Objective To investigate the surgical approaches indication, technique and efficacy for treatment of intracranial arachnoid cysts (IAC). Methods The clinical data of 56 patients with IAC who were diagnosed with MRI and/or CT techniques were retrospectively analyzed. According to the surgical approaches adopted, these cases were classified into 4 groups, group A with expectant treatment (12 cases), group B with endoscopic cystocisternal fenestration (15 cases), group C with cystoperitoneal shunting (21 cases), and group D with surgical cyst removal (8 cases). The outcomes of the patients were compared. Results The patients were followed up for 3 months to 46 months (mean 15 months). The rate of symptomatic improvement in group B was 14/15, in group C was 95.24%(20/21) and in group D was 5/8, and there was significant difference ( χ2=8.335, P<0.05). The rate of radiographic improvement in group B was 14/15, in group C was 90.48%(19/21) and in group D was 5/8, and there was significant difference (χ2=8.235, P<0.05). The rate of complication in group B was 1/15, in group C was 14.29%(3/21), and in group D was 5/8, and there was significant difference ( χ2=8.256, P<0.05) Conclusions In terms of therapeutic effect, surgical safety and minimal invasiveness, cystoperitoneal shunting and endoscopic cystocisternal fenestration should be the primary choice .

The use of tourniquets in total knee arthroplasty (TKA) helps to obtain a blood-free operating space and clear anatomical landmarks, reduce the operation time and intraoperative bleeding and facilitate bone cement fixation. Arterial calcification is relatively common in the elderly people. There is still no consensus on whether it is safe to use tourniquets in patients with arterial calcification in total knee arthroplasty. Some studies showed that the use of tourniquets in patients with vascular calcification does not increase postoperative complications. Other studies showed that the use of tourniquets in patients with calcification will increase postoperative vascular complications. This article reviews the use of tourniquets in patients with vascular calcification. It is recommended that patients with vascular calcification should undergo detailed lower extremity vascular and cardiovascular assessments before surgery. The consult to a vascular surgeon is important. For patients with extensive vascular calcification, It's better to avoid the use of tourniquets during TKA to reduce possible complications.

Objective To evaluate the relationship between Helicobac ter pylori (Hp) related gastritis and gastric mucosa content of interleukin - 8 (IL-8) and granuloeyte colony-stimulating factor ( G-CSF). Methods ELISA was used to detect the amount of IL-8 and G-CSF in supernate fluid of gastromucosal tissue culture from 60 patients, and to examine the difference between Hp infected and non-Hp infected cases. Objective To invetigate the application value of antigen‐antibody joint detection in increasing the detection rate of hepatitis C infection in the patients with hemodialysis .Methods The clinical data of 100 cases of hemodialysis were collected .Hep‐atitis C virus(HCV) core antigen ,HCV antibody and HCV‐RNA were detected by adopting ELISA ,Chemiluminescence and RT‐PCR respectively .The positive expression situation of each detection item in the same specimen was performed statistics .The detec‐tion rates of RNA positive specimen were compared among the single antigen detection ,single antibody detection and joint detection schemes ;at the same time the antigen level of antigen positive specimens ant its RNA copy number were performed the correlation analysis .The antibody levels in the positive antibody specimens were compared between the RNA positive group and the RNA neg‐ative group .Results The single antigen or single antibody detection could cause the missed detection of HCV infection in hemodial‐ysis patients .For the specimens of positive RNA ,the joint detection had the highest detection rate (P<0 .05) ,the antigen level in the antigen positive specimens was positively correlated with the RNA copy number in the corresponding specimens (r=0 .85 ,P<0 .05) ,while the antibody level in the antibody positive specimens had no statistical difference between the RNA positive group and the RNA negative group(P>0 .05) .Conclusion The joint detection of HCV antigen antibody can effectively increase the detection rate of hepatitis C infection in hemodialysis patients .At the same time the HCV core antigen detection also can be used as one of monitoring indicators for the HCV re -infection in the patients with antibody positive .

Objective To investigate the bacterial spectrum and drug resistance of gram-negative bacteria infection in intensive care unit(ICU).Methods A retrospective analysis was conducted on flora isolates from Jan 2003 to Dec 2003 to investigate the bacterial spectrum,bacterial resistance and clinical characteristics.Results The leading causative microorganisms isolated were Acinetobacter spp (22.1%),Pseudomonas aeruginosa(20.1%),and E.coli (16.9%). Gram-negative bacteria exhibited the lowest resistance rates to imipenem/cilastati and Amikacin in all antibiotics currently used.However Pseudomonas aeruginosa had the highest resistance rate to imipenem/cilastatin in all the gram-negative bacteria (12.1%).The respiratory tract was the most common compromised site in ICU.Conclusions The causative bacteria in infection in ICU have a higher resistance to antibiotics currently used including cephalosporins of third-generation.It is critical to adopt specific antibiotics based on bacteriology and susceptibility to prevent the development of drug resistance.

OBJECTIVE To investigate the role of brain-derived neurotrophic factor(BDNF) and its receptor TrkB in the development of allergic rhinitis(AR). METHODS SD rats were randomly divided into allergic rhinitis group(AR group, n=20) and control group (Con group, n=10) . A model of allergic rhinitis in health SD rat was established by using ovalbumin intraperitoneal immunization and nasal antigen challenge. The nasal mucosa obtained from 18 out of 20 AR models as well as 10 normal controls were studied with immunohistochemistry (Envision System) method to observe the expression and distribution of BDNF and TrkB. RESULTS Immunohistochemical staining showed that the distribution of BDNF and TrkB in nasal mucosa from normal rats was in accordance with that in the nasal mucosa from rats with allergic rhinitis on the whole. The study showed that immunoreactivity to BDNF (or to TrkB) was present both on the surface and in the cytoplasm of epithelial cells, glandular cells and their duct cells. The statistical analysis of imumunohistochemical staining showed that the IOD of BDNF in rat nasal mucosa with allergic rhinitis (77344.17?8567.02) was obviously higher than that in the normal rat nasal mucosa (54124.82? 18515). Objective To observe the curative effectiveness of kidney transplantation on uremic patients complicated with Thalassemia.Methods Forty-six cases of uremic patients complicated with Thalassemia(26 cases of alpha-thalassemia,20 cases of beta-thalassemia)subject to kidney transplan- tation were retrospectively studied as the anemia group.Incidence of delayed graft function restoration, rejection(hyperacute rejection,accelerated rejection and acute rejection)and anemia correction were observed.Time needed for renal function restoration was recorded and creatinine value(Cr)was mea- sured in those patients whose kidney function restored after kidney transplantation.Control group in- cluded 131 cases of kidney transplantation performed concurrently that accompanied with different con- tent of anemia other than thalassemia.Results Incidence of DGF was 26.1% and 23.7% in thalasse- mia group and control group,respectively.Six months after transplantation,39 cases in thalassemia group and 109 cases in control group with follow-up were host and kidney alive.In 6 months,rejection ratio was 30.8% in thalassemia group and 32.1% in control group respectively.Cr value was(121?20)?mol/L and(128?33)?mol/L in thalassemia group and control group,respectively.Ratio of anemia correction in thalassmia and control groups was 79.5% and 76.1%,respectively.No indexes above were found to be statically significantly different between these two groups.Conclusion Uremic patients complicated with thalassemia is suitable for kidney transplantation,with clinical effectiveness similar to those without thalassemia.

OBJECTIVEThis study aims to construct a long-term, osteogenesis-targeting HU-308 drug delivery implant by the layer-by-layer electrostatic self-assembly (LBL) technique, and observe the features of its delayed release in vitro.

METHODSA heparin (Hep) and chitosan (Chi) multilayer was coated on pure titanium using the LBL technique, and the titanium implants were dipped into the solution to load HU-308. The amount of loaded drug and release rates were measured using a UV-Vis spectrophotometer. The relationships between the loading efficiency, release time, and multilayer films were evaluated. The morphology of all the multilayers was characterized by scanning electron microscopy (SEM) and atomic force microscopy (AFM).

RESULTSThe results showed that the Hep/Chi self-assembly multilayer was gradually fabricated on the titanium surface. HU-308 was successfully loaded on the titanium implants. The amount of loaded drug increased with the amount of multilayer films, except in the T20 group. In vitro drug release study showed that drug release was more difficult in the system with thicker films, and large amounts of multilayer films decreased the release speed. Both SEM and AFM measurements showed typical LBL deposition of Hep and Chi.

CONCLUSIONThe HU-308 drug delivery implant is successfully fabricated via LBL technology. It could provide sustained release of HU-308 over 30 d. This type of implant may provide a new possi- bility of promoting implant-bone osseointegration for osteoporotic patients.

Objective To study the mechanism of hypoxia inducing factor-1α(HIF-1α)pathway in establishment of hypoxia inducing low endometrial receptivity.Methods RL95-2 cell lines.the ideal model of study ER,were cultured in hypoxia condition induced by CoCl2,and the expression of mRNA and protein of HIF-1α and tumor necrosis factor like weak inducer of apoptosis(TWEAK)were measured by reverse transcription-PCR and western blot. The apoptosis rate was analyzed by flow eytometry.Then the mechanism confirmed by comparing the two factors in endometrium and the ultra-appearance of inflammatory reaction and apoptosis between recurrent spontaneous abortion women and control women.Results (1)On difierent time point(0,12,24,48 hour),mRNA expression of HIF-1α were 0.272±0.010,0.354±0.020,0.591±0.020.0.890±0.020,while the expression of TWEAK were 0.104±0.010,0.510±0.020,1.021±0. 020, 1. 237 +0. 040, respectively, the expression level between 12, 24, 48 and 0 hour all showed significant differences (P＜0. 05 ). (2) Protein expression of HIF-1α were 0. 853 +0. 010, 0. 931 ±0. 030,1. 124±0.010, 1.317±0.0 20 respectively, while was 0.042±0.010, 0.091 ±0.010, 0. 131±0.020,0. 205 ±0. 030 in TWEAK expression, the different level were statistically significant ( P＜0. 05 ). ( 3 )With longer culture under hypoxia, the cell apoptosis rate increased obviously. The apoptosis rate of each time point were ( 3.2±1.4 ) %, ( 16. 2 ±3.2 ) %, ( 26. 3±3.5 ) %, ( 31.8±3.5 )%, the differences between 12, 24, 48 and 0 hour had significance (P ＜0. 05). (4) The positive rate of HIF-1α stained in epithelium cells and stroma cells of test group were 32. 3%, 8.4% and 16. 7%, 7. 3% in control group. The positive rate of TWEAK were 28. 3%, 3.9% in recurrent spontaneous abortion group and 11.6%, 2. 7% in control group ( P ＜0. 05 ). The ultra-appearance of inflammatory cell infiltrated and apoptosis were obvious in test group. Conclusions Cell inflammation reaction and apoptosis induced by HIF-1α pathway may participate the mechanism of hypoxia inducing low endometrial receptivity. HIF-1α might become a novel target for improving poor endometrial receptivity.
