1.Effect of ultrasonic scaling combined with vinegar for treatment the dental calculus

Objective To investigate the clinical efficacy of ultrasonic scaling combined with the vinegar-for treatment the patients with dental calculus.Methods 120 patients with dental calculus dental were randomly divided into two groups.Control group used ultrasonic scaling treatment,observation group was given ultrasonic scaling combined with the vinegar treatment.Results The clinical cure rate of the observation group was 96.67% (58/60),the incidence rate of complications was 6.67% (4/60).The clinical cure rate of the control group was 68.33% (41/ 60),the incidence rate of complications was 21.67% (13/60).There were statistically significant differences between the two groups about the cure rate and the incidence of complications (x2 =14.37,3.61,P < 0.05).After treatment,periodontal pocket depth of the observation group was significantly lower than that of the control group (P < 0.05).Sulcular fluid volume of the observation group was significantly lower than that of the control group (P < 0.05).The difference of oral sample results of bacterial culture after treatment was statistically significant between the two groups (P < 0.05).Conclusion The clinical effect of ultrasonic scaling combined with the vinegar in treatment patients with dental calculus is certain,which is worthy of further promotion.
2.Minimally invasive laparoscopic-assisted surgery of the thyroid nodule

Objective To study the feasibility, superiority and experience of minimally invasive laparoscopic-assisted surgery of the thyroid using ultrosonic scalpel. Methods In a period from January, 2006-August, 2008, 56 patients of the department of general surgery, the hospital of Heilongjiang Province were diagnosed as thyroid node. The size of the nodes ranged from 5.0 cm×4.2 cm to 3.5 cm×2.0 cm in uhrosonography. An incision of about 1.5-2.0 cm was made 1.0 cm above the suprasternal notch and the operation was performed with ultrosonic scalpel assisted with laparoscope. Partial thyroideetomy of lateral thyroid was performed in 20 cases, subtotal thyroidectomy in 25 cases, subtotal thyroidectomy of unilateral thyroid and partial thyroidectomy of contralateral thyroid respectively in 1 case, partial thyroidectomy of bilateral thyroid in 10 cases. Results All 56 cases have undergone minimally invasive laparoscopic-assisted surgery successfully in 50-146 min and no complications occurred. Fifty five cases were confirmed to be benign lesion pathologically. Conclusion Laparoscopic-assisted surgery of the thyroid is an effective with a good cosmetic result.
3.Effect of laparoscopic lymph node dissection on the radical degrees of advanced gastric carcinoma

Objective To clarify weather or not lymph node dissection can achieve the requests of gastric carcinoma radical degrees under laparoscopic operation for advanced gastric careinoma. Methods Sixty-five cases of advanced gastric carcinoma with under 1/3 stomach lesions and three cases with middle 1/3 stomach lesions. were randomly divided into tow groups: assisted laparoseopic group(n=33)and conventional gastric carcinoma D2 radical surgery group(n=35).The number of lymph node dissection, and the incidence of positive lymph nodes oecnred in damaged capsule. the margin necrosis analyzed. Results The average lymph node dissection of the laparoscopic group were 20.79±5.21 were ohscrved. the positive oases were 9.63±4.64.The average lymph nnde dissection of the control group were 21.20±5.04.the positivecases were 9.63±4.64. The two groups had no significant difference f (P>0.05)1. Positive-margin lymph node in the control gmup was 6 and 14 in the experimenl group ( P>0.05). The experlment gnmp margin appeared necrosis under eletronic microseope and the control group appeared degeneratinn. Conclusions Laparoscopic advanced gastric carcinoma lymph node dissectinn can D2 radical of advanced gastriC carcinoma demands(D>N), have significant difference with conventional surgery. Positive lymph node dissection margin of treatment is better than traditional surgery.

6.Clinical characteristics of neuromyelitis optica

Objective To explore the clinical characteristics of neuromyelitis optica(NMO).Methods The clinical data of 18 patients with NMO were analyzed retrospectively.Results Of the enrolled patients,15 cases(83.3%) were relapsing form of NMO,16 cases(88.9%) were female and the mean onset age was 36 years old.Simultaneous bilateral optic neuritis(ON) or ON occured sequentially and horizontal impairent of spinal cord were the main clinic features in these 18 patients.Of the patients who received spinal cord MRI examinations,4 cases showed the lesions located in cervical cord,6 cases in thoracic cord and 5 cases in both cervical and thoracic cord.13 cases revealed lesions in spinal cord extended across 3 or more vertebral segments.Furthermore,brain lesions were detected by MRI in 6 patients.The mean onset number of per patient was five.Seven patients presented with mild disability,7 patients with median disability,2 patients with severe disability and death of 2 patients.Conclusions Most patients with NMO are female and relapsing form accounts for the majority type.The cardinal manifestations are bilateral ON and longitudinally extensive transverse myelitis(LETM).Some patients show brain lesions that may be detected by MRI examination.Relatively high relapsing frequency of clinical event and severe impairing are common in NMO.

Objective To study molecular biology and clinicopathologic characters of peritoneal metastasis of advanced gastric cancer and their relationship to prognosis. Methods Based on the molecular biology and the clinicopathologic characters and prognosis single-factor analysis of relevant factors and risk assessment was made in 193 cases of gastric cancer.Results The younger the patient was with the earlier the time of occurrence of peritoneal metastasis of gastric cancer. There was a high expression of ICAM-1 and MMP-2,but low expression of TIMP-1 and p53 in gastric cancer patients with peritoneal metastasis.Conclusions ICAM-1,MMP-2,TIMP-1 and p53 can well predict the occurrence of metastasis and the prognosis of patients with gastric cancer,and these factors can be used for preoperative assessment of the development of metastasis of gastric cancer.
9.Clinical efficacy of trimebutine maleate combined with Morita therapy in treatment of patients with irritable bowel syndrome

AIM: To investigate the clinical efficacy of trimebutine maleate combined with Morita therapy in the treatment of patients with irritable bowel syndrome (IBS). METHODS: 108 patients were randomly assigned to two groups. Trimebutine maleate group (n=54) was given trimebutine maleate 200 mg, po, tid, combined with Mortia therapy. The patients were analyzed by self rating anxiety scale (SAS) and self rating depression scale (SDS) before and after the treatment. Control group (n=54) was only given trimebutine maleate 200 mg, po, tid. The course was 4 weeks. RESULTS: The total efficacy rates were 87.04 % and 72.22 % in trimebutine maleate group and in control group, respectively. 10.Treatment of dyslipidemia in the elderly

Dyslipidemia is a well-established risk factor for atherosclerosis. Treating dyslipidemia in elderly patients requires specific knowledge and understanding of common dyslipidemias and the relative safety of various pharmacologic agents in the presence of possible multiple comorbidities. Lifestyle modification remains the first step in the treatment of dyslipidemia; however, it can be difficult to sustain and achieve acceptable compliance in the elderly and it is best used in combination with drug therapy. Statins are widely accepted as the first-line therapy. Several recent studies have demonstrated that statins are safe and effective in the elderly. However, it is important to note that there is very limited data regarding the effects of dyslipidemia treatment on morbidity and mortality in patients over 85 years of age. In summary, the clinicians must recognize that the presence of dyslipidemia in the elderly poses substantial risk of coronary events and stroke. The available evidence has demonstrated that in most elderly patients who are at increased risk for cardiovascular morbidity and mortality, treatment of dyslipidemia with appropriate therapy reduces the risk, and when used carefully with close monitoring for safety, the treatment is generally well tolerated. With increasing life expectancy, it is critical for physicians to recognize the importance of detection and treatment of dyslipidemia in the elderly.
