1.CT perfusion : the clinical application in the diagnosis of breast diseases

Objective To assess the CT perfusion characteristics of breast diseases. Methods Twenty-two patients with fibroadenoma accompanied with cyst (n=1), other benign diseases (n=5), and carcinomas (n=16) underwent breast CT scan. CT perfusion was performed for breast with cine mode (0.5 s per rotation ) on a multislice CT scanner ,120 kV, 60 mA, 4?5 mm collimation. Contrast injection was done by using 50 ml nonionic contrast agent ( 300 mg I/ml ), at a flow rate of 4 ml/s with a power injector, 5 seconds delay, and data acquisition lasted for 50 seconds. These data were processed on a GE AW 4.1 workstation using perfusion 3 software package, and the mean BF, BV, MTT and PS were measured and statistically analyzed. Results The mean BF, BV, MTT and PS in carcinoma were (33.55? 28.40) ml?min -1 ? 100 g -1 , (5.60?3.08) ml/100 g, (15.37?6.41) s, (18.71?10.42) ml?min -1 ?100 g -1 . The mean BF, BV, MTT and PS in contralateral normal breast of carcinoma were (10.36?9.44) ml?min -1 ? 100 g -1 , (0.70?0.22) ml/100 g, (13.55?7.27) s, (1.54?1.74) ml?min -1 ?100 g -1 . The mean BF, BV, and PS between carcinoma and normal breast were statistically significant (P0.05). The mean BF, BV, MTT and PS in fibroadenoma were (62.57?19.62) ml?min -1 ?100 g -1 , (7.37?1.30) ml/100 g, (11.24?3.34) s, (27.63?3.87) ml?min -1 ?100 g -1 . The mean PS between carcinoma and fibroadenoma was statistically significant (P0.05). The mean BF, BV, MTT and PS in benign lesion were (7.65?10.24) ml?min -1 ?100 g -1 , (1.23?0.97) ml/100 g, (14.94?10.68) s, (1.81?1.72) ml?min -1 ?100 g -1 . The mean BF, BV, and PS between carcinoma and benign lesion were statistically significant (P0.05). Conclusion MSCT perfusion may reflect the features of breast diseases, and provide valuable hemodynamic message. These parameters have a value in diagnosis of breast diseases and are helpful for differential diagnosis.
2.Impacts on the life quality of the patients with allergic rhinitis treated with warming acupuncture in winter and summer.

OBJECTIVETo explore the impacts on the life quality and the effect mechanism in the patients of allergic rhinitis (AR) treated with warm acupuncture in winter and summer.
METHODSTwo hundred and forty patients of AR were randomized into a summer and winter acupuncture group, a non-summer and winter acupuncture group and a western medication group, 80 cases in each one. In the two acupuncture groups, Dazhui (GV 14), Fengmen (BL 12), Feishu (BL 13), Pishu (BL 20) and Shenshu (BL 23) were selected. In the summer and winter acupuncture group, the warm acupuncture started at the first day of the three periods of hot season and the first day of the third nine-day period after the winter solstice. The treatment was given once every two days, continuously for 15 times. Totally, 30 treatments were required a year. In the non-summer and winter acupuncture group, the warm acupuncture was applied out of the three periods of the hot season and the third nine-day period after the winter solstice. The treatment was given once every two days and 30 treatments for a year. In the western medication group, cetirizine was taken orally, continuously for 30 days as one session. In the three groups, the treatment for 1 year was taken as one session. The second session started in the next year. Totally, 2 sessions were required. The score of rhinoconjunctivitis quality of life questionnaire (RQLQ) and the level of serum immunoglobulin E (IgE) were compared in the patients' of each group before treatment and in 1 and 2 sessions of treatment.
RESULTSAfter treatment, the scores of 7 domains, named activities, common complaints, practical problems, sleep, ocular symptoms, nasal symptoms and emotions were all improved as compared with those before treatment, in the patients of the three groups (all P < 0.05). After 2 sessions treatment, the results in the summer and winter acupuncture group were better than those in the other two groups (all P < 0.05), and the results in the non-summer and winter acupuncture group were better than those in the western medication group (all P < 0.05). After treatment, serum IgE level was lower averagely than that before treatment in the three groups (all P < 0.05). After 2 sessions treatment, the result in the summer and winter acupuncture group was lower than that in the other two groups (both P < 0.05), and the result in the non-summer and winter acupuncture group was lower than that in the western medication group (P < 0.05). After treatment, the total effective rate was 92.3% (72/78) in the summer and winter acupuncture group, better than 80.5% (62/77) in the non-summer and winter acupuncture group and 69.7% (53/76) in the western medication group (both P < 0.05).
CONCLUSIONThe warm acupuncture in both summer and winter achieves the definite efficacy on AR and the effect mechanism is relevant with reducing serum IgE level.

3.The application of information technology in continuous nursing after laparoscopic gastrointestinal tumor surgery

Objective? To explore the application of information technology in continuous nursing after laparoscopic gastrointestinal tumor surgery. Methods? Using convenient sampling method, we selected a total of 118 patients who underwent laparoscopic gastrointestinal tumor surgery at the Second Affiliated Hospital of Fujian Medical University from April 2015 to April 2017. They were randomly assigned into the control group and the observation group, with 59 cases in each group. Patients in the control group were treated with traditional continuous care, while patients in the observation group were treated with information technology integrated continuous care. Self-efficacy and quality of life before and after intervention were compared between the two groups using the Chinese version of the Cancer Self-Efficacy Assessment Scale and the European Five-Dimensional Health Scale. Results? There was no significant difference in self-efficacy between the two groups before intervention (P>0.05). The overall and different dimensions of self-efficacy of the patients in the observation group were higher than those in the control group (P< 0.01). There was no significant difference in quality of life between the two groups before intervention (P>0.05). The daily activity and self-care ability of the observation group were higher than those of the control group (P<0.05). Conclusions? Compared with traditional continuous care, information technology based continuous care can improve the self-efficacy and quality of life of patients undergoing laparoscopic gastrointestinal tumor surgery, and its promotion is worthwhile.
4.Primary screening for breast diseases among 17618 women in Wufeng area, a region with high incidence of cervical cancer in China.

In this study, the current status for breast diseases in a region with high-incidence of cervical cancer were epidemiologically investigated. From March to August, 2009, 17618 women, from Wufeng area of Hubei province, China, were recruited to screen breast diseases by using breast infrared diagnostic apparatus. Other diagnostic methods, such as B-mode ultrasound, X-ray mammography, needle biopsy and pathological examination were, if necessary, used to further confirm the diagnosis. The screening showed that 5990 of 17618 cases (34.00%) had breast diseases, 5843 (33.16%) had mammary gland hyperplasia, 48 (0.27%) had breast fibroadenoma, 11 (0.06%) had breast carcinoma, and 88 (0.50%) had other breast diseases. The peak morbidity of breast cancer was found in the women aged 50-60 ages. The morbidity of breast cancer was significantly increased in women elder than or equal to 50 years old (n=8, 0.157%) in comparison with that in the subjects younger than 50 years old (n=3, 0.024%) (u=2.327, P<0.05). It was shown that the occurrence of breast diseases was concentrated in women aged 20-40 years, while the total morbidity reached its peak at the age of 30 years and then decreased sharply after age of 40. Compared with the patients elder than or equal to 40 years old (n=3289, 27.46%), the morbidity rate of breast diseases was significantly increased in women less than 40 years old (2648 cases, 47.18%; P<0.001). However, there was no significant difference in the morbidity of breast diseases between the age group of 20-29 years and that of 30-39 years (P=0.453), and both of them were high. There was no significant association between the morbidity of breast diseases and cervical cancer. Since the morbidity of breast diseases was higher among young women, more attention should be paid to the screening of breast diseases among young women for early diagnosis.
5.Best evidence summary for difficult peripheral intravenous catheterization in children

Objective:To search, evaluate, and summarize the best evidence for difficult peripheral intravenous catheterization in children.Methods:Following the "6S" evidence pyramid model, literature related to the management of difficult peripheral veins in children was searched in both English and Chinese databases including UpToDate, BMJ Best Practice, National Guidelines Clearinghouse, the Joanna Briggs Institute Evidence-Based Health Care Database, PubMed, Medlive, SinoMed, CNKI, and Wanfang Database. The search period was from the establishment of the database to January 2023. Two researchers trained in systematic evidence-based nursing, independently evaluated the quality of included literature and extracted relevant evidence.Results:Five articles were included: two guidelines, two expert consensuses, and one systematic review. 19 best evidence were summarized, covering five aspects: difficult vein quality management, difficult vein assessment, difficult intravenous catheterization site and needle type selection, difficult intravenous catheterization auxiliary methods, and handling of failed difficult intravenous catheterization.Conclusions:This study summarizes the best evidence for difficult peripheral intravenous catheterization in children, demonstrating clinical nursing practicality. It provides evidence-based guidance for pediatric nursing staff performing difficult intravenous catheterization.
