Cerebral small vessel disease (CVSD) is a group of small vascular diseases involving small arteries,arterioles,small veins,venules,and capillaries.The imaging findings were lacunar infarction,cerebral microbleeds,cerebral white matter lesions,and perivascular space expansion.In recent years,the incidence of CSVD is increasing,which brings different degree of economic burden to the families and society.It becomes the focus of research at present.The permeability change of blood-brain barrier is the main reason for the onset of CSVD.This article reviews the relationship between the integrity of blood-brain barrier and CSVD.

Objective To investigate the effect of cold atmospheric plasma (CAP) on the healing of skin ulcers using Balb/c mice. Methods Wounds with a diameter 6 mm were created on each side of the backs of BalB/c mice ( n = 150) using a punch bioptome. The mice were assigned randomly into a control group ( wounds healed naturally), a laser group (wounds treated with a He-Ne laser for 10 min daily) and a CAP group (wounds treated with CAP for 10 min daily). Wound healing was evaluated on postoperative days (PODs) 4, 7, 10 and 14 in terms of percent wound closure. Ten mice per group were sacrificed on each of the evaluation days. Both wounds were removed and a histological examination was conducted. A scoring system was used to evaluate the wounds. The expression of vascular endothelial growth factors (VEGFs) in the wounded tissue was detected by using immunohistochemical methods on POD 7. The results were quantified using an HPIAS-1000 system. Results Compared with the control group, the average percentage of wound healing was significantly greater in the CAP group on PODs 7 and 10. The average scores on the histological examination were significantly higher in the CAP group on PODs 7, 10 and 14. Compared with the other two groups, the expression of VEGF was up-regulated significantly in the CAP group.Conclusions CAP can positively affect the wound healing process. This might be related to the up-regulation of VEGF in the wounded tissues.

ObjectiveTo analyze the change in the incidence of papillary thyroid carcinoma (PTC) and follicular thyroid carcinoma (FTC) in the past 27 years and to probe the relationship between iodine and these carcinomas. MethodsStatistical analysis was performed on data of thyroid diseases, thyroid malignant tumors, PTC , FTC and their complications in Department of Pathology from 1978 to 2004 in General Hospital of Tianjin Medical University with Run test、Cox-Stuart test, ANOVA, t or t′ test and Chi-square test. ResultsIn 27 years there were 231837 cases of biopsies (average 8587cases/year). The biopsies were performed in the thyroid diseases as a whole 6276 cases (2.70%),thyroidmalignanttumors644cases(0.28%), PTCs 465 cases (0.20%), and FTCs 80 cases (0.03%) in 27 years. Biopsies in thyroid diseases as a whole showed a descending tendency since 1982 but an ascending tendency after 1995. Biopsy in malignant thyroid tumor did not show a significant change, and biopsy in PTC showed an ascending tendency and that in FTC showed a descending tendency after 1995. The average age, both in male and female patients, at the time of diagnosis of PTC and FTC showed no significant change during 27 years, but the mean age of male at the time of diagnosis was older than that of female. The ratios of male to female in PTC and FTC were 1:3.31、1:2.81 respectively,and showed no significant change either. There were more biopsies with lymph node metastasis in PTC (33.76%) than those in FTC (10.00%) and more biopsies with blood vessel involved or nodular goiter in FTC (11.25% and 13.75%) than those in PTC (0.65% and 4.52%). Mean age of patients with PTC and nodular goiter (44.3 years old) was older than those without goiter (38.1 years old). The ratios of male to female with PTC and FTC with lymph node metastasis were higher than those without lymph node metastasis. ConclusionThe incidence of thyroid malignant tumors in 27 years has no significant change. The contrary tendency of PTC and FTC suggests that the decreasing malignant degree of carcinoma. It suggests that this area is not in high iodine condition and the change have no relationship with the iodine intake.

Objective To construct an objective,comprehensive core competence evaluation index system for nursing postgraduates,and to provide evidence for postgraduates nursing education.Methods According to literature review,expert interviews,focus group discussions,Delphi and other research methods,we conducted two rounds of 32 experts consultation in 18 cities from 10 provinces to establish the final core competence evaluation index system.Results The two experts consultation derived core competence evaluation index system including 7 level-1 indexes,20 level-2 indexes and 53 entries.Level-1 indexes:theoretical knowledge and skills,critical thinking ability,research ability,clinical care ability,teaching skills,interpersonal communication skills,professional development skills.Conclusions This program is rigorous and reliable,with some theoretical and practical significance of its results,which provide a reference for the training program,evaluation of the quality,assessment of postgraduate nursing education.

Objective To assess the leading causes of death and disability adjusted life year (DALY) due to non-communicable disease (NCD) in China.Methods World Health Report 2004 published by the World Health Organization (WHO) was reviewed,including total deaths,deaths per 100 000,agestandardized death rate per 100 000,total DALYs,DALYs per 100 000 and age-standardized DALYs per 100 000 by cause and by member state.Diseases or injuries were assigned to three levels:communicable diseases,NCD and injuries (the first level); categories of disease or injure (the second level); specific diseases (the third level).R2.15 was used for data analysis.Results NCD causes 737.6 million deaths,141million total DALY years,627 age standardized mortality per 100 000,and 10 829 age-standardized DALYs per 100 000.NCD account for 79.4％ and 70.3％ total death or all-cause DALYs.Conclusions Cardiovascular disease,malignant neoplasm and respiratory disease were the leading causes of death,while neuropsychiatric disorder,cardiovascular disease and sense organ disease were the most important causes of DALYs.Among China,the United Kingdom,the United States,Canada,Japan,Korea and India,China ranked second in age-standardized mortality rate of chronic disease.DALYs of esophagus cancer and chronic obstructive pulmonary disease were 6 or 2 times of world average level.Besides,the increasing trend in the prevalence of diabetes mellitus remains impressive.

Objective To study the phenylpropanoid constituents from the roots of Euphorbia hylonoma Hand-Mazz. Methods The chemical constituents were isolated and purified by silica gel and Sephadex LH-20 column chromatography,and the structures were elucidated on the basis of chemical properties and spectral data. Results Three phenylpropanoid constituents were isolated from the acetone extracts of the roots of Euphorbia hylonoma Hand-Mazz,which were hexadecyl-3-methoxy-4-hydroxybenzeneacrylate Ⅰ,ethyl brevifolincarboxylate Ⅱ and(+) 3′-angeloyl-4′-isovalerylcis-khellactone peuformosin Ⅲ. Conclusion The above compounds were isolated from Euphorbia hylonoma Hand-Mazz for the first time,and the compound Ⅲ was the first obtained from the Euphorbiaceae.

Objective To analyze the consistency of individual examination diagnosis with terminologies expressed in the conclusion report of physical examinations by the chief inspection physician. Methods Based on the clinical classifications used in the Guidelines on Prevention and Treatment of Blood Lipid Abnormality for Chinese Adults, and related terminology descriptions of Dyslipidemias that were actually used in four-item blood lipid examinations diagnosis, a lexicon database of Hyperlipidemias was constructed with 39 terms used in the four-item blood lipid examination diagnosis and chief physician conclusions. Totally 11953 electronic chief physician inspection reports from 8 health check-up institutions were included. We investigated the terms of lexicon database using word frequency analysis method, calculated the positive rate in the diagnosis of four single examinations of serum lipid and the positive rate in the chief physician's conclusion. Consistency of chief physician's conclusion with single examination diagnosis was analyzed by Kappa test. Results (1) Among the 39 terms of lexicon database, there are 18 nonstandard terms used in single examination diagnosis, accounted for 46％ of all terms; (2) In word frequency analysis, there are only 1％ of terms that corresponded to clinical classifications of Hyperlipidemias accurately;(3) The positive rate of Hyperlipidemias in serum lipid four single examinations diagnosis was 47％, the positive rate of physician diagnosis was 35％. The consistency analysis of chief physician conclusions with single examination diagnosis showed Kappa=0.71(P<0.01). Conclusions Although the final conclusion written by the chief physician was not using standard terms strictly, most of synonymous terminology expressed in the physician conclusion is normative, which enabled the construction of lexicon database and text mining. Whether the single examination diagnosis are consistent with physician conclusions can be an evaluation indicator to assess the quality of chief physician conclusions for physical examinations. Kappa>0.75 may be suggested as a favorable value.

Objective To investigate the effect of recombinant murine interleukin-23(rIL-23)on systemic candidiasis in a murine model.Methods A cyclophosphamide-induced immunosuppressed murine model of systemic candidiasis was established.The mice were divided into control group and rIL-23 treatment group.Colony forming units(CFU)of the kidney and spleen were determined by using plating dilution method.The histopathological changes and degree of infection of the kidney and spleen were graded.Meanwhile,the levels of interferon gamma(IFN-γ)in the spleen were measured by enzyme-linked immunosorbent assay.Results On the 2nd,3rd and 7th day after Candida albicans infection the number of CFU of the fungi in the kidney in the control group was significantly greater than that in rIL-23 treatment group(P＜0.01).The number of CFU of the fungi on the 2nd,3rd and 7th day after Candida albicans infection in the spleen in control group was also greater than that in rIL-23 treatment group,but without statistically significant difference(P＞0.05).The scores of histopathological changes in the kidney in rIL-23 treatment group were lower than those in control group(P＜0.01),and the degree of infection was milder in rIL-23 treatment group.The scores of histopathological changes in the spleen in rIL-23 treatment group were also lower than those in control group,but without statistically significant difference(P＞0.05).The levels of IFN-γ in the spleen on the 2nd,3rd and 7th day after infection in rIL-23 treatment group were significantly higher than those in control group(P＜0.01).Conclusion rIL-23 has protective effect on murine systemic candidiasis.

In this study, the sterilizing effect of atmospheric pressure nonequilibrium plasmas (APNPs) on Neisseria gonorrhoeae (N. gonorrhoeae) was preliminarily examined and the possible mechanisms were explored. N. gonorrhoeae FA1090, FA19 and MS11 were treated by APNPs and their survival rate was analyzed by using CFUs counting and structurally studied by laser scanning confocal microscopy. The morphological changes of bacterial cell membrane and wall were studied under TEM. Our results showed that APNPs had strong sterilizing effect on N. gonorrhoeae. The survival rate of MS11 in N. gonorrhoeae liquid medium was 60.65% after disinfection with the APNPs for 5 min, whereas, the survival rate of FA19 was 92.60% and the rate of FA1090 was 96.40%. The survival rate of MS11 was 21.13% after exposure to APNPs for 6 min, whereas the survival rate of FA19 was 31.60% and the rate of FA1090 was 91.00%. N. gonorrhoeae was structurally damaged after treatment with APNPs. It is concluded that APNPs is able to effectively and quickly kill the N. gonorrhoeae, and the killing effect is related to the architectural damage of cell membrane.
