1.Diagnosis and treatment of arrhythmias in children with congenital heart disease after surgery

Arrhythmias in children with congenital heart disease may be associated either with the underlying abnormality or result from surgical intervention.The treatment rationale bases on the rapid and accurate diagnosis.As a general rule,risk-benefit analysis of drug therapy should be considered when facing an arrhythmia in a child,due to the negative inotropy.In the last decades,non-pharmacological therapy is progressively used in pediatric population.
2.Anterior corpectomy decompression and titanium mesh bone iraft fusion combined with titanium nate fixation for the treatment of the multilevel cervical spondylotic myelopathy.

OBJECTIVETo explore the clinical effects of anterior corpectomy decompression and titanium mesh bone graft fusion combined with titanium plate fixation in treatting multilevel cervical spondylotic myelopathy.
METHODSThe clinical data of 48 patients with multilevel cervical spondylotic myelopathy underwent surgical operation were retrospectively analyzed from October 2010 to January 2013. There were 37 males and 11 females, aged from 37 to 76 years old with an average of 54.6 years. Thirty-five cases were two-segment lesion, 7 cases were three-segment lesion, 6 cases were four-segment lesion. All the patients were treated by anterior corpectomy decompression and titanium mesh bone graft fusion combined with titanium plate fixation. ROM, JOA, VAS and SF-36 scores were recorded before and after operation(including 3, 6, 12 months after operation and final follow-up). Fusion degree and spinal canal decompression condition were observed by radiographic data.
RESULTSAll patients were followed up from 14 to 48 months, with an average of 27.3 months. At 12 months after surgery, radiographic data showed that all patients obtained bony fusion, spinal canal decompression were sufficient. Preoperative vertebral canal sagittal diameter of the most serious segment were (5.13 +/- 1.32) mm, 12 months after surgery were (9.94 +/- 1.22) mm, there was statistically significance (t=2.463, P=0.014); the degree of vertebral canal decompression were (92.15 +/- 2.35)%. Postoperative ROM, JOA, VAS and SF-36 scores were obviously improved than that of preoperative (P<0.05); there was no statistically significance of ROM, JOA, VAS and SF-36 scores in each time after operation (P>0.05).
CONCLUSIONAnterior corpectomy decompression and titanium mesh bone graft fusion combined with titanium plate fixation can obtain higher fusion rate, complete thoroughly decompression, improvement of clinical symptoms and well safety in treating multilevel cervical spondylotic myelopathy.

3.Diagnosis and treatment of 40 cases of hyperthyroidism in pregnancy

Objective: To study the diagnosis, treatm ent and outcome of infants in patients with hyperthyroidism in pregnancy. Methods: The levels of T3,T4,TSH,FT3,FT4 in 40 cases of hyperthyroid gravida and control group were measured before delivery, the r oute of delivery and the outcome of infants were studied, and the neonatal thyroid f unctions were evaluated by the levels of T3,T4,TSH in cord serum samples . Results: The levels of T3,T4,FT3,FT4 in non-treatm ent group were higher than that of treatment group and control(P<0.01); an i ncreased rate of forceps and cesarean in hyperthyroid gravida was seen compared with that in control group, about 90%(36/40); the body weights of infants in no n-treatment group were lower than that in treatment and control group. There wa s a case of neonatal hyperthyroidism in non-treatment group. Conclusion : Hyperthyroidism should be diagnosed and treated earlier and there is little side effects on the outcome of infants.
4.An analysis of factors for quality-of-life in patients with chronic cough

Objective To constitute a correlation with the subjective indicators by investigation of the causes and clinical features in patients with chronic cough.Methods Totally 100 patients with chronic cough were recruited followed a diagnostic program.Airway responsiveness[by methacholine challenge test (MCT)],Leicester cough questionnaire(LCQ),visual analogue scale(VAS),cough score,age,gender and disease duration were all recorded for analysis.Results The top five causes of chronic cough in these patients were variant asthma,post infectious cough,atopic cough,eosinophilic bronchitis and upper airway cough syndrome.LCQ total score was negatively correlated with age and the VAS score(r=-0.239 and -0.470 respectively,all P<0.05),while no difference was found among patients with different causes of disease or gender(F=1.233,t=1.918,all P>0.05)and no correlation was found with BMI(r=-0.029,P>0.05).The physiological and psychological field score in female patients significantly reduced(t=2.174,1.990,P<0.05),and LCQ total score of MCT positive patients obviously reduced than negative ones(t=-2.22,P<0.05).After the treatment of two weeks,LCQ three component field and total score could be improved significantly(all P<0.01).Conclusion Gender and age may have some impact on the quality of life in patients with chronic cough.LCQ,VAS and cough score should be used to assess cough severity and evaluate therapeutic effect in patients with chronic cough.

6.Progress in research on bacterial HD-GYP domain proteins

Bis-(3′,5′) cyclic di-guanylate (c-di-GMP) is an almost ubiquitous intracellular second messenger in bacteria.Now it is known to regulate complex physiological processes, including mobility, adhesion, virulence and biofilm formation.The level of c-di-GMP is regulated by diguanylate cyclases (DGCs) containing GGDEF domains and phosphodiesterases (PDEs) containing EAL or HD-GYP domains.Recent studies have demonstrated that HD-GYP domain protein is a novel phosphodiesterase, which is also involved in the regulation of c-di-GMP degradation.This review highlights recent advances in the structure and biochemical functions of HD-GYP domain proteins, which might help to further clarify the mechanism of c-di-GMP signal system.
7.Chemical constituents in leaves of Euphorbia soongarica

Objective To investigate chemical constituents in the leaves of Euphorbia soongarica(Xinjiang origin).Methods The leaves were extracted with 95% EtOH and partitioned with petroleum ether,CHCl3,and n-BuOH,respectively.Compounds were isolated by using repeated silical gel column chromatography and Sephadex LH-20 column chromatography.Results Eight compounds were identified on the basis of physiochemical properties and spectral data as chrysoeriol(Ⅰ),phthalic acid,bis-(2,5-dimethylhexyl) ester(Ⅱ),fraxetin(Ⅲ),3-(4-O-?-D-glucopyranosyloxy-3,5-dimethoxy)-phenyl-2E-propenol(Ⅳ),fraxin(Ⅴ),3-methylellagic acid-3′-O-?-D-xylopyranoside(Ⅵ),3,3′-dimethylellagic acid-4-O-sulphate(Ⅶ),and(+)-isolariciresinol-9-O-?-D-xylopyranoside(Ⅷ).Conclusion Compound Ⅶ is a new natural product,the other seven compounds are obtained from this plant for the first time.
8.The Evaluation of Therapy Efficacy of Gamma Knife on Pituitary Adenoma by Using MR

Objective To explore the therapy efficacy of gamma knife on pituitary adenoma based on the changes of MR imagings and hormone levels before and after gamma knife treatment .Methods MR imagings and hormone levels, clinical material were retrospectively analysed on 45 patients(altogether 59 times MR examinations)followed up from 1~28 months. Both plain and enhancement MR scans were performed on each MR examination. Results 59 cases were followed up for 1~28 months . The tumor size shranked in 31 cases (52.5%), no changs in 25 cases (42.4%), increased in 3 cases(5%). Of the 59 cases , intratumoral cyts were observed in 15 cases (25%). Functional pituitary adenomas were followed up in 23 cases. The hormone levels droped in 13 cases (56.5%), the number of hormone levels droped in about the sixth month after gamma knife treatment was 8.6% , in 1 year was 13% and in 2 years was 34.8%. Conclusion Gamma knife can effectively keep the pituitary adenomas from their growing up; avoid the overproduce of functional pituitary adenomas . The treatment effects of gamma knife can be enlarged with the time gone.
9.Clinicopathological characteristics of axillary lymph node metastasis in lung cancer

Objective:To investigate the clinicopathological characteristics of lung cancer patients with axillary lymph node metastasis (ALNM). Methods:The clinical data of 91 lung cancer patients with ALNM who were treated in Zhejiang Cancer Hospital from January 1st, 2007 to December 31st, 2013 were retrospectively analyzed. The relevance of the sites of the tumor site, local lymph node, and ax-illary lymph node was checked by contingency table. Survival rates were calculated by the Kaplan-Meier method and compared by a log-rank test. Cox proportional hazards model was applied to analyze the prognostic factors. Results:The proportion of lung cancer pa-tients with ALNM was 0.63%, and the patients were often presented with adenocarcinoma, peripheral tumor type, pleura invasion with pleural effusion, or chest wall invasion. A relationship between tumor sites, local lymph node sites, and axillary lymph node sites was observed. The median survival time of lung cancer patients with ALNM was 19.02 months, and the two-year survival rate is 62.64%. Patients identified with ALNM at the initial diagnosis reported poor prognosis (P=0.002). Cox regression analysis showed that the relative risk of death in patients with ALNM at initial diagnosis was elevated 2.18 times (95%CI:1.330?3.572, P=0.003). Conclu-sion:ALNM in lung cancer is rare, and it may involve through direct chest wall invasion and spread from supraclavicular and mediasti-nal lymph node metastasis or systemic origin. Patients detected with ALNM at the initial diagnosis indicated poor prognosis.
10.Clinical study of the correlation of obesity with chronic periodontitis

Objective:To investigate the relationship between obesity and chronic periodontitis.Methods:2 1 36 subjects (1 091 males and 1 045 females)were included.Body mass index(BMI)was calculated.Gingival index (GI),tooth pocket probing depth (PD)and attachment loss (AL)were measured at 6 ponint of each tooth according the quartation method.The correlation between obesity and the indexes of periodontitis was studied by Spearman analysis.Results:There was a significant positive correlation of BMI degree with chronic periodontitis;there was no correlation between BMI and GI,PD in men,but BMI was positively correlated with AL,BMI and GI in women.Conclusion:Progress of chronic periodontitis may be related to obesity,the relation is more significant in women.
