1.Clinical analysis of common abdominal operation complicated with pleural effusion

Pleural effusion is one of the common complications after abdominal surgery, which has a high incidence rate. It not only extends the length of hospital stay and increases the economic burden, but also may affect the prognosis of patients or even endanger lives. Therefore, early diagnosis and treatment of pleural effusion are important for postoperative rehabilitation. So far,the relationship between pleural effusion and clinical pathological factors of postoperation has not been revealed clearly and definitely. Preventive measures and the causes are summarized by reading a large number of literatures and combining with clinical experience to reduce incidence rate.
2.Recent advances of miRNAs in adipose tissues.

microRNAs (miRNAs), a class of endogenous non-coding RNA about 22 nucleotide long, regulate gene expression at the post-transcription level by inhibiting the translation or inducing the degradation of their target mRNAs in organisms. A lot of studies reveal that miRNAs in adipose tissues are involved in adipocyte differentiation and lipid metabolism and modulated by multiple transcription factors, adipocytokines and environmental factors, which form a complex regulatory network maintaining the homeostasis of adipose tissues. The discovery of circulating miRNAs adds new elements to the regulatory network. To study the metabolic diseases such as obesity, we should keep a new insight into the complex dynamic network. In this review, we summarize the latest studies of miRNAs in adipose tissues, which might provide new strategies for the treatment of obesity and other related diseases.

3.Impact of mild cognitive impairment on elderly well-being: the mediating effect of self-efficacy

Objective:To explore the effect of mild cognitive impairment on the subjective well-being of the elderly and the mediating role of self-efficacy in it. To provide an effective basis for health management and clinical intervention in the elderly with mild cognitive impairment.Methods:A total of 164 patients with mild cognitive impairment from December 2018 to June 2019 were surveyed by the General Self-efficacy Scale, the Montreal Cognitive Assessment Scale, and the Memorial University of Newfoundland Scale of Happiness Scale.Results:The overall score of self-efficacy was 21.41±3.98, and subjective well-being was 26.44±7.35. There was a positive correlation among cognitive function, self-efficacy and subjective well-being, except that there was no significant correlation between self-efficacy and cognitive function( r values were 0.263-0.687, P<0.01). Self-efficacy played a part in mediating between cognitive function and subjective well-being in patients with mild cognitive impairment, accounting for 37.8% of the total effect. Conclusions:Mild cognitive impairment can also have a negative impact on the subjective well-being of the elderly. Among them, self-efficacy plays a particularly important role in mediating. Interventions about self-efficacy on the elderly with mild cognitive impairment may have more significant effects.
4.Epidemiological characteristics and antiseptic resistance gene detection of Pseudomonas aeruginosa in children in Suzhou, Jiangsu Province

Abstract: Objective To investigate the clinical distribution characteristics, drug resistance trends and the carrying of antiseptic resistance gene of Pseudomonas aeruginosa infection in children in Suzhou, in order to provide theoretical basis for the prevention and treatment of Pseudomonas aeruginosa infection in children. Methods The clinical distribution characteristics and drug resistance trends of Pseudomonas aeruginosa isolated from Children's Hospital of Soochow University
from 2016 to 2021 were retrospectively analyzed. Forthermore, 101 strains of Pseudomonas aeruginosa were randomly selected to detect the expression of 9 antiseptic resistance genes (qacEΔ1-sul1, qacE, qacEΔ1, qacG, sugE(p), sugE©, emrE, ydgE, ydgF) by polymerase chain reaction. Results Pseudomonas aeruginosa in Soochow University Children's Hospital was mainly isolated from respiratory specimen (47.83%), pus (28.60%) and urine (11.72%); the main departments were intensive care unit(21.45%), general surgery department (15.71%) and respiratory department (12.31%). Patients were mainly aged from 1 month to 1 year old and older than 6 years old (34.31% and 25.38%). The top three drug resistance rates of Pseudomonas aeruginosa were imipenem (11.25%), aztreonam (9.26%) and meropenem (8.02%). Among the 853 strains of Pseudomonas aeruginosa, the drug-resistant strains were mainly from the intensive care unit (58/183), hematology department (33/91), neonatology department (31/96), and there were 57 strains of multi-drug-resistant strains with the detection rate of 6.68%. There were 98 strains (11.49%) of Carbapenem resistant Pseudomonas aeruginosa, and the annual detection rates were 22.06%, 8.40%, 3.60%, 5.67%, 9.85% and 17.20%, respectively. Among the 9 antiseptic resistance genes, the carrying rate of ydgF, sugE© and qacE was 98.02%, 94.06% and 0 respectively. Conclusion Pseudomonas aeruginosa has high resistance to some drugs, so attention should be paid to rational drug use. The carriage rates of of two antiseptic resistance genes exceeded 90%, indicating the need to strengthen research on the mechanism of antiseptic resistance research and rational use of disinfectants
5.Berberine promotes epirubicin-induced G0/G1 phase arrest in T24 bladder cancer cells

AIM:To observe the effects of the combination of berberin and epirubicin on the cell cycle of T24 bladder cancer cells and the underlying mechanisms.METHODS:The cancer cells were exposed to epirubicin in the presence or absence of different concentrations of berberin.The viability of the cancer cells was determined by MTT assay.The cell cycle distribution was detected by flow cytometry, and the protein levels of cyclin D1, CDK2, CDK4, P21 and P27 were detected by Western blot.RESULTS:Berberine markedly enhanced the inhibitory effect of epirubicin on the viability of T24 cells and promoted epirubicin-induced cell cycle arrest at G0/G1 phase as compared with the negative control cells.Epirubicin increased the protein expression of P27 and P21, both of which were enhanced by treatment with berberin.In contrast, berberin exposure further decreased the protein expression of cyclin D1, CDK2 and CDK4 in epirubicin-treated T24 cells.CONCLUSION:Berberine significantly promotes epirubicin-induced G0 /G1 phase arrest in human bladder cancer cells by up-regulating P27 and P21 expression and inhibiting the expression of cyclin D1, CDK2 and CDK4.

Objective To provide nutritional supportive scheme for patients with radiation injury through the treatment of the one exposed to Nanjing 192Ir source accident.Methods The reasonable nutrition treatment scheme was made on the basis of dietary survey and nutritional index monitoring during clinical stages of the patient,including body weight,body mass index(BMI),biochemical indexes,electrolyte,etc.,as well as metabolic cart determination of resting energy expenditure (REE).Results Patient on admission (days 5 post-irradiated) weighing 42.5 kg,172 days after the first irradiated (the first skin grafting) fell to a minimum of 36 kg,then gradually rise,hen rose back to normal range on days 383 before discharge.Normal admission hemoglobin was 135 g/L,172 d after irradiated to a minimum of 54 g/L,normal discharge;when lymphocytes admission low as 0.5 × 109/L,58 days back to normal after exposure,172 days after irradiated down to 0.4 × 109/L.Serum albumin was normal admission 41.2 g/L,172 days after irradiated down to 25.3 g/L.The normal level of serum prealbumin was 0.22 g/L,248 days to a minimum of 0.04 g/L,the basic return to normal at discharge was 0.17 g/L.Admission normal liver function,bilirubin index slightly higher,the all in one parenteral nutrition after about 2.5 months later,bilirubin and liver function indicators were gradually increased,the adjusted treatment and nutrition liver and gallbladder and other gradually returned to normal after treatment.REE and the body weight were determined by metabolic cart on days 294,308 and 342 for the energy requirements.Conclusions For patient with radiation injury,appropriate nutrition therapy is a key method for the clinical treatment and rehabilitation,which can maintain the nutritional status of patients and improve clinical treatment.
7.The effect of transcranial magnetic stimulation on the proliferation of neural stem cells and miR-9 in the brain after focal cerebral ischemia

Objective To investigate the effect of repetitive transcranial magnetic stimulation (rTMS) on the proliferation of neural stem cells in the subventricular zone (SVZ) and the expression of miR-9 in the ipsilesional hemisphere of rats with focal cerebral ischemia.Methods A rat model of acute focal cerebral ischemia was established using transient middle cerebral artery occlusion (tMCAO).Rats were then randomly assigned to a sham group,a control group or an rTMS group.Daily rTMS treatments (10 trains with 30 pulses per train and 50-second breaks between trains at 120% of the resting motor threshold) were targeted at the ipsilesional MI cortex beginning one day after the tMCAO.After the treatment the proliferation of neural stem cells in the ipsilesional SVZ was identified by double immunofluorescence staining (BrdU/nestin).The expression of miR-9 was evaluated using quantitative PCR.Results Compared with the other two groups,BrdU + Nestin + cells in the ipsilesional SVZs of the rTMS group increased and the expression of miR-9 decreased significantly.Conclusion rTMS can promote the proliferation of neural stem cells and inhibit the expression of miR-9 in the ipsilesional hemisphere after focal cerebral ischemia.
8. Development of a new type of ampoule cutter

Objective:
To develop a new practical ampule cutter with simple operation.
Methods:
According to the actual clinical situation, the structure of the ampoule cutting device were designed, which was mainly divided into three parts: the handle, the ampoule cutting tube and the ampoule cutting device.
Results:
The device can effectively cut the bottleneck of ampoule bottle and avoid the injury of sharp instrument and drug contamination caused by traditional breaking the bottleneck of ampoule bottle.
Conclusion
This kind of ampoule cutter can break the bottleneck of ampoule effectively, avoid the cross infection of medical staff, improve the work efficiency of medical staff and the effective utilization rate of medicine.
