Objective To investigate and compare the diagnostic value of fine needle aspiration biopsy(FNAB) and core needle biopsy(CNB) in the treatment of patients with invasive breast cancer.Methods Given 100 cases of patients with newly diagnosed breast cancer biopsy in our hospital from November 2015 to November 2016,50 cases of patients with FNAB into the observation group,50 cases of patients with CNB into the control group.Taken the pathologic results as a diagnostic gold standard and compared the diagnostic value of two diagnostic methods in invasive breast cancer.Detected the human epidermal growth factor receptor 2(Her-2),estrogen receptor(ER) and progesterone receptor(PR) preoperatively and compared the adverse reactions of puncture of patients in two groups.Results 30 cases of patients in the observation group showed axillary lymph node metastasis and 20 cases of patients had no axillary lymph node metastasis,21 cases of patients were positive cases of axillary lymph node metastasis accepted FNAB examination,the sensibility was 70.00％.29 cases of patients in the control group showed axillary lymph node metastasis while 21 cases of patients didn′t have axillary lymph node metastasis,23 cases of patients were positive cases of axillary lymph node metastasis accepted FNAB examination,the sensibility was 74.19％.Compared the ER、PR、Her-2 antibody with the testing result of primary tumor after operation.The diagnostic accordance rates in the observation group were 93.55％,91.67％,92.86％ respectively.The diagnostic accordance rates in the control group were 94.12％,92.31％,86.67％,the differences in two groups were not statistical significance(P>0.05).The adverse reaction rate was 4.00％ in the observation group and 0.00％ in the control group after puncture.Conclusion The diagnostic sensibility of FNAB is poorer than CNB in the diagnostic procedure of invasive breast cancer.ER,PR,Her-2 in two diagnostic methods keep a high consistency with the testing result of primary tumor after operation.

BACKGROUND: Blood vessel remodeling is the basis of the remodeling of orthodontic periodontal tissues. Can He-Ne laser irradiation promote blood vessel remodeling and then accelerate the process of the remodeling of orthodontic periodontal tissues or not?OBJECTIVE: To investigate the effects and action mechanisms of He-Ne laser irradiation on the blood vessel remodeling of orthodontic periodontal tissues of rabbits.DESIGN: Randomized controlled study.SETTING: Animal experimental center of Jilin University.MATERIALS: Thirty-five flap-eared rabbits weighting 2.0 kg were provided by Animal Experimental Center of Jilin University, license No. SYXK(Ji)2002-0002, production license No. SCXK2003-0003. Animal intervention met animal ethical standard. HNGSQ-1 type of He-Ne laser therapeutic instrument was produced in Shanghai. HPIAS-1000 image analytical system was produced by Tongji Medical University. CD34 monoclonal antibody with the product number of MAB-0034 and vascular endothelial growth factor monoclonal antibody with the product number of MS-350 were offered by Fuzhou Maixin Company.METHODS: Experiments were performed at the Provincial Laboratory of Animal Experimental Center of Jilin University from May to December 2003. ①Thirty-five flap-eared rabbits were randomly divided into seven groups, an unforced group and forced 1-, 3-, 5-, 7-, 14- and 21-day groups with 5 rabbits in each group. A stainless steel closing coil spring was deposited between the maxillary first molar and the corresponding maxillary incisor bilaterally, delivering 0.08 N force. The right maxillary of rabbits of each forced group was irradiated by He-Ne laser, and the left side was as control.The cheek corresponding to the right maxillary first molar was selected as irradiation spot and then received He-Ne laser local irradiation. He-Ne laser's wavelength is 632.8 nm, power output of 20.0 mW and energy density of 2.50 J/cm2.Animals of the unforced group did not proceed with anything, sacrificed after raising for a week. Animals of the forced groups were respectively sacrificed on days 1, 3, 5, 7, 14 and 21. ②The premaxillary bones, including the upper first molar, were cut into sagittal sections. Sections were preceded with CD34 and vascular endothelial growth factor immunohistochemical straining. Microvessel density and microvessel area were measured in the tension zone and the pressure zone of orthodontic periodontal tissues. The staining intensity for vascular endothelial growth factor was evaluated by the measurement of gray value using the Computer Image Analyzing System.MAIN OUTCOME MEASURES: ①Gray integral of vascular endothelial growth factor after immunohistochemical staining. ②Microvessel density and microvessel area.RESULTS: Thirty-five rabbits were involved in the result analysis. ①Microvessel density and microvessel area in He-Ne laser irradiated sides were all obviously larger than in the control sides in every forced group in the tension zone and the pressure zone (P ＜ 0.05-0.01), except for the 1-day group. ②In the 1-day group, the vascular endothelial growth factor expressions in periodontal tissues in pressure zone of He-Ne laser irradiated sides were significantly lower than that of control sides (P ＜ 0.05). In the 3- and 5-day groups, vascular endothelial growth factor expressions in pressure zone of irradiated sides were significantly higher than that of control sides (P ＜ 0.05). The vascular endothelial growth factor expressions in pressure zone of both irradiated sides and control sides reached a peak at 5-day group. The vascular endothelial growth factor expressions of irradiated sides were significantly higher than the corresponding control sides in tension zone at 3-, 5- and 7-day group (P ＜ 0.05). The vascular endothelial growth factor expressions in tension zone of both irradiated sides and control sides reached a peak at 14-day group.CONCLUSION: He-Ne laser irradiation promotes blood vessel remodeling of periodontal tissues, increases the vascular endothelial growth factor concentrations in periodontal tissues and makes the angiogenesis of periodontal tissues more active, followed by the remodeling of orthodontic periodontal tissues.

It systematically analyzed the basic framework and key elements of health sector governance,such as,accountability,transparency,participation,integrity and capacity of policy,based on the method of systematic literature review and related theory and practice of governance.It systematically expounded the concepts of each element,the associated policy and managed tools,as well as the authority-responsibility relationship of pluralistic subjects in the governance framework.These five key elements were mutually reinforcing,not only to build the basic framework of the health sector governance,also to determine its policy decision-making pattern,which aimed to identify actual or potential governance problems.Through forming an inclusive and mutually reinforcing governance framework,the coordination among the conflicting or different stakeholders was encouraged by the mode and mechanism which embodied their governing bodies' values.The cooperation and sustainable development way formed to improve the supply of the medical and health services,and the use efficiency of resources.

Objective To study the effect of berberine on the lipid metabolism and the expression of ox-LDL,CD36 and PPAR? in the peritoneal macrophage(PM) and alveolar macrophage(AM) from the type 2 diabetic rats. Methods Sprague-Dauley rats were randomly divided into groups of normal control,high-fat diet,type 2 diabetes and berberine treatment.The levels of blood glucose,insulin and lipid were measured biochemically,the content of ox-LDL in macrophage and the protein expression of CD36 and PPAR? in macrophage were assayed by ELISA,the mRNA expression of CD36 and PPAR? in macrophage was disclosed by RT-PCR. Results Compared with the control group, the levels of glucose,insulin and total cholesterol (TC),triglycerides(TG),low density lipoprotein-cholesterol (LDL-C) in blood were increased significantly(P0.05). Total hip arthroplasty (THA) has developed rapidly in recent years, which reconstructs the joint function of some terminal hip joint diseases and obtains favorable efficacy. However, the stress load alters in the proximal femur following THA, leading to "adaptable bone reconstruction", which is characterized by thin cortical bone, decreased bone density, and bone loss surrounding prosthesis, in particular calcar femorale. This paper analyzed the effects of cement and cementless prosthesis, coated prosthesis, and THA approaches on stress shielding in the proximal femur to reduce stress shielding effect by improving prosthesis design, low elastic modulus materials, decreased section area of the prosthesis, cement prosthesis, appropriate coated materials and improved THA approaches.

Objective: To analyze the influence of long-term swimming training on spatial learning-memory in rats and its relationship with cyclic adenosine monophosphate(cAMP) and cyclic guanosine monophosphate(cGMP) signal transduction pathway. Method: After 3 times adaptable swimming exercises (30min each time), 40 male SD rats were divided into 2 groups: control group (CR, n=20) and exercises, group (TR, n=20). CR group didn't swim, and TR group swam without burden (6 times/week, 60 min each time). After 8 weeks training, 10 rats were selected from both groups respectively for examing of Morris water maze test. Radioimmunoassay was used to measure the levels of cAMP and cGMP in hippocampus and prefrontal cortex of rats. Result: ①Compared with CR group, in TR group learning-memory improved in a certain extent: ②Compared with CR group, in TR group, the level of cAMP in hippocampus enhanced very obviously (P＜0.01), the cAMP/cGMP ratio enhanced obviously (P＜0.05); in prefrontal cortex the levels of cAMP and cAMP/cGMP ratio enhanced obviously (P＜0.05). Conclusion: Swimming training for 8 weeks improved the spatial learning-memory of rats in a certain extent, and elevated the level of cAMP and cAMP/cGMP ratio in hippocampus and prefrontal cortex of rats obviously. It is suggested that exercises might affect learning-memory by cAMP and cGMP signal transduction pathway.

Objective To investigate the CTA, MRA and DSA in the primary liver cancers and the value of the perfor-mance. Methods 29 cases of primary liver cancer patients were diagnosed through comparative analysis of CTA, MRA and DSA in the diagnosis of primary liver cancer between the common and the differences to explore the full and effective ways so as to facilitate the clinical work in a more effective diagnostic tool. Results For 29 cases of liver cancer patients, both the detection rate and accuracy rate of MSCTA were significandy higher than MRA. Conclusion At this stage, the MSCTA has features of shortcut, convenience, high detection rate, which is the preferred method of examination and has higher value in clinical application.

Objective To research the Alzheimer disease(AD) patients' hippocampal structure and cognitive function with the susceptibility weighted imaging (SWI) and morphological measurement,and explore the changing regularity and correlation.Methods Sixty patients (divided equally into mild,moderate and major group as the condition of the severity of the AD) and 20 healthy control groups were scanned with SWI and T1MR,then the differences between AD and healthy controls were analyzed.Results The hippocampal volumes of AD patients were smaller than that of control group.The biggest variability occured in the major group(the left was Dunnett-t =-7.03,P ＜ 0.01 ;the right was Dunnett-t =-7.52,P ＜ 0.01),the moderate group was next(the left was Dunnett-t =-6.71,P ＜ 0.01 ; the right was Dunnett-t =-6.62,P ＜ 0.01) ;and the least variability occurred in the mild group.They did not achieve statistical significance (the left was Dunnett-t =-0.86,P＞ 0.05 ;the right was Dunnett-t =-0.68,P ＞ 0.05).The hippocampus vein' s length,diameter had reduced significantly in the mild group(Dunnett-t were-5.62,-7.02,P＜ 0.001),but the number of the branches was slightly increased,which was close to the statistical significance.(Dunnett-t =1.86,P ＞ 0.05).With the progress of the AD,all of the hippocampus vein's length,diameter,and branches were reduced significantly(the Dunnett-t of the moderate were-10.10,-11.40,-2.83,P＜0.05;the Dunnett-tofthe major were-11.69,-14.68,-5.74,P＜ 0.001).AD patients blood vessel diameter,length and cognitive have correlation(r =-0.034 ～-0.517,P ＜0.05,P ＜ 0.01).Conclusion The hippocampus vein and cognitive function function change in AD patients happened in the mild stages of disease.An important supplement to the MRI conventional sequences.

Objective To analyze the correlation between the bleeding control time,treatment and prognosis in acute gastroesophageal variceal hemorrhage and to explore the predictive factors in treatment failure.Methods The clinical data of 309 patients with acute gsstroesophageal variceal hemorrhage was collected.The bleeding control was retrospectively analyzed within and at two hours after treatment,over two hours but less than six hours,over six hours but less than five days,at and after five days.The correlation between bleeding control time and mortality rate was analyzed.The differences in success rate of bleeding control and mortality rate between medication treatment alone and medication combined with endoscopic treatment were compared.Logistic regression mode was used to analyze the predictive factors of treatment failure.Results Of the 309 acute gastroesophageal variceal hemorrhage hospitalized patients,the failure of bleeding control within and at two hours,over two hours but less than six hours and over six hours but less than five days were 79 cases,25 cases and 26 cases respectively and there were one,four and 16 cases who died respectively.The success rate of bleeding control by medication treatment alone was 57.93％ (179/309) and the mortality was 10.36％ (32/309). The success rate of bleeding control by medication combined with treatment under endoscope was 82.35％ (28/34) and the mortality rate was 0.The results of Logistic regression mode analysis indicated that the admission systolic blood pressure (≤90 mm Hg,1 mm Hg=0.133 kPa),increase of total bilirubin,increase of Child-Pugh grade and ascites were the predictive factors of treatment failure.Conclusions The time of bleeding control failure mostly was within two hours.With time extended,the mortality increased. The success rate of bleeding control by medication combined with endoscopic treatment was higher and the mortality was lower.The admission systolic blood pressure lower than 90 mm Hg,increase of total bilirubin,increase of Child-Pugh grade and ascites may be the predictive factors of treatment failure.
