Objective To promote the cure rate of operation and to decrease the operative complications in patients with upper urinary tract stone Methods The data of 962 patients with upper urinary tract stone operatively treated in our department from 1975 to 2002 were reviewed and analyzed Results Residual stones were found in 109 out of 673 patients (11 3 %) Of the 109 cases, conservative treatment, nephrectomy and various methods of treatment were performed in 21, 12 and 76 cases, respectively Conclusion Strict control of operative complication, proper selection of the operation type and reasonable management of the complication are the important measures to improve the cure rate of upper urinary tract stone, to greatly decrease the occurrence of residual stones and to reduce the possibility of nephrectomy
2.β-Catenin,β-Trcp and CRD-BP in colorectal cancer

Wnt/β-catenin signaling pathway plays an important role in the initiation and progression of colorectal cancer. The key to its carcinogenicity is related to the inhibition of β-catenin degradation, and the subsequent cytoplasmic accumulation and nuclear translocation of β-catenin, which activates the transcription of downstream target genes such as coding region instability determinant-binding protein (CRD-BP). In cancercells ,the expression of CRD-BP mediates the stabilization of β-Trcp, which in normal cells regulates the degradation of β-catenin. The activation of Wnt/β-catenin signaling pathway, overexpression of β-catenin and CRDBP, and the upregulation of β-Trcp by CRD-BP play important roles in the initiation, progression, and metastasis of colorectal cancer.

4.Relationship between muscarinic receptor subtypes and detrusor hyperreflexia in spinal cord transected rat

Objective To investigate the relationship between muscarinic receptor subtypes and detrusor hyperreflexia. Methods Spinal cord was transected in male Wistar rats at L 1~2 level, changes of urodynamics were monitored 3 weeks later by cystometric studies.Spontaneous contraction frequency and strength of detrusor strip mustarded by carbachol (muscarinic receptor agonist), atropine (nonselective M antagonist), methoctramin (M 2 receptor selected antagonist) and 4 DAMP (M 3 receptor selected antagonist) were measured. Results Bladders after SCI showed littler capacity, lower compliance and hyperreflexia. Detrusor of all groups contracted at a definite tension. The contraction frequency in the hyperreflexia group (4.797?0.257 times/min) was higher than in the control's(3.227?0.173 times/min).There were no obvious changes while detrusor strips were mustarded by different muscarinic receptor antagonists.Carbachol could increase detrusor contraction frequency( 6.316 ?0.409 times/min) and the effect could be blocked by atropine(5.304?0.095 times/min) and 4 DAMP(5.046?0.089 times/min). Conclusions Detrusor is a rythmatic tissue at a difinite tension.Excitability rise of detrusor is the main cause of hyperrefexia. M 3 receptor can enhance the excitability of detrusor causing direct contraction.The function of M 2 receptor is still unclear.
5.Effects of noradrenaline, isoptin, prostaglandin E and in-domethacin on ureteral dynamics and musculoelectrical activities in dogs

The effects of noradrenaline,isoptin.prostaglandin E and indomethacin on the dynamics and the muscloelectrical activities of the ureter were sutdied in dogs.It was found that no-radrenaline exerted excitation on the smooth muscle of the upper ureter,isoptin could remarkably reduce both the electrical and contractile activities of the ureter,prostaglandin E could intensify the activeites of the ureter,and indomethacin could the inhibit the synthesis of intrinsic prostaglandins and in turn relax the smooth muscle of the ureter.
6.Clinical significance of detrusor leak point pressure study in evaluating damage of the upper urinary tract secondary to neurogenic voiding dysfunction

Objective To evaluate clinical significance of detrusor leak point pressure (DLPP) in the damage of the upper urinary tract secondary to neurogenic voiding dysfunction. Methods 38 cases of voiding dysfunction received the urodynamic test,especially detecting of detrusor leak point pressure. Results 38 cases were divided into 2 groups by DLPP of 4.0 kPa,26 in high DLPP group and 12 in low DLPP group.The bladder capacity of the high pressure group is (422.95?183.27)ml,significantly less than the low pressure group (464.83?106.43)ml,and its compliance is lower as well as with DSD.Clinical results of B ultrasound,IVU,BUN,Cr show that the high pressure group has more serious than that in the low pressure group. Conclusions DLPP is valuable in evaluating the damage of upper urinary tract secondary to neurogenic voiding dysfunction.
7.The correlation between mucosal healing and prognosis of ulcerative colitis

Objective To investigate the correlation between mucosal healing after treatment and prognosis of ulcerative colitis (UC).Methods UC patients who were remitted after treatment (n =82) were divided into MH group(A:DA1,0-1 ; Geboes,0-1) and non-MH group (B:DAI,0-1 ; Geboes,2-5) according to the assessment and were followed up for 2 years.The baseline characteristics,histological parameters,serologic indices (albumin,ESR,CRP,ANCA,IL-1 β,IL-6 and IL-15) at 0,12,24 months or recurrence and hospitalization,colon surgery,colon cancer were recorded.The correlation between mucosal healing and prognosis were assessed.Results There were no significant differences between group A and B in recurrence rate and recurrence time(P > 0.05).The multivariate Cox regression analysis showed that gender (female),past recurrence,pANCA and basal plasmacytosis were independent risk factors for recurrence of UC (P < 0.05).The rehospitalization rate [27.5% (group A) VS 44.1% (group B) ; P =0.018 ; OR=2.24,95%CI (1.11-3.98)] and colectomy rate [0% (group A) VS 17.6% (group B); P=0.035; OR =5.43,95% CI (2.14-7.64)] between groups have significant differences,however,rates of cancer [0% (group A) VS 2.7% (group B) ; P=0.643; OR =3.43,95%CI (0.14-7.64)] were not significantly different.Conclusion Mucosal healing after treatment is associated with UC prognosis.
8.Correlation between the expression of gastric cancer tumor markers in preoperative serum and the risk of radical resection

Objective:To study the relationship between the expression of tumor markers and radical resection in patients with gastric cancer.Methods:The medical records of gastric cancer patients undergoing surgery in Department of Gastrointestinal Surgery, Puji District, Dongguan People′s Hospital Affiliated to Southern Medical University from June 2018 to December 2019 were retrospectively analyzed, and 136 patients were selected.Collected patient data, including general information, the expression levels of tumor markers carcinoembryonic antigen(CEA), carbohydrate antigen 19-9(CA19-9), carbohydrate antigen 72-4(CA72-4), and the surgical method used.The relationship between the expression of tumor markers and radical resection was observed.Results:There was no statistically significant difference in the radical resection rate of gastric cancer in different genders, ages, and locations of gastric cancer (all P>0.05). TNM classification (tumor node metastasis classification) stage III patients had a lower radical resection rate (63.3%(38/60)) than stage I (100%(27/27)) and II (100%(49/49)) (χ 2=58.166 and 81.208, P<0.001). The radical resection rate of CEA(+ ) patients (47.8%(44/92)) was lower than that of CEA(-) patients (90.9%(40/44))(χ 2=23.394, P<0.001). The radical resection rate of CA19-9(+ ) patients (47.7%(42/88)) was lower than that of CA19-9(-) patients(87.5%(42/48))(χ 2=20.804, P<0.001). The radical resection rate of CA72-4(+ ) patients (54.3%(51/94)) was lower than that of CA72-4(-) patients (78.6%(33/42)) (χ 2=7.268, P=0.007). The variables with P<0.1 in univariate analysis were included in the logistic regression model, including 4 variables including TNM stage, CEA, CA19-9, and CA72-4.The results showed that TNM staging ( OR=1.169, 95% CI=0.925-1.634, P=0.001), CEA ( OR=1.067, 95% CI=1.364-4.338, P=0.024), CA19-9( OR=3.012, 95% CI=1.679-6.317, P=0.007), and CA72-4 were independent risk factors for radical resection( OR=5.364, 95% CI=0.675-3.224, P=0.004). The number of positive expressions of tumor markers was negatively correlated with the radical resection rate ( r=-0.621, P<0.05). The expression levels of CEA, CA19-9, and CA72-4 in patients with radical resection were 75.36(3.76, 198.20)μg/L, 152.76(34.81, 241.09)kU/L, 126.60(4.01, 218.07)kU/L, respectively.The expression levels of CEA, CA19-9, and CA72-4 in the radically resected patient group were 173.65(120.78, 254.87) μg/L, 255.88(102.45, 395.11) kU/L, 201.71(79.15, 325.92)kU/L.The expression level of CEA, CA19-9, CA72-4 in the group with radical resection were lower than that in the group with no radical resection, and the difference was statistically significant (the Z values were 10.672, 8.945, 9.862, all P<0.001). ROC curve showed that AUC of CEA, CA19-9 and CA72-4 were 0.627, 0.714 and 0.768, respectively.The best cut-off value of CA72-4 was 87.62 kU/L, the sensitivity was 88.1% (74/84) and the specificity was 90.4% (47/52). Conclusion:The expression levels and number of tumor markers CEA, CA19-9, CA72-4 in patients with gastric cancer were correlated with the risk of radical resection.
9.Expressions of calpain 1 and caspase-3 in lichen planus lesions and their relationship with apoptosis in keratinocytes

Objective To quantify the expression levels of calpain 1 and caspase-3 in lichen planus (LP) lesions and their relationship with the apoptosis in keratinocytes.Methods Biopsy samples were obtained from the lesions of 20 patients with LP and normal skin of 10 healthy controls,and embedded in paraffin.Terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling (TUNEL) was used to evaluate the apoptosis in keratinocytes,and immunohistochemical staining to detect the expressions of calpain 1 and caspase-3,in these tissue specimens.Data were processed by SPSS 13.0 software.Comparison analysis was carried out by t test for apoptosis index,and by rank sum test for the expressions of calpain 1 and caspase-3.Spearman correlation analysis was conducted to evaluate the relationship between these parameters.Results The apoptosis index of keratinocytes was higher in LP lesions than in the normal skin (67.59 ± 13.50 vs.28.26 ± 7.56,t =8.52,P < 0.01).Significantly increased expressions of calpain 1 and caspase-3 were observed in the epidermis of LP lesions compared with the normal skin (T =78.00 and 77.00,respectively,both P < 0.01).The expressions of both calpain 1 and caspase-3 were positively correlated with apoptosis index of keratinocytes (r =0.71 and 0.74,respectively,both P < 0.01).Conclusions The expressions of calpain 1 and caspase-3 are upregulated in LP lesions,which may be closely associated with the accelerated apoptosis in keratinocytes.
10.Inhibitory effect of SIN-1 on ICAM-1 expression in rat kidney after ischemia/reperfusion injury

Objective To explore the therapeutic mechanisms of nitric oxide donor (SIN 1) in ischemic acute renal failure (IARF). Methods The effects of SIN 1 on intercellular adhesion molecule 1 (ICAM 1) expression and inflammatory cell infiltration in rat kidney after ischemia/reperfusion (I/R) injury were examined using ICAM 1, ?2 integrin polyclonal antibody, and immunohistochemistry. The renal functions were measured simultaneously. Results Progressive increased expression of ICAM 1 was found in kidneys, especially in vasa recta, after IR injury. ?2 integrin positive cells increased simultaneously in the outer medulla. The two indexes reached the peak values at 24 h after I/R injury. SIN 1 infusion at the beginning of reperfusion could remarkably inhibit the enhanced ICAM 1 expression, reduce the local infiltration of inflammatory cells, and ameliorate the renal functions. Conclusion NO donor can inhibit the renal expression of ICAM 1 and the infiltration of leukocytes after ischemia reperfusion injury, which may play an important role in the treatment of ischemic acute renal failure.
