Objective To investigate the effect of a single dose of propofol at the end of surgery on pediatric anesthesia emergence delirium undergoing adenotonsillectomy during sevoflurane anesthesia. Methods Sixty pediatric patients for adenotonsillectomy were randomly divided into propofol group and control group (30 cases in each group), they were undergoing sevoflurane induction and maintaince. The pediatric patients in propofol group were received 1 mg/kg 10% of the propofol intravenous injection at the end of surgery, while the pediatric patients in control group were received the same volume physiological sailine intravenous injection. Duration of surgery, sevoflurane administration, anesthesia time, extubation time were recorded. The highest PAED score after extubation in 30 min were recorded. The time spent in PACU and the FLACC pain score were recorded. Results The PAED score in propofol group was significantly lower than that in control group [(7.5 ± 3.9) scores vs (10.9 ±4.5) scores, P<0.05]. The incidence rate of postoperative delirium in propofol group was significantly lower than that in control group [20.0% (6/30) vs 46.7% (14/30), P < 0.05 ] . Duration of extubation in propofol group was slightly longer than that in control group[(10.2 ± 1.1 ) min vs (9.1 ± 1 .5 ) min, P < 0.05].There was no statistical significant difference in duration of anesthesia, the time spent in PACU and the FLACC pain score (P >0.05).Conclusions The administration a single dose of 1 mg/kg propofol at the end of surgery can effectively reduce pediatric anesthesia emergence delirium undergoing adenotonsillectomy during sevoflurane anesthesia, while can't extend the duration of anesthesia and the time spent in PACU. It has some clinical value.

AIM To investigate the transdermal delivery effects of cyclosporine A solubilized in mixed micelles composed of phospholipid and different surfactants. METHODS When applied onto the excised abdominal skin of the mice occlusively, the enhancing effects of various mixed micelles on the penetration of cyclosporin A were assessed by an in vitro permeation technique. In vivo study was carried out by topical application of sodium cholate-phospholipid mixed micelles onto the mice skin and drug blood concentration was detected. RESULTS In vitro, mixed micelles containing different surfactants displayed distinct permeability and corresponded to the following order: sodium cholate > sodium deoxycholate > Trition X-100 > Tween-20. In vivo, peak drug concentration was detected at 5 h and after that the concentration fell down slowly. CONCLUSION Mixed micelles were shown to be efficient carrier for the transdermal delivery of the lipophilic polypeptide when kept in solution during the application process.

PurposePrimary chest wall rhabdomyosarcoma (RMS) is very rare with limited imaging characteristic studies in the literature. This paper analyzes the CT imaging features of chest wall RMS in children to improve the diagnostic accuracy.Materials and MethodsThe imaging data of contrast enhanced CT scan of pathology conifrmed chest wall RMS in ifve children were analyzed.ResultsThe lesion was located in the anterior chest wall in one case, in the posterior chest wall in two cases, and the lateral chest wall in two cases (axillary). The tumors were round or spindle in shape with shallow spiculation. Plain CT showed heterogeneous density with patchy low-density necrotic area in two cases, and homogeneous attenuation in three patients. In all ifve cases there was no calciifcation or fatty tissue. The tumor involvement of adjacent spinal canal was seen in one case. Visceral compression was evident including lung parenchyma in one case, heart and liver in one case. Tumor blood vessel growth was seen in two cases. All ifve lesions were adjacent to the ribs, humerus, scapula and the spine with bone destruction in one case. On contrast enhanced scan, all ifve cases demonstrated heterogeneous mild to moderate enhancement, more prominent in the periphery. There were enlarged feeding arteries. Necrotic areas did not enhance. In two cases there were pulmonary metastases. Pleural effusion and ascites were identiifed in one case. There was lymph node metastasis in one case.ConclusionThe CT manifestation of children's chest wall RMS for chest wall include large soft tissue mass, heterogeneous density, no calciifcation or fatty tissue, partial necrosis, adjacent tissue compression, lymph node or distant metastasis. Combining with clinical manifestations, comprehensive analysis of contrast enhanced CT imaging can improve diagnostic accuracy.
