1.The application of the post crowns with wings to the restoration of the subgingival residual roots and residual crowns of anterior maxillary teeth

Objective:To presents a new way of constructing post crowns for the anterior maxillary teeth which had the subginginval crown fracture more than 3 mm.To discusse the difficulties in the restoration of the subgingival residual roots and residual crowns of anterior maxillary teeth,and to explore the principles of the new restoration by the casting post-core and the porcelain fused mental crowns with wings. Methods: 23 patients(26 teeth) which had crown fracture more than 3 mm under the gingival were enrolled in this study.After those residual roots and crowns were endodontically treated following the conventional root canal therapy,the casting post-core and the porcelain fused mental crowns with wings were applied to reconstructing those missing crowns. Results: After 3 months to 5 years clinic observation,restored crowns were still stable with good retention and fracture resistance. Conclusion: Those residual roots and crowns can be preserved by this innovative way and can also acquire satisfactory effects in esthetics and functions.
2.Characters of occlusal contact in patients with severe tooth wear

Objective To analyze the characters of occlusal contact in patients with severe tooth wear. Methods The characters of occlusal contact in 20 cases with severe tooth wear and 20 persons with normal occlusion were measured with T-Scan Ⅱ occlusal analysis system. Results The offset degree in quantitative analysis showed that the offset in transversal and offset distance of center of force in the group with severe tooth wear[ ( 10. 5 ± 1.5 )mm, ( 12. 0 ± 1.3 ) mm ] were larger than control group [ (4. 5 ± 0. 8 )mm,(6. 5 ±0. 8)mm]. Occlusion time in severe tooth wear group[ (0.45 ±0.07)s]was shorter than control group(o)(0.78 ±0.06)s]. Lateral disclusion time in severe tooth wear group[ ( 1. 50 ±0. 08)s, ( 1.50 ±0.10) s ] was longer than control group [ ( 0. 93 ± 0. 07 ) s, ( 0.90 ± 0.11 ) s ]. Incidence rate of lateral occlusal interference in severe tooth wear group(70%) was higher than control group( 10%) (P <0.05). Offset in vertical of center of force [ ( 4.9 ± 1.0 ) mm ], protrusive disclusion time [( 1.02 ± 0. 09 ) s ], incidence rate of premature contact and protrusive occlusal contact interference( 20% ,30% ) showed no significantly statistical difference between two groups [ (4. 7 ± 0. 6 ) mm, (0. 96 ± 0. 06 ) s, 10% , 10% ] ( P >0. 05). Conclusion There were some differences in the characters of occlusal contact in patients with severe tooth wear.
3.Alterations in levels of CD8+ CD28- regulatory T cell in the peripheral blood of multiple sderosis patients

Objective To investigate the levels of peripheral blood CD8+ CD28- regulatory lymphocytes and their clinical values in the patients with multiple sclerosis (MS). Method From October 2005 to August 2008, 51 patients with active rehpsing-remitting MS were enrrolled from Department of Neurology of the First Affil-iated Hospital of Wenzliou Medical College. The diagnostic criteria for MS were the 2005 revisions to the "McDon-ald criteria". All the admitted patients received 1 g of methylprednisoione per day intravenously for 5 days, fol-lowed by 60 mg prednisone per day orally for 12 days,and tapered in 6 weeks. Fourteen patients were reevaluated after corticosteroid therapy. Twenty healthy individuals ,as normal controls,matched for age and sex with the MS patients were also enrolled in this study. The percentages of peripheral blood T cells (CD8+ CD28-, CD8+CD28+, CD8+, CD4+ CD8-) were measured by flow cytometric analysis. Parametric statistical analysis were per-formed using standard methods, and linear regression analysis was conducted using Pearson correlation test. Re-sults (1)Compared with controls,the patients with active MS had significantly lower percentage of CD8+ CD28-T cells [(18.48±9.89)% vs. (24.48±4.86)%, P <0.01], and higher percentage of CD8+ CD28+ T cells [(12.23±4.31) % vs. (8.55±3.49) %, P <0.01]. (2)The percentage of CD8+ CD28- T cells was negative-ly correlated with that of CD4+ CDS- T cells (r = -0.488, P < 0.01). (3) After corticosteroid therapy, the per-eentage of peripheral blood CD8+ CD28- / CD8+ CD28+ T cells didn' t significantly decrease or increase in 14 ac-tive MS patients (P > 0.05). Conclusions The decrease of peripheral blood CD8+ CD28- regulatory T cells might be associated with the pathogenesis of MS, and CD8+ CD28- regulatory T cells perhaps played their roles through CD4+ T cells. Corticosteroid therapy could not reverse the levels of CD8+ CD28- T cells.
4.Practice and experience of clinical teaching in pediatrics

Department entrance education will encourage the students to study pediatrics. Clinical teaching includes performing ward inspection and clinical practice, moreover, the teaching should be based on how to solve clinical problems and focused on pediatrics characteristics. The practice and experience shows that clinical study in department of pediatrics wiu improve the students' comprehensive ability and help them lay a solid foundation for further work.

6.Superselective renal artery embolization for traumatic renal hemorrhage: report of 38 cases

Objective To evaluate the clinical application of superselective renal artery embolization for the treatment of traumatic renal hemorrhage. Methods Thirty eight patients with traumatic renal hemorrhage were first demonstrated by renal arterigraphy under seldinger technique to ensure the rupture site and degree of renal artery and then followed by percutaneous cathetherized superselective renal arterial embolization through guidance of super slippery guide wire with gelfoam particles or steel coil. Results All 38 patients revealed occlusive abruption of bleeding arteries and disappearance of vextravasation staining after the superselective catheterized renal arterial embolization with gelfoam particles and steel spring coil under DSA. Clinically, all patients urine turned clear without macroscopic hematuria during 1 - 3 days after the procedure. Conclusions Superseclective renal arterial embolization is less aggressive and simultaneously with less complications, good hemostatic efficacy, maximal preservation of renal tissue and function for renal traumatic hemorrhage, which is worthy to be recommended.
7.Efficacy observation of occlusal rehabilitation in aged people with serious attrition

Objective:To evaluate the efficacy of occlusal rehabilitation in old patients with decreased vertical dimensions caused by serious attrition.Methods:Sixty-five patients with decreased vertical dimension caused by serious attrition were divided into 3 groups according to the defect of dentition and/or TMD.Group A included 42 patients with serious attrition and defection of dentition.Group B had 15 patients with heavy tooth wear,defection of dentition and TMD.Group C included 8 patients with serious attrition and functional TMD.All patients were conducted occlusal rehabilitation at increased vertical dimensions in normal intercuspital position(ICP) by removable denture with occlusal pad and removable occlusal splints.The time of follow-up visit of these patients ranged from 1.5 months to 9 years. Results:The masticatory efficacy of the patients was obviously improved,and all the symptoms of TMD were relieved.Furthermore,the symptoms of TMD were not observed after the treatment in patients without TMD.Conclusion:The results achieved by removable occlusal rehabilitation to aged people with serious attrition and decreased vertical dimension are satisfactory,and suggest that the occlusal rehabilitation is necessary for them.Furthermore,the removable occlusal rehabilitation can be acted as a reference and transition to permanent fixed occlusal rehabilitation.

9.EVALUATION OF BIOAVAILABILITY OF IRON IN Fe-ENRICHED YEASTII. FEEDING STUDY IN PRESCHOOL CHILDREN

135 children, aged 18-36 months, were selected from two nurseries and divided into 3 groups. To children in group A, Fe-enriched yeast fortified "Bao Bao Le" formula powder (40+5g per capita per day) was given instead of soybean milk and soybean curd in the ordinary nursery diet. Unfortified "Bao Bao Le" was given to group B as another test food. Children in group C, serving as control, were given ordinary nursery diet. This experiment lasted for three months. Dietary survey, Hb, FEP, SF, serum uric acid concentration, vitamin B2 and C loading tests (4 h) , and anthropometric measurements were determined before and after the experiment.No statistically significant differences were observed in nutrient intake, vitamin loading tests and serum uric acid (seeking for nuclecic acid metabolite) in each group before and after the experiment. At the end of the experiment, changes of Hb, FEP, SF concentration and FEP/Hb indicated that iron status of children in group A was much better than that in groups B and C, with no significant differences between the latter groups. Weight gain and height increment of children in group C were much less than those in groups A and B, with no significant differences between the latter groups too. Therefore we conclude that the improvement of iron status in group A is due to the effect of Fe-enriched yeast contained in the fortified "Bao Bao Le" formula powder.
10.Neonatal tuberous sclerosis complex in 8 cases

Objective To investigate the clinical characteristics of neonatal tuberous sclerosis complex (TSC).Methods A total of 134 patients were admitted and diagnosed as TSC in Beijing Children′s Hospital,Capital Medical University from September 2006 to September 2015.The clinical characteristics of TSC in 8 patients who had clinical symptoms initiating from the neonatal period (skin lesion,neurologic abnormality,etc.) were analyzed,so as to conduct the auxiliary examinations (skin biopsy,ultrasonic cardiogram,cranial imaging) and the follow-up results.Results Among 8 patients,4 were diagnosed as TSC in the neonatal period while other 4 were diagnosed at later period from 4 months to 14 years old.Six patients had skin lesions in neonatal period,accounting for 75% of 8 patients,with predominant symptoms:hypomelanotic macules (5 cases),angiofibroma (2 cases),and shagreen patch (1 case).Cardiac abnormalities in neonatal period mainly included cardiac rhabdomyoma,which were characterized by high incidence,multi-regional occurrence,and wide distribution,noticed in 4 patients (50%).Ultrasonic cardiogram showed a mass in 4 patients with moderate-strong echo,uniform texture and clear boundary,and slight effect on tricuspid valve blood flows (in 1 case).Electrocardiogram abnormalities were found in 1 case,like atrial premature beats with intraventricular aberrant conduction,and accelerated atrial escape.Neurologic abnormality of neonatal TSC could present convulsive seizures.Cranial imaging lesions showed subependymal nodules (3 cases),leukodystrophy (2 cases),and giant-cell astrocytoma (1 case).Evidence for TSC2 gene positive and heterozygous mutation was identified in 1 case.The nucleic acid mutation site was at c.268C>T (E4),and the amino acid mutation was p.90Q>X.The mutant effect was nonsense mutation,which could lead to premature termination of protein translation.Conclusion The clinical characteristics of neonatal TSC may involve multi-system lesions,most commonly seen in the skin,cardiac,and ner-vous system.Neonatal physical examinations,cranial CT/magnetic resonance imaging and cardiac ultrasound screening should be done for the suspected TSC patients,and genetic diagnosis may contribute to the early diagnosis of the disease.
