1.The application and nursing of the transanal drainage tube in decompression of acute left- sided obstruction of colorectum

Objective To explore the application of transanal drainage tube in decompression of acute left- sided colorectum obstruction and its nursing. Methods Twenty- one cases of patients, with acute left-sided colorectal obstruction, were enrolled in this study from 2011 and 2014, Results Of the 21 cases, 5 failed to drip, 6 was difficult to drain, 2 having loose stools around the anus, 2 with the tubes out of the anus, and 6 with obstructed tubes during the drainage. Nineteen finally underwent surgery, 16 of which with 1- stage operation, and 13 of the 16 with sufficient lymph node dissection, without stoma leakage. Conclusions Transanal drainage tube plays an important part in acute left- sided colorectal obstruction, and a professional nursing work could help maintain the unobstruction of the tube, which is of much importance in the procedure of the drainage and the decompression of the obstruction.
2.The expression and clinical significance of vascular endothelial growth factor-C and nm23-H1 in colorectal carcinoma

Objective To discuss the expression and clinical significance of vascular endothelial growth factor (VEGF)-C and nm23-H1 in colorectal carcinoma.Methods Immunohistochemical staining was employed to determine the expression of VEGF-C and nm23-H1 in the carcinoma tissues of 120 cases with colorectal carcinoma and in the adjacent mucosal tissues of 55 cases as control,and analyzed their correlation with cliniopathological features and prognosis.Results The positive expression of VEGF-C in the carcinoma tissues was 71.7% (86/120),significantly higher than that in the adjacent mucosal tissues [21.8 % (12/55)],and there was significant difference (x2 =35.186,P < 0.01).The positive expression of nm23-H1 in the carcinoma tissues was 57.5% (69/120),significantly lower than that in the adjacent mucosal tissues [90.9% (50/55)],and there was significant difference (x2 =18.351,P < 0.01).The expression of VEGF-C was significantly correlated with lymph node metastasis (P < 0.05),and the expression of nm23-H 1was significantly correlated with lymph node metastasis and pathological type (P <0.01 or <0.05).The expression of VEGF-C and nm23-H1 did not show a significant correlation with gender,age,tumor size,tumor site and depth of invasion (P > 0.05).Spearman correlation analysis showed that the expression of VEGF-C was negatively correlated with the expression of nm23-H 1 in colorectal carcinoma (r =-0.472,P <0.01).Conclusions The joint detection of VEGF-C and nm23-H1 expression is very promising in prediction of the prognosis of patients with colorectal carcinoma.However,whether it can be used as a marker in prognosis judgment need further investigation.
3.Determination of Syringin and Syringaresinol in Ciwujia Injection by RP-HPLC

Objective To establish the determination method of syringin and syringaresinol in Ciwujia Injection with RP-HPLC. Methods C18 ODS was used as a stationary phase. The mobile phase consists of water and acetonitrile. The gradient condition was 0→20 min, A:90%→50%. The flow rate was 1.0 mL/min. The detective wavelength was set at 220 nm. Results The linear range of syringin was 0.03~0.16 ?g (r =0.999 7, n=5) and the linear range of Syringaresinol was 0.02~0.13 ?g (r =0.999 3, n=5). The average recovery of syringin was 99.34% and RSD=0.58%. The average recovery of Syringaresinol was 99.22% and RSD=0.71%. Conclusions The method was simple, convenient and accurate, and can be used for content determination of syringin and syringaresinol in Giwujia injection.
4.Compliance of antihypertensive drug use in patients with hypertension

Objective To understand the compliance of antihypertensive drug use in patients with hypertension.Methods A retrospective analysis was conducted among 218 patients with hypertension to understand their drug use compliancy and influencing factors,including side effect of the drugs,drug type,educational level,economic status and drug use length.Results The factors including disease course,drug type,drug use length and drug side effects,the economy status,educational level,awareness of hypertension related knowledge and psychological reaction could significantly influence the compliance of antihypertensive drug use.Among the patients surveyed,86.67% of them with poor drug use compliance had only an educational level less than senior high school,77.33% had poor awareness of hypertension related knowledge.Conclusion The antihypertensive drug use compliance in patients with hypertension is directly related to the outcome of the disease in clinical treatment.It is necessary to take effective measures to improve the treatment compliance and maintain normal blood pressure level of the patients.
5.The thought and method of constructing diagnostic specification for syndrome/pattern of defensive qi deficiency

The syndrome/pattern of defensive qi deficiency is a common basic syndrome of traditional Chinese medicine in clinical practice. However,there is a lack of standardized and operable diagnostic specifications in practical applications. Based on the previous literature,this study proposed the idea of starting from the elements of the syndrome,qualitative diagnostic criteria for the syndrome/pattern of defensive qi deficiency oriented to the entire region of the disease were constructed based on the two dimensions of " deficient defensive qi failing to consolidate the exterior" and " qi deficiency" and constructing a set of quantitative evaluation criteria as the supporting content for the diagnostic items. The core members of the research group attempted to formulate the draft standard,then reached a consensus through the Delphi method expert questionnaire consultation and the Nominal group technique,and finally evaluated the reliability and validity of the standard through clinical verification to provide ideas for the standardization and normalization of research on syndromes.

SIRT6 belongs to the conserved NAD+-dependent deacetylase superfamily and mediates multiple biological and pathological processes. Targeting SIRT6 by allosteric modulators represents a novel direction for therapeutics, which can overcome the selectivity problem caused by the structural similarity of orthosteric sites among deacetylases. Here, developing a reversed allosteric strategy AlloReverse, we identified a cryptic allosteric site, Pocket Z, which was only induced by the bi-directional allosteric signal triggered upon orthosteric binding of NAD+. Based on Pocket Z, we discovered an SIRT6 allosteric inhibitor named JYQ-42. JYQ-42 selectively targets SIRT6 among other histone deacetylases and effectively inhibits SIRT6 deacetylation, with an IC50 of 2.33 μmol/L. JYQ-42 significantly suppresses SIRT6-mediated cancer cell migration and pro-inflammatory cytokine production. JYQ-42, to our knowledge, is the most potent and selective allosteric SIRT6 inhibitor. This study provides a novel strategy for allosteric drug design and will help in the challenging development of therapeutic agents that can selectively bind SIRT6.
