbital implant in muscle cone and four rectal muscles were sutured to the hole of the hydroxyapatite orbital implant. Mattress suture of fascia tissue with double staggered position was performed in all the implantations. Conjunctiva was sutured discontinuously. This implant provided satisfactory functional motility and stability. Two cases developed implant exposure, suggesting that hydroxyapatite was the implant of choice for cosmesis following surgical removal of the eyeball, or enucleation. The complication rate is low and implant exposure, if it occurs, can be easily managed.

Abstract: To report the diagnosis, treatment and outcome of 4 patients with hematological diseases complicated with Aeromonas hydrophila bloodstream infection in the Second Affiliated Hospital of Kunming Medical University, further clarify the importance of blood culture and deepen the clinical understanding of the disease. Four patients with hematological diseases complicated with Aeromonas hydrophila bloodstream infection treated in the Second Affiliated Hospital of Kunming Medical University from 2017 to 2021 were recruited as the study objects. The clinical manifestations, blood culture collection, detection time of Aeromonas hydrophila, laboratory examination, treatment and prognosis of the patients were retrospectively analyzed. In this study, 4 cases were male patients with hematological diseases, who were in myelosuppression after chemotherapy. After fever, blood culture was collected and Aeromonas hydrophila was detected. The positive time of blood culture in 4 cases ranged from 4 to 11 hours. The results of antibiotic sensitivity showed that it was highly sensitive to the second, third and fourth generation cephalosporins, quinolones and carbapenems. Four patients were treated with imipenem cilastatin sodium in the early stage, and one patient recovered after active anti infection and leukocyte raising treatment. One patient did not complete chemotherapy due to a request for discharged, and the follow-up was unknown. Two patients developed rapidly into necrotizing fasciitis and died later. Hematological diseases complicated with Aeromonas hydrophila bloodstream infection are rare, but the mortality rate is high. For patients with repeated fever and considering infection, blood culture should be carried out as soon as possible to confirm the pathogen and drug sensitivity test. During clinical treatment, the treatment should be adjusted in time in combination with the patient's situation. In addition to anti-infection treatment, the patient's immunity should be improved and the development of necrotizing fasciitis should be vigilant. BACKGROUND:Up to date,there is not an acceptable method for isolating,culture and amplifying human adipose-derived mesenchymal stem cells (hADSCs).OBJECTIVE:To explore the most effective way to obtain highly homogenous and undifferentiated hADSCs.DESIGN,TIME AND SETTING:The in vitro cytology experiment was performed at the Key Laboratory of Pathobiology,Ministry of Education,Jilin University from June to December 2008.MATERIALS:Human abdominal adipose tissue resected in the surgery was supplied by the Affiliated Hospital of Jilin University,The informed consent was obtained from patients.METHODS:Human adipose tissue was removed connective tissue and blood vessel,followed by incubation in 0.1% type I collagenase for 60 minutes at 37℃,filtrated then centrifuged.Consequently,the subnatant precipitation was cultured with LG-DMEM containing 10% fetal bovine serum,incubated at 6-well plate with density of 1×10~9/L,and placed in incubator of 5% CO_2 at 37 ℃.The cultured cells were passaged when the cells reached 80%-90% confiuency,and the 3~(rd) passage of cells were induced to osteogenic and adipogenic differentiation.MAIN OUTCOME MEASURES:Morphological characteristics of hADSCs were observed by laser scanning confocal microscope.Immunophenotypes,cell cycle and growth curve of hADSCs were detected by flow cytometry and immunofiuorescent techniques.In addition,the multiple differentiation potential of hADSCs was detected.RESULTS:hADSCs presented fibroblast-like morphological feature with a flocked array.The 3~(rd) passage of hADSCs had unique immunophenotypes and they were positive for CD73,CD44,CD166,CD105 and CD29,but negative for CD31,CD34,CD45 and HLA-DR.Cell cycle result showed that they had the typical growth characteristics of stem calls,namely,83.81% cells stayed at G_0/G_1 stage,only 16.19% cells were stayed at S+G_2/M stage;The latent phase of the primary culture cells was 2 days prior to and after incubation,followed by 3-6 days of logarithmical proliferation,reached a peak at day 6,and entering the growth platform phase with lower growth speed.The alkaline phosphatase was positive expressed at week 2 of osteogenic induction.And the positive expression of oil-O red staining could be seen at day 3 of adipogenic induction.CONCLUSION:Cells contamination can be reduced by removing connective tissue and blood vessel of adipose tissue,and 0.1% type Ⅰ collagenase for 60 minutes can effective separated stroma cell to matrix fiber,furthermore,ensure a sufficient contact between collagenase and tissures,which provide an supportive for harvesting highly homogenous hADSCs.

Objective To analyze the bloodstream infections caused by vancomycin‐resistant Enterococcus (VRE)in West China Hospital and examine the risk factors of such infections for best control of VRE infections .Methods Case‐control method was used to compare the bloodstream infections due to VRE with those caused by vancomycin‐susceptible Enterococcus(VSE)from August 2010 to August 2014 in West China Hospital .The data were compared by T‐test ,Chi‐square test (univariate analysis) and logistic regression analysis (multivariate analysis) using SPSS 19 .0 software .Results A total of 31 VRE strains were isolated from bloodstream infections in the study period ,including 5 (16 .1 % ) strains of vancomycin‐resistant Enterococcus faecalis and 26 (83 .9 % )strains of vancomycin‐resistant Enterococcus faecium .In the past four years ,the prevalence of vancomycin‐resistant E . f aecalis in bloodstream infections was 1 .5% ,1 .6% ,1 .8% ,and 1 .2% ;while the prevalence of vancomycin‐resistant E . f aecium in bloodstream infections was 3 .8% ,4 .4% ,5 .8% ,and 7 .1% .Such VRE bloodstream infection was mainly found in Intensive Care Unit (ICU)(13 ,41 .9% )and Department of Neurosurgery (4 ,12 .9% ) .More than 90% of the isolates were resistant to ampicillin and erythromycin .But less than 20% of the strains were resistant to quinupristin‐dalfopristin and linezolid . Univariate analysis indicated that prolonged hospital stay , ICU admission , venous catheter ,prior carbapenem exposure , prior vancomycin/norvancomycin exposure were associated with VRE bloodstream infections .Logistic regression analysis confirmed that venous catheter was an independent risk factor of VRE bloodstream infections .Conclusions Venous catheter is an independent risk factor for VRE bloodstream infections . Infection control measures should be strengthened to prevent the outbreak of VRE‐related bloodstream infections .
