2.Association of allogeneic peripheral blood stem cell transplantation with a low incidence of relapse

Objective To evaluate the relationship between transplantation of allogeneic peripheral blood stem cells (allo-PBSCs) and low incidence of leukemic relapse. Methods Thirty-seven adult patients with acute myeloid leukemia in first remission ( n =16) and chronic myeloid leukemia in chronic phase ( n =21) underwent allo-PBSCT from June 1997 to December 1999. A median number of CD34 + cells, CD3 + cells, CD4 + cells and CD8 + cells infused was 7.3 ?10 6/kg, 364?10 6/kg, 210?10 6/kg and 137?10 6/kg, respectively. Busulfan and cyclophosphamide were used as conditioning regimen (BU-CY2). All patients received cyclosporine and methotrexate for GVHD prophylaxis. Results Engraftment of neutrophil and platelet was achieved at a median of day +13(9~28) and day +12(7~19) respectively. Acute GVHD (aGVHD) occurred in 18 of 37 patients ( 48.6% ) with grade II~IV aGVHD ( 24.3% ). Chronic GVHD (cGVHD) developed in 22 (extensive 8, limited 14) out of 32 evaluable patients ( 68.8% ). 28 patients were still alive in complete remission at a median follow-up of 950 days (range 600~ 1 500 days). The main causes of death were aGVHD in two patients, CMV-IP in four patients, cGVHD in two patients, and relapse only in one patient. The probability of disease free survival at 3 years was 75.7% . Conclusion In our series, leukemia recurrence was seen only in one patient ( 2.7% ), suggesting that a high incidence of cGVHD may have an enhanced graft-versus-leukemia effect and hence reduce the incidence of relapse after transplantation.
3.Clinical Observation of Sertraline in the Treatment of Diabetes Patients Complicated with Depression

Objective:To investigate the effect and safety of sertraline tablets with oral administration in the diabetes patients com-plicated with depression. Methods:Totally 74 diabetes patients complicated with depression were randomly divided into 2 groups with 37 ones in each. All the patients were orally given dimethyldiguanide 500 mg,tid,and the observation group was given sertraline tablets additionally,50-100 mg,qd. The treatment course was 8 weeks. The changes of sugar metabolism and the scores of self-rating depres-sion scale( SDS)and quality of life scale( DMQLS)in the two groups were compared. Results:After the 8-week treatment,the blood sugar,SDS scores and DMQLS scores in the two groups were significantly improved when compared with those before the treatment( P<0. 05). The levels of FBG,2hPG and HbAlc and SDS scores in the observation group were significantly lower than those in the con-trol group(P<0. 05),while the scores of DMQLS disease,psychology,society and satisfaction dimension were significantly higher in the observation group(P<0. 05). The incidence of adverse reactions showed no statistically significant difference between the two groups(P>0. 05). Conclusion:Sertraline tablets can improve the depression of diabetes patients and enhance the blood sugar control and quality of life.

6.Expression and function of miRNA211 in human cutaneous melanoma

Objective To determine the expression of miRNA211 (miR-211) in the development of malignant melanoma,and to investigate the correlation between miR-211 and its target molecule,matrix metalloproteinase 16 (MMP-16).Methods Cultured A375 melanoma cells were divided into 3 groups:miR-211 overexpression group and mock-vehicle group transfected with miR-211 mimics and empty vehicle respectively,and negative control group receiving no treatment.TaqMan fluorescence-based quantitative PCR was performed to determine the expression of miR-211 in HER1 primary melanocytes,A375,C32 and G361malignant melanoma cell lines,as well as in nevus tissues (n =18) and melanoma tissues (n =41),and to evaluate changes of MMP-16 mRNA expression in A375 cells before and after the overexpression of miR-211.Sulforhodamine B (SRB) assay and flow cytometry were conducted to evaluate cellular proliferative activity and determine cell cycle distribution respectively,and methylcellulose assay and Transwell assay to evaluate colony formation and cell migration abilities respectively.The size of selected colonies was used to represent colony formation ability,while the ratio of the number of migrating cells to that of non-migrating cells to represent cell migration ability.Results There were significant differences in the expression level of miR-211 among the G361,C32 and A375 cells (0.09 ± 0.02 vs.0.000 52 ± 0.000 20 vs.0.000 03 ± 0.000 01,F =10 410,P < 0.01).The expression of miR-21 1 was significantly decreased in melanoma tissues compared with nevus tissues (0.17 ± 0.03 vs.0.87 ± 0.08,t =9.118,P < 0.01).No significant differences were observed in cellular proliferative activity or cell cycle distribution among the miR-211 overexpression group,mock-vehicle group and negative control group.Compared with the mock-vehicle group,the miR-211 overexpression group showed significantly suppressed colony formation (0.49 ± 0.05 vs.0.85 ± 0.09,t =2.19,P < 0.05) and cell migration (0.49 ± 0.06 vs.0.82 ± 0.09,t =3.15,P < 0.05) abilities,while no significant difference was observed between the mock-vehicle group and negative control group.Additionally,the mRNA expression of MMP-16 significantly decreased in the miR-211 overexpression group compared with the mock-vehicle group after transfeetion (24 hours:0.33 ± 0.02 vs.0.91 ± 0.03,t =11.30,P < 0.01;48 hours:0.52 ± 0.01 vs.0.96 ± 0.02,t =5.02,P < 0.05;72 hours:0.71 ± 0.01 vs.0.97 ± 0.03,t =3.85,P < 0.05),with no significant difference between the mock-vehicle group and negative control group at the above time points.Conclusions miR-211 was lowly expressed in both malignant melanoma cells and tissues,and it could inhibit both anchorage-independent growth and migration of melanoma cells.After up-regulation of miR-211 expression,the mRNA expression of MMP-16 decreased in A375 cells,suggesting that MMP-16 may be a downstream target of miR-211,and can influence melanoma metastasis.
7.Effects of Atorvastatin Combined with Probucol on the Vascular Elasticity in Patients with Hypertension

OBJECTIVE:To explore the effects and safety of atorvastatin combined with probucol on the vascular elasticity in patienits with hypertension. METHODS:246 hypertensive patients were randomly divided into control group and observation group,123 cases in each group. All patients received conventional antigypertensive treatment,based on it,control group was given 10 mg Atorvastatin tablet,qd;observation group was additionally given 0.25 g Probucol tablet,qd,on the basis of control group. They were treated for 1 year. Clinical efficacy,lipid levels(total cholesterol,triglyceride,low density lipoprotein cholesterol,high density lipoprotein cholesterol),cystatin C (Cys-C),C-reactive protein (CRP),elastic parameters of common carotid artery and lower limb artery(stiffness,pressure-strain elastic modulus,compliance,augmentation index,pulse wave velocity)before and af-ter treatment in 2 groups were observed,and the incidence of adverse reactions was compared. RESULTS:Before treatment,there were no significant differences in lipid levels,Cys-C,CRP and elastic parameters of common carotid artery and lower between 2 groups (P>0.05);after treatment,the above-mentioned indexes were significantly improved,and the improvement degree in ob-servation group was superior to control group,the differences were statistically significant(P<0.05). And there were no severe ad-verse reactions during treatment in both 2 groups. CONCLUSIONS:Atorvastatin combined with probucol can improve lipid level and elasticity of common carotid artery and lower limb artery,with good safety.
8.Analysis of factors influencing 18F-FDG metabolic parameters in PET/CT scan for lung cancer

Objective:To explore the effects of body mass index (BMI) and gender on primary lung cancer 18F-FDG uptake param-eters, standardized uptake value (SUV), and standard uptake value of lean body mass (SUL). Methods:Data of 50 patients with prima-ry lung cancer confirmed by 18F-FDG positron emission tomography (PET)/computed tomography (CT) were retrospectively analyzed. AW4.6 workstation was employed to measure the SUVmean and SUVmax. Meanwhile, PETVCAR (PET Volume Computed Assisted Reading, GE Healthcare) software was used to automatically measure the SULmean, SULmax, and SULpeak. The SUVmean, SUV-max, SULmean, and SULmax of the liver (central region of the right lobe) were also measured automatically by PETVCAR. Afterward, T/N ratios (lesion SUVmax/liver SUVmean, lesion SULmax/liver SULmean, and lesion SULpeak/liver SULmean) of the lung cancer lesions were calculated. Correlations of the 18F-FDG metabolic parameters with BMI and gender of the patients were analyzed. Results:Liver SUVmean and SUVmax demonstrated significant positive correlations with BMI in all the patients (γ=0.38 and 0.36, P<0.05), and the SUVmean and SUVmax were positively correlated with BMI in male and female groups (γ=0.47 and 0.44, P<0.05), respective-ly. By contrast, no correlation existed between the liver SULmean and SULmax and BMI (P>0.05). No significant correlation was not-ed between the SUVmean, SUVmax, SULmean, SULmax, and SULpeak of the lung cancer lesions and BMI (P>0.05). The correlation trend is the same as that in different gender groups. Only the SUVmax T/N ratio of the lung cancer lesions showed a significant nega-tive correlation with BMI (γ=?0.29, P<0.05). The T/N ratios did not correlate with BMI in the different gender groups (P>0.05). Con-clusion:Patient BMI and gender mainly affect SUV values, particularly SUVmax, by contrast, patient BMI and gender did not signifi-cantly influence SUL and T/N ratio (SUL). Hence, SUL can be more suitable to quantitatively analyze and assess treatment response ob-jectively. This result will be helpful to the clinical application and promotion of PERCIST, which evaluates treatment response mainly by SUL.
9.Analgesic effects of intraarticular cocktail versus intravenous parecoxib injection after total hip arthroplasty

BACKGROUND:Intraarticular cocktail analgesic injection is a popular postoperative analgesia method and can effectively control postoperative pain and relieve side effects after total hip arthroplasty.
OBJECTIVE:To compare and assess the effectiveness and safety of intraarticular analgesic injection or intravenous injection of parecoxib after total hip arthroplasty.
METHODS:A total of 60 patients undergoing total hip arthroplasty were randomly assigned to:treatment group (intraarticular cocktail analgesic injection with morphine, bupivacaine, and compound betamethasone), and control group (intravenous injection of parecoxib). Al patients received tramadol hydrochloride at 24 hours after replacement. Analgesic consumption, visual analog scale at rest and during activity, range of motion, and postoperative complication of patients in each group were recorded.
RESULTS AND CONCLUSION:Intraarticular cocktail analgesic injection significantly reduced analgesic consumption. When comparing visual analog scale scores, rest pain scores were significantly less in the treatment group at 12, 24 and 48 hours after replacement than that in the control group (P<0.05). Scores on range of motion were significantly less in the treatment group at 24 and 36 hours than that in the control group (P<0.05). No significant differences in total complications were detectable between the treatment and control groups (P>0.05). Results suggested that intraarticular cocktail analgesic injection lessened analgesic consumption after replacement, relieved early pain after replacement, and contributed to early rehabilitation of patients. Moreover, no significant adverse reactions were visible.
10.Investigation and analysis on professional identity of nursing students with different educational backgrounds

Objective To investigate and analyze the status quo of professional identity of nursing students with different educational backgrounds.Methods With stratified-cluster random sampling,635 students were surveyed using the nursing students' professional identity questionnaire.Results The total score of professional identity of junior college nursing students was significantly higher than that of undergraduate students.Junior college nursing students had a higher score in professional knowledge dimension than undergraduate and graduate students,while the score of nursing undergraduate students was higher than that of graduate students.Junior college nursing students had a higher score in professional will dimension than undergraduate and graduate students.Nursing graduate students had a higher score in professional expectation dimension than junior college and undergraduate students,while the score of undergraduate students was higher than that of junior college students.Whatever the educational background,professional skill and professional expectation were the top two dimensions,while professional knowledge and professional emotion were the last two dimensions.The difference was statistically significant between the answers of nursing students with different educational backgrounds to the following questions,such as which is the most probable factor affecting your learning enthusiasm,who has the greatest impact on your professional thought about nursing and which nursing field are you most willing to go in for.There was significant difference in professional identity score between different grades of junior college and undergraduate nursing students,while there is no significant grade difference in graduate students.Conclusions There were some differences of professional identity of nursing students with different educational backgrounds.Nursing educators should carry out the professional education according to the student's educational backgrounds.
