1.Transplantation of mobilized peripheral blood mononuclear cells for peripheral arterial occlusive disease of the lower extremity

Objectives To assess the clinical efficacy, safety, and feasibility of autologous transplantation of mobilized peripheral blood mononuclear cells (PBMNCs) for patients with peripheral arterial occlusive disease (PAOD) of the lower extremity. Methods A total of 152 patients with PAOD of the lower extremity were enrolled into this non-controlled observational study from November 2003 to March 2006. All patients received subcutaneous injections of recombinant human granulocyte colony-stimulating factor (G-CSF, 450600 μg/day) for 5 days in order to mobilize stem/progenitor cells; their PBMNCs were collected and transplanted by multiple intramuscular injections into ischemic limbs. Patients were followed up for at least 12 weeks. Results At 12 weeks, primarymanifestations,including lower limb pain and coldness, were significantly improved in 137 (90.1%) of the patients; limb ulcers improved or healed in 46 (86.8%) of the 53 patients, while 25 of the 48 (47.9%) patients with limb gangrene remained steady or improved. Ankle-brachial index (ABI) improved in 33 (22%) of the cases, and TcPO2 increased in 45 (30%) of the cases. Angiography before treatment, and at 12 weeks after treatment, was performed in 10 of the patients and showed formation of new collateral vessels. No severe adverse effects or complications specifically related to cell transplantation were observed. Conclusion Autologous transplantation of G-CSF-mobilized PBMNCs might be a safe and effective treatment for lower limb ischemic disorder.(J Geriatr Cardiol 2006; 3:178-80.)
2.Clinical study on the use of the curved cutter stapler in anterior resection of rectal cancer

Objective To evaluate the application of curved cutter stapler(Contour TM)in the anterior resection of rectal cancer.Methods The clinical data of 57 patients of rectal cancer, who received anterior resection with the curved cutter stapler during the period between November 2005 and August 2006,were analyzed retrospectively.Of the 51 cases who received double stapling technique anterior resection.Of the 6 patients who underwent Hartmann procedure.Results Among above 51patients,41 patients(80.4%)were uhralow anastomosis and no local recurrence occurred.Anastomotic leakage was found in 1 patient,anastomotic bleeding occurred in 3 patients and rectovaginal fistula in 2.In the 6 patients who received Hartmann's procedure,the average distance from cutting and closing line to anal verge was 2.8 cm.Conclusion The ContourTm curved cutter stapler is superior in the anus-preserving surgery of lower rectal cancer when compared with linear stapler.

4.Anatomical study on Jingming (BL 1).

OBJECTIVETo explore the anatomical structures, and depth and direction of needling at Jingming (BL 1), so as to provide anatomical basis for its clinical application.
METHODSForty-eight adult orbital specimens were observed by dissection.
RESULTSWhen a needle was vertically inserted into Jingming (BL 1), the needle tip will past through the skin, subcutaneous tissue, medial palpebral ligament, medialis rectus and orbital adipose body. Above the body of the needle, there are ophthalmic artery, anterior ethmoidal artery and nasociliary nerve. The average distance between the skin at the punctured point and the anterior ethmoidal artery is (18.25 +/- 4.45) mm, with an angle of (12.5 +/- 5.5) degrees, and the average distance between the skin at the punctured point and the optic nerve tunnel frontal point is (43.37 +/- 7.84) mm.
CONCLUSIONTo avoid bleeding caused by injuring the anterior ethmoidal artery, acupuncture at Jingming (BL 1) should avoid deeply inserting needled back-upwards and upwards, and the needling depth should not exceed 30.36 mm to avoid injury of the optic nerve tunnel frontal point.

5.Treatment of infantile asthma in remission stage with Chinese medicine and new moxibustion-massage apparatus.

OBJECTIVETo compare differences of effects between new moxibustion-massage apparatus and suspended moxibustion for infantile asthma.
METHODSSixty patients were divided into 2 groups randomly, observation group and control group, 30 cases in each. Tiaoyuan Yuping Decoction, a decoction made by the hospital, was applied for both groups. New moxibustion-massage apparatus was applied for the observation group, and suspended moxibustion was adopted for the control group. Moxibustion was applied on acupoints of Fengmen (BL 12), Feishu (BL 13), Geshu (BL 17), Pishu (BL 20), Juque (CV 14) and Zhongwan (CV 12) for 3 months, and therapeutic effect was observed.
RESULTSThe remarkably effective rate of the observe group is 66.7% (20/30) and the total effective rate is 90.0% (27/30), which are better than 40.0% (12/30) and 83.3% (25/30) of the control group. The asthma of attack of both groups after the treatment obviously decreased (both P < 0.05). The asthma of attack of the observation group is obviously lower than that of the control group (P < 0.05). And the effect is approved without side-effects such as dermal infection.
CONCLUSIONIt is held that the combination of new moxibustion-massage apparatus and Chinese medicine for infantile asthma in remission stage has obvious therapeutic effect, which can reduce the attack of asthma and alleviate symptoms. The result is better than suspended moxibustion.

6.Study on the HIV infection status, characteristics of AIDS-related behaviors and risk factors among men attending the sexually transmitted disease clinics

Objective To understand the status on AIDS awareness,AIDS-related behaviors,risk factors on HIV infection status among 15-90 years or older men at the sexually transmitted disease clinics.Methods Data from the 2009 and 2010 national sentinel surveillance system,regarding men attending sexually transmitted disease clinics was collected from Guangdong,Guangxi,Henan,Sichuan,Yunnan and Jiangxi provinces,where the AIDS epidemic among 15-90 years or older population was serious.Data was uploaded to National Center for AIDS/STD Control and Prevention (NCAIDS) through the comprehensive AIDS control and prevention information system.Data was then analyzed by SPSS 18.0.Results A total of 64 003 pieces of data were collected.Among them,8783 ( 13.7% ) were related to men at ≥ 50 years or older,and to men 15-49 years older were 55 220.The rates on the awareness of AIDS knowledge were from 69.6% vs.80.1%,on frequently having had commercial sexual contacts in the last three months were between 34.1%vs.36.6%,on having had casual sexual contact in the last three months were 18.7% vs.28.4%,on having had homosexual anal intercourse as 0.7% vs.1.4%.The rates of taking HIV antibody testing in the last year (14.3% vs.17.1% ) among this population were all significantly lower than the rate among the 15 to 49 years age group.However,the HlV-positive rate among the older age group (fifty years of age or older) was significantly higher than the rate among 15 to 49 year age group ( 1.1% vs.0.7% ).Regard the fifty years of age or older men.Data from the multivariate logistic regression analysis demonstrated that factors as:having regular partner (OR=0.588,P=0.034),having homosexual anal intercourse (OR=5.226,P=0.006) were associated with positivities of HIV antibody.Conclusion High-risk sexual behaviors,including homosexual anal intercourse were the major risk factors for men at ≥50 years or older age,related to the infection of HIV.

8.Celastrol in the inhibition of neovascularization.

OBJECTIVETo study the inhibition effect of celastrol on neovascularization.
METHODSThe effect of celastrol on the in vitro proliferation of endothelial cell of vessel (ECV) was examined by MTT assay. The effect of celastrol on endothelial cell migration, tube formation on Matrigel and Chick chorioallantoic membrane angiogenesis was also examined. Matrigel plug assay was used to evaluate the effect of celastrol on angiogenesis in vivo.
RESULTSThe proliferation of ECV was inhibited significantly by celastrol with IC(50) being 1.33 microg/ml. Celastrol inhibited endothelial cell migration and tube formation in a dose-dependent manner. Celastrol also inhibited angiogenesis both in Matrigel plug of mouse model and in chick chorioallantoic membranes.
CONCLUSIONCelastrol, which can inhibit angiogenesis, could be developed as an antiangiogenic drug.

10.The study of combining high-risk human papillomavirus types checking and cytologic test in the screening of cervical lesions

Objective To study the relationship between thinprep cytologic test and the types of human papilloma virus (HPV) infection in cervical precancerous lesion screening. Methods To perform high-risk HPV types test in 1375 samples. Choose 256 positive samples to take thinprep cytologic test (TCT) and directed biopsies under colposcopy. Adopting two-channels real time PCR to genotype and quantify eight high risk HPV DNA (high risk types: HPV 16,18,45,31 ; intermediate risk types: HPV 33,52,58,67). Results There are 256 positive samples in High risk HPV DNA test ( 18. 62% ). WNL rate for TCT is 16.41% (42/256), ASCUS and above rate for TCT is 83.59% (214/256). There is no statistically significant difference in the viral loads of HPV infection rate between the TCT negative patients and positive patients ( P > 0. 5 ). Positive correspondence rate for TCT and biopsy are 92. 86% ( 39/42),81.36% (48/59) ,85.19% (23/27)and 9/10. Conclusion High-risk HPV types checking combined with TCT and biopsy can raise positive rate significantly. It should be used as a reliable method for early diagnosis in cervical cancer and CIN screening.
