1.Effect of concanavalin A on mouse islet transplantation

To test whether Con A has antirejection effect on mouse islet allograft, 600-700 handpicked islets of C57BL/6 mice was transplanted beneath the kidney capsule of the streptozotocin induced diabetic BALB/C recipients. In control group the mean survival time (MST) of the grafts was 14.1?2.4 days. Treatment with Con A significantly prolong the MST of the islet allografts. In group with Con A treatment of the donor islets prior to the transplantation the MST was 25?5.4 days. The MST increased sigificantly after ip injection of Con A to recipient mice. The combined treatment of islet and recipient with Con A resulted in a prolongation of MST to 25.2?4.4 days. The experiments concerning the mechanisms of the antirejection effect of Con A showed that Con A was able to combine with the islet cell membrane well masking perhaps the histocmpatibility antigen. Furthermore the lymphocytes of mice after i. p Con A injection suppressed the lymphocyte proliferation reaction of the svnergic mice to mitogen.
2.Surgical management of the complications after endoscopic sphincterotomy

ObjectiveTo review surgical management of the complications caused by iatrogenic endoscopic sphincterotomy(EST).MethodsEleven patients with complications of EST were surgically treated, including 4 patients with hemorrhage, 3 patients with perforation, 3 patients with perforation plus hemorrhage, and one patient with perforation plus severe pancreatitis. ResultsTwo patients with perforation plus hemorrhage and the patient with perforation plus severe pancreatitis died, other patients survived.ConclusionEarly diagnosis and surgical intervention was very important to lower the mortality. For patients suffering from perforation and hemorrhage or pancreatitis, it′s very important to make an unobstructed peritoneal drainage as well as other maneuvers.

Objective To explore serum caspase-3 activity in pulmonary tuberculosis patients.Methods 38 tuberculosis pa-tients in Infectious Disease Department in the First Affiliated Hospital of Guangxi Medical University were recruited and the peripheral venous blood was collected in December,2010.Among them,sputum smear-positive patients which were first treated were 13,sputum smear-negative patients which were first treated were 15,10 cases were drug resistance.In addition, 22 unrelated healthy individuals from a routine health survey were enrolled as controls.Spectrophotometry was used to de-tect serum caspase-3 activity in tuberculosis patients,which was absorbancy-A of 4-nitroaniline generated by caspase-3.Re-sults The activity of caspase-3 in sputum smear-positive patients,sputum smear-negative patients,drug resistance patients, and healthy controls were 37.15±5.06,50.38±6.29,45.32±3.32 and 23.89±2.29,respectively.The caspase-3 activity was significantly different from each other (F=10.68,P<0.01).Compared with the healthy controls,the caspase-3 activity in sputum smear-positive patients (q=3.51,P=0.016),sputum smear-negative patients (q=7.5,P<0.001),drug resist-ance patients (q=5.34,P=0.001)was significantly different.There was no statistical difference among sputum smear-posi-tive patients,sputum smear-negative patients and drug resistance pulmonary tuberculosis patients (P>0.05).In the same way,there was no significant difference in male/female ratio[χ2=0.38 (DL=3),P=0.94]and mean age (F=0.82,P=0.49)among the four group.Conclusion Caspase-3 closely associated with the development of tuberculosis occurrence, which might promote apoptosis of cells through the activation of caspase-3 and produce the body immune process of tubercu-losis.
4.Research on relationship between coronary lesion and blood stasis syndrome based on coronary angiography.

OBJECTIVETo explore the relationship between blood stasis (BS) syndrome and coronary lesion in patients with coronary heart disease (CHD).
METHODSSyndrome types of 500 patients collected from multiple centers whose diagnosis of CHD confirmed by coronary angiography were differentiated. And the relationship between BS syndrome, its subtypes, and coronary lesion (affected branches, degree of constriction) were analyzed.
RESULTSThe affected branches of coronary artery in patients of BS syndrome was 2.28 +/- 0.28, while that in the non-BS syndrome patients was 2.07 +/- 0.86, showing significant difference between them (P < 0.05); as compared to patients of non-BS syndrome, the coronary lesions in patients of BS syndrome were mostly multi-vascular, and of more severe degree (P < 0.05). In patients of various BS syndrome subtypes, the average number of affected coronary branches in patients of yang-deficiency subtype was 2.58 +/- 0.65, which was significantly more than the number in patients of other BS syndrome subtypes. The constriction degree of coronary lesions in patients of yang-deficiency BS syndrome subtype were mostly severe or moderate, and single branch lesion was rarely seen, as compared with those in patients of phlegm-stasis obstruction subtype, the difference was significant (P < 0.05). The corresponding correlative analysis showed that close correlation was found between yang-deficiency subtype of BS syndrome and severe coronary constriction with the correlation distance of 0.1899.
CONCLUSIONRelationship between BS syndrome and coronary lesion (its number of branches and degree of constriction) truly exists to a certain extent.

Safety issues of traditional Chinese medicine injections has been heated debate. There are two diametrically opposed views: it should be used reasonable and developed healthily or be forbidden to use. Some people have many misunderstandings and prejudices about the safety of traditional Chinese medicine injections. Compared with western medicine,traditional Chinese medicine has its own particularity. Traditional Chinese medicine has complex components. Its research and clinical application is different from western medicine. Adverse reactions of traditional Chinese medicine injections are related to many factors,such as a large number of irrational use,blind use of traditional Chinese medicine injections and western medicine injections,counterfeit and substandard drugs,incorrect methods of intravenous infusion,toxicity of supplementary materials,drug ingredients. Developing traditional Chinese medicine injection is the need for curing sickness to save patients. The purposeful, targeted, organized and planned systematic research of traditional Chinese medicine injections should be strengthened,especially the safety of traditional Chinese medicine. Strengthen supervision and control of rational drug use.Strengthen the examination and approval,supervision and management of all aspects to ensure the safety of patients.
6.Study on the Chinese medicine syndrome characteristics and coronary artery lesion specialty of female coronary heart disease.

OBJECTIVETo study the clinical feature, coronary artery lesion specialty and Chinese medicine syndrome characteristics of coronary heart disease (CHD) in females.
METHODSThe clinical materials of 297 female patients with CHD, whose diagnosis had been confirmed by coronary angiography, were analyzed and compared with those of 772 male patients.
RESULTSCHD in females was characterized by later occurrence, frequently complicated with diabetes mellitus and blood lipid disorder, and rather serious coronary lesion, mainly the multi- or double-branch lesion, involving the anterior descend branch, right coronary artery and convolution branch; with the often encountered syndrome factors and their combinations of qi deficiency, blood stasis, yin deficiency, blood stasis due to qi deficiency,deficiency of both qi and yin,turbid phlegm, yang-deficiency, etc.; the figures of tongue and pulse were generally dark-red or dark-purple or pale-red colored corpulent tongue proper, with waterless yellow or less coating, even without coating, cyanosed varicose sublingual veins,thin-weak or thin- rapid pulse.
CONCLUSIONFemale CHD has its special clinical feature, coronary specialty and Chinese medicine syndrome characteristics. Proper treatment methods for it are supplementing qi, activating blood, nourishing yin in dominance, with dissolving phlegm and accessing yang as accessory.

7.Study on improving the speed and quality of making rat myocardial infarction model by coronary artery ligation.

OBJECTIVETo establish the methods for improving the speed and quality of making rat myocardial infarction model by coronary artery ligation.
METHODSWith the precondition of having no effect on rat myocardial infarction size: (1) To compare the thoracotomy time, the ligation time of the way of coronary artery ligation by tying one knot with the way by tying two knots. (2) To compare the survival rate of making model, the onset time of anesthesia, the awakening time, the effect on myocardial infarction size between using hydration chlorine aldehyde and using sodium pentobarbital.
RESULTSIn case of having no significant effect on the myocardial infarction size (P > 0.05), the way of tying one knot could significantly shorten the thoracotomy time and the ligation time (P < 0.01). There was no statistical difference in the onset time of anesthesia or the myocardial infarction size between the model made by sodium pentobarbital and that made by hydration chlorine aldehyde (P > 0.05). But the awakening time of the model made by sodium pentobarbital was obviously shortened (P < 0.01). The clarity rate of myocardial infarction size was improved.
CONCLUSIONSThe way of tying one knot could improve the speed of model making. Sodium pentobarbital as the anesthetic for in vivo rat myocardial infarction model could improve the clarity rate of myocardial infarction area.

8.Efficacy of Quxie Capsule in Metastatic Colorectal Cancer: Long-Term Survival Update of A Double-Blind, Randomized, Placebo Controlled Trial.

OBJECTIVE:
To verify the efficacy of Quxie Capsule in patients with metastatic colorectal cancer (mCRC).
METHODS:
It was a block randomized, double-blind, placebo-controlled trial. Sixty patients with mCRC were randomized into 2 groups at a 1:1 ratio. The patients in the treatment group received conventional therapy including chemotherapy, radiotherapy, targeted therapy and supportive care, and Chinese herbal medicine combined with Quxie Capsule (each capsule of 0.4 g was orally administered at 50 mg/kg, twice daily, day 1-20, in a 30-day course) for 3 months. The patients in the control group received conventional therapy and Chinese herbal medicine combined with placebo for 3 months. Main outcome measures were overall survival (OS) and progression-free survival (PFS). Subgroup analysis was performed according to therapy lines, right or left-sided colon, targeted therapy and RAS gene status to determine the differences in PFS and OS between the two groups. Patients were followed up every 3 months until December 31st, 2018.
RESULTS:
Median follow-up time was 19.4 months. The median OS was 23.9 months in the treatment group [95% confidence interval (CI) 15.9-28.5] vs. 14.3 months in the control group (95% CI 11.3-21.4, P<;0.05). Hazard ratio (95% CI) was 0.55 (0.31-0.95, P=0.040). There were no significant differences between the two groups in PFS (P>0.05). In the subgroups of ⩾second-line therapy, non-targeted therapy, RAS gene wild type and left-sided colon, the treatment group showed a significant survival benefit compared with the control group (P<;0.05 or P<;0.01), respectively.
CONCLUSION
Quxie Capsule can reduce the risk of death and prolong the OS of patients with mCRC. (Registration No. ChiCTR-IOR-16009733).

9.Current Situation and Suggestion of Comprehensive Clinical Evaluation of Medical Products in China in Recent Ten Years

Through analyzing the current situation of clinical comprehensive evaluation researches of domestic medical products from 2013 to 2023, it is found that there is a phenomenon of “four more, four less”. Specifically, there is a greater emphasis on conducting research on drug clinical comprehensive evaluation, but fewer comprehensive evaluation opinions provided; more research data come from publicly available information, but fewer derived from real world; more options for similar reference drugs, but less pre-defined research on reference drugs; the research process involves more steps, yet there are fewer studies strictly implementing quality control. Based on this, suggestions are proposed in three aspects: improving the quality of clinical comprehensive evaluation of medical products, enhancing the transformation and dissemination of results, and improving the standardization of clinical comprehensive evaluation of medical products. Regarding the current state of clinical comprehensive evaluation research on Chinese patent medicine, reflections are presented on elevating original research evidence and highlighting the characteristics and advantages of Chinese patent medicine, so as to promote better transformation and application of the results of clinical comprehensive evaluation of medical products.
10.Dietary therapy of qi-yin-reinforcing porridge for the alleviation of chemotherapy related symptoms of gastrointestinal tumors: a single-case randomized controlled study.

OBJECTIVETo investigate the efficacy of dietary therapy of qi-yin-reinforcing porridge for the alleviation of clinical symptoms during FOLFOX chemotherapy.
METHODSA single-case randomized controlled study was carried out. Fourteen patients with gastrointestinal tumors accepting FOLFOX chemotherapy were enrolled using self-crossover control design. On days 1-7 of chemotherapy, the dietary therapy experimental group and the placebo control group were given dietary therapy of qi-yin-reinforcing porridge and dextrin-prepared porridge which had the same appearance, smell, color and taste as the qi-yin-reinforcing porridge, respectively. Fourteen clinical symptoms, including debility, vomiting and nausea, etc. were observed.
RESULTSTen patients completed the study and were estimable. The qi-yin-reinforcing porridge demonstrated certain efficacy in alleviating clinical symptoms of patients with gastrointestinal tumors during the period of chemotherapy. Better effect of alleviating debility was found in the dietary therapy experimental group than in the placebo control group, and the difference was statistically significant (Z=2.27, P=0.02). No statistically significant difference was found between the experimental group and the control group with respect to the effect of alleviating the other 13 clinical symptoms including anorexia, nausea, vomiting, dry stool, loose stool, etc. and body weight (P>0.05).
CONCLUSIONDietary therapy of qi-yin-reinforcing porridge shows an additional benefit in alleviating debility of patients during chemotherapy and deserves clinical use and popularization.
