1.Research in effect of centralized training of basic operational skills for nursing students before nursing practice

Objective To discuss effect of centralized training of basic operational skills for nursing students before nursing practice. Methods 176 student nurses in 2007 were set as the control group,320 nursing students in 2008 as the experimental group. The control group began practice after routine pre -practice education, based upon this, the experimental group carried out centralized training of basic operational skills for one week, only those who passed the examination could began practice, those who failed the examination should continued the training till they passed the examination. Time cost to master the operational skills, operational assessment results and evaluation of nursing practice by teachers were compared between the two groups. Results Compared to the control group, time cost to master the operational skills shortened, operational assessment results and evaluation of nursing practice by teachers improved in the experimental group. Conclusions Through centralized training of basic operational skills, time cost to master the operating skills of basic nursing by nursing students shortened, operational capability of nursing students can be improved, and the clinical nursing teaching quality can be promoted.
2.Differences and risk factors of regimen modification in acquired immunodeficiency syndrome patients who initiated antiretroviral treatment

Objective To compare the rates of regimen modification between patients with different initial antiretroviral therapy, and to investigate risk factors associated with drug toxicity-related regimen modification.Methods A two-years retrospective cohort study was conducted in 14 060 patients who initiated antiretroviral treatment with Zidovudine (AZT)/Tenofovir disoproxil (TDF)+Lamivudine (3TC)+Efavirenz (EFV) since 2012.There were 5 126 patients initiated TDF+3TC+EFV therapy (TDF group) and 8 934 patients initiated AZT+3TC+EFV therapy (AZT group).Chi-square test was used to compare the rate of first-line regimen modification and the rate of toxicity-related regimen modification between two groups.Cox proportional hazard model was used to investigate the risk factors associated with regimen modification.Results A total of 14 060 acquired immunodeficiency syndrome patients were observed for a median period of 1.85 person-years.There were 2 795 patients who changed their initial antiretroviral regimen and the rate of initial regimen modification was 19.9%.Two hundred patients who changed their initial regimen due to pregnancy were excluded.There were 2 070 patients in AZT group who changed their initial regimen with a rate of 23.5%.Among them, 1 652 patients changed their regimen due to drug toxicity and the rate was 18.8%.There were 525 patients in TDF group who changed their initial regimen with a rate of 10.4% and the rate of toxicity-related regimen modification was 6.2%.The differences between two groups were statistical significance (χ2=366.68 and 416.89, respectively, both P<0.01).The risk of regimen modification in AZT group were significantly higher than that in TDF group (aHR=2.89, 95%CI: 2.57-3.24).The risk of toxicity-related regimen modification in AZT group was also significantly higher than that in TDF group (aHR=3.85, 95%CI: 3.34-4.45).Conclusions Patients initiated antiretroviral treatment with AZT+3TC+EFV are more likely to change their initial regimen than those who initiated treatment with TDF+3TC+EFV.Female, age >45 years old, BMI<18.5 kg/cm2 and baseline CD4+ T cell count<200/mL were risk factors associated with regimen modification.
3.Swelling property of common hydrophilic polymers and their use in push-pull osmotic-pump tablets.

OBJECTIVETo investigate the feasibility of swelling hydrogel instead of polyethylene oxide as swelling polymer in push-layer of push-pull osmotically controlled-release tablets.
METHODThe swelling patterns of tablets made of pure polymers were studied by immerging the tablets into purified water and testing their size at different time points. The push-pull osmotic-pump tablets were prepared and their release patterns in vitro were studied and compared by their similar factor (f2).
RESULTTablets with different swelling materials all showed satisfying release pattern in vitro and their release ratio at 12 h were all above 80%.
CONCLUSIONWith its release rate and cumulative release percentage at 12 h, the mixture of HPMC K15M and CMCNa in ratio of 1:1 is the best choice instead of polyethylene oxide as swelling polymer in push-layer.

4.Clinical value of vitamin D receptor gene polymorphism in lung cancer screening

Objective To investigate the clinical value of vitamin D receptor ( VDR) gene polymorphism in lung cancer screening .Methods From January 2017 to September 2017 ,2000 lung cancer screening patients in the Fifth People's Hospital of Foshan were selected in the research .The VDR gene polymorphism was determined by phenol-chloroform method .DNA was extracted from the peripheral blood of patients ,different VDR genotypes [ Bsm1 bb and Bb),Apal(aa,Aa and AA)] were analyzed by univariate analysis to determine the correlation between lung cancer and VDR gene polymorphism and lung cancer incidence by multivariate non -conditional logistic regression analysis, thus to explore the relationship between different VDR genotypes and risk factors of lung cancer . Results The level of serum vitamin D in the non -lung cancer patients was (46.5 ±2.3) ng/L,which was signifi-cantly higher than (26.5 ±1.1)ng/L in the lung cancer patients (t=49.614,P=0.000).The Bsm1bp genotype, Apal aa genotype and Apal Aa genotype were the risk factors of lung cancer .The Bsm1bp and Apal Aa genotypes were independent risk factors of lung cancer .Conclusion The Bsm1 locus and Apal locus Aa genotype in VDR receptor are high risk population of lung cancer .Therefore,we should pay more attention to the clinical screening and avoid misdiagnosis and missed diagnosis .
5.The three-dimensional culture of adult mesenchymal stem cells for intervertebral disc tissue engineering.

Intervertebral disc (IVD) degeneration is one of the major causes of low back pain. As current clinical treatments are aimed at restoring biomechanical function and providing symptomatic relief, the methods focused on biological repair have aroused interest and several tissue engineering approaches using different cell types have been proposed. Owing to the unsuitable nature of degenerate cells for tissue engineering, attention has been given to the use of mesenchymal stem cells (MSCs). In this connection, we have made a study on the characteristics of MSCs derived from adult bone marrow and on the feasibility of constructing IVD tissue-engineering cell under a Three-Dimensional Pellet Culture System. The human bone marrow MSCs were isolated and purified with density gradient solution and attachment-independent culture system. MSCs isolated using this method are a homogeneous population as indicated by morphology and other criteria. They have the capacity for self-renewal and proliferation, and the multilineage potential to differentiate.

6.Treatment progress of myelodysplastic syndromes

The effective treatment methods of myelodysplastic syndromes (MDS) are limited. The patient's treatment plan is mainly based on individual differences and different risk levels [by revised International Prognostic Points System (IPSS-R)], including observation, erythropoiesis stimulating agents, iron elimination, immunosuppressive agents, lenalidomide, hypomethylating agents and hematopoietic stem cell transplantation (HSCT). The limitations of the treatment strategies are important exploration directions for future clinical trials. At present, multiple clinical trials for the treatment of MDS are underway, but it is still necessary to optimize therapies through integrating molecular and genetic data and applying them to clinical practice. This article reviews the current treatment approaches of MDS and looks forward to future research directions.
7.Reliability and validity of Chinese version of sensory-motor function scale during awake craniotomy

Objective To translate English version of sensory-motor profile awake ( SMP-a) into Chinese version (the Chinese Version of SMP-a),and analyze the reliability and validity of the scale before and after craniotomy under awakening anesthesia. Methods Eighty-one patients whose tumors were located near or already in sensory-motor functional area were included in this study. Before and after awake cranioto-my,the Chinese version of SMP-a was used to accurately assess the sensory-motor function of each patient. Finally, the reliability and validity of the scale were analyzed by SPSS statistical software. Results Cronbach's α coefficient in the Chinese version of SMP-a was 0. 971,and Cronbach's α coefficient in the four subscales of face,hand,leg and sensation was 0. 965,0. 989,0. 981 and 0. 970,respectively. The test-retest reliability of the Chinese version of sensorimotor assessment scale was 0. 910,0. 904,0. 884,0. 898 and 0. 695 (total,face,hands,legs and sensory score respectively). The raters' consistency reliability was above 0. 949,0. 960,0. 934,0. 887 and 0. 660,respectively. The Pearson correlation coefficients of sensorimotor function score with SF-36 physiological function factors and KPS score were 0. 868 and 0. 790,respectively. Conclusion Before or after operation,the Chinese version of SMP-a has preferable reliability,internal con-sistency reliability and structural validity. It is feasible in awakening anesthesia craniotomy,and the degree of damage can be determined by repeated measurement of the sensorimotor sites that may be impaired by the patient.
