1.The study of the angitensin-converting enzyme gene polymorphism in hepatorenal syndrome

Objective To investigate the insertion/deletion(I/D) polymorphism of ACE gene in uncompen-sated hepatocirrhosis with hepatorenal syndrome(HRS). Methods ACE I/D polymorphism was detected by poly-merase chain reaction amplification of DNA fragment in 96 cases of uncompensated bepatocirrhnsis with HRS, every group as controls were involved in this study. At the same time,GPT,GOT,sarum creatinine(SCr), blood urea nitro-gen(BUN) and glomerular filtration rate(GFR) of every case were measured and the difference of these targets among different genotypes were compared. Results There was no significant difference for every genotype and allele fre-quency between the HRS group and every group as controls(P >0.05). The I allele frequencies were higher than the one of D allele for every groups except other liver disease group(P < 0.01). But in the every group as controls, there was no significant difference for the genotype frequency among three groups and in the HRS group, the Ⅱ genotype frequency was higher than the one of ID and DD(P < 0.05). SCr etc. of the Ⅱ genotype were higher than the one of ID and DD(P<0.05) and GFR of the Ⅱ genotype was lower than the one of ID and DD in the HRS group(P<0.05). Conclusion There was relationship between ACE gene polymorphism and the incidence of uncompensated bepatocirrhosis with HRS.
2.Evaluation of the efficiency of medical resource utilization in public traditional Chinese medicine hospitals in Gansu province

Objective:To analyze the efficiency of medical resource utilization in public traditional Chinese medicine (TCM) hospitals in Gansu province from 2016 to 2020, so as to provide decision-making reference.Methods:The number of in-service staff, actual number of open beds, number of diagnosis and treatment, and number of discharge from TCM hospitals in Gansu province from 2016 to 2020 were extracted, and their technical efficiency, pure technical efficiency, scale efficiency, and returns to scale were analyzed by data envelopment analysis.Results:From 2016 to 2020, the average technical efficiency, pure technical efficiency, and scale efficiency of the sample hospitals were 0.647, 0.680, and 0.952, respectively. Among them, 213 hospitals (48.2%) were in a decreasing state of returns to scale, 54 hospitals (12.2%) were in a constant state of returns to scale, and 175 hospitals (39.6%) were in an increasing state of returns to scale; Out of the 45 tertiary hospitals, 42 (93.3%) were in the stage of diminishing returns to scale, while 226 (56.9%) of 397 secondary and lower hospitals were in a state of constant or increasing returns to scale.Conclusions:The utilization efficiency of medical resources in public TCM hospitals in Gansu province is relatively low, and there is a significant gap between different levels of TCM hospitals.
3.Analysis of distribution characteristics on exercise behavior stages and its predictive factors in patients with chronic obstructive pulmonary disease after discharge

Objective To understand the characteristics and the predictors of the distribution of exercise behavior stages in COPD patients after discharge from the hospital,and to provide a reference basis for the implementation of the whole process and continuous rehabilitation care decision-making.Methods Using the convenience sampling method,380 COPD patients who were hospitalized in the departments of respiratory medicine or geriatrics in 2 tertiary-level hospitals in Chongqing from February 2022 to February 2023 were selected for the study,and were followed up until 6 months after the patients were discharged from the hospital.Questionnaires were administered using the self-designed General and Disease-Related Information Questionnaire,Exercise Stages of Change Scale,Exercise Benefits and Barriers Scale,Exercise Self-Regulatory Efficacy Scale,Social Support Rate Scale and Hospital Anxiety and Depression Scale.Characteristics and predictors of exercise behavior in COPD patients after hospital discharge were analyzed using univariate analysis and ordered multicategorical logistic regression.Results The included 380 COPD patients of post-discharge exercise behavior were 53 cases(14.0%)in the pre-intentional stage,97 cases(25.5%)in the intentional stage,103 cases(27.1%)in the preparatory stage,75 cases(19.7%)in the action stage,and 52 cases(13.7%)in the maintenance stage.The age,monthly household income,daily chores,sleep,exercise habits,and history of the current inpatient rehabilitation,number of acute episodes in the last year,whether home oxygen therapy and regular use of medication for respiratory diseases,degree of dyspnea,perceived exercise benefit/impairment,social support,and depression were the main predictors of the distribution of the stages of exercise behavior in COPD patients after discharge from the hospital.Conclusion Most COPD patients did not develop regular exercise behavior habits after discharge,and the level of their exercise behavior was affected by a variety of factors,including individual characteristics,disease factors,decision-making balance,social support,and psychology,suggesting that healthcare professionals should develop personalized early exercise interventions according to the characteristics of the behavioral stage in which the patient is located in order to increase the long-term effects of the intervention.

COVID-19 is caused by a novel coronavirus-severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2), which has being spreading around the world, posing a serious threat to human health and lives. Neutralizing antibodies and small molecule inhibitors for virus replication cycle are the main antiviral treatment for novel coronavirus recommended in China. To further promote the rational use of antiviral therapy in clinical practice, the National Center for Infectious Diseases (Beijing Ditan Hospital Capital Medical University and the First Affiliated Hospital, Zhejiang University School of Medicine) invited experts in fields of infectious diseases, respiratory and intensive care to develop an Expert Consensus on Antiviral Therapy of COVID-19 based on the Diagnosis and Treatment Guideline for COVID-19 ( trial version 10) and experiences in the diagnosis and treatment of COVID-19 in China. The consensus is concise, practical and highly operable, hopefully it would improve the understanding of antiviral therapy for clinicians and provide suggestions for standardized medication in treatment of COVID-19.
