1.Rehabilitation of swallowing disorders after acute stroke

目的探讨对急性脑卒中后吞咽障碍的康复。方法急性脑卒中后吞咽障碍的患者52例,对其中26例患者在给予药物治疗的同时,按不同程度制定不同的训练计划。对照组仅进行神经内科常规治疗和护理。结果对内囊以及以上部位病损引起的吞咽障碍,观察组康复效果好,多在10天内改善,优于对照组。结论急性脑卒中后吞咽障碍患者接受康复护理,能改善吞咽功能。
2.Comparation of Total Iridoids in Valerian from Different Habitats

Objective To determine and compare the contents of total iridoids in valerian from different origins.Methods The total iridoids in valerian was detected by reaction with alkaline hydroxylamine-perchloric acid iron reagent,and the absorbance was measured at 522 nm by spectrophotometry. Results The content of valepotriate and its absorption were linear within the range of 0. 05-1. 40 mg·mL-1 . The average recovery was 98. 50%,and RSD was 1. 02%. There was significant difference between the contents of iridoid in valerian from different habitats. Conclusion This method is accurate and reliable for evaluating the quality of valerianand. The contents of valeriana is related to its habitats.

4.Endoscopic resection of gastrointestinal submucosal tumors

Objective To study on the endoscopic resection of submucosal tumors (SMT) in gastro-intestinal tract with especial regards to the effectiveness and safety, and the value of endoscopic ultrasonogra phy ( EUS) in SMT before endoscopic resection. Methods Endoscopic therapy was performed in 71 patients with gastrointestinal SMT diagnosed by endoscopy, and of them 64 patients (90. 1 % ) were examined by EUS before the procedure. The SMT located in the esophagus 36 cases, stomach 29 cases, duodenum 3 cases and rectum 3 cases. The median tumor diameter was 14. 2 mm ( ranged 6-20 mm). Endoscopic mucosal resection was performed using a two-channel endoscope after submucosal injection of saline solution in 55 cases, then the SMT resected. In 16 patients, the SMT was resected completely with one-step standard procedure using cap-fitted endoscope ( 10 cases, the SMT less than 10 mm in diameter) or two-channel endoscope (6 cases). Results Sixty-eight of seventy-one solid SMT were removed completely under endoscopy and 2 SMT (one ectopic pancreas and one gastric leiomyoma) were resected incompletely; the findings were showed at 4th week by repeated endoscopy. One patient with rectal leiomyoma required surgical resection after unsuccessful endoscopic resection. The follow up period was 18. 7 months on average, no recurrences were found in 67 cases. Histological results showed 51 (71. 8% ) leiomyomas, one granular cell tumor, 4 fibromas, 3 ectopic pancreas, 3 lipomas, 2 stromal tumors and 2 carcinoids in the SMT. Five tumors of mesenchymal tissue , their histological origin cannot be firmly defined due to lack of immunoassayed specimen. Bleeding occurred in only 9 of 71 cases and was easily managed with endoscopic haemostatic measures. One patient with stromal tumor suffered from gastric perforation after the procedure. Conclusions Endoscopic resection of gastrointestinal SMT is a safe and effective procedure and obtains the histological diagnosis simultaneously. EUS is helpful in selecting indicated cases for endoscopic resection.
5.Effects of hypoxia on hemodynamics and hemorrheology of pulmonary circulation in the dog

The change of hemodynamics and hemorrheology induced by hypoxia challenge in the dog were investigated. The results showed that hypoxia caused significant decrease of cardiac output,increased pulmonary artery pressure and pulmonary vascular resistance. Hypoxia also caused increase of blood viscosity. In terms of changes of the reduced viscosity is directly related to the deformation and agglutination properties of red blood cells.
6.Selection on the optimal extraction of Weining-Dispersible Tablet by central composite design-response surface methodology

AIM: To optimize the extraction of Weining-Dispersible Tablet(Radix et Rhizoma notoginseng,Radix et Rhizoma ginseng) with the help of central composite design-response surface methodology. METHODS: The main influencing factors of extraction effect included the quantity of ethanol,the concentration of ethanol and extraction time and the extraction conditions were evaluated by the extract yield and the contents of total notoginseng saponins.The optimal extraction of Weining-Dispersible Tablet was selected by central composite design-response surface methodology. RESULTS: The optimal conditions consisted of extraction two times,2 h each time with ten fold of 70% ethanol. CONCLUSION: It is simple,convenient and high predictive to optimize the extraction craft of Weining-Dispersible Tablet by central composite design-response surface methodology.
7.Evaluation of Bacterial Drug Resistance Detection in Lower Respiratory Tract Infection Therapy

OBJECTIVE To evaluate bacterial drug resistance detection value in lower respiratory tract infection treatment.METHODS The data of distribution and antimicrobial resistance of clinical strains in sputum samples in patients with lower respiratory tract infection within some periods were collected and analyzed by WHONET software.Meanwhile,the delivery rates and detection positive rates of clinical species and antibiotics application status in clinic were retrospectively analyzed.RESULTS The delivery rates and detection positive rates of clinical species in patients with lower respiratory tract infection within some periods were 67.8% and 55.9%,respectively.The proportion of Gram-negative bacilli and Gram-positive cocci was 82.39% and 14.18%,respectively.Different drug resistance was displayed in different pathogenic bacteria.Resistant rate of Enterobacteriaceae bacteria and non-ferment bacteria against ciprofloxacin and piperacillin/tazobactam was 47.8%,34.8%and 31.5%,44.7%,respectively and resistant rate of S.aureus against levofloxacin was 63.9%.CONCLUSIONS Although the pathogenic bacteria in patients with lower respiratory tract infection show higher drug resistance to general antibiotics,most of patients with lower respiratory tract infection are treated according to clinical experience of doctors,not based on the results of bacterial culture and antimicrobial susceptibility test.Therefore,the results of bacterial culture and antimicrobial susceptibility test do not play a proper role to direct anti-infective therapy in clinic.
8.Practical value of TERC gene amplification by FISH in the differential diagnosis of cervical intraepithelial neoplasias 1 and 2

Purpose To explore the practical significance of TERC gene amplification by fluorescence in situ hybridization ( FISH) on the differential diagnosis of cervical intraepithelial neoplasias 1 and 2 by tissue microarray. Methods A total of 42 cases of cervical tissue samples were selected, including 20 cases of normal cervix, 22 cases of cervical intraepithelial neoplasia (CIN) 1/2 (8 cases of CIN1 and 14 CIN2). Homemade tissue microarray was prepared. TERC gene amplification was detected with FISH method. Results TERC gene showed no amplification in the normal cervical squamous epithelium. TERC gene amplification was detected in 22. 7%(5/22)CIN1 and 77. 3%(17/22) in CIN2. 1/8 cases of TERC gene amplification in CIN1, 4/14 cases of TERC gene amplification in CIN2. There were significant differences in TERC gene amplification between those groups (P<0. 05). Conclusion The method is feasible and reliable in the detection of TERC gene amplification in CIN1/2 on paraffin sections. It has practical values in differential diagnosis between CIN1 and CIN2.
9.Specific clinical and imaging features of osteogenesis imperfecta V

Objective To evaluate specific clinical and imaging features of osteogenesis imperfecta V and to improve diagnostic accuracy of this disease. Methods Data of 15 patients with osteogenesis imperfecta type V were retrospectively analyzed for their clinical and imaging features. There were 10 males and 5 females, aged from 1 year and 30 years old (median age,12.5 years ). All 15 patients had plain X-ray, and 4 of 15 had CT. All data were analyzed by 3 experienced deputy chief doctors in OI according to OI V standard. Results X-ray:calcification of the interosseous membrane between radius-ulna was detected in all patients and calcification of the interosseous membrane between tibia-fibula was detected in 2 of 15 patients. Dislocation of the radial head was seen in 13 of 15 patients,bilateral in 9 and utilateral in 4.All patients showed restriction in the pronation and supination of the forearm and restricton in the flexion and extention of the elbow joint. Patients with dislocation of raidal head were associated with large coronoid process and olecranon of the ulna. Hyperplastic callus of the extremities were detected in 7 of 15 patients (7 at femur , 3 at humerus, 1 at tibia.In early stage, hyperplastic callus showed thin cortice, and clear boundares with the diaphysis showing and low density, irregular, mesh-like lamellation inside. In the later stage, there were dense calcification inside hyperplastic callus, and no difference in density with the diaphysis. Diaphysis surrounded by hyperplastic callus had clear boundaries with the hyperplastic callus. No cortical destruction was detected. CT:there were sparse needle-dot calcification inside hyperplastic callus, with the patterns of granular, ring-and-arch,irregular streaky mineralization. The cross section of proximal femoral shaft showed irregular shape , flat square shape and tiny medullary cavity, with no calcification on the edge of hyperplastic callus. CT value:-91 HU inside hyperplastic callus; 283 HU in femoral shaft. Conclusions Interosseous membrane between radius-ulna or tibia-fibula, hyperplastic callus ,dislocation of the radial head are specific features in osteogenesis imperfecta V. X-ray can make a definitive diagnosis of osteogenesis imperfecta V. CT scan is helpful in the differential diagnosis of osteogenesis imperfecta V from osteosarcoma.
10.A study on the relationship between family typology and perceived social support in spouses of pa-tient after stroke events

Objective To study family typology of stroke survivors′spouse and its relationship with perceived social support and offer a basic reference for family intervention. Methods A total of 327 qualified spouses of stroke survivors were investigated in the Family Cohesion and Adaptability Questionnaire (FACESII-CV) and Perceived Social Support Scale (PSSS) for their family typology proportion and relationship with PSSS. Results Compared with normal group, the ratios of extremity type family and middle family type were increased in this study: 23.9%(78/327)vs. 16.4%(20/122), 45.0%(147/327) vs. 29.5%(36/122), while the equilibrium family type were decreased:31.1% (102/327) vs. 54.1%(66/122), and there were significant differences (χ2=23.089, P<0.01). PSSS scores of spouses were (61.99±11.90) points, which included family support (21.71±4.27) points and social support (40.28±8.31) points. The scores were all in high level. Inter-comparison of three types of family showed the scores of PSSS were higher in extremity type families than those in middle and equilibrium type families: (68.27±11.27) points vs. (61.04±12.18), (59.56±10.34) points, and there were significant differences (F=5.636, P=0.005), but no significant difference between the last two (P > 0.05). Family typology was the most important factor to PSSS (β=0.212, P<0.05) in spouses of stroke survivors, followed by education degree of spouses (β=-0.190, P<0.05). There were no significant differences in the patients' spouses the baseline indicators in three types of families (P> 0.05). Conclusions Among the spouses of stroke survivors, the ratios of extremity and middle family type are increased but equilibrium family type are decreased. Family typology is the most important factor to PSSS of survivors′spouse. The PSSS of survivors′spouse in extremity type families is higher than that in middle and equilibrium type families. Intervention should be offered to the families, guiding the conversion of family typology from other types to extremity.
