Medication has become the first-line option for the management of lower urinary tract symptoms induced by benign prostatic hyperplasia (LUTS/BPH) for its advantages in controlling the symptoms, inhibiting BPH progression, and reducing serious complications and surgical risks. Recent years have witnessed remarkable achievement in the studies of phosphodiesterase type 5 inhibitors (PDE5-Is) in the treatment of LUTS/BPH. PDE5-Is can effectively alleviate LUTS/BPH, with even better efficacy when combined with al-ARAs.

Chinese Journal of Tissue Engineering Research 2009;13(4):793-796
BACKGROUND:Knowledge of retropedtoneal space communications might influence catheter placement, and understanding the normal anatomy of the retroperitoneal space is a prerequisite for predicting the distribution of inflammation or other fluid collections in this region. Until recent years, the media/ attachment of the posterior renal fascia remained controversial. The multiple detector spiral CT can show the abdominal anatomic details. So,using the multiple-detector spiral CT to study the anatomy of posterior renal fascia has clinical significance. OBJECTIVE:To describe the medial attachment of the posterior renal fascia by using multiple-detector spiral CT. DESIGN,TIME AND SETTING:A retrospective case analysis was performed at Department of Radiology,Affiliated Hospital of Weifang Medical College between June 2003 and November 2007. PARTICIPANTS:A total of 52 patients with retropedtoneal inflammatory diseases were retrospectively reviewed through analysis of their CT data. METHODS:Toshiba Akuilion 16-detector spiral CT was employed for scanning. Of the 52 patients,15 were proved by clinical and laboratory findings and 37 were proved by surgery and pathology. Among the 52 patients,17 suffered from appendicitis,1 from ureteritis,2 from abscesses in the perirenal space,3 from abscesses in the posterior pararenal space, and 29 from pancreatltis. MAIN OUTCOME MEASURES:Medial attachment of the bilateral posterior renal fascia. RESULTS:At the level of the upper pole of kidney, the posterior renal fascia fused with the fascia of the ipsilateral quadratus lumborum muscle. Forty-six patients manifested the attachment site of the left posterior renal fascia transforming from the quadratus lumborum muscle fasciae to the psoas major muscle fascia at the level of the lower pole of kidney or the infrarenal space. Fifty patients showed the attachment site of the right posterior renal fascia transforming from the quadratus lumborum muscle fascia to the psoas major muscle fascia at the level of the lower pole of kidney or the infrarenal space. CONCLUSION:The posterior renal fascia attachment site is not the same all the time. At different levels, the attachment site of the posterior renal fascia is distinct.

Objective To investigate changes in the expression of prepro-orexin and orexin receptor-1 ( OX1R) following permanent middle cerebral artery occlusion ( MCAO ) with or without preconditioning through electrical stimulation of the cerebellar fastigial nucleus (FNS). Methods Wistar rats were subjected to permanent MCAO and randomly divided into 5 groups: a sham-operated control group (PO), an FNS preconditioning + shamoperated control group (FNS-PO) , an ischemia group, an FNS preconditioning + ischemia group (FNS-PI) and a cerebellar fastigial nucleus injury + FNS preconditioning + ischemia group (FNL-FNS-PI). Each group was divided into 5 subgroups according to the time at which the animals were sacrificed after the MCAO ( 1, 3, 6, 12 and 24 h).RT-PCR was used to detect expression of OX1R mRNA, and ELISA to measure the levels of orexin-A in the hypothalamus and plasma. Results The immunoreactivity of prepro-orexin decreased significantly in the PI groups, with further decreases over time. At the 12th h after MCAO, the immunoreactivity of prepro-orexin reached a minimum. There were significant differences between the rats in the PO and FNS-PO groups. On the contrary, the immunoreactivity of OX1R increased significantly in the PI groups, with further increases continuing over time, peaking at 12 h after the MCAO. There were significant differences between the PO and FNS-PO groups. In the rats with FNS preconditioning (PI-FNS) , the decrease in prepro-orexin and the increase in OX1R were significantly inhibited compared to the PI subgroups at the 6th and 12th hour. There was no significant difference between the FNL-PIFNS group and the PI group. The expression of OX1R mRNA increased significantly in the PI group, with further increases continuing over time, peaking at 24 hours. The plasma levels of orexin-A were not significantly different among the groups, but the levels of orexin-A in the hypothalamus decreased significantly in the PI and FNL-PI-FNS groups, with further decreases continuing over time. At the 12th h after the MCAO the levels were significantly different compared with the PO and PO-FNS groups. While in the rats with FNS preconditioning (PI-FNS) , the decrease in orexin-A level was reversed and there was no significant difference compared with PO and PO-FNS groups. Conclusions The orexinergic system is altered following cerebral ischaemia. FNS preconditioning may be able to regulate these changes.

Objective:
To investigate the level of arsenic exposure in rural drinking water and to assess the health risk caused by exposure to arsenic in Tongzhou District, Beijing Municipality in 2019, so as to provide insights into improving the quality of rural drinking water.
Methods:
Water samples were collected from self-provided wells in 196 villages of 7 townships in Tongzhou District from April to June, 2019, and the arsenic levels were measured in drinking water according to Standard examination methods for drinking water-Metal parameters (GB/T 5750.6-2006). The carcinogenic and non-carcinogenic risks of arsenic were evaluated in drinking water using the environmental risk assessment model recommended by the United States Environmental Protection Agency (EPA).
Results:
Totally 520 water samples were collected, and the median arsenic concentration was 0.001 2-0.050 0 mg/L. There were 67 water samples with arsenic levels exceeding the defined standard level (12.88%), and the proportions of arsenic levels exceeding the defined standard level were 57.69%, 17.24%, 12.20%, 6.52% and 1.31% in Songzhuang, Huoxian, Lucheng, Zhangjiawan and Yongdedian townships, and the arsenic levels did not exceed the defined standard level in Majuqiao or Taihu townships. The carcinogenic risk of arsenic was 3.135 7×10-4/a in drinking water, which exceeded the maximum acceptable risk, and the carcinogenic risk of arsenic all exceeded the maximum acceptable risk, with the highest value seen in Songzhuang Township (9.648 2×10-4/a). A high carcinogenic risk of arsenic was seen in men at ages of 60 to 80 years (3.012 8×10-4/a) and in women at ages of 80 years and older (2.949 0×10-4/a). The non-carcinogenic risk of arsenic was 0.696 8 in drinking water, indicating a low risk, and were 1.009 5 and 2.144 1 in Huoxian and Songzhuang townships, indicating a high risk. In addition, a high non-carcinogenic risk of arsenic was seen in men at ages of 60 to 80 years (0.669 5) and in women at ages of 80 years and older (0.655 3).
Conclusion
There was a high proportion of arsenic levels exceeding the defined standard level in rural drinking water in Tongzhou District, 2019, and there was a health risk, notably with the highest carcinogenic risk seen in residents at ages of 60 years and older. Effective interventions are needed to reduce arsenic levels in drinking water.

Journal of Preventive Medicine 2022;34(6):581-585
Objective:
To investigate the effects of the link between overweight/obesity and type 2 diabetes mellitus (T2DM) on leptin and visfatin levels.
Methods:
Males without T2DM and male patients with T2DM hospitalized in Lishui Municipal Central Hospital from January to June, 2017 were enrolled. Subjects' age and medical history of diseases were collected. The height and body weight were measured, and the body mass index (BMI) was estimated. The leptin and visfatin levels were determined, and compared between patients with and without T2DM, and between patients with and without overweight/obesity. The effect of the link between overweight/obesity and T2DM on leptin and visfatin levels was examined using a generalized linear regression model.
Results:
There were 66 patients with T2DM, with a mean age of (49.70±9.45) years and a mean diabetes duration of (4.99±4.46) years, and there were 64 patients without T2DM, with a mean age of (43.89±0.20) years. The leptin [ (3.17±0.36) vs. (3.03±0.30) ng/mL; t=2.387, P=0.018] and visfatin levels [ (29.14±3.16) vs. (21.81±3.32) ng/mL; t=12.900, P<0.001] were significantly greater in T2DM patients than in patients without T2DM. The leptin level was significantly greater in patients with overweight/obesity than in those without overweight/obesity [ (3.27±0.32) vs. (2.92±0.26) ng/mL; t=6.634, P<0.001], and the visfatin level was significantly lower in patients with overweight/obesity than in those without overweight/obesity [(24.38±5.14) vs. (26.71±4.36) ng/mL; t=2.780, P=0.006]. Generalized linear regression analysis showed interacting effects of overweight/obesity and T2DM on leptin (β=0.286, P=0.003) and visfatin levels (β=2.709, P=0.008).
Conclusion
The interaction between overweight/obesity and T2DM affects leptin and visfatin levels.
