2.THE INVESTIGATION OF THE ANESTHETIZATION OF FOREIGN BODY EXTRACTION IN RESPIRATORY TRACT OF INFANTS

Objective:To investigate the methods of the anesthetization of the foreign body extraction in respiratory tract of infants.Methods:This clinical study compared two anesthetic methods in foreign body extraction of respiratory tract of infants:Ketamine+Sodium r-hydroxybutyrate intravenous (group A) and Ketamine+propofol intravenous (group B) and analyzed the advantages and disadvantages of both methods.Results:Compared with group B,the extent of the changes of heart rate and blood oxygen saturation in patients of group A is littler,and the incidence rate of breath holding and asphyxiation is littler too.Conclusions:The anesthetic effect of Ketamine+Sodium r-hydroxybutyrate intravenous in better than that of Ketmaine+propcfol intravenous.
3.Management of Anesthesia of Hirschsprung's Disease Radical Operation By Abdominoscope

Objective To investigate the effect of the general anesthesia combined with peridural anesthesia on hirschsprung's radical operation through children abdominoscope. Methods 40 children patients with hirschsperung were elective eradicative operated by using endotrachal intubation complex vein general anesthesia combined with continuons peridural anesthesia.ECG,HR,MAP,SPO 2,P ET CO 2 were monitored, and blood gas were recorded and analyzed before pneumoperitoneum,15 minutes and 40 minutes after pneumoperitoneum,1 minute after deflation,before decannulation at the operation termination.Results Compared with before pneumoperitoneum date,HR inereased obviously 40 minutes after pneumoperitoneum,and before decannulation at the operation termination,peak pressure in airway increased obviously after pneumoperitoneum,the difference was notable,SPO 2 decreased after pneumoperitoneum,the difference was also significant.pH decreased gradually with the time prolongation,there was obviously difference.The trend of P ET CO 2 and PaCO 2 changes were analogous,P ET CO 2 had much notable difference after pneumoperitoneum,PaCO 2 had notable difference at the operation termination,and before decannulation,it was decreased to the basic value.Conlusions Hirschsprung's Disease eradical operation using general anesthesia combined with peridural anesthesia under abdominoscope guide,can immobilize ill children completely,relieve pain perfectly,muscular relaxation satisfactory,the physical interference to body is little and less complication.
4.Prevention of post-ERCP pancreatitis and hyperamylasemia by combination of proton pump inhibitors and octreotide

Objective To assess the efficacy of proton pump inhibitors for preventing post-endoscopio retrograde cholangiopancreatography (post-ERCP) pancreatitis and hyperamylasemia. Methods A total of 250 patients, who underwent successful ERCP, were randomized into control group (n = 126) or treatment group (n=124) after ERCP. All patients received octreotide, the patients in treatment group received omeprazole 40 mg i. v. every 12 h; while the patients in control group received same amount of normal saline. The serum level of amylase at 4, 24 h and TNF-a were determined and the incidence of post-ERCP pancreatitis was documented. Results There were no differences in age, sex and proportion of therapeutic procedures performed between the two groups. The serum amylase and TNF-a levels at 4, 24h were (221 ± 31) U/L, (181 ±39) U/L, (0.264 ±0.052) ng/ml and (0.257 ±0.071) ng/ml in treatment group, respectively; which were significantly lower than (272 ±32) U/L, (227 ±30) U/L, (0. 372 ±0.047) ng/ml and (0.422 ±0.026) ng/ml in the control group (P<0.05). The incidence of pancreatitis in proton pump inhibitors group was 1.6%, which was significantly lower than that in control group (5.6% , P = 0. 04) . Conclusions Combination of proton pump inhibitors and octreotide could effectively decrease the incidence of post-ERCP hyperamylasemia and pancreatitis.
5.The effect of B7-H3 on human hepatocellular carcinoma cell line HepG2 mediating regulation on activation,cell cycle and IL-17 secretion of human peripheral blood CD8+T cells

Objective To investigate the effect of B7-H3 on human hepatocellular carcinoma cell line HepG2 mediating regulation on human peripheral blood CD8+T cell activation,cell cycle and secretion of IL-17.Methods The expression of the B7-H3 on HepG2 cells was detected by RT-PCR and FCM respectively.B7-H3 was silenced by PGPU6/GFP/neo-B7-H3shRNA plasmid via cathodolyte liposome transfection method.CD8+T cells were sorted from healthy human peripheral blood with immunomagetic beads.The effect of HepG2 cells on activation,cell cycle and cytokine secretion of CD8+T cells which was stimulated by PHA or PMA respectively were analyzed by FCM.Results B7-H3 was highly expressed on HepG2 cells,and PGPU6/GFP/neo-B7-H3shRNA plasmid could effectively block down its expression.Otherwise,HepG2 cells could inhibit the expression of CD69,the early activation phenotype of T cell,blockade B7-H3 on HepG2 cells could significantly attenuate the inhibitory effects.Likewise,blockade B7-H3 on HepG2 cells apparently reversed the inhibitory effects of HepG2 cells on CD8+T cell cycle through down-regulating the cell number in G0/G1 phase and up-regulating the cell number in S phase;Moreover,HepG2 cells caused a sharp increase of IL-17 which was secreted by CD8+T cells and the level of IL-17 was further up-regulated after blocking down B7-H3.Conclusion HepG2 cells highly expressed B7-H3 that could promote the inhibitory the effect of HepG2 on expression of CD69 and cell cycle of CD8+T cells.HepG2 cells were able to up-regulate the level of IL-17 secreted by CD8+T cells,in which B7-H3 played an inhibitory role.
6.Clinical Observation of Abdominal Acupuncture Combined with Chinese Medicine for Treatment of Qi-deficiency Functional Constipation

Objective To observe the therapeutic effect of abdominal acupuncture combined with Chinese medicine for the treatment of functional constipation. Methods Ninety-three qi-deficiency functional constipation patients were evenly randomized into western medicine group, Chinese medicine group and combination group. The western medicine group was given oral use of Dupbalac, Chinese medicine group was given modified Buzhong Yiqi decoction, and the combination group was given abdominal acupuncture and oral use of modified Buzhong Yiqi decoction. The treatment of the three groups covered 3 months. After treatment, the therapeutic effect was evaluated accord ing to the symptom scores and the examination results of gastrointestinal kinetics. Results ( 1) The total effective rate for relieving symptoms was 100.00% in combination group, 90.32% in Chinese medicine group, and 38.71% in western medicine group, and the effect was the best in the combination group ( P<0.05). ( 2) The combination group had stronger effect on relieving dyskinesis of anal sphincter, rectal sensibility, colonic transportation than Chinese medicine group and western medicine group (P<0.05). Conclusion Abdominal acupuncture combined with Chinese medicine shows certain therapeutic effect for the treatment of qi-deficiency functional constipation, and the effect is superior to that of Chinese medicine or Dupbalac.
7.Analysis on Usage Situation of Traditional Chinese Medicine Injections in Our Hospital

Objective To know the usage situation of traditional Chinese medicine (TCM) injections;To improve the level of rational administration of TCM injections.Methods Totally 2686 medical records of inpatients from Aug. 2012 were randomly selected. Patient genders, ages, medication types, dosage and solvent, etc. were analyzed respectively.Results There are 38 varieties of traditional Chinese medicine injections in this survey. There are 22 varieties of traditional Chinese medicine injections involving the unreasonable usage in solvent, a total of 453 times. The total qualified rate was 80.59%. There are 9 varieties of traditional Chinese medicine injections involving the unreasonable dosage. The total qualified rate was 88.77%.Conclusion In order to guarantee the medication safety, the reasonable usage of TCM injections should be strengthened in the clinic, which can be realized by reading package inserts carefully, prescribing through syndrome differentiation.
8.HPLC fingerprint analysis of Jiketing Granules

AIM: To establish the fingerprint analysis method of Jiketing Granules(Fructus Forsythiae,Radix Scutellariae,Radix Bupleuri,etc) by HPLC-UV.METHODS: Aanalysis was performed on an alltima C_(18)(4.6 mm?250 mm,5 ?m) column with a acetonitrile-0.1% acetic acid gradient.Detection time was 55 min.The flow rate was 1.0 mL/min.The montoring wavelength was changed.The colunm temperature was at 30℃. RESULTS: 21 peaks were separated on HPLC fingerprint in Jiketing Granules. CONCLUSION: The method is reliable,accurate and can be used as a quality control for Jiketing Granules.
9.RASSF1A expression mediated by lentivirus inhibits growth of small-cell lung cancer cell line H446

AIM: To explore the inhibitory effect of Ras-association domain family 1A ( RASSF1A) on the small-cell lung cancer cell growth .METHODS:The lentiviral expression vector containing RASSF1A gene was constructed and used to infect the small-cell lung cell line H446.The growth curve and cell cycle were detected by MTT assay and flow cytometry.The mRNA and protein levels of cell cycle-associated proteins were determined by real-time PCR and Western blotting.RESULTS:We obtained the H446 cells in which RASSF1A was stably expressed (named RASSF1A-H446). Compared with normal cell group and negative cell group , RASSF1A inhibited the proliferation of H446 cells, and arrested H446 cells in G1 phase.The expression of p21 and p27 was significantly increased , and E2F1 was significantly decreased in RASSF1A-H446 cells.CONCLUSION:RASSF1A inhibits the H446 cell growth by increasing the expressions of p 21 and p27, and decreasing the expression of E 2F1.
