1.Comparison of real-time genotyping and quantitative PCR,multiplex-PCR and sequence analysis for hepatitis B virus genotypes B and C

Objective To evaluate the real-time genotyping and quantitative PCR(RT-GQ-PCR)method by comparing it with direct sequence analysis and the multiplex-PCR method.Methods RT-GQ-PCR,direct sequence analysis and the multiplex-PCR method were used to detect HBV genotypes of 113 patient samples with HBV-DNA positive.ResultsThe detection rate of RT-GQ-PCR and direct sequence analysis was 100%,and the multiplex-PCR is 94.69%.The concordance between RT-GQ-PCR and the multiplex-PCR is perfect(Kappa value =0.915),and the consistency of RT-GQ-PCR and direct sequence analysis is pretty good(Kappa value = 0.742),specially at detecting single genotype.Twenty-eight samples with genotypes B and C dual infections were detected by RT-GQ-PCR,but only 19 samples by the multiplexPCR and 13 samples by direct sequence analysis.Conclusion The RT-GQ-PCR is convenient,rapid and accurate in HBV genotyping,especially more sensitive than direct sequence analysis and the multiplex-PCR for detecting dual genotypes.The method is applicable for large-scale epidemiological study.
2.A case study of primary testicular lymphoma:diagnosis and treatment strategies

Primary testicular lymphoma comprises 1% to 9% of testicular neoplasms and represents 1% to 2% of all non-Hodg-kin lymphomas. Histologically, the majority of the tumor consists of diffuse large B-cell non-Hodgkin lymphomas that are of intermedi-ate- or high-grade neoplasm. Clinically, the disease typically presents as a painless testicular swelling that develops over a span of weeks to months. B symptoms such as fever, weight loss, and anorexia are present in 25% to 41% of the patients. This tumor is an ag-gressive type, with frequent invasion of the epididymis, spermatic cord, and scrotum, as well as a marked tendency to relapse, especial-ly in the CNS. The treatment is mainly based on orchiectomy (mostly in stages ⅠE and ⅡE) regardless of its association with prophy-lactic irradiation of the scrotum and administration of intrathecal chemotherapy, cyclophosphamide, doxorubicin, vincristine, and pred-nisone regimen chemotherapy plus rituximab (R-CHOP) (stages ⅢE and ⅣE) and radiotherapy. The multi-modality treatment marked-ly improved progression-free and overall survival. We introduce as reference one case that received a multidisciplinary comprehensive discussion in the Department Lymphoma, Tianjin Medical University Cancer Hospital.
3.Analysis and insights from the survey on current clinical practice on violence risk assessment in top grade psychiatric hospitals in beijing-tianjin-hebei region

Objective:To investigate the status of violence and aggression suffered by nurses in psychiatric hospitals in beijing-tianjin-hebei region and explore the development trend of violence assessment.Methods:A total of 539 psychiatric nurses from three top grade psychiatric hospitals (Beijing Huilongguan Hospital, Tianjin Anding Hospital, Mental Health Center of Hebei Province) in Beijing-Tianjin-Hebei region were surveyed by a general data questionnaire, Workplace Violence Frequency Measurement Scale (WVS), Violence Cognition Qquestionnaire, Knowledge and Needs on Violence Risk Management Questionnaire.Results:A high level of workplace violence (6.21±2.94) was found with nurses in different wards ( χ2 value was 32.149, P<0.05), being in different wards (χ 2 value was 32.149, P<0.05), whether participating psychiatric training ( χ2 value was 11.588, P<0.05) and whether working on different schedule ( χ2 value was 21.017, P<0.05), suffered from distinct level of workplace violence. Participants believed that the change in the patients′ condition (68.6%, 370/539) and symptom fluctuation (65.9%, 355/539) were the main factor of violence from patients′ aspect. While a lack of assessment (57.9%, 312/539) and poor communication among nurses (56.8%, 306/539) was cited as the main causes of violence from nurse′s point. Conclusion:The management and control of patients′ violence should be improved through staff training program, introduction and employment of validated assessment tools, the construction of violent prevention and control system. Only through the joint efforts above could we reduce the occurrence of psychiatry violence injuries and ensure a safety practice environment for the maintenance and improvement of nurses′ health in mental health department.
4. Identification of unclassified influenza A virus using high-throughput sequencing technology

Objective:
To identify the avian influenza virus subtype from the avian and environmental samples using the Ion Torrent new-generation semiconductor sequencing technology and to establish a high-throughput sequencing method to identify unclassified influenza A virus.
Methods:
Virus RNA was extracted from the nine avian swab and environmental samples and real-time RT-PCR was carried out to detect universal fluA, H5N1, H7N9 and H9N2. The whole genome of influenza A virus was amplified by PathAmpFluA kit. Sequencing library was prepared using Next Fast DNA Fragmentation & Library Prep Set for Ion Torrent kit and high-throughput sequencing was done by Ion Torrent Personal Genome Machine(PGM). Data from the PGM was processed and quality evaluated using Ion TorrentSuite v3.0 software. Sequence assembly and influenza database blast were carried out by FluAtyping v4.0 and PathogenAnalyzer bioinformatics software to identify the influenza A virus subtype of these nine samples.
Results:
The results of real-time RT-PCR for universal fluA of these nine samples were positive but the results for H5N1, H7N9 and H9N2 were negative. Seven subtypes of influenza A virus were identified by high-throughput sequencing and bioinformatics analysis: six samples were H2N3, H5N6, H5N8, H7N1, H7N7, H11N3 subtype respectively and three samples were H6N6 subtype.
Conclusions
Avian influenza virus has many subtypes in the environment of Zhejiang province. Ion Torrent semiconductor sequencing technology is suitable for fast identification of unclassified influenza virus for avian influenza environment monitoring.
5.Analysis of hospital management projects funded by National Natural Science Foundation of China during 2010—2020

Objective:To analyze the characteristics and hot spots of hospital management projects funded by National Natural Science Foundation of China(NSFC), for the reference for furthering improving the modernization level of hospital management.Methods:The NSFC funded research projects in hospital management during 2010-2020 was searched. Project name, keywords, approval year, affiliation and other informations were obtained. CiteSpace was used for bibliometric analysis of these research projects, while all the data acquired were subject to descriptive analysis and frequency analysis.Results:The number of hospital management projects funded by NSFC increased from 4 in 2010(0.93 million yuan) to 32(9.94 million yuan) in 2020, totaling 192 projects and 72.76 million yuan. These numbers consisted of 90 general projects(46.9%) and 79(41.1%) youth fund projects respectively. All the funded projects were distributed in sub-disciplines under the NSFC first-level discipline of macro management and policy, namely health management and policy(154) and medical/drug management and policy(38). The hotspots of the funded projects covered performance evaluation, payment method, public health system, health economics, hierarchical diagnosis and treatment, utility theory, regional medical alliance, essential medicine system, capitation and incentive strategy, and so on.Conclusions:Recent years are witnessing a rising trend in the number and grants of NSFC projects on hospital management. More and more funds are granted to universities, especially young researchers, and a hospital management research system has been initially taken shape. Hospital management in China should take into full account of the management characteristics and the status quo of the medical and health field, focus on performance appraisal incentive mechanism, hierarchical medical system, construction of medical alliances. and patient medical experience as well. In addition, the cooperation between hospitals and universities should be strengthened in order to promote the translation of research results.
6. Establishment of a high throughput sequencing technology for detection of SFTS virus genome

Objective:
The objective of this study was to establish a next generation sequencing (NGS) method for severe fever with thrombocytopenia syndrome virus(SFTS).
Methods:
SFTS virus RNA was extracted from the patient serum inoculated and isolated by Vero cells. Two methods of random primer sequencing and oligo(dT) beads selection sequencing were used for library construction. The libraries were built based on the best amplification and purification conditions. Whole genome sequencing was performed on NGS platform.
Results:
There were significant differences in data of 3 virus between the two methods.The sample was sequenced by random primer sequencing showed low coverage and depth. However, three samples were sequenced by oligo(dT) beads selection showed coverage was over 99% and depth was over 900.The alignment rate of database from NCBI was more than 90%. The initial detection quality of this method was 300ng RNA.
Conclusions
In this study, we used the method of oligo(dT) beads selection to build libraries, and established a SFTS virus genome detection based on next generation sequencing.
