1.The influence factors and treatment effect of abnormal umbilical blood flow

Objective To retrospectively analyze the main factors of abnormal umbilical blood flow in different stage after 24 weeks' gestation,and to evaluate its treatment outcomes.Methods 256 cases with abnormal umbilical blood flow were selected.The abnormality happened in different stages after 24 weeks' gestation and they were inspected and treated routinely.Latency factors of the different stage were analyzed to discover the primary reason,and the treatment outcomes were compared.Results The significant differences of constructional proportion were observed in different stage(x2 =25.60,P < 0.05).The influence factors of different stages were as following:hypertension relative disease (33.33 %) between 24 and 27 weeks' gestation,the umbilical cord factor (16.67 %) ; the incidence rate of abnormal umbilical blood was 42% between 28 and 36 weeks' gestation,including hypertension disease(28.70%),the umbilical cord factor(15.74%) ;umbilical cord (31.37%) between 37 and 40 weeks' gestation,although gestation period hypertension disease reduced,but still accounted for 22.54%,the placenta factor was 12.74%.After treatment,the cure rate had significant differences (x2 =11.16,P <0.01),89.58% (24-27 week),80.55% (28-36 week),69.60% (37-40 week).Conclusion The hypertension relative diseases and abnormal cord are the primary factors which caused abnormal umbilical blood,24-37 gestation week is the key stage for the inspection and treatment,especially for the hypertension relative diseases.
2.Study of the effect of Astragalus injection on the cell apoptosis of intracerebral hemorrhage in rats

Objective To investigate the rule of cell apoptosis after intracerebral hemorrhage (ICH) in rats, and to analyze the effect of Astragalus injection on the brain tissue water content and cell apoptosis perihematoma.Methods Experimental ICH models of rats were produced in by injection of Collagenase into right caudate nucleus using stereotaxis technique. After operation, 160 SD rats were randomly divided into 4 test groups according to different intervention time (operation meanwhile group, 6 h group,12 h group and 24 h group after operation, 32 rats in each group) and hemorrhagic control group (32 rats). The rats in test groups were treated with Astragalus injection intraperitoneally once a day. All ICH animals were killed at 4 d or 7 d after ICH. 12 rats from the above each group were used to measure brain tissue water content (dry-wet method). The effect of different initiation time point of treatment on brain tissue water content and cell apoptosis perihematoma was observed. 3.The experimental study of ginkgolide B effectual on neuronal cell apoptosis in the rat with intracerebral hemorrhage

Objective To investigate the effects of ginkgolide B on neuronal cell apoptosis,superoxide dismutase activity,malondialdehyde,interleukin-1beta,tumor necrosis factor-alpha,and interleukin-6 levels in serum of rats with intracerebral hemorrhage in order to explore the role of ginkgolide B in suppressing the neuronal cell apoptosis.Methods A total of 175 male Wistar rats were randomly (random number)divided into sham operation group,intracerebral hemorrhage group,as well as low,medium and high dose treatment groups.The rat model of intracerebral hemorrhage was made with infusion of autologous whole blood to caudate nucleus in the right basal ganglia region.Ginkgolide B in dose of 5 mg/kg,10 mg/kg and 20 mg/kg was given to rats in the low,middle and high dose treatment groups by intraperitoneal injection once a day for 5 days after intracerebral hemorrhage.The rats with intracerebral hemorrhage in the sham operation groups received intraperitoneal administration of 1 mL saline.Animals were sacrificed by decapitation 2,6,12,24,48,72 h and 5 days after intracerebral hemorrhage.Brains were taken and blood samples were collected.Neuronal cell apoptosis was measured by using terminal deoxynucleotidyl transferase biotin-dUTP nick end labeling(TUNEL),and superoxide dismutase activity in serum was determined by using xanthine oxidase method,and serum malondialdehyde level was detected by using thiobarbituric acid reactive substance assay,and interleukin-1beta,tumor necrosis factor-alpha,and interleukin-6 levels in serum were assayed with enzyme linked immunosorbent assay(ELISA).Statistical analysis was carried out by using one-way analysis of variance and least-significant difference test.Results As 2 h,6 h,12 h,24 h,48 h,72 h,and 5 days after intracerebral hemorrhage,the differences in the number of apoptotic neuronal cell,superoxide dismutase activity in serum,serum malondialdehyde,interleukin-1 beta,tumor necrosis factor-alpha and interleukin-6 levels between the low dose treatment group and intracerebral hemorrhage group were not significant statistically(P >0.05).As 12 h,24 h,72 h,and 5 days after intracerebral hemorrhage,the number of apoptotic neuronal cell,superoxide dismutase activity in serum,serum malondialdehyde,interleukin-1 beta,tumor necrosis factor-alpha and interleukin-6 levels in the medium dose and high dose treatment groups were significantly statistically lower than those in the intracerebral hemorrhage group(P < 0.05),but these differences in above biomarkers were not significant statistically among these three groups 2 and 6 hours after intracerebral hemorrhage(P > 0.05).Conclusions Ginkgolide B may lessen neuronal cell apoptosis by means of inhibition of free radical production and inflammatory reactions after intracerebral hemorrhage.

5.Post-traumatic stress disorder in the survivors of the tremendous explosion

BACKGROUND: Posttraumatic stress disorder(PTSD) arises as a delayed and /or protracted response to a stressful event or situation of an exceptionally threatening or catastrophic nature. There have been reports about natural disasters causing PTSD, but there have been few reports about PTSDcaused by technological disasters. Our study investigated the mental status of those survivors after a serious explosion.OBJECTIVE: To investigate the characteristics and the factors of impact of PTSD caused by technological disasters.DESIGN: A case controlled study of the survivors of a momentous explosion.SETTING: A provincial mental health center.PARTICIPANTS: A serious explosion happened in Urumqi on September 8, 2000. Twenty-eight survivors(as the study group) who were still hospitalized for treatment 3 -5 months after the accident were involved in this study. And in the control group were 30 normal persons whose general demographic data were the same as those patients.METHODS: A psychiatrist told the subjects the following: The objective and meaning of the test, and explained the self-making questionnaire about their general condition, symptom checklist (SCL-90), self-rating anxiety scale (SAS), self-rating depression scale(SDS), cornell medical index(CMI), the questionnaire on dealing styles, the questionnaire on social support, and others-evaluating scale, incident effect scale(IES); a list of stress reaction symptoms.MAIN OUTCOME MEASURES:Main resulta:①Clinical feature of PTSD caused by the technological disaster.②the related factors to PTSD in the technologal disaster.Secondary results: Comparison of the results between the two groups in IES, CMI, SDS and SAS.RESULTS: A total of 22(79% ) survivors met the criteria as PTSD. There were significant differences in all the assessments(IES, CMI, SDS, SAS,SCL-90) between the study group and the control group (t =3.62-8.17, P < 0.01 ) . The diagnosis of PTSD was positively correlated with the level of traumatic exposure( r = 0. 420, P < 0. 05), and negatively correlated with the degree of satisfaction with the post-event solutions( r = 0. 420, P< 0.05), positively correlated with IES scoring and the level of heart pain ( r = 0. 389 - 0. 665, P < 0. 01 ) . The total scores of IES were positively correlated with the level of exposure ( r = 0. 478, P < 0.05 ).CONCLUSION: The incidence of PTSD after explosion was high, and its occurrence was related with the degree of exposure in the event and that of the satisfaction with the post-event solutions.
6.Application of pure brilliance imaging in diagnosis of benign and malignant breast neoplasms

Objective To evaluate the application of pure brilliance imaging(PB1) in diagnosis of benign and malignant breast neoplasms. Methods Ninty-seven breast neoplasms were respectively examined by high frequency ultrasound and PBI before operation,then correlated with pathological results. Results The accuracy of high frequency ultrasound for breast neoplasms was 73.2% with the sensitivity 77.8% ,specificity 69.2%. The detectable rate of spiculate margin, halo,calcification were much higher in PBI than those in high frequency ultrasound(P<0.05). And the accuracy,sensitivity and specificity of PBI were 86.6%, 91.1%, 82.6%. Conclusions PBI can significantly increase the detectable rate of ultrasonographie marginal features of breast neoplasms which were also helpful to the differential diagnosis.
7.Analysis on grade evaluation system of physician competency in Shenzhen pilot project

In order to reform the medical talent evaluation system, establish evaluation systems of physician competency, and implement physician resources management systems according to work position, and the compensa-tion system which reflects the value of medical services, The basic principles and standards of different physician tiers and grades are created in Shenzhen based on literature review, expert consultation and the methods from the American Centers for Medicare and Medicaid Services ( CMS) . Some results have achieved, including medical competency as-sessment of a certain number of physicians in the pilot project ( the coincidence rate of special hospitals is higher than general hospitals, respectively 78. 9% and 44. 8% ), comprehensive personnel systems reforms in public hospi-tals, and the trial selection of medical talents. The paper also provides some implications:The cognition of all sectors of society and physician themselves should be improved, Transitional policies for position recruitment and performance pay needs further improvement. Meanwhile, the grade evaluation system of non-physicians must also be given greater attention.
8.Recent status of heart transplantation in Chinese multi-centers

ObjectiveTo know the recent status of heart transplantation in Chinese multicenters.MethodsThe retrospectively data from 24 centers before 2010 (438 cases) and the data from China heart transplant Registry database between 2010 (149 cases from 15 centers)-2011(148 cases from 19 centers) were analyzed.Results In 2010,15 centers performed heart transplantation,including one large-scale center (60 transplants per year),3 moderate-scale centers (10 to 30 transplants per year),and 6 small-scale center (2-8 transplants per year).The rest 5 centers had one transplant per year each.In 2011,19 centers performed heart transplantation,including one large-scale center (52 transplants per year),3 moderate-scale centers (10 to 30 transplants per year),and 5 small-scale centers (2-8 transplants).The rest 8 centers had one transplant per year each.In 2010 and 2011,the median ages of heart transplant recipients were 44.6 and 42.9 years,respectively.Nonischemic cardiomyopathy was the leading indication for heart transplantation.The median donor age in 2010 and 2011was 30.2 and 30.8 years respectively.In 2010 and 2011,up to 99.3% and 97.3% of patients were treated with immune induction therapy,respectively.In 2010,the mortality at discharge was 10%,and that was 6% in 2011.ConclusionAlthough total volume of heart transplants is still small in recent years,the discharge survival in China is similar to ISHLT report.
