1.Electroacupuncture applied at the Sanyinjiao point can relieve stress incontinence by up-regulating the expression of spinal NMDA and α 2 adrenergic receptors

Objective:To explore the effect of electroacupuncture on the expression of N-methyl-D-aspartic acid (NMDA) in the spine and the activity of α 2 adrenergic receptors in rats with stress urinary incontinence (SUI). Methods:In all, 48 adult female rats were randomly divided into a sham operation group, a model group and an acupuncture group. The sham operation group received sham vaginal distention. Both the model and acupuncture groups had SUI induced using endovaginal balloon dilatation. The former group then had no further intervention, while the latter was given acupuncture on the sanyinjiao point for one week. Urodynamic testing and leak point pressure (LPP) determination were then conducted with all three groups, as well as urethral sphincter electromyography. The expression of NMDA in the L 6-S 1 spinal segments using the polymerase chain reaction and α 2 adrenergic receptor activity was analyzed using western blotting. Results:No significant differences in the urodynamic variables were observed among the three groups. However, compared with the sham operation group, a significant decrease in the average LPP and the frequency and amplitude of the external urethral sphincter electromyography was observed in the model group. Moreover, compared with the model group, the acupuncture group showed significantly increased average LPP and higher electromyographic frequency and amplitude. The average mRNA and protein expression of NMDA in the spine were also significantly higher in the acupuncture group, as was the average α 2 receptor activity. Conclusions:Vaginal distention is an effective method of establishing urinary incontinence in rats. Electroacupuncture can not only improve the ability to store urine, it also increases urination, which may be related to the up-regulation of the expression of NMDA and α 2 adrenergic receptors.
2.Analysis of the risk factors on survival rate in 185 patients who received cardiopulmonary resuscitation with return of spontaneous circulation after discharge

Objective To investigate the survival rate and status of cerebral performance category in patients who received cardiopulmonary resuscitation (CPR) with return of spontaneous circulation (ROSC) (abbreviated by ROSC-CPR) after discharge,and to analyze the risk factors which influencing the prognosis of these patients.Methods A retrospective analysis of the clinic data coming from the patients who received cardiopulmonary resuscitation with return of spontaneous circulation and admitted to the Department of Intensive Care Unit from January 2009 to December 2016 was carried out to find corresponding risk factors influencing the prognosis.Results A total of 185 patients who received ROSC-CPR with 59.5% male,the average age of (67.15 ± 17.64) years old and the average Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score (28.80 ± 7.16) were divided into two groups,the survival group (n =56) and the death group (n =129).The total survival rate was 56/185 (30.3%).Compared to the death group,the usage rate of ventilation,blood purification and target temperature management in survival group was no significant (P > 0.05).The most common aetiology of the survival patients was cardiogenic disease,but the number of those patients with the good cerebral performance category (CPC 1/2) discharged from hospital were 16 cases (8.6%).According to logistic analysis,high value of APACHE Ⅱ score,the duration from CPR to ROSC over 10 minutes,admission took place out-of-hospital were unfavorable predictors for the prognosis (P < 0.05),while admission took place in the second four years and cases with cardiogenic aetiology were favorable predictors for the prognosis of these patients who suffered from ROSC-CPR.Conclusions The survival rate of the patients who received CPR with ROSC after discharge was still low,especially the rate of good status of CPC was very low.To shorten the duration from CPR to ROSC as possible as we can,and to strengthen target temperature management would improve the prognosis of the patients who received CPR with ROSC in our hospital in future days.
3.Proteomics analysis of mouse lens supernatant treated with hydrogen peroxide in vitro

Objective:To study the protein leakage of mouse lens treated with hydrogen peroxide in vitro, and to identify and carry out bioinformatics analysis of the leaked proteins. Methods:Intact binocular lenses of 42 healthy male C57BL/6J mice were enucleated following sacrifice, and 28 lenses were arbitrarily combined as one sample, with 3 samples in total.Mouse lenses were cultured with medium containing 2 mmol/L hydrogen peroxide for 24 hours to establish a cataract model in vitro.Blank medium M-199 was taken as control sample.Supernatant was subjected to polyacrylamide gel electrophoresis (PAGE). The structure of the supernatant proteins was identified by liquid chromatography coupled with tandem mass spectrometry, and the high-abundant proteins were quantitatively analyzed by label-free quantitative mass spectrometry.The biological information of the identified proteins was analyzed by METASCAPE database.Content of cytokines in the supernatant was determined using the multiplex bead immunoassay system.This study protocol was approved by an Ethics Committee of The First Affiliated Hospital of Chongqing Medical University (No.2019-173), and the use and care of experimental animals complied with the ARVO statement. Results:After 24-hour culturing, the lenses became turbid, and the capsules were intact.The protein concentration in the supernatant was (3.73±0.59)μg/μl.PAGE analysis of the supernatant protein profile revealed that protein bands were below 35 000.A total of 675 proteins were identified with mass spectrometry.Sixteen crystallin proteins accounted for (86.1±0.8)% of the total protein abundance; the biological processes those proteins participated in mainly included cell-cell adhesion, cell transformation, oxidation-reduction, inhibition of apoptosis, and protein translocation; the molecular functions of those proteins mainly involved the activity of hydrolase, oxidoreductase, catalysis, translation initiation factor, and GTPase; the subcellular localization of proteins were mainly in cytoplasm, extracellular exosome, the nucleus, the plasma membrane, and the intercellular space.The results of an antibody-based multiplex bead immunoassay presented 9 cytokines in the supernatant, which included monocyte chemotactic protein-1 and transforming growth factor-β 2 highly expressed in intraocular fluid. Conclusions:There are protein leakages in lens treated with hydrogen peroxide in vitro, and it may have influences on metabolism and/or functions of other intraocular tissues.
4.Exploring the Disease Mechanism and Treatment of Ischemic Stroke Based on Ascending and Descending of Qi Movement from the Perspective of Xiang Thinking

Xiang thinking is the key way of thinking to construct the life model of human body in traditional Chinese medicine (TCM), and the theory of ascending and descending of qi movement is an important manifestation of xiang thinking in the theory of TCM. Based on the theory of qi movement, this paper interpreted the mechanism of ischemic stroke through the perspective of xiang thinking "earth weakness - wood constraint - fire hyperactivity", as "earth weakness in the central and dampness accumulated to phlegm" "wood constraint and stirring wind led to blood stasis" and "fire hyperactivity and fire toxin showed flaming upward" due to disorder of qi movement. Combined with the "xiang of medicinal properties and therapy methods" to discuss the treatment and prescriptions of ischaemic stroke, applying wind medicinals to elevate ji-earth (己土) and yi-wood (乙木), so that phlegm and stasis can be eliminated, and cold medicinals to descend jia-wood (甲木) and wu-earth (戊土) so that fire toxin can be cleared, with a view to restore ascending and descending of qi movement for ischaemic stroke.
5.Clinical Application of Supplementing Essence and Boosting Marrow Method in the Treatment of Encephalopathy Based on the Marrow Sea Theory

As the guiding theory for the diagnosis and treatment of encephalopathy in traditional Chinese medicine (TCM), The marrow sea theory has important theoretical connotation and clinical value. This paper summarized the clinical research literature on the differentiation and treatment of common encephalopathy based on the marrow sea theory published in recent years, analyzed the treatment method and effects from eight aspects in terms of stroke, dizziness, insomnia, headache, constraint syndrome, dementia, tremor syndrome, and atrophy syndrome, and discussed the possible mechanism based on the relevant basic research. It is believed that marrow sea depletion is the common pathogenesis of encephalopathy in TCM. Guided by the method of supplementing essence and boosting marrow, the corresponding formulas and medicinals are recommended in accordance with differentiated syndromes, which can effectively improve the symptoms of the disease, delay the progression, increase the daily life ability of the patients, and improve the quality of life. Based on the marrow sea theory, the method of supplementing essence and boosting marrow, rectifying healthy qi and dispelling pathogen can be used to highlight the advantages of TCM and provide ideas for the diagnosis and treatment of encephalopathy in TCM.
6.Impact of prostaglandin F2α and endothelin, pulsation index and resistance index of uterine artery blood flow on dysmenorrhea patients of cold stagnation and blood stasis syndrome with Dingkun Dan.

This research apply Dingkun Dan to treat patients with dysmenorrhea of cold stagnation and blood stasis syndrome. This study observed its effectiveness and safety of the treatment of the disease and its influence on the serum prostaglandin F2α, endothefin, pulsatility index and resistant index of uterine artery blood, to explore the possible mechanism of effect of Dingkun Dan in the treatment of dysmenorrhea and provide scientific basis for clinical application. The 75 patients with dysmenorrhea of cold stagnation and blood stasis who met the inclusion criteria, were divided into treatment group (n=37) and control group (n=38) by using random number remainder grouping method. In the treatment group patients were treated with Dingkun Dan, the other group were given Fuke Zaizao Jiaonang. Two groups have same time to take the medicine, three days prior to the menstruation for ten days. Medication for three menstrual cycles was seen as a course of treatment. To observe and compare the two groups of patients before and after treatment VAS score, syndrome integral, serum levels of prostaglandin F2α and endothelin, pulsation index and resistance index of uterine artery blood flow and related safety index changes. Finally makes statistical analysis. It has been identified that, Treatment group and control group can reduce pain symptom of dysmenorrhea patients and improve the syndromes scores, compare with control group, effect of the treatment group is more significant(P<0.01). VAS pain curative effect: the treatment group and control group total effective rate respectively were, 97.22%, 69.44%, markedly effective rate were 83.33%, 30.56%, comparison between two groups, treatment group is better than that of control group(P<0.01). Syndromes curative effect: the treatment group and control group total effective rate respectively were 97.22%, 94.44%, markedly effective rate was 66.67%, 2.78%, respectively. The comparison between two groups, the total effective rate has no obvious difference, but markedly effective rate of treatment group is better than that of control group(P<0.01). The treatment group can significantly reduce the patients' serum level of prostaglandin F2α(P<0.01), but no obvious difference was found in the control group before and after treatment. Both groups can significantly reduce the serum level of endothelin(P<0.01), comparison between two groups, the treatment group is more significant(P<0.01).Both treatment group and control group were significantly lower left and right pulsation index and resistance index of uterus artery blood flow(P<0.01). Between groups to compare the effect, the treatment group is more significant(P<0.01). Both treatment group and control group in the security check before and after treatment found no significant anomalies. Dingkun Dan in treating dysmenorrhea with cold stagnation and blood stasis syndrome seems to have remarkable clinical curative effect and safety, which may be achieved by significantly reducing the serum level of prostaglandin F2αand endothefin of the patients, and reducing the pulsation index and resistance index of uterine artery blood flow of the patients, to improve uterine artery condition of blood, and correcting local tissue ischemia to relieve pain.
7.Application of kidney deficiency and blood stasis and disharmony between heart and kidney theories in patients with sleep disorders in perimenopause.

Perimenopausal syndrome refers to a series of physical and mental symptoms, caused by the fluctuation of the sex hormones in the menopause, which is one of the common diseases in gynecology. The incidence of sleep disorders in perimenopausal women is increasing significantly. Both Chinese medicine and Western medicine are trying to exploring its pathologic mechanism to find the safe and effective methods of treatment. Menopausal sleeping disorder is the same as the perimenopause syndrome and insomnia category in traditional Chinese medicine. Kidney deficiency is the basic characteristic of perimenopause, whether kidney essence, kidney Qi, kidney Yin or kidney Yang can lead to blood stasis. It is believed that this disease results from kidney deficiency in the root and blood stasis in the tip, and on this basis women affected by many factors will result in disharmony between heart and kidney. In clinical practice, The method of tonifying kidney and activating blood applied to treat perimenopause syndrome with insomnia was testified to be effective. Therefore the author thought that kidney deficiency and blood stasis and disharmony between heart and kidney were the key factors of patients with insomnia in perimenopause.
8.Infertility caused by salpingitis treated based on theory of kidney deficiency and blood stasis.

Infertility caused by salpingitis is one of the main causes of secondary infertility in women. In recent years,the incidence has been increasing year by year. Modern medicine believes that this disease is a complication due to incomplete or delayed treatment of acute and chronic salpingitis,with no satisfactory drug therapy at present. Clinical therapies mainly include surgical treatment,interventional treatment and assisted reproductive technology. After summarizing the experience of predecessors and the clinical practice of treating infertility for many years,the author considers that the disease location is the cell veins,and the nature is mostly mixed of deficiency and excess. Kidney deficiency and blood stasis are the main pathogenesis of infertility caused by salpingitis. Blood stasis is the pathological basis,while kidney deficiency is the fundamental pathogenesis. Long-term kidney deficiency will lead to blood stasis,and blood stasis will aggravate kidney deficiency. Both of them are cause and effect to each other. Infertility caused by salpingitis is difficult to cure. Based on the theory of kidney deficiency and blood stasis,the basic principles of clinical treatment are tonifying kidney and activating blood circulation,removing blood stasis and dredging collaterals. Oral administration with traditional Chinese medicine combined with external therapies,such as enema,external application,acupuncture and moxibustion,have been achieved a good efficacy in repairing fallopian tube function and improving pregnancy rate. Therefore,the treatment of infertility caused by salpingitis based on " kidney deficiency and blood stasis" is worthy of further discussion in both clinical and experimental aspects.

9.Clinical experience of Bushen Huoxue therapy in treatment of infertility due to endometriosis.

Endometriosis is a common and difficult gynecological disease. The incidence of endometriosis has been increasing year by year. Because endometriosis mostly occurs in childbearing age,it can cause persistent damage to the fertility of patients,and is an important cause of infertility. Although endometriosis is a benign disease,it has malignant behavior,and is easy for relapse and metastasis and difficult to treat in clinic. Early diagnosis,comprehensive evaluation,formulation of programs and timely treatment play an important role in protecting patients' fertility. The pathogenesis of endometriosis is still unclear in modern medicine. Drugs,surgery and assisted reproductive technology are the main therapies. The author has achieved a good efficacy in the long-term treatment of infertility due to endometriosis. She believes that the disease is caused by kidney deficiency and blood stasis. They are cause and effect to each other,which form a vicious circle. In view of the basic pathogenesis of kidney deficiency and blood stasis,we should make good use of the method of Bushen Huoxue,and emphasize the treatment by stages: promoting blood circulation and removing blood stasis,and treating diarrhea with purgative in the menstrual stage; nourishing kidney and blood,and regulating thoroughfare and conception vessels in the postmenstrual stage; warming kidney and supporting Yang,and dredging collaterals and hastening excretion in the interval stage;and tonifying kidney and spleen,and managing Qi and activating blood in the premenstruum stage. Leech is commonly adopted in clinic prescriptions to remove accumulation of persistent blood stasis and regulate mood,and combined with enema of traditional Chinese medicine,hot compress and other external therapies to enhance the curative effect.
