1.Literature Analysis of ADR Caused by Antigout Agents

OBJECTIVE:To investigate rules and characteristics of ADR induced by antigout agents,and to provide reference for clinical rational drug use. METHODS:A total of 146 literatures,167 putients of ADR induced by antigout agents,which were reported during Jan. 1979-Dec. 2014. RESULTS:Of 167 ADR cases,the ratio of male to female was 5.4:1,and ADR patients aged above 70 years occupied the highest proportion(34.14%);Most ADR were induced by allopurinol(137 cases)ADR induced by antigout agents mainly were dermal system injury(60.00%),systemic reaction(9.19%),urinary system injury(5.95%),etc. and the specific performance manifested as erythra (51 cases),dermatitis exfoliativa (38 cases),drug hypersensitivity syndrome (13 cases),bullous epidermal necrolysis type of drug eruption (11 cases),Stevens-Johnson syndrome (9 cases) and so on. New and severe ADR were 66 cases,13 cases of severe ADR were dead. CONCLUSIONS:ADR induced by antigout agents is common in clinical and should be treated promptly,patient's care also should be strengthen to ensure patient medication safety and improve clinical treatment.
2.Expression and function of CD40 on rat peritoneal mesothelial cells

Objective To determine whether peritoneal mesothelial cells express CD40 and to investigate potential mechanism by which CD40-CD40 ligand(CD40L) interaction may be involved in the inflammation of peritoneal membrane. Methods Rat peritoneal mesothelial cells (MC) were harvested from peritoneal cavity and maintained under defined in vitro conditions. Expression of CD40 on MC under normal culture or stimulation with interferon-?(IFN-?), tumor necrosis factor-?(TNF-?), interleukin(IL)-1 was examined by RT-PCR and FACS analysis. After activation with CD40 mAb, the expression of intercellular adhesion molecule-1 (ICAM-1) on MC was analyzed by FCAS. Results MC cultured in vitro expressed CD40 constitutively. The expression of CD40mRNA and CD40 protein up-regulated markedly following the stimulations with IFN-?, but not with IL-1, TNF-?. The expression of ICAM-1 on MC was significantly increased after activation of CD40 with IFN-? and CD40mAb. Conclusions MC functionally expresses CD40. The interaction of CD40 on MC and CD40L+ cells in peritoneal cavity may play an important role in peritoneal local defense and may be involved in the inflammation process of peritoneum.
3.Analysis on clinical curative effect of compound polymyxin B ointment combined with basic fibroblast growth factor to treat the wound surface of deepⅡdegree burn of children

Objective To observe the clinical curative effect of compound polymyxin B ointment combined with basic fibroblast growth factor (bFGF) on wound surface of deep second-degree burn in children, and provide reference for the clinical treatment. Methods Eighty cases of children patients with deep second-degree burn were included in this study, who were admitted in Tianjin 4th Center Hospital from March 2015 to March 2016. The cases were divided into control group and combined treatment group, 40 cases for each group. Wherein, the control group was treated with bFGF, the combined treatment group was received compound polymyxin B ointment and bFGF. The occurrence rate of infection of burn wound surface, wound healing time and occurrence rate of scar after healing were compared between two groups. Results There were lower infection rates on day 7, 10, 15 and 20 after burn in combined treatment group than those in control group (P<0.05). The healing time was significantly shorter in combined treatment group than that in control group [(21.53±1.33) d vs. (25.76±1.50) d, t=13.345, P<0.01]. The scar occurrence was significantly lessin combined group than that of control group (U=5.077,P<0.05). Conclusion The compound polymyxin B ointment combined with bFGF show a certain effect of anti-infection, acceleration of wound surface healing and preventing the scar from generating for wound surface on deep second-degree burn in children, which having a significant clinical curative effect.
4.Perceptions of medical staff on uncomfortable symptoms of leukemia children during chemotherapy

Objective To investigate the perceptions of medical staff about chemotherapeutic symptoms in children with leukemia.Method Sixty-one doctors and nurses were involved in the investigation using a self-designed questionnaire on uncomfortable symptom scale.Result The main symptoms of chemotherapy included hair loss,poor appetite,nausea,vomiting, oral mucositis and fatigue.Conclusion Chemotherapeutic symptoms are common in children with leukemia and need appropriate nursing management.
5.Correlations between molecular subtypes and clinical pathological features in 621 breast invasive ductal carcinoma patients

Objective To investigate the distribution of breast invasive ductal carcinoma with different molecular subtypes ,and to study the relationship between molecular subtypes and clinical pathological features in Xinjiang area .Methods A total of 621 patients with IDC from January 2013 to January 2014 was classified into Luminal A type,Luminal B type,HER-2 overexpression type and Basal -like type,different molecular subtypes according to the estrogen receptor(ER),progesterone receptor(PR),epidermal growth factor receptor -2(HER-2) and Ki-67,and compared the statistics combining with the clinical pathological characteristics of IDC .Re-sults The result showed that Luminal B type accounted for 53.1%,Luminal A type,HER-2 overexpression type and Basal-like type accounted for 14.5%,15.9%,16.4%,respectively.There were significant differences between the molecular subtypes and tumor size ,histological grade ,tumor pathological stage ,the expression of ER , PR and HER-2(P<0.05 for all).There were no correlations between the molecular subtypes and age ,as well as lymph node metastasis(P>0.05).Conclusion There is a close relationship between invasive ductal carcino-ma of different molecular subtypes and clinical pathological characteristics .The molecular subtypes are beneficial to treatment guidelines for clinical individual .
6.Literature Analysis of 132 Cases of Adverse Drug Reaction Induced by Monoclonal Antibody

OBJECTIVE:To investigate rules and characteristics of the adverse drug reactions(ADR)induced by monoclonal antibodies,and provide reference for rational drug use. METHODS:The literatures of ADR induced by monoclonal antibodies from Jan. 1979 to Dec. 2014 were retrieved and analyzed. RESULTS:Among the 132 ADRs induced by monoclonal antibodies,57 were male(43.18%),75 were female(56.82%),the ratio of male to female was 1∶1.3,the youngest was 2 years old,and oldest was 111 years old;Adalimumab dominated monoclonal antibody drugs(70 cases,53.03%);there were a maximum of ADR(44) within 1 d,accounting for 33.33%;ADR induced by monoclonal antibodies mainly were skin lesions (20.72%),systemic reac-tions(18.42%)and cardiovascular system damage(16.78%),with the specific performance of rash,itching,chills,fever,palpi-tation,rapid heartbeat and so on. CONCLUSIONS:ADR induced by monoclonal antibodies is more common in the clinic. The clin-ic should timely promote and strengthen the patients' care to ensure safe drug use and improve level of clinical treatment.

Objective After an experimental exposure to low dosage of Cd 2+(39*!?mol/L) for 30 days,the changes of Cd 2+ on ultrastructure of the hepatopancreas cells of the freshwater crab (Sinopotamon yangtsekiense♂)at three different times were studied. Methods Sixty freshwater crabs(♂)were divided into two groups:control(n=30) and test(n=30)groups.The test(n=30) group was exposed to low dosage of Cd 2+(39*!?mol/L)while the control crabs were in water without cadmium.During the 5*!d,15*!d,30*!d,the ultrastructure of the hepatopancreas cells was observed by using transmission electro-microscopy(TEM). Results In the course of the experiment,the nuclear membrane changed from spreading all over the place to disintegrated,and the penetration of the mitochondrial outer membrane and inner membrane was changed.The mitochondria showed swelling and vacuolating.Mitochondrial cristae partly disintegrated,then fully disintegrated.Besides,the Cd 2+ exposure affected the gross form and the structure of the rough endoplasmic reticulum(RER).At first,the RER became the different sized vesicles,then,the ribosomes on the little vesicles began to fall,and finally,the RER turned to the concentric round lamellar structures.But the smooth endoplasmic reticulum(SER)increased markedly.In addition,the numbers and types of lysosome were increased in the mean time and some lysosomes became void vesicles,the autophagosomes or the myeloid bodies(MD).The microvilli showed partly falling and vacuolating.Conclusion After the cadmium treatment the detoxification mechanism of the freshwater crab's hepatopancreas cells lost,the structural and functional integrality of the mostly organelles,such as the nucleous,the mitochondria and the rough endoplasmic reticulum,were destroyed,so that the normal physiological function of hepatopancreas cells was affected.
9.Simultaneous Determination of 7 Active Constituents in Tangshen Qingdu Granule by HPLC-DAD

OBJECTIVE:To establish a method for the simultaneous determination of 7 active constituents in Tangshen qing-du granule. METHODS:HPLC was performed on the column of SHIMADZU Inert Sustain C18 with mobile phase of acetonitrile-0.1%phosphoric acid at a flow rate of 1.0 mL/min,detection wavelength was 327 nm for chlorogenic acid and caffeic acid,280 nm for baicalin,228 nm for arctiin and 276 nm for wogonoside,baicalein and wogonin,column temperature was 35℃,and injection volume was 10 μL. RESULTS:The linear range was 4.830-154.6 μg/mL for chlorogenic acid and(r=0.9998),0.750-24.1 μg/mL for caffeic acid(r=0.9997),22.859-731.5 μg/mL for baicalin(r=0.9997),8.491-271.7 μg/mL for arctiin(r=0.9993),2.471-79.0μg/mL for wogonoside(r=0.9996),6.656-213.0 μg/mL for baicalein(r=0.9994) and 2.756-88.2 μg/mL for wogonin (r=0.9998);RSDs of precision,stability and reproducibility tests were lower than 2.0%,recoveries were 96.86%-100.82%(RSD=1.46%,n=6),98.79%-101.09%(RSD=0.93%,n=6),97.57%-101.51%(RSD=1.37%,n=6),97.76%-99.63%(RSD=0.77%,n=6),97.99%-100.12%(RSD=0.76%,n=6),96.54%-101.07%(RSD=1.87%,n=6) and 96.60%-99.59%(RSD=1.14%,n=6). CONCLUSIONS:The method is simple with good precision,stability and reproducibilty,and can be used for the simultaneous determination of 7 active constituents in Tangshen qingdu granule.
10.Shape variation of optic nerve head by mechanical analysis using three-dimensional finite element model

BACKGROUND:The measure and prediction of the shape of optic nerve head are the key issues for early diagnosis and duration prediction of glaucoma. Therefore, it is significant to research the shape variation of optic nerve head under high intraocular pressure.
OBJECTIVE:To establish three-dimensional finite element model of optic nerve head and to analyze the deformation of optic nerve head and the change in retina thickness after acute high intraocular pressure.
METHODS:Animal model of acute ocular hypertension was established by methods of anterior chamber perfusion. The tomographic images of the optic nerve head of a cat were obtained at the normal intraocular pressure and 5 320 Pa, 7 980 Pa, 10 640 Pa, 13 300 Pa, 15 960 Pa intraocular pressures using optical coherence tomography. Then, we measured the variation of retinal thickness at typical location. Basing on the tomographic images of optic nerve head of a cat at normal intraocular pressure, we obtained three-dimensional structure of retina and choroid using software MIMICS. Then three-dimensional model of optic nerve head was established by assembling the retina and choroid. The deformation of optic nerve head and the change in retinal thickness under high intraocular pressure were observed using software ABAQUS. The effectiveness of the model was verified by the experimental result.
RESULTS AND CONCLUSION:With the increase of intraocular pressure, the retinal thickness is thinner, the optic nerve head depth, width and the ratio of the depth to width are gradual y increased. It suggests that the acute high intraocular pressure causes retinal thinning, optic nerve head widening and deepening. It is feasible to establish optic nerve head modeling in vivo by using optical coherence tomography, mechanical analysis can be applied to predict the shape variation of optic nerve head, which is significant to further deduce the pathological process of glaucoma.
