Objective To compare the efficacy and safety of Saccharomyces boulardii sachets and bismuth in the treatment of chronic gastritis and duodenal ulcer patients with Helicobacter pylori (Hp)infection.Methods A total of 176 patients with chronic gastritis or duodenal ulcer with Hp infection were enrolled.Patients with chronic gastritis (n=116) were randomly divided into the treatment group (58 cases) and the control group (58 cases): treatment group received triple therapy(rabeprazole+amoxicillin +levofloxacin) and Saccharomyces boulardii sachets for 14 days;control group received triple therapy and colloidal bismuth pectin for 14 days.Patients with duodenal ulcer (n=60) were ulcer group,given with bismuth-containing quadruple therapy for 14 days,then rabeprazole for 14 days.All patients were tested for 13 C urea breath test four weeks after stopping treatment.Results One hundred and seventy cases completed the trial.Hp eradication rate was 84.5%in treatment group,87.9% in control group,and 90.0% in ulcer group (analysis by intent of treatment,ITT analysis).The difference between the three groups was not statistically significant(P>0.05).The incidence of adverse events was significantly lower in treatment group (10.3%)than in control group (20.7%)and ulcer group (21.7%)(P<0.05).Conclusion Quadruple therapy with Saccharomyces boulardii sachets can effectively eradicate Hp.It can reduce the occurrence of adverse reactions.

OBJECTIVE:To establish a preparative separation method for euphol reference substance in Euphorbia pekinensis. METHODS:Parts of petroleum ether extraction from E. pekinensis ethanol extract were separated by silica gel column chromatogra-phy with petroleum ether-ethyl acetate(95∶5-70∶30,V/V)by gradient elution. The enriched fractions of euphol were collected. With the methanol repeated recrystallization,nuclear magnetic resonance spectroscopy(NMR)method,mass spectrometry(MS)meth-od and other spectroscopic methods were used to identify the chemical structures,and thin layer chromatography (TLC),UV, HPLC-UV,HPLC-MS were combined to detect the mass fraction. RESULTS:The mass fraction of euphol reference substance sepa-rated from E. pekinensis was>99%. CONCLUSIONS:The reference substance of euphol acquired by this method meet the relative requirements of the chemical reference substance in the content dertermination of TCM new drug quality standard. It provides chemi-cal reference substance for the quality control of E. pekinensis and prescription preparations containing E. pekinensis and the basic research of effective substances.

Objective To research the estrogen-like activity of the DDTs and the extract of vehicle exhaust by recombinant yeast system.Methods Being added into the yeast cell culture fluid,the objects reached the final concentration of 10-9～10-15 mol/L for 17?-estradiol,10-4～10-9 mol/L for DDTs(p,p'-DDT,o,p'-DDT,p,p'-DDD,p,p'-DDE),0.25～8.00 ml/ml for the extract of vehicle exhaust,then the cultivation was performed for 4 hours,the quantitative assay of ?-galactosidase activity accounts for the estrogenlike effect of the samples was carried out.Results DDTs were a group of environmental estrogens hormones,which could integrate estrogen receptors as the stimulators do and express the estrogen-like effect.Being tested separately,the DDTs showed the estrogen-like activity,which was stronger for o,p'-DDT and weaker for p,p'-DDE.The?-galactosidase activity of gasoline-fueled vehicle exhaust at the final concentration of 2.00～4.00 ml/ml was significantly different from that of the DMSO(P

Purpose To assess the left ventricular global systolic function changes using three-dimensional speckle tracking imaging (3D-STI) in female patients with subclinical hypothyroidism (SHT) undergoing L-thyroxine treatment. Materials and Methods Thirty-eight female patients with SHT and 40 healthy female volunteers of the same age (control group) were selected, all the SHT patients received L-thyroxine therapy and were followed for 1 year after euthyroid status was achieved; all the participants underwent blood biochemical examinations, complete conventional echocardiographic and 3D-STI examinations, left ventricular end-diastolic diameter (LVEDd), left ventricular end-systolic diameter (LVEDs), interventricular septal depth (IVSd), left ventricular posterior wall depth (LVPWd), left atrial diameter (LAD), diastolic mitral flow spectrum of A peak, left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), and left ventricular ejection fraction (LVEF) of the two groups were compared, and the correlation of parameters of three dimensional left ventricular global longitudinal strain (GLS), global circumferential strain (GCS), global radial strain (GRS), global area strain (GAS), and thyroid stimulating hormone (TSH) with each parameter was analyzed. Results IVSd and LVPWd in the study group were higher than those of the control group (t=3.30 and 3.64, P<0.05). Compared with the control group, GLS, GCS, GRS and GAS of left ventricular of SHT patients in the study group were significantly lower (t=8.60, 11.95, 9.78 and 5.92, P<0.05) before treatment. GLS, GCS, GRS and GAS of SHT patients improved after L-thyroxine therapy, and the difference was statistically significant (t=6.91, 9.41, 6.46 and 4.31, P<0.05).TSH level was negatively correlated with E/A ratio and E (r= - 0.39 and - 0.42, P<0.05), and also negatively correlated with GLS, GCS, GRS and GAS (r= - 0.38, - 0.56, - 0.33 and - 0.41, P<0.05). Conclusion Left ventricular global systolic function changes of SHT patients before and after L-thyroxine treatment can be evaluated properly using 3D-STI.

Objective To investigate the characteristics of blood pressure variability during 24 hours and pre-thrombotic state(PTS) in elderly patients with essential hypertension.Methods One hundred cases of old patients with essential hypertension in the department of Cardiology (hypertension group) and 65 controls with normal blood pressure from Punan Health Check Center (control group) were recruited.The systolic and diastolic blood pressure variability (SBPV,DBPV) during 24 h daytime and nighttime were monitored for all subjects by ambulatory blood pressure monitor.The plasma levels of P-selectin (PS),fibrinogen (Fg) and plasminogen activator inhibitor-1 (PAI-1) were measured.Results The systolic blood pressure variability (SBPV) in hypertension group was significantly larger than that of control group (P ＜ 0.05) in daytime,and no significant difference regarding of the diastolic blood pressure variability (DBPV) between two groups.While in nighttime,SBPV and DBPV in hypertension group were significantly higher than those of control group (P ＜ 0.05).The levels of plasma Fg and PAI-1 in hypertension group were significantly higher than those of control group ((2.8 ±0.9) g/L vs.(2.3 ±0.7) g/L and (29.8 ±2.7) μg/L vs.(25.6 ± 1.2) μg/L,respectively).While no significant difference was found between two groups for PS (P ＞ 0.05).Conclusion That indicated pre-thrombotic state including higher blood pressure variability during 24 hours,especially during nighttime in elderly patients with essential hypertension and plasma fibrinogen.

The study reports the detection of neuroprotective effect of 10 kinds of coumarin derivatives and explores their possible mechanism. MTT method was used to screen the neuroprotective effect of 10 coumarin derivatives on neurotoxic agents (Aβ25-35 and rotenone) or OGD (oxygen-glucose deprivation). A compound with better protective effect was obtained. Then the effect of this compound on neurotoxic agents on PC12 was detected by the morphological observation. Furthermore, the effect of compound 3 on microglia with lipopolysaccharide (LPS) induced inflammation was detected. And the inflammatory factor was tested. Finally, direct free radical scavenging ability was detected. Compound 3 was found to be the best compound through three neurons toxic models. Not only compound 3 ameliorated cell viability reduced by three neurons toxic models, but also significantly inhibited the production of inflammatory factor (TNF-α and IL-1β). And its free radical scavenging ability is very good, especially the effect on superoxide anion, which is comparable with vitamin C. The significant scavenging effect of compound 3 on superoxide anion might be the mechanism of the neuroprotection. Compound 3 as a potential neural cell protective agent merits further investigation.

Objective To investigate the security and effect of endovascular stents placement in treating atherosclerotic stenosis of the iliac arteries.Methods 15 cases with iliac arterial stenosis caused by atherosclerosis were treated by a big amount of thrombolysis in combination with iliac arterial stent placement.The length of lesions was from 4.3 cm to 9.5 cm(mean of 6.7 cm),the claudicant distance was from 100 m to 300 m(mean of 185 m).Clinical follow-up included CT angiography,color Doppler sonography and clinical evaluation with the ankle-brachial index(ABI).Results Eighteen stents were placed in 18 limbs of 15 patients.The technical successful rate was 100%.The clinical symptoms(claudication or the rest pain)were improved,and ABI increased from 0.25?0.18 to 0.85?0.15 after stent implantation in all cases.All the patients were followed up for 3~47 months,one patient was death during the follow-up periods because of brain stem ischemic stroke.All patients maintained uninterrupted stent patency until the final follow-up.Conclusion Iliac arterial stent placement is a safe treatment with favorable long term patency for treating arterial stenosis caused by atherosclerosis.

Objective To investigate the application of echocardiography in the diagnosis of neonatal ductus arteriosus aneurysm (DAA).Methods A total of 27 neonates with DAA were detected by echocardiography.Echocardiographic features in these cases were explored and outcome of these neonates were followed up.Results There were four crucial features in DAA detected by echocardiography:① A vessel bump was shown on the left side of the origination of left pulmonary artery(LPA) in the high parasternal short-axis view.②A horizontal shunt jet from the bump to the origination of LPA was shown when interrogate the top of the bump.③The bump joined the descending arota when interrogate it down and back orientately.A ‘V' shaped sign was shown when the aortic arch appeared on the right side and the bump on the left side.④A slow red flow signal was found in bump when the scale of color Doppler was cut down.The follow-up echocardiography showed spontaneous closure of the DAA when 3 months of age except one case lost.There were no abnormality in the figure and function in heart.Conclusions Echocardiography can be a valuable method for the diagnosis of neonatal ductus arteriosus aneurysm.Neonate with DAA have a good outcome.

Objective To investigate the relationship of pancreatic β-cell function and insulin resistance with microalbuminuria in a cross -sectional study of patients with type 2 diabetes.Methods A total of 524 partici-pants with type 2 diabetes were recruited in this cross -sectional study.All subjects'height,weight,waist circumfer-ence and blood pressure were measured.Venous blood samples were drawn to measure fasting plasma glucose (FPG), fasting lipids,glycated hemoglobin A1c (HbA1c),fasting C -peptide (FPC).24h -urine was collected to measure urinary albumin excretion rate (UAER).Homeostasis model assessment of pancreatic β-cell function (HOMA -B) and insulin resistance (HOMA -IR)were estimated using fasting plasma C -peptide.According to HOMA -B quar-tile,the subjects were divided into four groups,including q1 -q4.According to HOMA -IR,the subjects were also divided into four groups,including Q1 -Q4.We assessed the crude associations across quartiles of these data with demographic and clinical parameters using a nonparametric test for trend across ordered groups (trend using Stata software).Multivariable logistic regression analysis was performed to assess the relationships of pancreatic β-cell function and insulin resistance with microalbuminuria in patients with type 2 diabetes.Results Trend test showed that UAER gradually reduced with increase of HOMA -B.The UAER values in subjects with q1,q2,q3 and q4 were 8.92(5.53 -28.65),8.55(5.52 -20.95),7.57(4.79 -19.83)and 7.84(5.23 -14.38)μg/min,respectively, and the trend was statistically significant(z =-2.1,P <0.05 ).With HOMA -IR increasing,UAER gradually increased.The UAER values in subjects with Q1,Q2,Q3 and Q4 were 6.73(4.85 -16.52),8.61 (5.2 -20.37), 8.31(4.88 -27.04),8.75(6.03 -25.21)μg/min,respectively,and the trend was also statistically significant(z =2.41,P <0.05).Multivariable logistic regression analysis showed that subjects with the highest quartile of HOMA -B had lower possibility of microalbuminuria than patients with the lowest quartile of HOMA -B (adjusted OR q4 vs. q1 =0.39,95% CI:0.20 -0.76,Wald =7.59,P =0.006).Subjects with the highest quartile of HOMA -IR had higher risk of microalbuminuria than those with the lowest quartile of HOMA -IR (adjusted OR Q4 vs.Q1 =2.00, 95% CI:1.08 -3.72,Wald =4.84,P =0.028).Conclusion Insulin resistance is associated with an increased prevalence of microalbuminuria in type 2 diabetes,while improved pancreatic β-cell function is linked to decreased rates of microalbuminuria for those patients.

Objective To evaluate the frequency of pncA gene mutations in pyrazinamide-resistant (PZA-resistant)Mycobacterium tuberculosis(M.TB)isolates.Methods The isolates were tested for PZA susceptibility with absolute concentration method.The pncA gene was amplified and the products were cloned into T-vector,followed with sequencing.Results In all the 36 M.TB isolates,there were 25 PZA-resistant strains and 11 PZA-susceptible strains.Mutations of the pncA gene were found in 10 PZA-resistant isolates,four of which belonging to high PZA-resistant strains.The wild type pncA sequence was present in 3 PZA-susceptible strains, synonymous mutations of pncA gene were detected in two PZA-susceptible strains.Additionally,11 PZA-resistant strains and 2 PZA-susceptible ones showed putative regulatory mutations in the upsteam of pncA gene.Condusions The mutation of the pncA gene can cause the PZA resistance.However.there ale other drug resistance mechanism except this mutation.
