1.Reasons for Clinical Shortage of Certain Drugs

OBJECTIVE: To draw the attention of our country and the concerned party on the shortage of certain drugs. METHODS: Based on the drug shortage condition in our hospital in recent years, we analyzed the reasons for drug shortage, put forward the solution and summarized the clinically indispensable categories. RESULTS & CONCLUSIONS: We should draw experiences from US by solving shortage problem of certain drugs through the co-efforts of government, manufactures and medical institutions to satisfy patients' needs of disease prevention and treatment.
2.Correlation between polymorphism in the promoter of DNA methyltransferase-3B gene and susceptibility of colorectal cancer

Objective To observe and analyze the correlation between the DNA methyltransferase-3B (DNMT3B) gene single nucleotide polymorphisms (SNP) rs 1569686 and the susceptibility to colorectal cancer (CRC) in Tianjin Han population.Methods A case-control study was performed.89 CRC patients and 94 normal controls were examined,and Sequenom MassArray system was applied to detect the genotype of rs1569686 in DNMT3B gene polymorphisms.The association results were analyzed of the SNP and CRC.Results The frequencies of genotypes TT,GT and GG at SNP rs1569686 were 74.2%,24.7% and 1.1% in CRC patients respectively,which there were no significant differences from that in controls(73.4%,23.4% and 3.2%; P>0.05).Conclusion It suggests that the SNP rs1569686 of DNMT3B gene may not be related to susceptibility of CRC in Tianjin Han population.
3.Protective effect of omapatrilat on human umbilical vein endothelial cell injury induced by angiotensin Ⅱ in culture

Aim To investigate the protective effects of omapatrilat on human umbilical vein endothelial cells (HUVECs) injury induced by angiotensin Ⅱ (AngⅡ) in culture. Methods Cultured HUVECs were randomly divided into 4 groups:①control; ②AngⅡ;③omapatrilat and ④AngⅡ+omapatrilat. Lactate dehydrogenase (LDH) leakage was evaluated by spectrophotometer and cell cycle by flow cytometry (FCM); Nitric oxide (NO) was measured by colorimetry and endothelin-1 (ET-1) by radioimmunoassay. Results 10 -7 mol?L -1 AngⅡ significantly increased LDH leakage and ET-1 release, and this increase was inhibited by omapatrilat (10 -6 mol?L -1). Omapatrilat promoted HUVECs proliferation and NO release. Conclusion Omapatrilat can protect human umbilical vein endothelial cells injury induced by angiotensin Ⅱ in culture, suggesting that it may play an important role in prevention and treatment of vascular diseases.
4.Study on the risk factors of urolithiasis in chongqing area

Objective To investigate the association of age,precipitating factors and seasons of symptom onset of urinary stones with patients who have or have no family history. Methods The questionaires were used to survey 360 urinary stone cases with family history,1200 cases without family history and 1000 normal controls in Chongqing Area. Results In the 360 patients with family history,the morbidity rates of the first degree relatives and second degree relatives were 10.71%(147/1372) and 1.68%(54/3214), respectively.But in those without family history,the morbidity rates of the first degree relatives and second degree relatives was 0.66%(23/3480) and 0.12%(9/7345),respectively.Among the patients with family history,the mean age of morbidity for the males (26.35 years) was younger than that of the females (36.4 years, t=1.75,P 0.05).There was no significant difference of the precipitating factors and symptom onset seasons between patients with or without family history,while the enviropmental factors had significant effects on the morbidity of urinary stone patients,without regard to family history(? 2=0.23, P

6.To observe the effect of psychological intervention on patients with advanced ovarian cancer chemotherapy

Objective To explore the effect of psychological intervention on patients with advanced ovarian cancer. Methods A total of 80 patients with advanced ovarian cancer patients combined with carboplatin or Western him paclitaxel combined with cisplatin in the treatment of patients, the implementation of grouping according to nursing, patients in the control group for routine care, patients in the observation group in the control group on the basis of psychological nursing. The two groups were observed and compared the clinical effect of intervention. Results The observation group the effective rate of the treatment and nursing satisfaction rate were significantly higher than the control group; the implementation of group nursing, observation group patients psychological status and improve the psychological status of patients in the observation group after treatment were better, and the control group have obvious difference. Conclusion For patients with advanced ovarian cancer chemotherapy, combined with psychological intervention on the basis of routine nursing, can significantly improve the treatment efficiency, improve the psychological status of patients, is worthy of clinical application

Objective To investigate the clinical significance of changes in vascular endothelial cells during the development of glucocorticoid-induced avascular necrosis of femoral head(GANFH),and the preventive and therapeutic effects of "Cugusu Keli",a mixture of traditional Chinese medicines,on GANFH in rabbit.Methods Forty-eight New Zealand white rabbits were randomly assigned to four groups,12 in each,as control group,model group,therapy group and prevention group.The contents of plasma von Willebrand's factor(vWF),6-keto-prostaglandin F1?(6-keto-PGF1?)and the activity of plasminogen activator inhibitor 1(PAI-1)were determined at the 2nd,4th,6th,8th and 12th week after treatment.Meanwhile,rabbits were sacrificed at the 4th,8th and 12th week,and both femoral heads of all rabbits were obtained for histopathological examination.Results Compared with control group,the level of vWF and activity of PAI-1 in model group were elevated since the 4th and the 2nd week,while the level of 6-keto-PGF1? increased since the 4th week(P0.05).Compared with the model group,the level of vWF and activity of PAI-1 in therapy group was lowered,while the level of 6-keto-PGF1? increased(P
8.THE EXPERIMENTAL STUDY OF ESTROGEN EFFECT ON PRECANCEROUS LESION AND K-ras MUTATION IN RATS ' PANCREATIC ACINAR CELLULAR CANCER

Objective: To set up the animal model of precancerous lesion of rats' pancreatic acinar cellular cancer,and to learn estrogen's effect on this lesion's biological feature and K-ras gene mutation. Methods: male Wistar rats were injected Azaser-ine 30mg/kg(ip). Some rats received Estradiol-17 P Valerate(0.18mg/100g/2weeks, im) .4 months later, rats'pancreas were autopsied and K-ras gene mutation were detected by PCR-SSCP method. Results : The incidence of AACN were very high. The incidence of K-ras gene mutation was 42.1% in rats without estrogen and 25.6% with estrogen. Conclusion: Azaserine could induce precancerous lesion of rats' pancreatic acinar cancer successfully, K-ras gene mutation was early change during this process Estrogen could inhibit this precancerous lesion's happen and development to some degree.
9.Therapeutic effect comparison between unipolar and bipolar radiofrequency ablation in treating atrial fibrillation in elderly patients

Objective To compare clinical effect of unipolar versus bipolar radiofrequency ablation in treating atrial fibrillation (AF) in elderly patients and discuss the correlated risk factors.Methods From October 2008 to December 2013,a total of 50 elderly patients with organic heart disease and AF underwent radiofrequency ablation surgery.All patients were divided into 2 groups of unipolar (group A,n=20) and bipolar (group B,n=30 patients) radiofrequency ablations.The variations of electrocardiogram (ECG) and ultrasonic cardiogram (UCG) in patients were collected and the complications and the NYHA class of the patients were recorded at 3,6 and 12 months after surgery.The total 43 elderly patients were divided into sinus group and non-sinus group according to their ECG at 12 months after surgery.Results The recovery rates of sinus rhythm at 3,6 and 12 months after surgery were 73.7%(14/19),66.7%(12/18) and 61.1% (11/18)respectively in group A and 82.8%(24/29),85.2%(23/27)and 88.0%(22/25) respectively in group B.Two groups of the recovery rate of sinus rhythm had a statistically significant difference between the two groups (P<0.05) at 12 months after surgery.The complication rate in group A was higher than in group B [55.0% (11 cases) vs.26.7% (8 cases),P<0.05].Preoperative history of AF and left atrial diameter were the influencing factors for the sinus rhythm recovery rates (P<0.05).Conclusions Bipolar radiofrequency ablation for AF is safe and has less complications and high success rate of restoring sinus rhythm.The patients with long preoperative history of AF and left atrium distension have a low success rate of restoring sinus rhythm with a high recurrence rate.
10.Investigation on parent's and preschool children's dental health behavior in pudong new area of shanghai

Objective To get informed of the parents' oral health behaviors,children's oral hygiene habits and the correlation between the two,to lay the foundation of building up joint educational pattern of the oral health between pre-school children and their parents.Methods The questionnaire between preschool children and their parents was applied.By cluster sampling,124 pre-school children from 5 kindergartens in the local community were selected to participate the survey.The oral health behaviors of parents and their kids were investigated and the results underwent analysis.Results Parents’ oral health behaviors were closely related to children's,which was embodied by three parts,such as the starting age of brushing teeth,the duration of brushing and the habits of gargling after meals.As to parents,the knowledge sources of oral health mainly came from TV and broadcast programs (82.3%),newspapers and periodicals,magazines and popular science readings (51.6%),relatives and friends (27.4%),but the anticipated knowledge sources were from medical personnel (90.3%),TV and broadcast programs (53.2%),community activities on health education (51.6%).Conclusions Pre-school children's oral health behaviors are currently good; more emphasis should be laid on the effect of parents' behaviors to their children's oral health; it's necessary to hold more community activities on health education.
