1.Comparison of the effect and safety of candesartan cilexetil and enalapril in the treatment of senile hypertension

Objective To compare the efficacy and safety of candesartan cilexetil and enalapril in the treatment of senile hypertension.Methods 100 patients with hypertension were randomly divided into the treatment group and control group according to digital table method.The treatment group were received candesartan cilexetil 4 ~ 8mg once daily for 8 weeks,and control group were received enalapril 10 ~ 20mg once daily for 8 weeks,and the efficacy and adverse reactions were watched.Results The SBP and DBP were (134.42 ± 6.39) mmHg and (82.00 ± 5.05) mmHg for candesartan cilexetil and (137.70 ± 5.27) mmHg and (81.76 ± 5.03) mmHg for enalapril after 8 weeks.A significant reduction of blood pressure was achieved in both groups than before treatment (tcan =35.85,30.88;tena =37.92,31.67,all P < 0.01).After treatment pulse pressure was (56.06 ± 4.91)mmHg for candesartan cilexetil and (60.04 ± 4.40) mmHg for enalapril.Compared with before treatment,pulse pressure was significantly reduced in the treatment group (t =16.93,P < 0.01) and was no significant difference in control group (t =6.34,P > 0.05).The incidence of adverse reactions of patients with treatment group was obviously lower than that of the control group.Conclusion Candesartan cilexetil is effective and safe in senile hypertension patients,and it could be used as a first-line treatment.
2.Retrospective Analysis of Anti-infective Treatment for CAP Patients in Our Hospital

OBJECTIVE:To provide reference for rational use of antibiotics in community-acquired pneumonia (CAP) pa-tients. METHODS:Medical records of 342 CAP inpatients in our hospital during Jan.-Dec. in 2014 were analyzed retrospectively by DDDs and SPSS 19.0 statistical software. According to the age,the patients were divided into youth group,the young group, middle age group and elderly group,and clinical information and the use of antibiotics were analyzed statistically. RESULTS:Mi-crobiological examination of 320 of 342 CAP inpatients were performed with detection rate of 93.57%;sputum culture,blood cul-ture and PCT were conducted in 67 case to find out pathogenic bacteria with negative rate of 20.18%. 338 patients had used antibi-otics (98.83%),and other 4 patients refused anti-infective treatment. Selected empirical antibiotics of our hospital were mainlyβ-lactam (50.07%),quinolones (35.70%) and macrolides (9.80%). CAP inpatients were mainly given two-drug therapy,of which cephalosporin combined with quinolone were most common,including 178 cases in total (53.61%). During treatment,17 patients suffered from ADR,with incidence of 5.03%. CONCLUSIONS:The detection rate from patients with CAP in our hospital is high in microbiological examination,but culture results should be analyzed dialectically according to clinical signs. Empirical an-ti-infective therapy mainly include β-lactams,quinolones and macrolides antibiotics alone or in combination with,basically meet the guideline for CAP Diagnosis and Therapy Guideline.
3.Research on ECG de-noising method based on ensemble empirical mode decomposition and wavelet transform using improved threshold function.

A de-noising method for electrocardiogram (ECG) based on ensemble empirical mode decomposition (EEMD) and wavelet threshold de-noising theory is proposed in our school. We decomposed noised ECG signals with the proposed method using the EEMD and calculated a series of intrinsic mode functions (IMFs). Then we selected IMFs and reconstructed them to realize the de-noising for ECG. The processed ECG signals were filtered again with wavelet transform using improved threshold function. In the experiments, MIT-BIH ECG database was used for evaluating the performance of the proposed method, contrasting with de-noising method based on EEMD and wavelet transform with improved threshold function alone in parameters of signal to noise ratio (SNR) and mean square error (MSE). The results showed that the ECG waveforms de-noised with the proposed method were smooth and the amplitudes of ECG features did not attenuate. In conclusion, the method discussed in this paper can realize the ECG denoising and meanwhile keep the characteristics of original ECG signal.

4.Acupuncture combined with ganglioside and mecobalamine injections can relieve the symptoms of idiopathic facial palsy

Objective To observe the clinical effect of acupuncture combined with ganglioside and mecobalamine injections on idiopathic facial palsy (IFP).Methods A total of 120 IFP patients were divided into a ganglioside group,a mecobalamine group and a consociation group,each of 40.All were given routine drug and acupuncture treatment,while the ganglioside and mecoblamine groups were additionally provided with the corresponding injections of ganglioside and mecobalamine,respectively.The treatment was administered once daily for a total of 14 days.All the groups were evaluated using the House-Brackmann facial nerve grading system before and after the interventions of 14 days ended.The compound muscle action potential (CMAP) wave amplitude and R1 incubation,the motor nerve conduction velocity (MCV) and the sensory nerve conduction velocity (SCV) of the affected facial nerves were observed and compared before and after the 14 days of treatment.Results The total effectiveness rate of the consociation group (95.0%) was significantly higher than that of the ganglioside (90.0%) or the mecobalamine group (87.5%).After the treatment the CMAP wave amplitude had improved significantly and the R1 latency had shortened significantly in the consociation group compared to the other 2 groups.The average MCV and SCV of the consociation group had also improved significantly compared with the other two groups.Conclusion Acupuncture combined with ganglioside and mecobalamine injections is an effective therapy for idiopathic facial palsy.It can improve the patient's clinical symptoms,shorten the CMAP wave amplitude and R1 latency on the affected side and improve MCV and SCV.
5.Retrospective Analysis of the Safety of Tigecycline in a Hospital

OBJECTIVE:To provide reference for rational application of tigecyclinet and alert to the occurrence of severe ADR.METHODS:Fifity patients receiving tigecycline in a level 3 general hospital during 2013-2015 were analyzed retrospectively to observe the change of symptom,sign and lab indexes after using Tigecyclinet injfection.Possible ADR of tigecycline,processing and outcomes were summarized.RESULTS:Among 50 patients,there were 24 cases of ADR,including 10 cases of inducing or aggravating blood coagulation abnormality (41.67%),9 cases of liver function injury (37.50%),4 cases of vomiting and other gastrointestinal discomfort (16.67%),1 case of red erythra with itching (4.17%).ADR of digestive system were mild and recovered after symptomatic treatment as inhibiting acid,antiemetic.Severe ADR as Liver function injury could not be recovered after symptomatic treatment as protecting liver,reducing enzyme.Nine cases of liver function injury mainly manifested as the elevation of TBIL,DBIL,ALP,LDH;8 of which suffered from liver function injury before medication and the symptom was aggravated after medication;liver function injury appeared in 3 cases on 10th day after medication and in 2 cases on 9th day after medication.Ten cases suffered from coagulation function disorder before medication and the symptom was aggravated after medication,which mainly manifested as the prolongation of APTT and TT and the elevation of INR,PT,D-dimer,etc.The coagulation function disorder was aggravated abnormally on 2nd-22th day after using tigecycline,mainly appearing on 2nd-5th day (70.00%).CONCLUSIONS:Great importance should be attached to severe ADR as coagulation function disorder,liver function injury when using tigecycline,in order to ensure the safety of drug use.
6.Diagnosis of neonatal infections

Objective To investiGate the value of procalcitonin( PCT ),C reactive protein( CRP ), white blood cell( WBC)and platelet in the diaGnosis of neonatal infection. Methods The clinical data of 57 cases with neonatal infection in the First Hospital of Hefei were retrospectively analyzed. Select 65 healthy neonates in the same period as control Group. Compared two Groups of serum PCT,CRP,WBC and platelet levels, and diaGnosed of sensitivity,specificity of neonatal infection. Results There were statistical siGnificant differences between infection Group and control Group interms of CRP,WBC and PCT in newborn health neonates ( P<0. 05),but except the platelet( P>0. 05). The sensitivity and specificity of PCT were 54. 4% and 92. 3%, of CRP were 43. 9% and 84. 6%,of WBC was 29. 8% and 90. 8%,and of PLT were 38. 6% and 69. 2%. Conclusion PCT,CRP,WBC are of diaGnostic siGnificance for early diaGnosis of neonatal infection,and PCT has hiGh sensitivity and specificity of diaGnosis of neonatal infectious diseases.

8.Differentiation of benign from malignant lesions of heart and pericardium: the feasibility of Fluorine-18 fluorodeoxyglucose positron emission tomography CT

Objective To assess the feasibility of integrated 18F-FDG PET-CT for the differentiation of malignancy from benign lesions of heart and pericardium. Methods A total of 23 cases (malignancy∶benign= 13∶10) with cardiac and pericardial lesions confirmed by pathology or clinic were analyzed in the present study. All lesions were evaluated semi-quantitatively using maximum standard uptake values (SUVmax) and SUVmax lesion/blood, and the density of the heart and pericardium lesions and the relationship with surrounding tissues were evaluated. The differences of SUVmax and SUVmax lesion/blood between benign and malignant lesions were analyzed using Mann-Whitney test. Subsequently, the diagnostic sensitivity, specificity, accuracy, positive predictive value (PPV) and negative predictive value (NPV) were calculated for CT and PET-CT respectively. Results The maximum SUV showed significant difference between malignancy(6. 5 ) and benign ( 1.5 ) ( Z = - 3. 601, P < 0. 01 ), the SUVmax Lesion/Blood of malignancy and benign were 3.4 and 0. 9 respectively, also with significant difference(Z = -3. 600, P <0. 01 ). The optimal cut-off value of SUVmax is 3.5-4. 0 and SUVmax Lesion/Blood is 1.3-2. 0. The sensitivity, specificity, accuracy, PPV and NPV of CT and PET-CT were 76. 9% ( 10/13 ), 100. 0% ( 10/10) ,87.0% (20/23), 100. 0% ( 10/10 ), 76. 9% ( 10/13 ) and 100. 0% ( 13/13 ), 90. 0% (9/10), 95.7%(22/23),92.9% (13/14),100.0% (9/9) respectively. Conclusion 18F-FDG PET-CT can correctly differentiate benignity and malignancy of cardiac and pericardial lesions.
9.Correlation between reduction quality of femoral neck fracture and femoral head necrosis evaluated by three-dimensional measurement method

BACKGROUND:Reduction quality is essential for the healing and function of limb. Traditional X-ray and CT scans have some limitationsin quantitatively and visualy observing the reduction quality of the remnant fracture in three-dimensional displacements. Mimics have advantages in bone rapid modeling and three-dimensional measurement, which offers the chance in quantifying the reduction quality of the displacement in three-dimensional methods.
OBJECTIVE:To measure residual displacement in femoral neck fracture patients with satisfactory reduction by using three-dimensional measurement method, and to evaluate the association between residual displacement and femoral head necrosis.
METHODS:We enroled 141 patients with femoral neck fracture, who were treated in the Third Department of Joint, Affiliated Hospital of Logistics University of People’s Armed Police Force from January 2012 to December 2013. Al patients were divided into undisplaced fracture group (n=44; Garden1&2) and displaced fracture group (n=97; Garden3&4). Based on DICOM data of CT files, three-dimensional models were constructed in each patient. Three parameters were measured: displacement of femoral head’s center, fossa capitis femoris and rotational displacement. Femoral head necrosis was folowed up. The correlation between residual displacement and femoral head necrosis was explored.
RESULTS AND CONCLUSION:(1) The duration of folow-up in those patients ranged from 25 to 43 months. Femoral head necrosis occurred in 7 cases from the undisplaced fracture group and 21 cases from the displaced fracture group. (2) The three parameters (displacement of femoral head’s center, fossa capitis femoris and rotational displacement) were respectively (7.44±2.84) mm, (11.75±4.12) mm and (17.68±8.99)° in the undisplaced group, and (6.51±3.75) mm, (9.34±5.27) mm and (14.67±7.43)° in the displaced group. Except displacement of femoral head’s center, significant differences in the other two parameters were detected between the two groups (P< 0.05). (3) These findings suggest that poor reduction quality is the main factor causing femoral head necrosis. X ray is insufficient to describe the spatial shift and the angle, and has limitations in assessing the reductionquality of femoral neck fracture. Three-dimensional measurement has certain advantages to assess quality of fracture reduction.
10.Research for GC fingerprint chromatography of Yujin Injection

AIM: To establish the fingerprint chromatography of Yujin Injection (Herba Houttuyniae, Flos Lonicerae) by GC. In the paper the authors examine the similarity among standard and sample chromatogra-phies. METHODS: GC was used to analyze the volatile ingredients, SGE 30QC3 colume (30m? 0.32mm? 0.5?m) was used with column temperature from 100℃ to 170℃ with 2℃?min -1 below 150℃ and 5℃?min -1 above 150℃, flow rate at 1.0mL?min -1 and detector temperature at 200℃. RESULTS: Standard fingerprint consisted of 17 marker peaks, the comparison of similarity's RSD to the injection of different batch had no more than 2%. CONCLUSION: According to the selected chromatographic conditions, a good fingerprint of the injection has been described. The method is simple, accurate with good reproducibility. It may be practical for the quality control of Yujin Injection.
