Pancreatic cancer is a highly malignant, fast progressive digestive system neoplasm with poor prognosis, and the incidence has increased significantly in the recent years. The treatment mode of pancreatic cancer has been transformed from surgery-first approach to multidisciplinary approach including surgery, chemotherapy, radiotherapy and other comprehensive treatment modalities, accompanying with significant improvement in clinical efficacy. During the last decade, the molecular mechanism of pancreatic cancer has been explored in depth. Indeed, we had a clearer understanding of the disease, which provides clues for translation of basic research into clinical practice. This review summarized recent high quality studies on pancreatic cancer including pathogenic factor, advances in basic research, and clinical treatment.

Objective To investigate the expression of sodium iodide symporter (NIS ) mRNA in differentiated thyroid Carcinoma (DTC)and further explore its value in clinical diagnosis and therapy of DTC. Methods The expression of NIS mRNA was detected and analyzed in 21 nodular goiter and 45 cases of DTC (including 35 cases of papillary thyroid carcinoma and 10 cases of follicular thyroid carcinoma)by using real-time fluorescent quantitative reverse transcription polymerase chain reaction (real-time RT-PCR).Results Compared with that in nodular goiter, the mRNA expression of NIS in DTC tissue was significantly decreased (P < 0.05 ). Moreover, the mRNA expression of NIS was significantly correlated with lymph node metastasis and AJCC stage, respectively. The expression of NIS mRNA in DTC with lymph node metastasis was significantly decreased compared with that in DTC without lymph node metastasis (P <0.05).In addition, the expression of NIS mRNA in Ⅱ-Ⅳstage DTC was significantly decreased compared with that in Ⅰ - Ⅱ stage DTC (P < 0.05 ).Conclusion Differential expression of NIS can provide evidence for individual 1 3 1 I therapy for DTC.

Objective compare the changes in condyle positions, face type lateral position and facial soft tissue before and after clinical correction of adult ClassⅡ Division Ⅰ malocclusion patients accompanied with the temporomandibular disorder. Method there were 7 adult Class Ⅱ Division Ⅰmalocclusion patients with accompanied with temporomandibular disorder, they accepted orthodontic extraction treatment routinely with MBT straight wire appliance technology, by which the occlusion was opened, the front teeth was received within correction, the occlusion was finely adjusted to canine fossa staggered relationship and the overbite cover was proper. Results The face type facial soft tissue profile was changed from raised face before treatment to straight face type and 1/3 of height under the face was increased and the temporomandibular disorder was made better. Conclusion the molars occlusion relationship of adult Class Ⅱ Division Ⅰ malocclusion patients accompanied with temporomandibular disorder was adjusted by opening the occlusion and adducting the upright front teeth , the condylar process was moved forward , the posterior teeth alveolar bone height was increased and the temporomandibular disorder was substantially improved.

Objective To evaluate the value on screening of diabetic retinopathy with digital fundus photography.Design Prospective observational case series.Participant Consecutive 108 patients with typeⅠand typeⅡdiabetes mellitus, without proliferat- ed diabetic retinopathy determined by dilated ophthalmoscopy in Beijing Tongren Hospital. Methods All the patients received six-fields color fundus mydriatic photography after mydriasis and fundus fluorescein angiography.Diagnosis and grading were made based on fun- dus photography of one-field,two-fields,and six-fields.Fundus fluorescein angiography served as the reference standard.Main Out- come Measures Sensitivity, specificity and kappa value of kappa analyses. Results In comparison with fundus fluorescein angiography, if taking mild nonproliferative diabetic retinopathy as referable diabetic retinopathy,one-field fundus photography gave a sensitivity of 73.21%,a specificity of 88.64%,and a kappa statistic of 0.46,while two-fields fundus photography gave a sensitivity of 75.00%,speci- ficity of 86.36%,and a kappa statistic of 0.47,and the six-fields fundus photography gave a sensitivity of 76.05%,specificity of 87.80% and a kappa statistic of 0.48.If taking moderate nonproliferative diabetic retinopathy as referable diabetic retinopathy,one-field fundus photography gave a sensitivity of 72.46%,a specificity of 93.01%,and a kappa statistic of 0.68,while two-fields fundus photography gave a sensitivity of 73.91%,specificity of 93.01%,and a kappa statistic of 0.69,and the six-fields mydriatic digital photography gave a sensitivity of 75.36%,specificity of 92.81% and a kappa statistic of 0.70.In comparison with six-fields fundus photography,one-field fundus photography gave a kappa statistic of 0.97.Conclusion One-field mydriatic digital fundus photography centered at fovea for screening of early diabetic retinopathy especially stage DR2 or more severe is feasible

Object To explore the beneficial effect of phytoecdysone (EDS) on myocardial infarction and its mechanism of action Methods Rat myocardial infarction model was prepared by ligating the left anterior descending coronary artery, and EDS was injected ip for seven consecutive days Serum creatine phosphokinase (CPK) glutamic oxalacetic transaminase (GOT), lactic dehydrogenase (LDH) activities, infarct size(IS), coronary blood flow, capillary vessel density and vascular endothelial growth factor (VEGF) expression were determined Results 0 5, 5, and 50 mg/kg of phytoecdysone were able to effect the activities of serum CPK, GOT, LDH in a dose depending manner with an optimal effect for improving cardiac zymogram at the dose of 5 mg/kg ip At this dosage EDS can markedly reduce IS, increase coronary blood flow, capillary vessel density and the expression of VEGF Conclusion ESD can alleviate myocardial infarction symptoms The mechanism of such beneficial effect may due to its ability to promote VEGF expression regeneration of capillary vessels and increase coronary blood flow
