2.Influences of different mechanical ventilation modes on ventilator-induced diaphragm dysfunction

Objective To investigate the influences of different mechanical ventilation modes on ventilator?induced diaphragm dysfunction (VIDD). Methods The patients received invasive mechanical ventilation in our ICU after lower abdomen surgery were enrolled into 3 groups according to different ventilation modes. The data of the 3 groups were analyzed. According to the offline results, patients were divided into two groups, the success of weaning as the dependent variable, the clinical indicators with statistical significance as the independent variable, the Logistic regression analysis was performed to analyze the ventilator weaning. Results The mechanical ventilation time, 48 h reintubation rate and the length of ICU stay in the A/C mode group were significantly higher than those in the SIMV mode group and the PSV mode group. The weaning success rate in the A/C mode group was significantly lower than that in the SIMV mode group and the PSV mode group, with significant difference (P<0.05). Weaning success group and weaning failure group had significant differences in the proportion of three ventilation modes (P < 0.05). With weaning as dependent variable, APACHE Ⅱ score, ALB and ventilation mode as independent variables, logistic regression analysis was performed using the enter method. APACHE Ⅱ and ventilation mode were independent influencing factors for weaning. Conclusion Compared with the A/C mode, the SIMV mode and the PSV mode have less influence on VIDD and high weaning success rate in the mechanical ventilation process of patients post?lower abdomen surgery. No significant difference was found between the SIMV mode and the PSV mode in their influence on weaning.
3.Study on gatifloxacin in situ pH-sensitive gel release in vitro

purpose To investigate the release properties of gatifloxacin in situ pH-sensitive gel in vitro.Methods The improved paddle method and the membraneless model were applied in assessing the drug release behavior.Results The gel erosion and drug release were increased with the increase of surface area and shaking frequency.The cumulative quantities of gel erosion were well correlated with the cumulative release of drug loaded in the gel.Conclusion Gatifloxacin was released from in situ pH-sensitive gel with zero-order kinetics characters,and drug release was mainly controlled by gel erosion.
4.Progress in the application of rituximab in treating primary mediastinal B-cell lymphoma

Primary mediastinal large B-cell lymphoma (PMBCL) is morphologically similar to diffuse large B-cell lymphoma (DLBCL) and nodular sclerosis Hodgkin lymphoma.For most PMBCL patients, chemotherapy plus consolidation radiotherapy showed that the latter could improve PMBCL responsiveness and progression-free survival (PFS), and its combined use with chemotherapy demonstrated higher therapeutic efficacy.Recent clinical studies suggested that rituximab and anthracycline chemotherapy regimens could increase PMBCL treatment efficacy, reduce early treatment failure, enhance PFS and overall survival, and improve prognosis.Although rituximab combined with some high-intensity chemotherapy without radiotherapy have achieved good results, many studies still support the use of post-immunochemotherapy consolidation mediastinal radiotherapy.Based on the results of a few studies with a small sample size, patients who were assessed as complete metabolic remission by PET following high-intensity immunochemotherapy may omit consolidation radiotherapy.However, these results will need to be further confirmed by large-sample multicenter clinical trials.Consolidation radiotherapy is recommended for patients with poor prognostic factors or PET score>3.
5.Orthodontic force effects on the expression of periodontal interleukin-6 mRNA in rats

BACKGROUND:As an important cytokine, interleukin-6 regulates immune responses in inflammation sites and has an autocrine/paracrine activity that stimulates osteoclast formation and bone resorption, which is related to bone remodeling during orthodontic tooth movement.
OBJECTIVE:To investigate the effects of orthodontic force on the expression of interleukin-6 mRNA in the periodontal tissue of rats.
METHODS:In situ hybridization was performed to measure the expression of interleukin-6 mRNA at 1, 3, 5, 7, 10 and 14 days after the application of orthodontic force on the maxil ary first molars of rats.
RESULTS AND CONCLUSION:The expression of interleukin-6 mRNA was observed at a low level in the normal periodontal tissue of rats. After the application of force, the induction of interleukin-6 mRNA was observed to reach a maximum on day 3 and to decline thereafter. The expression of interleukin-6 mRNA can be evoked by orthodontic force but with a certain self-limiting. As a multifunctional cytokine, interleukin-6 plays a very important role in periodontal remodeling during orthodontic tooth movement.
6.Research advances in radiation?induced heart disease

Radiation?induced heart disease ( RIHD) is a common type of radiation?induced damages in chest radiotherapy. There are no obvious short?term symptoms in patients with RIHD. However, RIHD causes irreversible permanent damages to the heart over time, which undermines the quality of life. Patients with severe RIHD even have a risk of death from myocardial infarction caused by coronary atherosclerosis. This paper summarizes the research advances in epidemiology, diagnosis, mechanisms of radiation?induced injury in various parts of the heart, radiotherapy techniques, and treatment. Reduction in radiation range and dose, early diagnosis, and early treatment are recommended for patients to reduce heart injury and improve the quality of life.
7.Research progress of oxytocin receptor ligands and its role in central nerve system and peripheral organs

Oxytocin ( OT ) is a cyclic neuropeptide containing nine amino acids residues, in addition to the traditional roles of uterine contraction and lactation, it also plays important roles in the central nervous system and other peripheral organs, such as improving schizophrenia, autism - related psychiatric and psy-chological symptoms. Oxytocin exhibits its physiological func-tions by binding to its receptor (oxytocin receptor,OTR). Cur-
rently researchers are manipulating OT system by developing new OTR ligand ( agonists and antagonists ) , hoping to prevent and treat OTR related diseases. This paper reviews the latest devel-opment of OTR agonists, antagonists and its physiological roles in central nerve system and peripheral organs.
8.Effect of hyperoxygenated solution on myocardial injury in rats with acute carbon monoxide poisoning

Objective To evaluate the effect of hyperoxygenated solution on myocardial injury in the rats with acute carbon monoxide (CO) poisoning.Methods Thirty pathogen-free adult male SpragueDawley rats,weighing 250-300 g,were randomly divided into 5 groups (n=6 each) using a random number table:control group (C group),acute CO poisoning group (ACP group),and different doses of hyperoxygenated solution groups (HP1-3 groups).CO 120 ml/kg was injected intraperitoneally to establish the model of acute CO poisoning.Hyperoxygenated solution 10,15 and 20 ml/kg were infused via the caudal vein at 1 h after intraperitoneal injection of CO in HP1-3 groups,respectively.At 24 h after intraperitoneal injection of CO,blood samples were collected from the caudal vein for determination of plasma creatine kinase (CK),creatine kinase-MB (CK-MB),lactic dehydrogenase (LDH) and alpha-hydroxybutyrate acid dehydrogenase (α-HBDH) activities using the automatic biochemical analyzer.The rats were then sacrificed,and myocardial specimens were obtained for examination of the pathological changes with a light microscope.Results Compared with group C,the plasma LDH,α-HBDH,CK and CK-MB activities were significantly increased in ACP and HP1-3 groups (P<0.01).Compared with group ACP,the plasma LDH,α-HBDH,CK and CK-MB activities were significantly decreased in HP1-3 groups (P<0.05 or 0.01).Compared with group HP1,the plasma LDH,α-HBDH,CK and CK-MB activities were significantly decreased in HP2,3 groups (P<0.05).The pathological changes of myocardium were significantly attenuated in HP1-3 groups as compared with group ACP.Conclusion Hyperoxygenated solution can attenuate myocardial injury in the rats with acute CO poisoning.
9.Effects ofDanshen Tongluo Detoxication Decoction on Inflammatory Response of Rats with Bone Marrow Stem Cell Transplantation in Acute Myocardial Infarction

Objective To explore the effects ofDanshen Tongluo Detoxication Decoction on the inflammatory response of rats with bone marrow stem cell (BMSC) transplantation in acute myocardial infarction (AMI); To discuss its mechanism of action.Methods The whole bone marrow adherent method was adopted for BMSCs separation and culture. The AMI model was established by closing the left anterior descending coronary artery of SD rats. After the modeling, SD rats were randomly divided into sham-operation group, model group and BMSCs group (BMSCs transplantation group),Danshen Tongluo Detoxication Decoction group, Danshen Tongluo Detoxication Decoction + BMSCs group, 10 rats in each group. BMSCs cell suspension was injected directly into the edge of the myocardial tissue infarction area; Chinese medicine or normal saline were administered for gavage. 4 weeks later, the contents of MCP-1 and sICAM-1 in each group were detected by ELISA. TLR-4 expression was measured by Western blot method, and HE staining was used to observe the myocardial tissue pathological changes.Results Compared with the model group, the contents of MCP-1 and sICAM-1 and the protein expression of TLR-4 in BMSCs group,Danshen Tongluo Detoxication Decoction group, andDanshen Tongluo Detoxication Decoction + BMSCs group decreased, Danshen Tongluo Detoxication Decoction group was better than BMSCs group (P<0.05,P<0.01), andDanshen Tongluo Detoxication Decoction + BMSCs group was better than BMSCs group andDanshen Tongluo Detoxication Decoction group(P<0.05,P<0.01).Conclusion BMSCs transplantation combined withDanshen Tongluo Detoxication Decoction can restrain the inflammatory response of AMI model rats and repair ischemic myocardium issue, which mechanism may be related to regulating TLR-4 induced inflammatory response.
10.Relationship between the methylation status of STAT3 promoter DNA in peripheral blood CD4+ T cells from patients after allo-HSCT and aGVHD

Objective:To study the relationship between acute graft versus host disease (aGVHD) and the methylation status of the STAT3 promoter in peripheral blood CD4+ T cells from patients after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods:We collected 40 patients who underwent allo-HSCT from HLA-identical sibling donors.Serum IL-10,TGF-β1,IL-17A and IL-17F levels were detected by ELISA.Foxp3 cytotoxic T-lymphocyte-associated protein 4 (CTLA4),IL-10,TG F-β 1,RO Rγt,IL-17A and IL-17F mRNA levels in CD4+ T cells were measured by real-time PCR.STAT3 expression levels were detected by real-time PCR and Western blot,and promoter DNA methylation was analyzed by bisulfite sequencing PCR (BSP).Results:IL-10 and TGF-β1 levels were significantly down-regulated,while IL-17A and IL-17F levels were significantly up-regulated in patients with aGVHD compared with patients without aGVHD.Foxp3,CTLA4,IL-10,TGF-β1 mRNA levels were significantly down-regulated,while RORγt,IL-17A,IL-17F mRNA levels were significantly up-regulated in patients with aGVHD compared with patients without aGVHD.STAT3 expression was increased,while STAT3 promoter DNA was hypomethylated in patients with aGVHD compared with those without aGVHD.The STAT3 mRNA level was negatively correlated with STAT3 promoter DNA methylation.Conclusion:The imbalance of Treg/fh17 in CD4+ T cells from patients after allo-HSCT is a key factor for triggering aGVHD,and the DNA hypomethylation of STAT3 promoter could promote its expression in CD4+ T cells and contribute to the imbalance.
