1.Effects of acupuncture intervention at different stages on urinary function reconstruction of neurogenic bladder after spinal cord injury.

OBJECTIVETo explore the effects of early intervention of acupuncture on urinary function reconstruction of neurogenic bladder after spinal cord injury, so as to find the optimal treatment timing of acupuncture.
METHODSFifty-three patients of neurogenic bladder after spinal cord injury were randomly divided into a prior-to-catheter group (25 cases) and a posterior-to-catheter group (28 cases). For the prior-to-catheter group, one week before the removal of catheter, acupoints on the abdomen and back such as Guanyuan (CV 4), Zhongji (CV 3), Baliao (Shangliao (BL 31), Ciliao (BL 32), Zhongliao (BL 33), Xialiao (BL 34), Jiaji (EX-B 2) and acupoints at distal end such as Zusanli (ST 36) and Sanyinjiao (SP 6) were selected; after the catheter removal, simple method. of water column was used to measure bladder capacity pressure to classify the type of bladder, and then different acupuncture manipulations were given; intermittent catheterization was performed if condition was allowed. The posterior-to-catheter group applied identical treatment after removal of catheter as the prior-to-catheter group. The treatment was given 5 times per week; after one-month treatment, five aspects including the time of first effective urination, time of establishing reflex urination, average residual volume of urine, time of residual urine less than 100 mL and quality of life (QOL) were evaluated.
RESULTSAfter treatment, the time of first effective urination, time of establishing reflex urination, average residual volume of urine and time of residual urineless than 100 mL in the prior-to-catheter group were all superior to those in the posterior-to-catheter group (all P<0. 05) ; the score of QOL was improved in two groups after treatment (both P<0. 01), but the difference between two groups was not statistically significant (P>0. 05); each index of urination function was not significant among patients with different injured segments of spinal cord (all P>0. 05).
CONCLUSIONThe early intervention of acupuncture (prior to catheter) has obvious improving function on establishing balanced bladder in patients with neurogenic bladder after spinal cord injury, which is superior to acupuncture intervention after removal of catheter, however, the effects of different injured segments on establishing balanced bladder are not different.

2.The effect of transcranial magnetic stimulation on the proliferation of neural stem cells and miR-9 in the brain after focal cerebral ischemia

Objective To investigate the effect of repetitive transcranial magnetic stimulation (rTMS) on the proliferation of neural stem cells in the subventricular zone (SVZ) and the expression of miR-9 in the ipsilesional hemisphere of rats with focal cerebral ischemia.Methods A rat model of acute focal cerebral ischemia was established using transient middle cerebral artery occlusion (tMCAO).Rats were then randomly assigned to a sham group,a control group or an rTMS group.Daily rTMS treatments (10 trains with 30 pulses per train and 50-second breaks between trains at 120% of the resting motor threshold) were targeted at the ipsilesional MI cortex beginning one day after the tMCAO.After the treatment the proliferation of neural stem cells in the ipsilesional SVZ was identified by double immunofluorescence staining (BrdU/nestin).The expression of miR-9 was evaluated using quantitative PCR.Results Compared with the other two groups,BrdU + Nestin + cells in the ipsilesional SVZs of the rTMS group increased and the expression of miR-9 decreased significantly.Conclusion rTMS can promote the proliferation of neural stem cells and inhibit the expression of miR-9 in the ipsilesional hemisphere after focal cerebral ischemia.
3.Discussion on the Isomorphism of TCM Syndrome and Biological Entropy

Syndrome, a very important concept in traditional Chinese medicine (TCM), is the essence and characteristics of TCM theory. Progress of related studies on syndrome will drive the development of TCM. Therefore, clinical and basic researches on syndrome have focused on the hotspot of TCM. In recent 60 years, many scholars have discussed the concept, essence and the establishment of standardization of syndrome from different angles and layers. Although many results have been obtained, there is still a certain one sidedness and dispute. Therefore, looking for a new theoretical tool to reveal the scientific connotation of TCM syndrome is imperative. TCM syndrome and biological entropy belong to two different subject concepts, but can also reveal the physiological and pathological state in the changes of life body growth. This article discussed isomorphism of TCM syndrome and biological entropy from the concept, theoretical background and characteristics, with a purpose to provide certain guiding significance to study and promote standardized researches on TCM syndrome.
4."Nutrition treatment scheme for a patient exposed to Nanjing ""5.7"" 192Ir source accident"

Objective To provide nutritional supportive scheme for patients with radiation injury through the treatment of the one exposed to Nanjing 192Ir source accident.Methods The reasonable nutrition treatment scheme was made on the basis of dietary survey and nutritional index monitoring during clinical stages of the patient,including body weight,body mass index(BMI),biochemical indexes,electrolyte,etc.,as well as metabolic cart determination of resting energy expenditure (REE).Results Patient on admission (days 5 post-irradiated) weighing 42.5 kg,172 days after the first irradiated (the first skin grafting) fell to a minimum of 36 kg,then gradually rise,hen rose back to normal range on days 383 before discharge.Normal admission hemoglobin was 135 g/L,172 d after irradiated to a minimum of 54 g/L,normal discharge;when lymphocytes admission low as 0.5 × 109/L,58 days back to normal after exposure,172 days after irradiated down to 0.4 × 109/L.Serum albumin was normal admission 41.2 g/L,172 days after irradiated down to 25.3 g/L.The normal level of serum prealbumin was 0.22 g/L,248 days to a minimum of 0.04 g/L,the basic return to normal at discharge was 0.17 g/L.Admission normal liver function,bilirubin index slightly higher,the all in one parenteral nutrition after about 2.5 months later,bilirubin and liver function indicators were gradually increased,the adjusted treatment and nutrition liver and gallbladder and other gradually returned to normal after treatment.REE and the body weight were determined by metabolic cart on days 294,308 and 342 for the energy requirements.Conclusions For patient with radiation injury,appropriate nutrition therapy is a key method for the clinical treatment and rehabilitation,which can maintain the nutritional status of patients and improve clinical treatment.
5.Ancient Classical Prescriptions Treating Middle and Advanced Primary Liver Cancer:A Meta-analysis of Randomized Controlled Trials

Objective To evaluate the efficacy of ancient classical prescriptions treating middle and advanced primary liver cancer.Methods Articles were searched from Pubmed, Embase, SCI, Cochrane Liarary and CNKI, VIP, WanFang Data, CBM databases. Randomized controlled trials about ancient classical prescriptions treating advanced primary liver cancer were collected. Results 17 studies were included, a total of 994 patients. The recent efficiency of ancient classical prescription combined with symptomatic therapy increased by 28% compared with single supportive and symptomatic therapy;the stable rate of life quality increased by 23%;the efficiency of TCM syndrome increased by 29%;the survival rates of 3 months, 6 months and 1 year increased by 16%, 36.7% and 58.5% respectively;the life quality score of later increased by 6.29 on average. Conclusion Ancient classical prescription combined with supportive and symptomatic therapy in advanced primary liver cancers is superior to single supportive and symptomatic therapy on recent efficiency, survival rate (3 months/6 months/1 year), quality of life and TCM syndrome. The conclusion of this study needs randomized controlled trials with larger samples, multicenter and high quality for further verification.
6.A qualitative study on psychological experience of patients with neurogenic bladder in spinal cord injury undergoing intermittent catheterization

ObjectiveTo explore the psychological experience of patients with neurogenic bladder in spinal cord injury undergoing intermittent catheterization and provide references for better implementation of bladder function training.Methods Using the phenomenological method of qualitative research, 12 patients with neurogenic bladder in spinal cord injury in orthopedic department of one hospital in Beijing from July 2016 to October 2016 were surveyed by in-depth interviews. Experience and feelings of patients in the process of catheterization were analyzed.Results Experience and demands were mainly divided into three themes. First, patients undergoing intermittent catheterization had different types of psychological reactions including exclusion and disgust, fear and anger, depression and loneliness, acceptance and customization. Second, different intermittent catheterization effects resulted in different feelings including frustration and happy due to failing and success. Third, patients had expectation of establishing a perfect medical system and coping with support mechanisms of family and society.Conclusions Psychological experience of patients has a process of changing. They behave different psychological feelings with failing or success of catheterization. The doctors and nurses should provide health education and technical guidance as well as psychological counseling for the patients, understand and respect their emotional needs,establish appropriate supporting system and improve their compliance.
7.Research of urinary patterns in patients with cervical spinal cord injury

Objective To investigate the urinary patterns in cervical spinal cord injury patients with different courses and degrees of injury.Methods A total of 49 patients with cervical spinal cord injury in the First Affiliated Hospital of PLA General Hospital from January 2014 to October 2016 were enrolled in this study using purposive sampling. Medical data and normal information of patients were recorded. A number of 5 hospitalized patients were interviewed face-to-face, while 44 patients were interviewed by telephone to record the urinary patterns and time of changing urination pattern.Results Indwelling catheter was the main urinary pattern and accounted for 31.0%-61.2% of patients. Patients with different injury degrees had different urinary patterns. Indwelling was the main urinary pattern of patients with complete cervical spinal cord injury and accounted for 42.9%; 14.3% of patients with complete cervical spinal cord injury could urinate by themselves. The main urinary pattern of patients with incomplete injury was self-urination, which accounted for 34.6%; there were 26.9% of these patients with indwelling catheterization.Conclusions The course and degree of injury affect the urinary pattern. Patients with cervical spinal cord injury should have individual urinary patterns to improve their bladder function and restore spontaneous urination so as to improve their quality of life to return to society.
8. Development and application of real-time qPCR assay for detecting Sendai virus gene copy number

Objective:
To develop a real-time quantitative PCR (qPCR) assay for detecting Sendai virus gene copy number.
Methods:
The recombinant plasmid was constructed and a reference used to establish the real-time qPCR method with the TaqMan probe and verified for reproducibility and sensitivity.
Results:
The linear range of the developed real-time qPCR assay was 1.024 × 103 ~5 × 1010 copies/μl, with an R2 value of more than 0. 99. The standard recovery of the tested samples was 84.56%~119.48%.
Conclusions
Compared with traditional hernagglutination assay for particle number detection, this method has higher sensitivity, better repeatability and high quantitative accuracy. It can be used for the detection of particle number of vaccine with sendai virus as the carrier, so as to detect the specific activity of vaccine products.
9.Impact of glycemic variability on left ventricular function in patients with acute ST-segment elevation myocardial infarction and type 2 diabetes

Objective To assess the impact of glycemic variability on left ventricular function in patients with acute ST-segment elevation myocardial infarction (STEMI) and type 2 diabetes.Methods Three hundred and three patients with type 2 diabetes and first STEMI between May 2014 and December 2016 in Beijing Anzhen Hospital,Capital Medical University were seclected continuously.All participants' continuous glucose monitoring system (CGMS) parameters,echocardiogram and biochemical characteristics were measured at baseline.According to the level of mean amplitude of glycemic excursion (MAGE) which is the gold indicator to present glycemic variability patients were classified into low MAGE group (n=182) and high MAGE group (n =117).Impact of glycemic variability on left ventricular function in patients with acute ST-segment elevation myocardial infarction and type 2 diabetes were analysed.Results (1) Cardiac function evaluation:The level of left ventricular ejection fraction (LVEF) were significantly lower in high MAGE group than in low MAGE group ((43.8± 7.2) vs.(52.3± 8.5) %,t =4.912,P< 0.001).There were significant differences between the two groups in Killip classification (x2 =49.931,P< 0.001).(2) Pearson correlation analysis shows that LVEF negatively correlated with the levels of MAGE(r=-0.367,P<0.001),postprandial glucose excursion (PPGE) (r=-0.274,P=0.003),Hemoglobin A1c(HbA1c) (r=-0.238,P =0.010),serum highsensitive C-reactive protein (hs-CRP) via logarithmic transformation (r =-0.245,P =0.008) and fasting plasma glucose (FPG) (r =-0.229,P =0.021).Killip classification positively correlated with the levels of MAGE (r =0.301,P < 0.001),PPGE (r =0.228,P =0.022),hs-CRP via logarithmic transformation (r =0.234,P =0.019),H bA 1 c (r =0.195,P =0.041) and FPG (r =0.193,P =0.045).(3) Multiple stepwise regression analysis and multivariate Logistic regression analysis indicated that the level of MAGE was independent risk factor of LVEF (t =-2.279,P =0.005,95% CI(-3.160 -0.219)) and the level of MAGE was an independent risk factor of Killip classification (Waldx2 =5.673,OR=1.665,95%CI(1.095-2.534),P=0.017).Conclusion Glycemic variability is associated with the presence and severity of left ventricular function in patients with STEMI and type 2 diabetes.
10.Effect of citicoline sodium combined with Eurekline on the expression of miR-17-5p and miR-29b in patients with ischemic stroke

Objective To investigate the effects of citicoline sodium and Ureicrin on the expression of miR-17-5p and miR-29b in patients with ischemic stroke.Methods A total of 100 patients with ischemic stroke treated in the hospital were divided into control group and combination group by random table method.From February 2023 to February 2024,they were respectively given Eurecrine monotherapy and citicoline sodium and eurecrine combination therapy.Cerebral blood perfusion indexes,inflammatory factors,oxidative stress,miR-17-5p,miR-29b levels and clinical efficacy were detected in the two groups,and adverse reactions were statistically recorded.Results There was no significant difference in CBF,CBV,hs-CRP,miR-29b,IL-6,TNF-α and miR-17-5p between the two groups before treatment(P>0.05).After treatment,the levels of CBF,CBV,miR-29b,SOD and GSH-Px increased,while the levels of hs-CRP,IL-6,TNF-α,miR-17-5p and MDA decreased,and the levels of CBF,CBV,miR-29b,SOD and GSH-Px in the combined group were higher than those in the control group.The levels of hs-CRP,TNF-α,IL-6,miR-17-5p and MDA were significantly different from those of the control group(P<0.05).The incidence of adverse reactions and the total effective rate of treatment in combination group were higher than those in control group(P<0.05),and the adverse reactions were mild and tolerable.Conclusion The combination of citicoline sodium and Ureicrin can improve the clinical efficacy and prognosis of patients.
