Chinese Journal of Ultrasonography 2003;0(11):-
Objective To compare the reduction in left ventricular hypertrophy (LVH) caused by monotherapy with ramipril or losartan or the combination of these two drugs,in patients with hypertensive LVH and to evaluate the value of three-dimensional echocardiography (3DE) on LVH regression. Methods Ninety-six essential hypertensive patients with LVH were enrolled in this study. All patients were randomly divided into 3 groups: ramipril group, losartan group, combination group with ramipril and losartan.Left ventricular mass(LVM) was calculated by M-mode echocardiography,two-dimensional echocardiography and 3DE at baseline and after 6 months treatment with ramipril or losartan. Results Of 96 patients enrolled,59 patients have complete follow-up datum.LVM of ramipril, losartan and combination group decreased 16.9% , 17.9% and 18.8% compared with before treatment.LVM by 3DE decreased 11.6% , 14.7% and 13.6% respectively. The difference of LVM derived from 3DE showed obviously lower than derived from M-mode echocardiography. Conclusions Applying M-mode echocardiography and two-dimensional echocardiography to evaluate the regression of LVH will overestimate the effects of drugs.Three-dimensional echocardiography may detect the changes of LVM more accurately.

Methods The serum levels of IL-32 and chemokine CXCL13 were meas-ured by ELISA methods in 102 patients with RA and 50 healthy controls;analyzed the correlation among the IL-32, chemokine CXCL13 and the clinical data, inflammation maker of RA, such as X-ray stage, the bone destruction, the number of tender joints, the number of swollen joints, DAS28,ESR,CRP,CCP,RF,TNF-α,IL-6, etc. Results The serum levels of IL-32 and chemokine CXCL13 were significantly higher in RA patients than that in healthy controls(P<0.05). Moreover, the serum levels of IL-32 and chemokine CXCL13 those patients who had bone de-struction were higher than those with no or mild bone destructions(P<0.05);there was a positive correlation be-tween IL-32, chemokine CXCL13 and X-ray stage, the bone destruction, DAS28, ESR,CRP,CCP,TNF-α,IL-6. Especially, there was a positive correlation between IL-32 and chemokine CXCL13 (r=0.509,P=0.000). How-ever, there was no relationship with the number of swollen joints, tender joints, and RF. Conclusion The serum levels of IL-32 and chemokine CXCL13 were increased significantly, which was closly related with the disease activ-ity and inflammation maker. The results indicate that the abnormality of IL-32 and chemokine CXCL13 may be use-ful for the evaluation of disease activity and monitoring the progress of the disease.

70% . The patients were mechanically ventilated. VT was set at 10 ml? kg-1 and respiratory rate at 12 bpm. End-tidal PCO2 was maintained at 30-35 mm Hg. End-tidal PCO2, SpO2, ECG and BP were continuously monitored. The respiratory function and mechanics were measured using respiratory monitor CP-100 (BICORE) before and after pneumoperitoneum (intra-abdominal pressure reached 15 mm Hg) and 5,10,15,20,25,30,35 min after vecuronium. The respiratory parameters measured included inspiratory and expiratory VT ( VTi, VTe) , minute ventilation (VE), respiratory rate (RR), peak inspiratory and expiratory flow rate (PIFR,PEFR), PEEP, auto-PEEP, pressure-time product (PTP), inspiratory-time ratio (TI/TTOT ), rate/VT ratio, average airway pressure (PAWM), esophageal pressure (PES), peak inspiratory pressure (PIP), dynamic compliance (CDyn), airway resistance (RAw ) and work of breathing. Results There was no significant difference in the effect of different doses of vecuronium on respiratory function and mechanics including all parameters measured at all time points among the three groups. Mean airway pressure, esophageal pressure peak inspiratory pressure, airway resistance and work of breathing increased significantly but dynamic compliance decreased significantly after CO2 penumoperitoneum. Conclusion Intra-abdominal CO2 insufflation significantly attect respiratory mecnanics. me effect of pneumoperitoneum on respiratory mechanics can not be prevented or attenuated by increasing the dose of muscle relaxant.
