Objective To investigate the effects of nonadrenergic noncholinergic (NANC) neuro transmitter on the frequency and strength of the automatic contractility of detrusor strip. Methods Cystometry was performed to observe the occurance of detrusor instability (DI) after 6 weeks of Wistar rats LUT. Then the rats were sacrified and their detrusors were made into strips to observe the effect of NANC vasoactive intestinal peptide (VIP) and adenosine triphoate(ATP) on the frequency and the strength of automatic contractility. Results The occurrence rate of DI was 62% after 6 weeks of obstruction of LUT; VIP and ATP could significantly inhibit the automatic contraction ability of detrusor strip, decreasing the frequency and the strength of the automatic contraction of detrusor ( P 0.05). Conclusions VIP and ATP can significantly inhibit the frequency and strength of the automatic contractibility of normal and obstructive detrusor.

Objective: To develop a molecular screening test for genetic defects on hearing loss related genes has significant impacts on early identification of hereditary hearing loss and genetic susceptibility to aminoglycoside ototoxicity. Early identification of pre-lingual hearing loss is very important for patient's language development, academic achievement, and social skill. Two common mutations, the 235delC in GJB2 gene and the mutation A1555G in mitochondrial DNA, are included in the newly developed screening panel for Chinese population. Methods: A molecular genetic assay, based on fluorescent labeled multiplex PCR and automatic DNA fragment analyzing techniques, was developed to detect both mutations simultaneously. Results: This assay was able to detect both mutations from patient's samples, and pooled DNA tests, as well as suitable to detect mutation from the DNA extracted from dried blood spot and buccal swab. Conclusion: This assay could be a useful tool for newborn screening and carrier screening for the hereditary hearing loss for the Chinese population.

Objective To explore the mechanism of the referred pains in pelvic pain due to chronic prostatitis and the relation between pain due to prostatitis and pelvic floor muscle dysfunction. Methods The retrograde fluorescent double labeling and immunohistochemistry were used. A total of 24 Wistar rats were divided two groups, 12 in each group. In the first group, Propidium Iodide(PI) was injected into the prostate, Bisbenzimide(Bb)into the external anal sphincter. In the second group, PI was injected into the prostate, Bb into genitofemoral, iliohypogastric, ilioinguinal, femoral and lateral femoral cutaneous nerves. Results The fluorescent double labeled cells were found in the L 1 L 2 and L 6~S 3 dorsal root ganglia(DRG) and some of them were confirmed to contain calcitonin gene related peptide(CGRP) and P substance (SP) by immunohistochemistry. Conclusion The findings confirm that the peripheral process of DRG cells dichotomizes into prostate, external anal sphincter and somatic parties, and some of these cells contain CGRP and SP, which indicates the referred pains in pelvic floor may be caused by axon reflex in the peripheral process of DRG cells. Meanwhile, neurogenic inflammation might play an important role in this persistent pain status. Pain due to prostatitis is correlated with pelvic floor muscle dysfunction.

Objective To elucidate the neurological mechanism of the referred pains in pelvic floor associated with the prostate. Methods The rat prostate was electrically stimulated, then the elicited action potentials (AP) of the lumbar and sacral nerves and the compound muscle action potentials from the external sphincter muscle of anus were detected. Results Action potentials from the iliohypogastric, genitofemoral, lateral femoral cutaneous, femoral, sciatic nerves compound muscle action potentials (CMAP) from the anal external sphincter were recorded. Conclusion Electric stimulation of the prostate can induce action potentials of the lumbar and sacral efferent nerves through the spinal reflex, resulting in referred pains in the perineal and pelvic areas due to muscle contraction.

Background:Gastrointestinal dysmotility is commonly seen in individuals exposed to acute plateau hypoxia. Its pathogenic mechanism is still not clear and intervention study is rarely performed. Aims:To investigate the influence of plateau hypoxia on small intestinal motility of rats, its possible mechanism, and the intervention effect of raw Atractylodes macrocephala. Methods:Seventy Wistar rats were randomly divided into 7 groups:control group, two high altitude model groups(3 500 m and 5 000 m),trimebutine and raw Atractylodes macrocephala groups at 3 500 m or 5 000 m altitude. Rats in six experiment groups were placed in a hypobaric chamber mimicking 3 500 m or 5 000 m altitude and given trimebutine/ raw Atractylodes macrocephala/ saline intragastrically for 3 days. Then all the rats were lavaged with 2 mL ink and sacrificed 30 minutes later. The propulsion rate of ink in small intestine was measured, the pathological changes of small intestine tissue were examined, and the expression of 5-HT4 receptor was determined by immunohistochemistry. Results:Compared with control group, the propulsion rate of small intestine and immunopositive area of 5-HT4 receptor were reduced in model group at 3 500 m altitude, while those in model group at 5 000 m altitude were increased(P ﹤0. 05). Moderate-to-severe mucosal injury was observed in two model groups. In model rats treated with raw Atractylodes macrocephala, the abnormalities in small intestine propulsion and 5-HT4 receptor induced by high altitude(3 500 m and 5 000 m)returned to the control level(P ﹤ 0. 05),and the mucosal injury ameliorated simultaneously. Efficacy of raw Atractylodes macrocephala was prior to trimebutine,a positive control. Conclusions:Acute plateau hypoxia may induce small intestine dysmotility with diverse manifestations, and altitude is a crucial determinant in this process. Raw Atractylodes macrocephala can exert therapeutic effect on this dysmotility by modulating 5-HT4 receptor, and it is also effective in repairing mucosal injury.

Objective:To investigate the clinical efficacy of laparoscopic resection of retrorectal cystic lesions. Methods:The retrospective and descriptive study was conducted. The clinicopathological data of 58 patients undergoing laparoscopic resection of retrorectal cystic lesions in the Peking Union Medical College Hospital, Chinese Academy of Medical Sciences from August 2012 to August 2019 were collected. There were 5 males and 53 females, aged from 15 to 70 years, with a median age of 38 years. All the 58 patients underwent laparoscopic resection of retrorectal cystic lesions and the combined operation through the transsacral approach was chosen according to the patient condition. Observation indicators: (1) surgical situations; (2) postoperative situations; (3) postoperative histopathological examination; (4) follow-up. Patients were followed up regularly using outpatient examination once every 6 months during the first postoperative year and once every 12 months after the first postoperative year. The recurrence of cysts was evaluated by computed tomography or magnetic resonance imaging examinations during the follow-up up to August 2020. Measurement data with normal distribution were represented as Mean± SD and measurement data with skewed distribution were described as M(range). Count data were described as absolute numbers. Results:(1) Surgical situations: of the 58 patients, 54 cases underwent laparoscopic resection of retrorectal cystic lesions and 4 cases underwent laparoscopic resection of retrorectal cystic lesions combined with the transsacral approach operation. One of the 58 patients who had a huge cyst surrounding the rectum underwent transverse colostomy after repairing the damage of separated posterior wall of rectum. Two cases underwent preventive transverse colostomy because the external rectal wall heat injury could not be excluded after separation of the tight adhesion between cyst and rectum. The operation time and volume of intraoperative blood loss were (123±56)minutes, 20 mL(range, 5?500 mL) of 54 cases who underwent laparoscopic resection of retrorectal cystic lesions and (232±38)minutes, 90 mL(range, 30?800 mL) of 4 cases who underwent laparoscopic resection of retrorectal cystic lesions combined with the transsacral approach operation, respectively. (2) Postoperative situations: 7 of the 58 patients had complica-tions. Of the 7 patients, 2 cases had postoperative rectal fistula and were cured after the treatment of transverse colostomy combined with pelvic drainage, 2 cases had postoperative urinary tract infection and were relieved after anti-infection treatment, 2 cases had urinary retention after removal of catheter and were recovered after 3 weeks of re-indwelling catheter, and 1 case had poor incision healing of transsacral and was healed after wound dressing change. The duration of postoperative hospital stay of the 58 patients was (7±4)days. (3) Postoperative histopathological examination: results of the postoperative histopathological examination showed that there were 26 of 58 patients with epidermoid cyst, 20 patients with teratoma (2 cases with mature teratoma accompanied by mucinous adenocarcinoma and 1 case with mature teratoma accompanied by neuroendocrine carcinoma), 10 patients with dermoid cyst, and 2 patients with tailgut cyst. (4) Follow-up: 57 of the 58 patients were followed up for 2-85 months, with a median follow-up time of 51 months. Of the 57 patients who were followed up, 1 patient was diagnosed with buttock subcutaneous cyst at postoperative 8 months and treated with local excision, 1 patient was diagnosed with a small presacral cyst recurrence by pelvic magnetic resonance imaging at postoperative 6 months and continued follow-up as the cyst without obvious enlargement, and the other 55 patients had no cyst recurrence. Conclusion:The laparoscopic resection of retrorectal cystic lesions is safe and feasible.
