1.Hyperbaric oxygen on left ventricular ejection fraction preserved by the influence of left ventricular remodeling in patients with heart failure

Objective To study hyperbaric oxygen on left ventricular ejection fraction preserved by the influence of left ventricular remodeling in patients with heart failure. Methods A total of 110 patients with heart failure and normal ejection fraction were randomly allocated into the control group (n=55) and the HBO group (n=55). The control group were given the routine therapy, the HBO group were treated with hyperbaric oxygen on the basis of conventional drug. The application of color doppler ultrasound before and after treatment for 3 months left ventricular structure indicators. Results Left ventricular structure indicators were significantly decreased (LVDd、IVSD、LVPWD、LVMI)(P<0.01). Compared with the control group the difference was statistically significant (P<0.05). Follow-up of 3 months, The treatment group composite cardiovascular events was fewer than the control group and had significant difference (P<0.05). Conclusion Hyperbaric oxygen therapy can significantly improve left ventricular ejection fraction preserved by heart failure of left ventricular diastolic and systolic function and reverse left ventricular remodeling,And can reduce the happening of cardiovascular events.

Objective To study the characteristics of auditory brainstem response by chirp and click stimuli in normal hearing children.Methods A total of 97 children (194 ears)with normal hearing were recruited,including 50 male (100 ears),and 47 female (94 ears).The CE-chirp and the click acoustic stimulation were used to,and the ABR waveforms and latencies were obtained.ResuIts The chirp-ABR Ⅰ wave recognition rate (36%)was significantly lower than that of click-ABR (100%).At 80 dB nHL,chirp-ABR V wave latency was shorter than click-ABR,the difference was statistically significant (P< 0.05 ).The chirp and click ABR V wave response thresholds showed no significant difference (P>0.05).At 80 dB nHL,no statistical significance of two kinds of a-coustic stimulation of ABR V wave amplitude difference (P>0.05).At 50 dB nHL,chirp-ABR significantly in-creased than click-ABR V wave amplitude with statistically significant differences (P<0.05).At 80,and 50 dB nHL,chirp-ABR showed no differences in the amplitude of V wave (P>0.05 ).ConcIusion The chirp acoustic stimulation produced the larger response amplitude,and was faster,demonstrating the noticeable advantages to tes-ting children.
3.Effects of ultraviolet B-induced autophagy on apoptosis in human skin fibroblasts: a preliminary study

[Objective] To observe the effects of autophagy induced by different doses of ultraviolet B (UVB) irradiation on the apeptosis in human skin fibroblasts.[Methods] Skin fibroblasts were isolated from the circumcision specimen of a 23-year-old healthy male,and subjected to a primary culture.After 3 to 10 passages,the cells were collected and applied in the following experiment.Methyl thiazolyl tetrazolium (MTT) assay was performed to evaluate the proliferation of some fibroblasts treated with different concentrations (0,0.5,2.0,5.0and 10.0 mmol/L) of 3-methyladenine (3-MA).To qualitatively and quantitatively detect the autophagy in fibroblasts treated with different concentrations of 3-MA and in fibroblasts treated with 3-MA of 0.5 mmol/Lfollowing UVB irradiation,monodansylcadaverine (MDC) staining was carried out,and immunofluorescence was used to detect the expression of microtubule-associated protein 1 light chain (LC3).Some fibroblasts were classified into 8 groups to remain untreated,be irradiated with UVB of 30,50 and 100 mJ/cm2 alone,treated with 3-MA of 0.5 mmol/L alone,or treated with 0.5 mmol/L 3-MA following irradiation with UVB of 30,50 and 100 mJ/cm2,respectively,then,cell apoptosis was qualitatively detected by Hoechst and propidium iodide (PI)staining,and quantitatively detected by flow cytometry with annexin V and PI.[Results] The percentage of autophagic cells was (63.037 ± 5.876) % in fibroblasts treated with starvation condition,significantly decreased to (34.425 ± 5.183) % in fibroblasts treated with 3-MA of 0.5 mmol/L.The expression of LC3 showed a gradually increasing trend from untreated fibroblasts,to fibroblasts irradiated with UVB of 30,50 and 100 mJ/cm2,while the increase was attenuated by the 4-hour treatment with 3-MA immediately after the irradiation.Compared with the other concentrations,the 3-MA of 0.5 mmol/L showed the least influence on the viability of fibroblasts.The addition of 3-MA of 0.5 mmol/L increased the percentage of cells both positive for Hoechst and PI staining in fibroblasts irradiated with UVB of 50 mJ/cm2,but decreased that in fibroblasts irradiated with UVB of 100 mJ/cm2.Similarly,the percentage of middle and late apoptotic cells was significantly higher in fibroblasts irradiated with UVB of 50 mJ/cm2 followed by treatment with 3-MA of 0.5 mmol/L than in those irradiated with UVB of 50 mJ/cm2alone ((10.933 ± 0.839) % vs.(7.267 ± 0.473) %,t =5.20,P< 0.05),but lower in fibroblasts irradiated with UVB of 100 mJ/cm2 followed by treatment with 3-MA of 0.5 mmol/L than in those irradiated with UVB of 100 mJ/cm2alone ( (7.100 ± 0.781 ) % vs.( 1 0.133 ± 0.681 ) %,t =6.29,P < 0.05 ).[Conclusion]s The irradiation with UVB of 50 mJ/cm2 may protect fibroblasts by inducing autophagy and suppressing apoptosis,while the high level of autophagy induced by UVB of 100 mJ/cm2 may lead to autophagic cell death in fibroblasts.
4.Correlation of carotid intima-media thickness and cardiac diastolic dysfunction in patients with type 2 diabetes

Objective:This study explored the correlation between carotid intima-media thickness (CIMT) and cardiac diastolic dysfunction in patients with type 2 diabetes mellitus (T2DM).Methods:381 T2DM patients hospitalized in the endocrinology department of the Second Affiliated Hospital of Guangzhou Medical University from January 2019 to January 2020 were selected. According to E/A value, they were divided into normal diastolic function group (149 cases) and incomplete diastolic function group (232 cases); According to the CIMT value, they were divided into CIMT normal group (213 cases) and CIMT thickening group (168 cases). The general data, blood lipid, blood glucose, liver and kidney function, cardiac color Doppler ultrasound and carotid color Doppler ultrasound were compared and analyzed. Multivariate logistic analysis was used to analyze the influencing factors of cardiac diastolic dysfunction in patients with T2DM. Receiver operating characteristic curve (ROC) was used to analyze the ability of CIMT value to predict cardiac diastolic dysfunction in T2DM patients.Results:Multivariate regression analysis showed that age ( OR=1.067, P<0.001), increased CIMT ( OR=1.863, P=0.011) and systolic blood pressure ( OR=1.014, P=0.016) were the risk factors for occurrence of cardiac diastolic dysfunction in patients with T2DM ( P<0.05). ROC analysis demonstrated that the area under the curve (AUC) for cardiac diastolic dysfunction diagnosed by CIMT was 0.625 (95% CI: 0.568-0.683, P<0.001). The cut-off value was 0.875, with a sensitivity of 52.6% and specificity of 69.1%. Conclusions:Increased CIMT is an independent risk factor for cardiac diastolic dysfunction in patients with T2DM. The level of CIMT has certain predictive value for the occurrence of cardiac diastolic dysfunction.
5.Medical decision-making in children with autism spectrum disorder from the perspective of caregivers: a systematic analysis

Objective:To systematically review the medical decision-making for children with autism spectrum disorder (ASD) from the perspective of caregivers.Methods:It was a systematic review. Using search terms as “Autism”“loneliness disorder”“autism spectrum disorder”“parents”“caregiver”“medical decision making”, etc., qualitative studies on medical decision-making of ASD caregivers in the databases of PubMed, Web of Science, Cochrane Library, Embase, Scopus, CNKI.com, Wanfang Database, VIP database, and Chinese Biomedical Literature database were searched. The search was conducted from the inception of the databases to July 31, 2022, and the data was organized from August 1 to September 1, 2022. The literature quality was evaluated by two researchers according to the quality evaluation criteria for qualitative research at the Centre for Evidence-based Health Care, Joanna Briggs Institute, Australia. Pooled integration method was used to integrate the study results and evaluate the medical decision-making of children with ASD from the perspective of caregivers.Results:Seventeen studies were included, with two studies rated as quality grade A and fifteen studies rated as quality grade B. A total of 36 research findings were extracted. These findings were grouped into ten new categories based on similarities, which were further synthesized into three overarching results: the symptom-driven medical decision-making process, delays in medical decision-making due to individual, family, and societal factors, and the influence of personal experiences and multiple external pressures on medical decision-making.Conclusion:The factors that lead to the delay or promotion of medical decision of caregivers of children with ASD are complicated, and comprehensive intervention measures should be taken at individual, family and social levels to promote early medical diagnosis and intervention of the children with ASD.
6.Clinical analysis of 51 female children with condyloma acuminatum

Objective To analyze clinical features and treatment outcomes of female children with condyloma acuminatum.Methods Female outpatients with condyloma acuminatum aged less than 12 years were enrolled into this study.Through a questionnaire survey,medical histories were collected,and a physical examination was performed in these patients.Polymerase chain reaction (PCR) was conducted to determine the genotypes of human papillomavirus (HPV).Descriptive statistical methods were used to identify high risk factors for condyloma acuminatum,manifestations of skin lesions,and results of HPV test.The number of sessions of laser therapy and photodynamic therapy was also recorded.Results A total of 51 female children aged under 12 years with a diagnosis of condyloma acuminatum were enrolled into this study.Their median age and course of disease (M [P25-P75]) were 3 (2-5) years and 6 (4-8) months respectively.Of the 51 patients,29 (56.9%) lived in the small towns or suburb of a city,and 16 (31.4%) lived in the rural areas.Only 2 children's parents had a definite HPV infection history.Twelve children (23.6%) usually wore open-seat pants,24 (47%) often took a bath in public bath houses,and only 1 child had a definite history of sexual assault.Among these patients,39 (76.5%) mainly had perianal skin lesions,and all cauliflower-like lesions occurred on the perianal area.A total of 35 patients underwent the HPV test in the exfoliated cells.The positive rate of HPV was 71.4%,and all the HPV-positive patients were infected by low-risk HPV types (HPV6/11).During the treatment,5 patients were lost to follow-up.In the remaining 46 patients,the median number of sessions of laser therapy and photodynamic therapy was 2 (1-4) and 3 (3-4) respectively.The median course of treatment was 4 (2-6) months.After the treatment,all the remaining 46 patients were cured with a recovery rate of 90.2% (46/51).Conclusions In these female children with condyloma acuminatum,the source of infection may maninly come from the environment,and skin lesions mostly occur on the perianal area.The prognosis is good after laser and photodynamic therapy.
7.Comparison of the effects between low-level assisted ventilation and T-piece method on respiratory mechanics during weaning of mechanically ventilated patients

Objective:To compare the difference of low-level assisted ventilation and T-piece method on respiratory mechanics of patients with invasive mechanical ventilation during spontaneous breathing trial (SBT) within 3 days before extubation.Methods:A retrospective observational study was conducted. Twenty-five patients with difficulty in weaning or delayed weaning from invasive mechanical ventilation who were admitted to department of critical care medicine of the First Affiliated Hospital of Guangzhou Medical University from December 2018 to June 2020, and were in stable condition and entered the weaning stage after more than 72 hours of invasive mechanical ventilation were studied. A total of 119 cases of respiratory mechanical indexes were collected, which were divided into the low-level assisted ventilation group and the T-piece group according to the ventilator method and parameters used during the data collection. The different ventilation modes related respiratory mechanics indexes such as the esophageal pressure (Pes), the gastric pressure (Pga), the transdiaphragmatic pressure (Pdi), the maximum Pdi (Pdimax), Pdi/Pdimax ratio, the esophageal pressure-time product (PTPes), the gastric pressure-time product (PTPga), the transdiaphragmatic pressure-time product (PTPdi), the diaphragmatic electromyography (EMGdi), the maximum diaphragmatic electromyography (EMGdimax), PTPdi/PTPes ratio, Pes/Pdi ratio, the inspiratory time (Ti), the expiratory time (Te) and the total time respiratory cycle (Ttot) at the end of monitoring were recorded and compared between the two groups.Results:Compared with the T-piece group, Pes, PTPes, PTPdi/PTPes ratio, Pes/Pdi ratio and Te were higher in low-level assisted ventilation group [Pes (cmH 2O, 1 cmH 2O = 0.098 kPa): 2.84 (-1.80, 5.83) vs. -0.94 (-8.50, 2.06), PTPes (cmH 2O·s·min -1): 1.87 (-2.50, 5.93) vs. -0.95 (-9.71, 2.56), PTPdi/PTPes ratio: 0.07 (-1.74, 1.65) vs. -1.82 (-4.15, -1.25), Pes/Pdi ratio: 0.17 (-0.43, 0.64) vs. -0.47 (-0.65, -0.11), Te (s): 1.65 (1.36, 2.18) vs. 1.33 (1.05, 1.75), all P < 0.05], there were no significant differences in Pga, Pdi, Pdimax, Pdi/Pdimax ratio, PTPga, PTPdi, EMGdi, EMGdimax, Ti and Ttot between the T-piece group and the low-level assisted pressure ventilation group [Pga (cmH 2O): 6.96 (3.54,7.60) vs. 7.74 (4.37, 11.30), Pdi (cmH 2O): 9.24 (4.58, 17.31) vs. 6.18 (2.98, 11.96), Pdimax (cmH 2O): 47.20 (20.60, 52.30) vs. 29.95 (21.50, 47.20), Pdi/Pdimax ratio: 0.25 (0.01, 0.34) vs. 0.25 (0.12, 0.41), PTPga (cmH 2O·s·min -1): 7.20 (2.54, 9.97) vs. 7.97 (5.74, 13.07), PTPdi (cmH 2O·s·min -1): 12.15 (2.95, 19.86) vs. 6.87 (2.50, 12.63), EMGdi (μV): 0.05 (0.03, 0.07) vs. 0.04 (0.02, 0.06), EMGdimax (μV): 0.07 (0.05, 0.09) vs. 0.07 (0.04, 0.09), Ti (s): 1.20 (0.95, 1.33) vs. 1.07 (0.95, 1.33), Ttot (s): 2.59 (2.22, 3.09) vs. 2.77 (2.35, 3.24), all P > 0.05]. Conclusions:When mechanically ventilated patients undergo SBT, the use of T-piece method increases the work of breathing compared with low-level assisted ventilation method. Therefore, long-term use of T-piece should be avoided during SBT.
8.Therapy of Dispelling Stasis, Removing Toxin, and Promoting Urination (Xuebijing Injection Combined with Modified Linggui Zhugantang) Affects Prognosis of Sepsis-induced Cardiomyopathy

ObjectiveTo evaluate the effect of the therapy of dispelling stasis, removing toxin, and promoting urination (modified Linggui Zhugantang combined with Xuebijing injection) on the prognosis of sepsis-induced cardiomyopathy (SICM). MethodA total of 96 patients were randomly assigned into an observation group and a control group, with 48 patients in each group. The patients in the control group received sepsis bundle, and those in the observation group additionally received the therapy of dispelling stasis, removing toxin, and promoting urination (intravenous drip of Xuebijing injection and oral administration of modified Linggui Zhugantang). The course of treatment in both groups was 7 days. The disease and prognosis indicators [28-day mortality, intensive care unit (ICU) length of stay, major adverse cardiac events (MACE), acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ), sequential organ failure assessment (SOFA) score, and mortality in emergency department sepsis (MEDS) score], cardiac function indicators [left ventricular ejection fraction (LVEF), E/A ratio of peak velocity blood flow from left ventricular relaxation in early diastole (the E wave) to peak velocity flow in late diastole caused by atrial contraction (the A wave), E/e′ ratio of mitral peak velocity of early filling (E) to early diastolic mitral annular velocity (e′), and afterload-corrected cardiac performance (ACP)], myocardial injury markers [high-sensitivity cardiac troponin T (hs-cTnT), N-terminal pro-brain natriuretic peptide (NT-proBNP), heart-type fatty acid-binding protein (H-FABP), and high mobility group box-1 (HMGB-1)], hemodynamic indicators [extravascular lung water index (EVLWI), global end-diastolic volume index (GEDVI), cardiac index (CI), and systemic vascular resistance index (SVRI)], and TCM syndrome scores were assessed and compared between the two groups. ResultThe 28-day mortality and the incidence of MACE in the observation group were slightly lower than those in the control group. The ICU length of stay in the observation group was shorter than that in the control group (P<0.05). After treatment, APACHE Ⅱ, SOFA, MEDS, syndrome score of stasis-caused internal obstruction, E/e′ ratio, hs-cTnT, NT-proBNP, H-FABP, and HMGB1 decreased compared with those before treatment (P<0.05), while LVEF, E/A ratio, and ACP increased (P<0.05). Moreover, the changes were more significant in the observation group (P<0.05). On days 3, 5, and 7 after treatment, the EVLWI and SVRI in the observation group were lower than those in the control group (P<0.05), while CI showed an opposite trend (P<0.05). The observation group had higher GEDVI than the control group on days 3 and 5 after treatment (P<0.05). ConclusionOn the basis of conventional bundle therapy, modified Linggui Zhugantang combined with Xuebijing injection with the effect of dispelling stasis, removing toxin, and promoting urination can inhibit the generation of myocardial injury markers and improve hemodynamics to shorten the length of ICU stay, mitigate the TCM syndrome, and reduce the risk of death, thereby improving the prognosis of SICM.
9.A comparative study on the clinical effects of hip arthroplasty through direct anterior approach in lateral decubitus in the treatment of hip osteoarthritis caused by Kaschin-Beck disease and congenital acetabular dysplasia

Objective:To compare the clinical effects of hip arthroplasty through direct anterior approach (DAA) in lateral decubitus in the treatment of hip osteoarthritis caused by Kaschin-Beck disease and congenital acetabular dysplasia.Methods:The prospective study method was used to select the patients who needed hip arthroplasty in the Fourth Department of Orthopedics, the Second Affiliated Hospital of Harbin Medical University from January 2015 to December 2019. All of them were operated with lateral decubitus DAA. According to the inclusion criteria, they were divided into Kacshin-Beck disease hip osteoarthritis group (group A) and congenital acetabular dysplasia hip osteoarthritis group (group B). Hip Harris score, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, visual analogue scale (VAS) score were conducted, and hip abduction angle and flexion angle were measured before surgery, 3, 14 days and 1, 3, and 12 months after surgery.Results:Nineteen and twenty-two patients were included in group A and group B, respectively. All patients successfully completed the surgery. There was no significant difference in Harris score between the two groups before surgery, 3, 14 days, and 1, 12 months after surgery ( P > 0.05). There were no significant differences in WOMAC score, VAS score, hip abduction angle and hip flexion angle between the two groups before surgery and each time point after surgery ( P > 0.05). In the same group, there were significant differences in Harris score, WOMAC score, VAS score, hip abduction angle and hip flexion angle at different time points ( P < 0.001). All postoperative indicators were significantly improved compared with those before surgery. Conclusions:There is no significant difference in the clinical effects of hip arthroplasty through lateral decubitus DAA in the treatment of hip osteoarthritis caused by Kaschin-Beck disease and congenital acetabular dysplasia. This surgical method has good therapeutic effect on both types of hip osteoarthritis.
10.Effect and mechanism of emodin intervention on intestinal nerve injury in rats with severe acute pancreatitis

Objective:To explore the role and the intervention effect of emodin in intestinal neuropathy in rats with severe acute pancreatitis (SAP) through the nucleotide binding oligomerization domain like receptor protein 3/cysteine containing aspartic acid protease-1 (NLRP3/Caspase-1) pathway.Methods:Forty male healthy SD rats aged 6-8 weeks with a weight of approximately 200g were randomly divided into control group, SAP model group, emodin treatment (EMO) group, and NLRP3 knockdown group. SAP were induced by retrograde injection of sodium deoxycholate into the pancreatic duct of rats and serum amylase of which were detected. The effective NLRP3 knockdown sequence was screened for NLRP3 knockdown animal experiments. Fluorescence quantitative polymerase chain reaction was used to detect the expression of NLRP3, Caspase-1, gasdermin-D (GSDMD), interleukin (IL)-1β, IL-18 and tumor necrosis factor-α(TNF-α) in the small intestine of each group. Immunofluorescence staining was used to detect the expression of glial fibrillary acidic protein (GFAP) in the small intestine of each group.Results:The amylase levels of the control group, SAP group, EMO group, and NLRP3 knockdown group were (277.73±24.92) U/L, (1018.57±282.89) U/L, (625.43±134.40) U/L, and (391.01±27.63) U/L, respectively. The SAP and EMO groups were significantly higher than the control group ( P<0.001), while the EMO and NLRP3 knockdown groups were significantly lower than the SAP group (all P<0.001). Compared with control group, the expression levels of NLRP3, Caspase-1, IL-1β, IL-18, TNF-α and GSDMD in SAP group were increased, with statistical significance (all P<0.001). Compared with SAP group, the NLRP3 knockdown group showed the expressionlevels of the above 6 genes were all decreased, and EMO group showed decreased gene expressing levels of NLRP3, IL-1β, IL-18 and TNF-α, with statistical significance (all P<0.05). The relative expression of GFAP in small intestine of control group, SAP group, EMO group and NLRP3 knockdown group were (1.00±0), (1.66±0.11), (1.13±0.02) and (1.13±0.02), respectively. Among them, the expression of GFAP in SAP group was increased compared with the control group; The expression of GFAP in EMO group and NLRP3 knockdown group was lower than that in model group, and the differences were statistically significant (all P<0.05). Conclusions:Emodin and knocking down NLRP3 can both promote the repair of SAP small intestine injury through the NLRP3/Caspase-1 signaling pathway, and thus play a protective role in the intestine.
