1.Literature Analysis of 132 Cases of Adverse Drug Reaction Induced by Monoclonal Antibody

OBJECTIVE:To investigate rules and characteristics of the adverse drug reactions(ADR)induced by monoclonal antibodies,and provide reference for rational drug use. METHODS:The literatures of ADR induced by monoclonal antibodies from Jan. 1979 to Dec. 2014 were retrieved and analyzed. RESULTS:Among the 132 ADRs induced by monoclonal antibodies,57 were male(43.18%),75 were female(56.82%),the ratio of male to female was 1∶1.3,the youngest was 2 years old,and oldest was 111 years old;Adalimumab dominated monoclonal antibody drugs(70 cases,53.03%);there were a maximum of ADR(44) within 1 d,accounting for 33.33%;ADR induced by monoclonal antibodies mainly were skin lesions (20.72%),systemic reac-tions(18.42%)and cardiovascular system damage(16.78%),with the specific performance of rash,itching,chills,fever,palpi-tation,rapid heartbeat and so on. CONCLUSIONS:ADR induced by monoclonal antibodies is more common in the clinic. The clin-ic should timely promote and strengthen the patients' care to ensure safe drug use and improve level of clinical treatment.
2.Simultaneous Determination of 7 Active Constituents in Tangshen Qingdu Granule by HPLC-DAD

OBJECTIVE:To establish a method for the simultaneous determination of 7 active constituents in Tangshen qing-du granule. METHODS:HPLC was performed on the column of SHIMADZU Inert Sustain C18 with mobile phase of acetonitrile-0.1%phosphoric acid at a flow rate of 1.0 mL/min,detection wavelength was 327 nm for chlorogenic acid and caffeic acid,280 nm for baicalin,228 nm for arctiin and 276 nm for wogonoside,baicalein and wogonin,column temperature was 35℃,and injection volume was 10 μL. RESULTS:The linear range was 4.830-154.6 μg/mL for chlorogenic acid and(r=0.9998),0.750-24.1 μg/mL for caffeic acid(r=0.9997),22.859-731.5 μg/mL for baicalin(r=0.9997),8.491-271.7 μg/mL for arctiin(r=0.9993),2.471-79.0μg/mL for wogonoside(r=0.9996),6.656-213.0 μg/mL for baicalein(r=0.9994) and 2.756-88.2 μg/mL for wogonin (r=0.9998);RSDs of precision,stability and reproducibility tests were lower than 2.0%,recoveries were 96.86%-100.82%(RSD=1.46%,n=6),98.79%-101.09%(RSD=0.93%,n=6),97.57%-101.51%(RSD=1.37%,n=6),97.76%-99.63%(RSD=0.77%,n=6),97.99%-100.12%(RSD=0.76%,n=6),96.54%-101.07%(RSD=1.87%,n=6) and 96.60%-99.59%(RSD=1.14%,n=6). CONCLUSIONS:The method is simple with good precision,stability and reproducibilty,and can be used for the simultaneous determination of 7 active constituents in Tangshen qingdu granule.

4.Study on Water Extraction Technology of Yinju Jiedu Oral Liquid

OBJECTIVE:To optimize the water extraction technology of Yinju jiedu oral liquid,and provide reference for the industrial production of the preparation. METHODS:According to the investigation of extraction time-extraction rate curves of chlo-rohenic acid of Yinju jiedu formula and extraction rate of chlorohenic acid in Lonicera japonica and other combined medicinal mate-rials in the formula,decoction methods and time of L. japonica were determined. Using the comprehensive scores of linarin,harpa-goside,(R,S)-epigoitrin,psoralen+angelicin contents and dry extraction yield as indexes,L9(34)orthogonal test was designed to detect the effects of adding water amount,decoction time times and optimize the extraction technology of the residues and other me-dicinal materials. Verification test was conducted. RESULTS:The optimal technology was L. japonica decocted first for 30 min with 8-fold water;the residues and other medicinal materials were decocted with 8-fold water for 3 times,1 h each time;combin-ing all the syrups. In verification test,the average contents of chlorohenic acid,linarin,harpagoside,(R,S)-epigoitrin,psoralen+angelicin were respectively 34.51,10.31,1.97,0.21,9.79 mg/g(RSD=1.24%,1.19%,1.40%,1.71%,1.28%,n=3);aver-age dry extraction yield was 25.4%(RSD=1.64%,n=3);average extraction rate of chlorohenic acid was 78.95%(RSD=1.24%,n=3). CONCLUSIONS:In the optimized water extraction technology,both the extraction rate of chlorohenic acid and contents of other ingredients are relatively high. The technology is stable and feasible.
5.Bacteriological Efficacy of Amoxicillin/sulbactam on Acute Bacterial Infections

0.05);the bacterial sensitive rates of amoxicillin/(sulbactam),(amoxicillin)/clavulanate,amoxicillin,ampicillin/sulbactam and cefotaxime were 88.42%,86.78%,57.02%,(86.78%) and 85.95%,respectively;the antibacterial potency of amoxicillin/sulbactam was higher than(amoxicillin)/clavulanate that of from the results of MIC_(90).CONCLUSIONS Amoxicillin/sulbactam has good (bacteriological) efficacy.
6.Correlation analysis of bacterial biofilm formation and bacterial culture in chronic otitis media.

OBJECTIVE:
To study the correlation between the bacterial biofilm formation and bacterial culture in chronic otitis media.
METHOD:
As a prospective reserch, we used scanning electron microscopy to examinate patients samples which collected from 32 cases of patients with chronic suppurative otitis media and middle ear cholesteatoma in the operations, and performed the middle ear secretions bacterial culture. According to the different types of chronic otitis media group, we analysised the relationship between chronic otitis media bacterial biofilm formation and the bacterial culture results.
RESULT:
Chronic suppurative otitis media (activity) and middle ear cholesteatoma bacterial biofilm formation rate were 87.5%, 81.3%, chi-square (P > 0.05). Compared bacterial biofilm results with the results of bacterial cultured in chronic otitis media, sensitivity was 70.37%, specificity was 60.00%, the misdiagnosis rate was 40.00%, the missed diagnosis was 29.63%, positive predictive value was 90. 46%, negative predictive value was 27.27%, accuracy was 68.75%. Youden index was 30. 37%, and Pearson correlation coefficient was 0.232 (P > 0.05).
CONCLUSION
Chronic suppurative otitis media (activity) and middle ear cholesteatoma bacteria had a higher biofilm formation rate. The routine bacterial culture results can't reflecte bacterial biofilm formation in chronic otitis media. We need to explore more reliable experimental methods to accurately reveal the infection status of chronic otitis media.

7.Diagnosis and surgical management of tumors primarily in the pterygopalatine fossa

OBJECTIVE To evaluate the clinical features of tumors primarily in the pterygopalatine fossa, and the efficiency of surgical management for these lesions. METHODS The clinical data of 7 cases with tumors primarily in the pterygopalatine fossa were retrospectively studied. Three of them were primary diseases, i.e. fibrous histiocytoma, neurofibroma and cholesteatoma. The other 4 cases were secondary tumors mainly located in pterygopalatine fossa. There were 1 case with epithelial-myoepithelial carcinoma, 1 case with adenoid cystic carcinoma , 1 case with recurrent inverted nasal papilloma, 1 case with recurrent malignant fibrous histiocytoma. Approaches to tumors in pterygopalatine fossa lesions included lateral rhinotomy, and transnasal or transantrum approaches under the nasal endoscope. RESULTS The patient suffered from adenoid cystic carcinoma developed local recurrence 4 months after operation, and extended resection of the recurrent tumor with laser was performed again. No further recurrence was found after following-up for 3 years. Neither local recurrence nor regional metastasis was found in the remaining 6 cases with a follow-up period of 2 to 4 years. The main complication was oronasal fistula. CONCLUSION CT scan or MRI is the main method to the early diagnosis of pterygopalatine fossa tumors. Lateral rhinotomy, endoscopic trasnnasal or transantrum approaches are feasible procedures to resect the tumors.
8.Effect of arotinolol on right ventricular function in patients with dilated cardiomyopathy

Objective Dilated cardiomyopathy (DCM) is generally considered to be accompanied by both left and right ventricular dysfunction,but most studies only analyze the left ventricular function. In this study, we evaluated the effect of arotinolol on right ventricular function in patients with DCM. Methods Right ventricular ejection fraction (RVEF) and right ventricular diameter (RVD) were measured by two-dimensional echocardiography (2-DE) in 33 DCM patients; RVEF measured by first-pass radionuclide angiography (FPRA) was compared with that by 2-DE. Results The treatment with arotinolol for one year resulted in a reduction in the right ventricular diameter (baseline, 23.0 ± 8.3 mm vs after one-year treatment, 20.7 ± 5.4 mm; P=0.004 ) and an associated increase in ejection fraction (baseline, 36.9 ± 10.3% vs after one-year treatment, 45.8 ± 9.6%; P < 0.001 ); there is a high correlation between the 2-DE method and radionuclide ventriculographic method. The correlation coefficient is 0.933 (P<0.001). Conclusion Arotinolol therapy could not only improve left ventricular function, but also improve right ventricular function in DCM patients.
9.Diagnosis, treatment and characteristics of adult Moyamoya disease in countryside in the southeast of Hubei province

Objective To analyze the clinical features,diagnosis and treatment of adult Moyamoya disease in countryside of the southeast of Hubei province.Methods Sixty-eight adult patients with Moyamoya disease,selected in countryside of the southeast of Hubei Province from May 2010 to May 2015,were enrolled.The clinical data,including age,gender,address (surrounding of residence),living habit (special hobby),health of family members,past medical history,symptom,confirmation related factors,treatment methods and prognoses,were retrospectively analyzed.Results These patients had high and low incidences in distribution.The ratio of male to female was 1.09:1.The peak age of onset was 35 to 44 years.There were 42 bleeding patients,16 ischemia patients,and 7 patients with atypical symptom,and 3 patients were asymptomatic.Thirty-eight patients (55.9%) had early diagnosis and 30 (44.1%) had late conformed diagnosis;patients with early diagnosis had significantly higher percentages of first diagnosis in the tertiary hospitals,hemorrhagic apoplexy as first onset,high education level and economic level than patients with late conformed diagnosis (P<0.05).Ten patients died.Eight patients received vascular reconstruction,accounting for 11.8% (8/58).Conclusions The distributions of adult moyamoya disease are regional cluster.The main age of onset is at the life prime.The initial symptom is hemorrhagic stroke.The time for diagnosis was short in the tertiary hospitals.The ratio of patients receiving vascular reconstruction is low.
10.Study on the correlation of integr in associated protein and disease activity and bone destruction in patients with rheumatoid arthritis

Objective:To analyze the serum levels of integrin-associated proteins (CD47) in patients with rheumatoid arthritis (RA), and to explore its association with disease activity and bone destruction in RA.Methods:Serum and clinical data were collected from 65 RA patients and 25 healthy subjects. RA patients were grouped into low, moderate, and high bone erosion groups according to 7-joint ultrasonography score (US7). The levels of serum CD47, thrombospondin-1 (TSP-1) and receptor activator of nuclear factor-κB ligand (RANKL) were measured by enzyme-linked immunosorbnent assay (ELISA) in patients with RA and healthy subjects. The statistical analysis was carried out with independent t-test, analysis of variance, nonparametric rank sum test, pearson or Spearman correlation and logistic regression. Results:① The Serum levels of CD47, TSP-1, and RANKL were higher in the RA group than in the healthy controls ( P<0.01). ② In RA patients, serum CD47 level was positively correlated with disease course ( r=0.301, P<0.05), C-reactionprotein (CRP)( r=0.316, P<0.05), number of tender joints (TJC) ( r=0.254, P<0.05), number of swollen joints (SJC) ( r=0.316, P<0.05), disease activity score in 28 joints (DAS28) ( r=0.255, P<0.05), RANKL ( r=0.252, P<0.05) and TSP-1 ( r=0.260, P<0.05). Serum TSP-1 level was positively correlated with CRP ( r=0.299, P<0.05), TJC ( r=0.335, P<0.01), DAS28 ( r=0.315, P<0.05), RANKL ( r=0.305, P<0.05). ③ The disease course [ OR(95% CI)=1.048(1.033, 1.017)] and TSP-1 [ OR(95% CI)=1.013(1.000, 1.026)] were independently relevant factors affecting bone destruction. Conclusion:CD47 levels is significantly higher in RA patients than in healthy controls, and is associated with disease activity and bone destruction. CD47 may be involved in the bone destruction process of RA by acting on TSP-1.
