AIM: To study the effect of IL-12 on T lymphocytes apoptosis, the expression of Fas/FasL and TNFR/TNF?. METHODS: Terminal dUTP nick end labeling(TUNEL) and Annexin V assay were used. Anti-TNFR were labeled with FITC, anti-CD95 was labeled with PE and Anti-FasL with biotin. Three kinds of T cells (HTB176,TIB152 and human normal T cells) were analysed through flow cytometry. RESULTS: At 1st hour after being treated with IL-12, the expression of FasL protein and FasL mRNA in HTB176 and TIB152 began to increase and reached peak value in 24 hours. In the normal T cells, FasL just began to increase in 1 hour and maintained stability in 6, 12 and 24 hours through the later experiment period. All three kinds of T cells displayed no change in the expression of CD95 and TNFR/TNF? under the stimulation of IL-12. CONCLUSION: Expression of such apoptosis regulating factors were different in the apoptosis of T cells induced by IL-12.

Objective To investigate the risk factors affecting prognosis in elderly patients with septic shock and explore the effective treatment. Methods A retrospective study of 147 patients with septic shock from the Affiliated Hospital of Hebei United University and Affiliated People's Hospital from Jan. 2011to Jan. 2013 was conducted. Early fluid loading was defined as the administration with an initial fluid bolus within 6 h in ICU. Conservative fluid management was defined as negative fluid balance measured in at least 1 d during the first 3 d as the case group, while the daily fluid balance and cumulative daily fluid balance of the first 3 d was as the control group. The effects of early fluid management on prognosis of elderly patients septic with shock combining the living index within 7 d in ICU were recorded. Results In the case group with 7 d ICU,21. 4%(15 / 70)patients were in progressed and 37. 1%(26 / 70)patients were dead, while 25. 9%(20 / 77)patients were in progressed and 59. 7%(46 / 77)patients were dead in control group(χ2 = 13. 17,P = 0. 00). Univariate analysis showed that APACHEⅡ scores,SIRS score, organ failure, patients with chronic diseases related were associated with 7 d survival rate in elderly patients with septic shock. Multivariate Logistic regression analysis showed that higher APACHEⅡ scores and SIRS score, organ failure, patients with chronic diseases related (have)were the risk factors(OR = 1. 88,5. 34,1. 96 and 4. 76;P < 0. 05). The restrictive fluid management strategy have related with the septic shock patients in elderly that the OR of death rate of fluid management in elderly patients with septic shock was 0. 657(P = 0. 032). Conclusion The prognosis in elderly patients septic with shock is associated with APACHE Ⅱ scores,SIRS score, organ failure, patients with chronic diseases related. Early fluid management can improve to the prognosis of elderly patients with septic shock.

Objective To evaluate specific clinical and imaging features of osteogenesis imperfecta V and to improve diagnostic accuracy of this disease. Methods Data of 15 patients with osteogenesis imperfecta type V were retrospectively analyzed for their clinical and imaging features. There were 10 males and 5 females, aged from 1 year and 30 years old (median age,12.5 years ). All 15 patients had plain X-ray, and 4 of 15 had CT. All data were analyzed by 3 experienced deputy chief doctors in OI according to OI V standard. Results X-ray:calcification of the interosseous membrane between radius-ulna was detected in all patients and calcification of the interosseous membrane between tibia-fibula was detected in 2 of 15 patients. Dislocation of the radial head was seen in 13 of 15 patients, bilateral in 9 and utilateral in 4.All patients showed restriction in the pronation and supination of the forearm and restricton in the flexion and extention of the elbow joint. Patients with dislocation of raidal head were associated with large coronoid process and olecranon of the ulna. Hyperplastic callus of the extremities were detected in 7 of 15 patients (7 at femur , 3 at humerus, 1 at tibia. In early stage, hyperplastic callus showed thin cortice, and clear boundares with the diaphysis showing and low density, irregular, mesh-like lamellation inside. In the later stage, there were dense calcification inside hyperplastic callus, and no difference in density with the diaphysis. Diaphysis surrounded by hyperplastic callus had clear boundaries with the hyperplastic callus. No cortical destruction was detected. CT:there were sparse needle-dot calcification inside hyperplastic callus, with the patterns of granular, ring-and-arch,irregular streaky mineralization. The cross section of proximal femoral shaft showed irregular shape , flat square shape and tiny medullary cavity, with no calcification on the edge of hyperplastic callus. CT value:-91 HU inside hyperplastic callus; 283 HU in femoral shaft. Conclusions Interosseous membrane between radius-ulna or tibia-fibula, hyperplastic callus ,dislocation of the radial head are specific features in osteogenesis imperfecta V. X-ray can make a definitive diagnosis of osteogenesis imperfecta V. CT scan is helpful in the differential diagnosis of osteogenesis imperfecta V from osteosarcoma.
